Evolution of Large-Scale Filmless Full-PACS in Korea

  • Filmless full-PACS in korea has rapidly been growing, since government had supported collaborative PACS project between industry and university hospital in late of 1995. At the same time, a small company had started PACS business, while the Korea PACS society was being formed. In the beginning, PACS societies had focused on developing peripheral solutions such as DICOM gateway for image acquisition, x-ray film digitizer, and viewing software for research or management of personal image data, while Samsung Medical Center had started installing an imported partial PACS system which had recently upgraded with a new system. In similar time frame, a few hospitals had started developing and installing domestic large scale full-PACS system. Several years later, many hospitals have installed full-PACS system with national policy of reimbursement for PACS exams in November 1999. It is believed that Korea is the first country that adopted PACS reimbursement for filmless full-PACS as a national policy. Both experiences of full-PACS installation and national policy generated tremendous intellectual and technological expertise about PACS at all levels, clinical, hospital management, education, and industrial sectors. There are currently three types of PACS system which includes domestic, imported, and hybrid PACS system with imported solution for core system and domestic solution for peripheral system. There are more than 20 domestic PACS companies and they have now enough experiences so that they are capable of installing a truly full-PACS system for large-scale teaching hospitals. PACS societies in Korea understand how to design, implement, install, manage, sustain, and provide good services for large-scale full-PACS. PACS society has also strength for the highest integration technology of the Hospital Information. However, further understanding and timely implementation of continuously evolving international standard and integrated healthcare enterprise concepts may be necessary for international leading of PACS technologies for the future.

Dental PACS development in Korea

  • Picture archiving and communication system (PACS) is an image information technology system for the transmission and storage of medical images. In Korea the first full PACS was installed at Samsung Medical Center in 1994, but, the rate of distribution was very slow. The government's approval for the medical insurance reimbursement for full PACS examinations in November 1999 became the turning point. Thereafter the number of hospitals with full PACS has steeply increased. In September of this year, PACS was installed at 906 medical institutes, including most of university hospitals and general hospitals. The first full dental PACS was installed at Wonkwang University Dental Hospital in 2002. Now ten out of eleven university dental hospitals implemented full dental PACS. The current status and technological factors of dental PACS in Korean university dental hospitals and the future perspectives of dental PACS are described.   • The purpose of this paper is to describe the transition of a 1,100 beds teritary hospital from 50% softcopy operation to full PACS operation. For the past 2 years, radiologists and clinicians have been using PACS to provide softcopy services to the outpatient clinics and inpatient wards of orthopedics surgery, neurosurgery and neurology as well as emergency room, surgical intensive care unit, medical intensive unit, pediatrics intensive care unit and neonatal intensive care unit. The examinations requested by these departments account for about 50% of hospital's radiological exams. In September 1996, we began the second phase of PACS implementation and installed additional workstations (102) in the remaining wards and clinics, interfaced to PACS additional imaging modalites, and increased the capacity of both the image server (256 Gbytes) and optical juke boxes (3 Tbytes). As of January 1997, we are in the final phase of moving away from conventional film system to full PACS operation.

  • The full-field digital mammography (FFDM), which has been known as a digital breast imaging system, carries out more outstanding performance than the screen-film mammography in overall image quality, skin & nipple, description of pectoral muscle and expression of micro-calcification. Thus, in this thesis, I perform experiments for both the enhancement of image quality and accurate estimation of the result in question, when detecting the very tiny-sized lesions in mammography. The image of digital breast X-rays is the important diagnostic tool for detecting early breast cancer and micro calcification lesion. The experiment of how much compression rate has an effect on the result of diagnosis in the case of microcalcification lesion, with JPEG2000 40:1 compression and over 50% enlargement led to obscure or definitely unacceptable diagnostic results is performed. And in another study of assessment of PSNR degree. I recognized the importance of standardized management system in mammography, where not to mention the accurate reading of the image has the most crucial role in diagnosis

Current status and installation standard of dental PACS

  • Purpose: Picture Archiving and Communication System (PACS) is difficult to implement in the best of situations, but evidence is growing that the benefits are significant. The aims of this study are to analyze the current status of full PACS and establish successful installation standard of dental PACS. Materials and Methods : Materials and methods were based on the investigation of current working status and installation standard of PACS, and observation of variable issues to installation of dental PACS. Results: By September 30,2004, full PACS implementations in their facilities were 88.1 % in specialized general hospitals (37 installations out of total 42 hospitals), 59.8% in general hospitals (144 installations out of total 241 hospitals), 12.3% in medical hospitals (116 installations out of total 941 hospitals) and 3.6% in dental hospitals (4 installations out of total 11 0 hospitals) Only 4 university dental hospitals currently have installed and are operating full PACS. Major obstacle to wide spread of dental PACS is initial high investments. Conclusions: Clinical environments of dental PACS differed from medical situation. Because of characteristic dental practice, the initial investments for dental PACS are generally much greater than those of medical PACS. Also new economic crisis makes users scruple. The best way to overcome these limitations is to establish an economic installation standard for dental PACS. Also the clear technical communication between the customer and the supplier before both sides are committed to the obstacles are critical to its success.

  • This thesis describes the effective interfacing methods of PACS Modality based on the system installation andoperating experiences. PACS(Picture Archiving and Communication Systems) is a system for medical image archiving and communication using large storage device and high-speed network. The standard communication protocol of PACS is DICOM(Digital Imaging and Communication in Medicine) based on TCP/IP and point-to-point protocol. However, there are many Non-DICOM Modalities and DICOM Modalities having problems. First, we had interfaced almost modalities. Fuji CR, GE CT, MRI, Angio, Fluoro, Phillips Angio, Shimadzu Fluoro. Ultrasound PACS, with the main PACS in the Seoul S Hospital as large scale hospital. And we manipulated the intelligent image distribution and the CT. MRI Interfaces never experienced beforein the Anyang J Hospital and the Chungju C Hospital as mid or small scale hospital. Technically, we developed both the DICOM Interface and the Non-DICOM Interface. At the last, the DICOM Worklist and the DICOM Print Interface were implemented in the Seoul B Hospital, the Bucheon SJ Hospital and the Seoul K Hospital independently with PACS. The Oracle, Sybase and MS-SQL are used as database, and UNIX, Macintosh, MS Windows as operating systems. And the Visual C++ and UNIX C are the main programming tools. We have used UTP, coaxial and fiber optic cable under 10/100 mbps LAN for networking.

  • Recently, dental PACS construction in dental hospital is fairly general. For this reason, inspection request works of dental PACS fee with Health Insurance Review & Assessment Service are more important for the financial stability in hospital and from now on status. But several departments in dental hospital, enforce a incoherent order system so inspection request situation is full of mistakes and omissions. As a result, financial stability in hospital rapidly change for the worse, as the case stands establishment of proper order system and match system is essential to resolve the issue. Consequently, we plan to establish the proposed idealized scheme.

  • In recent years, medical procedures have become more complex, while financial pressures for shortened hospital stays and increased efficiency in patient care have increased. As a result, several shortcomings of present film-based systems for managing medical images have become apparent. Maintaining film space is labor intensive and consumes valuable space. Because only single copies of radiological examinations exist, they are prone to being lost or misplaced, thereby consuming additional valuable time and expense. In this paper, MINI-PACS for image archiving, transmission, and viewing offers a solution to these problems. Proposed MINI-PACS consists of mainly four parts such as Web Module, Client-Server Module, Internal Module, Acquisition Module. In addition, MINI-PACS system includes DICOM Converter that Non-DICOM file format converts standard file format. In Client-Server Module case, Proposed system is combined both SCU(Service Class User: Client) part and SCP(Service Class Provider: Server)part therefore this system provides the high resolution image processing techniques based on windows platform. Because general PACS system is too expensive for Medium and Small hospitals to install and operate the full-PACS. Also, we constructed Web Module for database connection through the WWW.
