임상검체에서 포도알균의 분리 빈도 (Isolation Frequency of Staphylococcus species from Clinical Materials)

  • The total number of clinical materials collected from Dec 1, 2005 to Nov 30, 2006 in C. hospital was 63,133. Among these materials, the isolation of Staphylococcus spp. was 5,252 strains. The proportions of Staphylococcus species infection were as follows ; 46.7% in S. aureus, 39.3% in S. epidermidis, 4.7% in S. hominis, 3.8% in S. haemolyticus, 2.9% in S. capitis and 2.5% in others. Seventeen strains of Staphylococcus species were identified with an isolation range of 0.02-46.7%. The isolation ratios of Staphylococcus species were 7.8% (2,134/27,362) in female, 8.7% (3,118/35,807) in male and 8.3% (5,252/63,133) in both. The age-related frequencies of Staphylococcus species were 22.1% in the 60-69 year group, 19.3% in the 70-79 year group and 17.1% in the 50-59 year group. The isolation ratio of the 0-49 year group to the 50-over 80 year group were 1.74 times in female, 1.92 in male and 1.85 in both. The isolation frequency of Staphylococcus spp. was at its highest in April (9.9%) and at its lowest in February (6.8%) according to monthly analysis. According to seasonal variation, the isolation frequencies of Staphylococcus spp. were 27.0% in Summer, 25.9% in Spring, 23.9% in Fall and 23.2% in Winter. The clinical departments showing over 9% isolation rate against Staphylococcus spp. were critical care medicine (12.9%), department of pediatrics (11.6%), department of urology (9.8%) and department of neuro-surgery (9.1%). On the other hand, the lowest number was observed in the department of family medicine. The clinical material showing over 15% isolation rate against Staphylococcus spp. were 32.1% in urine (random urine and catheterized urine), 20.6% in blood and 16.0% in sputum.

High prevalence of Enterococcus spp. from dogs with otitis externa

  • Otitis externa (OE) is a frequent disease in the ear canals of dogs. To identify the pathogens causing OE in dogs and to determine their antimicrobial resistances, specimens were collected from animal hospitals in Daejeon. The isolates were examined by morphological and biochemical tests, 16S rRNA analysis and antimicrobial susceptibility tests. We analyzed correlation between the isolated pathogens and external factors of dogs such as breed, age, gender, ear mite, hair in ears and experience with antibiotic therapy. Thirty three strains of bacteria were isolated from 26 of the 68 heads of dogs with OE. The most isolated bacteria were Enterococcus faecalis (E. faecalis) followed by Staphylococcus aureus (Sta. aureus), Sta. pseudointermedius, E. faecium, E. avium and Streptococcus canis (Strep. canis) in order of frequency of occurrence. Isolation frequency of Enterococcus spp. and Staphylococcus spp. were 51.5% and 45.5%, respectively. E. faecalis and E. faecium isolates showed VanB phenotype, which is resistant to vancomycin but sensitive to teicoplanin were 58% and 25%, respectively. Nine isolates among total twelve isolates of E. faecalis were isolated from the dogs treated with antibiotics. There was no methicillin-resistant Sta. aureus (MRSA), but were MR-Sta. pseudointermedius (MRSP) (57.1%) and vancomycin-resistant (VR)-Sta. pseudointermedius (14.3%) (VRSP) showing VanB phenotype. However, vanA, vanB and vanC genes were not detected in VR isolates from the dogs. Taken together, VR-Enterococcus spp. (VRE) is one of the major pathogens in domestic animals, as well as community-and hospital-acquired infection.

Enterobacteriaceae and Related Microorganisms Isolated from Rump of Raw Beefs

  • In this study, 50 rump samples of raw beef obtained from local Korean supermarkets were analyzed to survey microbial distributions. As results, mesophilic microorganisms ranged from $(1.4{\pm}0.01){\times}10^2$ to $(1.6{\pm}0.05){\times}10^5\;CFU/g$, and total coliforms ranged from 0 to $(1.3{\pm}0.04){\times}10^4\;CFU/g$. Major foodborne pathogens, including Listeria monocytogenes, Escherichia coli O157:H7, and Salmonella spp., were not found among the samples. However, Staphylococcus aureus was isolated with 4% frequency. Other isolated microorganisms included Enterobacter amnigenus (4%), Enterobacter cloacae (24%), E. coli (24%), Listeria innocua (8%), Staphylococcus saprophyticus (56%), Staphylococcus xylosus (10%), and Staphylococcus warneri (8%).

여성의 손 위생관리에 관한 연구 (A Study on the Hand Hygiene Practices among Females)

  • Objectives: This study was performed to investigate hand washing awareness of females and load of indicator bacteria on their hands. This study focused on the variation according to their age. Methods: A self-administered questionnaire survey and bacterial analysis of indicator bacteria were carried out for 100 Korean women in their age from 20 s to 60 s. Hand samples were collected through a modified glovejuice method. Results: In the survey, significant difference (p < 0.05) was found among the age groups in the use of hand washing agents. The levels of aerobic colony count (ACC) were the highest in both hands among the 20s (p < 0.05). The levels of Escherichia coli were higher in both hands in their 20s and 30s. No significant difference was found in the levels of Staphylococcus aureus and Salmonella spp. However, the positive rates of S. aureus (left hand, 37.5~47.1%; right hand, 58.5~62.5%) and Salmonella spp. (left hand, 25.0~52.9%; right hand, 37.5~64.7%) were higher in the hands of the 20s and 30s, and then showed decreasing trend according to increase of age. The effect of hand washing frequency on the ACC level of hands was significant (p < 0.001). Conclusions: These results indicate that there was no remarkable difference of hand hygiene awareness among female age groups. The detection of S. aureus and Salmonella spp. on the hands of some females in each age group revealed poor hand hygiene practices. The significant effect of hand washing frequency on the ACC level suggests that frequent hand washing is helpful to reduce hand contamination.

일개 고등학교 환경에서 메치실린 내성 포도알균의 오염도 조사 (Study on the Contamination of Methicillin-Resistant Staphylococcus (MRS) in a High School Environment)

  • 메치실린 내성 포도알균(methicillin resistant Staphylococci, MRS)는 인체 여러 부위에서 집락화 될 수 있으며 의료기관과 연관된 사람에서 흔히 분리된다. 이 연구는 밀접한 단체생활을 하는 일개 고등학교 한 개 반의 학생들 손과 그들이 사용하는 책상에서 MRS 균의 오염 정도를 평가하고자 하였다. Staphylococcus aureus가 28명의 학생의 손 중에서 2 균주가 분리되었으며 모두 메치실린에 감수성이었다. 응고인자 음성 포도알균(coagulase negative Staphylococci, CoNS)는 26 균주가(26/28, 92.9%) 분리되었으며 이들 균주 중 14 균주는 메치실린 내성균(MRCoNS)이었다. 14 MRCoNS 중 S. warneri가 가장 흔하였으며(8/14, 57%), 이들은 대부분의 $non-{\beta}-lactam$ 항생제에 감수성 이었다. 31개의 책상에서는 S. aureus는 분리되지 않았으나 26 CoNS (26/31, 83.9%)가 분리되었다. 손과 책상에서 분리된 포도알균 이외의 균은 Micrococcus와 Bacillus spp.이었다. 결론적으로 MRSA는 이번 연구에서 분리되지 않았으나 mecA 유전자를 갖고 있는 MRCoNS는 학생들의 손에서 높은 비율로 분리되었다. 손씻기와 같은 예방 교육을 강화할 뿐만 아니라 이들 균의 오염 및 보균율 등의 조사와 같은 능동적 감시 등이 이들 균의 감염예방과 전파 차단을 위해 필요할 것으로 사료된다.

서울시내 중학생의 손오염에 관한 세균학적 연구 (Bacterial Contamination on the hands of the Middle School Students in Seoul City)

  • 활동성이 강한 중학생을 대상으로 남$\cdot$여, 학생에 따른 세균 오염현황을 파악함으로써 중학생의 보건 교육에 관심을 높이고자 하여 1985년 2월 4일부터 2월 15일까지 서울 시내 중학교에 재학중인 학생 29명을 대상으로 양손으로 부터의 세균을 분리하여 다음과 같은 결과를 얻었다. 1. 손에 오염된 평균 세균수는 1학년 남학생 $3.08\times10^4$개 여학생$2.48\times10^4$개, 2학년 남학생 $3.32\times10^4$, 여학생 $1.64\times10^4$개로 2학년 여학생에 있어 특히 적은 수가 나타남을 알 수 있었다. 2. 중학생 손에서 검출된 세균의 속명은 Staphylococcus, Streptococcus, Micrococcus, Bacillus, Neisseria spp., Branhamella, Acinetobacter, Moraxella, Chromobacterim, Alcaligenes, Flavobacterium, Pseudomonas, Actinobacillus, Pasteurella, Vibrio, Enterobacteria와 Plesiomonas로 17속으로 나타났다. 3. 속명이 밝혀진 균주 중 종명까지 밝혀진 것은 Staphylococcus aureus, Staphylococcus epidemidis, Micrococcus varians, Branhamella catarrhalis, Acinetobacter anitratus, Moraxella bovis, Moraxella urethralis, Pseudomonas aeruginosa와 plesiomonas shigelloid로 9종이 밝혀졌다. 4. Genus 중 전체 학생중 나타난 출현 빈도가 가장 높은 것은 Staphylococcus $(96.6\%)$이었고 그 다음 Enterobacterial $(14.4\%)$ Bacillus$(20.7\%)$, Branhamella$(17.2\%)$순으로 나타났다.

한방사료 첨가제의 항균성 및 재래닭에서의 Campylobacter jejuni 방제효과 (Antimicrobial Activity and Preventive Effect of Oriental Herbal Medicine Feed Additives for Campylobacter jejuni in Korean Native Chickens)

  • In this study, antimicrobial activity of oriental herbal medicine extract (OHME) was tested for some organisms and the preventive effects of OHME for the colonization of Campylobacter jejuni on epithelium of small intestine were examined in Korean native broiler chickens fed a forage added 1.0% OHME. The isolated Campylobacter spp were biotyped, serotyped and the susceptiblility of isolates to antimicrobial agent were examined. The growth of Staphylococcus aureus was inhibited in 0.25% OHME. C. jejuni and C. coli were inhibited in 0.1% OHME, and Salmonella spp, Lactobacillus acidophilus and Escheichia coli 0157 were inhibited in 2.0% OHME. For the application of forage added 1.0% OHME in broiler chicken farm, the frequency of Campylobacter spp from feces, liver and spleen sample of chickens were examined during 2 weeks interval. The frequence of Campylobacter spp in feces from chickens fed assorted forage (control group) was increased from 25% in first week to 75% in seventh week. But the frequence of Campylobacter spp in feces sample from chickens 134 forage added OHME was slightly reduced from 25% in first week to 15% in seventh week. The frequence of Campylobacter spp in liver, and spleen was 13.7% and 10% respectively after seventh week in control group, but the Campylobacter spp was not isolated after fifth week in live and spleen from chickens fed forage added OHME. Isolated 56 strains of thermophilic Campyiobacter from Korean native chickens was classified as C. jejuni (76.7%), C. coli (214%) and C. laridis (1.6%). The majority of 43 isolates of C. jejuni was classified on biotype I (60.4%), II (30.2%). Most of 12 isolates of C.coli were biotype I (83.3%). Isolated 31 strains C. jejuni of showed 11 different serotype, and serotype 36 (18.6%), 17 (13.9%)were most frequent. Isolated 10 strains of C. coli showed 5 different serotypes and serotype 31 (33.3%) and 21 (25%) were relatively common. Isolated Campylobacter spp were highly susceptible to nalidixic acid, amikacin, gentamicin, colistin and chloramphenucol.

공중전화기의 미생물학적 분포에 관한 연구 (A study on the microbiological distribution on the transmitters of public telephone)

  • The microbial distribution on 115 transmitter of telephone in 5 site were studied during the period Mar to May 1996. The result were follows: 1. The public telephone at Kangnam terminal showed highest contamination of total bacteria (1.5 $\times $ 10$^{4}$/cm$^{2}$ and the telephone at home showed lowest contamination at the mean 4.6/cm$^{2}$ 2. The most frequent bacteria was staphylococcus epidermides and staptylococcus aureus which is pathogen of food poison was frequently isolated. 3. The public telephone at Seoul showed the highest contamination of fungi and the telephone at home showed lowest contamination of Fungi 4. In fungi 631 strains of 16 species were isolated that of highest frequency is confirmed to be Asperillus fumigatus which is pathogen of Aspergillosis, Nigar and Penicillium spp. were frequently isolated.

일반인과 식품취급자의 손 위생관리에 관한 비교 (A Study on Hand Hygiene Practices: A Comparison of Food Handlers with General Population)

  • Objectives: This study was performed to investigate hygienic behavior of food handlers and general population focusing on awareness of hand-washing and the microbial load of their hands. Methods: A questionnaire survey and microbiological analysis were carried out for sixty-four people each. Samples for microbiological analysis were collected through the glove-juice method from the hands, and were analyzed for the presence of aerobic plate counts, total coliform, fecal coliform, Escherichia coli, Staphylococcus aureus, and Salmonella spp. according to the Food Code of Korea. Results: In the survey, significant differences between the food handlers and general population (p < 0.01) were found in hand-washing frequency, duration, use of hand-washing agents, washing parts of hands, hand-drying method, and method of turning off water. In eight different situations among the ten particular situations in their daily life, more food handlers responded to wash their hands than general population (p < 0.05). Bacterial load on hands with general population was consistently higher than with food handlers (p < 0.05), however, percentages of positive hands of S. aureus and Salmonella spp. were not. Conclusions: Poor hand hygiene practices were indicated by the positive results for E. coli, S. aureus, and Salmonella spp. on the hands of some respondents in both groups. This study reveals that there is the need for programs or campaigns to increase hand-washing practices of both groups.

Microbial isolates and antibiotic sensitivity in patients hospitalized with odontogenic infections at a tertiary center over 10 years

  • Objectives: This study investigated causative strains and their antibiotic sensitivity in patients who were hospitalized for maxillofacial odontogenic infections at a tertiary center in South Korea over the past 10 years with the aim of providing guidelines for the selection of appropriate empirical antibiotics. Materials and Methods: Patients with head and neck fascial space abscesses due to odontogenic infections who underwent incision and drainage surgery with pus culture tests between 2013 and 2022 at the Department of Oral and Maxillofacial Surgery, Dankook University Hospital were included. The bacterial isolates and antibiotic sensitivity of each strain were analyzed for 2013-2022, 2013-2017, and 2018-2022. The affected fascial spaces were classified into primary, secondary, and deep neck spaces. Results: In the 192 patients included in this study, 302 strains were detected. Viridans streptococcus had the highest frequency (51.7%), followed by Prevotella spp. (16.9%), Staphylococcus spp. (5.6%), and Klebsiella pneumoniae (4.6%). The identification rate of viridans streptococcus significantly increased from 41.8% in 2013-2017 to 60.9% in 2018-2022. Viridans streptococcus showed an antibiotic sensitivity of 80.5% to ampicillin; the sensitivity to penicillin antibiotics decreased over the study period. Antibiotic susceptibility was approximately 94% for third-generation cephalosporins. K. pneumoniae, which was identified at a high percentage in patients with deep neck space infection, showed increasing antibiotic resistance to most antibiotics over the study period. Conclusion: Viridans streptococcus was identified in head and neck fascial space abscesses with the highest frequency. Empirical antibiotics should be effective against this strain; penicillin antibiotics are considered inappropriate. For effective treatment of deep neck space abscesses, bacterial culture and antibiotic sensitivity tests performed as soon as possible are essential.
