임플란트 고정성 보철물을 이용한 가철성 국소의치의 합병증에 관한 임상적 평가 (Clinical evaluation of the removable partial dentures with implant fixed prostheses)

  • 목적: 임플란트 지지 서베이드 고정성 보철물을 이용한 가철성 국소의치의 합병증을 임상적으로 조사하고, 임플란트 식립위치, 보철물 연결고정여부, 국소의치의 유지장치 종류, 케네디 분류, 대합치에 따른 연관 관계를 조사하고자 하였다. 대상 및 방법: 2000년부터 2016년까지 임플란트 고정성 서베이드 보철물을 이용한 가철성 국소의치로 수복을 받은 환자 11명(남자 7명, 여자 4명)을 대상으로 하였다. 평균 연령은 67.5세였으며, 상악 7개, 하악 4개의 국소의치가 제작되었다. 장착된 국소의치 11개와 이를 지지하는 37개의 임플란트 고정성 보철물의 기계적 합병증, 잔존 자연치아 상태, 주변 연조직 상태 등 모든 합병증과 그와 관련된 요소를 조사하였다. 결과: 장착된 국소의치 11개를 지지하는 총 임플란트는 37개로 국소의치마다 평균 3.4 개의 임플란트 지지 서베이드 고정성 보철물이 이용되었다. 평균 41.5 개월의 추시 기간 동안 발생한 합병증은 임시시멘트 접착 탈락, 대합치 파절/동요, 나사 파절, 나사 풀림, 클래스프 헐거움, 상부 도재파절, 임플란트 주위 골 흡수/동요, 국소의치 인공치아 파절 순으로 발생하였다. 이러한 합병증은 구치부보다 전치부에서, 연결한 보철물 보다 연결하지 않은 보철물에서, 가공선 클래스프를 유지장치로 이용한 보철물에서, 대합치가 의치를 포함하는 보철물보다 자연치열인 경우 합병증 발생이 좀 더 높은 빈도로 나타난 것을 확인 할 수 있었다. 케네디 분류에 따른 합병증 발생 빈도 차이는 유의한 차이를 확인할 수 없었다. 결론: 임플란트 융합 국소의치는 장기간 사용되고 있고 충분히 효과적으로 기능하고 있었다. 하지만 아직까지 이러한 치료의 예후에 대한 축적된 임상 결과는 많이 부족한 실정이므로 좀 더 장기적인 임상 연구가 필요한 상황이다.

적층가공과 절삭가공으로 제작한 의치상과 직접 첨상용 레진 간의 인장결합강도 비교 (Tensile bond strength of chairside reline resin to denture bases fabricated by subtractive and additive manufacturing)

  • 목적:본 연구의 목적은 절삭가공과 적층가공으로 제작한 의치상과 직접 첨상용 레진과의 인장결합강도를 열중합형 의치상과 직접 첨상용 레진의 인장 결합강도와 비교 및 평가하여 절삭가공과 적층가공으로 제작한 의치상의 직접 첨상을 임상에 활용하고자 하는 것이다. 재료 및 방법:열중합형 의치상 레진(Lucitone 199), 절삭가공용 의치상 레진(VITA VIONIC BASE), 적층가공용 의치상 레진(NextDent Base)을 이용해 가로 25 mm × 세로 25 mm × 높이 3 mm의 직육면체 형태로 제작하였다. 제작한 의치상 레진 시편을 30일간 37℃ 증류수에 보관한 뒤, 건조하여 자가중합형 polyethyl methacrylate (PEMA) 직접 첨상용 레진(REBASE II Normal)을 사용해 결합하였다. 절삭가공과 적층가공용 의치상 레진을 실험군으로, 열중합형 의치상 레진을 대조군으로 설정하고 각 군 당 10개의 시편을 제작하였다. 모든 시편을 24시간 동안 37℃ 증류수에 보관한 뒤 꺼내어 만능시험기를 이용해 10 mm/min의 cross head speed로 인장결합강도를 측정하였고, 파절 양상을 관찰하여 접착 파절, 응집 파절, 혼합 파절로 분류하였다. 의치상의 제작 방법에 따른 직접 첨상용 레진과의 인장결합강도를 일원배치 분산분석으로 분석하였고 사후검정(Bonferroni's method)을 시행하였다 (α= .05). 결과:절삭가공용 의치상 레진과 직접 첨상용 레진과의 인장결합강도(2.33 ± 0.39 MPa)는 열중합형 의치상과 직접 첨상용 레진과의 인장결합강도(2.45 ± 0.39 MPa)와 통계적으로 유의성 있는 차이가 없었다 (P > .999). 적층가공한 의치상 레진과 직접 첨상용 레진과의 인장결합강도(1.23 ± 0.36 MPa)는 나머지 두 군보다 유의성 있게 낮았다 (P < .001). 열중합형과 절삭가공한 의치상에서는 혼합 파절이 가장 많이 나타났으며, 적층가공한 의치상에서는 혼합 파절과 접착 파절이 동일한 빈도로 나타났다. 결론:직접 첨상용 레진과 다양한 방법으로 제작한 의치상의 인장결합강도를 비교하였을 때 적층가공으로 제작한 의치상은 절삭가공으로 제작한 의치상, 열중합형 의치상보다 유의하게 낮은 인장결합강도를 보였다.

STRENGTH OF GLASS FIBER REINFORCED PMMA RESIN AND SURFACE ROUGHNESS CHANGE AFTER ABRASION TEST

  • Statement of the problem. The fracture of acrylic resin dentures remains an unsolved problem. Therefore, many investigations have been performed and various approaches to strengthening acrylic resin, for example, the reinforcement of heat-cured acrylic resin using glass fibers, have been suggested over the years. But problems such as poor workability, rough surface, poor adhesion of glass fiber resin complex are not solved yet. Purpose. The aim of the present study was to investigate the effect of short glass fibers on the transverse strength of heat-polymerized denture base acrylic resin and roughness of resin complex after abrasion test. Material and methods. To avoid fiber bunching and achieve even fiber distribution, glass fiber bundles were mixed with acrylic resin powder in conventional mixer with a non-cutting blade, to produce the glass fiber($10{\mu}m$ diameter, 3mm length, silane treated) resin composite. Glass fibers were incorporated at 0%, 3%, 6% and 9% by weight. Transverse strength were measured. After abrasion test, surface roughness was evaluated and scanning electron microscope view was taken for clinical application. Results. 1. 6% and 9% incorporation of 3mm glass fibers in the acrylic resin enhanced the transverse strength of the test specimens(p<0.05). 2. Before abrasion test, incorporation of 0%, 3%, 9% glass fiber in the resin showed no dirrerence in roughness statisticaly(p>0.05). 3. After abrasion test, incorporation of 0%, 3%, 6% glass fiber in the resin showed same surface roughness value statistically(p>0.05). 4. In SEM, surface roughness increased as the percentage of the fibers increased. 5. In the areas where glass fiber bunchings are formated, a remarkably high roughness was noticed. Conclusion. 6% and 9% addition of silane-treated short glass fibers into denture base acrylic resin increased transverse strength significantly. Before and after abrasion test, incorporation of 0%, 3%, 6% glass fiber in the resin showed same surface roughness value statistically.

치과보철물의 평균수명에 관한 연구 (A Study on the Life Expectancy of the Dental Prosthetic Restorations)

  • This study was performed to investigate the mean life expectancy of dental prosthetic restorations. The author has examined 352 dental prosthesis clinically and radiologically, and decided the success(survival) and failure(mortality) of the dental prosthesis. The dental prosthesis which had been treated in the Seoul National University Dental Hospital, two private clinics in Seoul, one university dental hospital, and two private clinics in local province were included in this study. The survival analysis using product limit estimator was used and the mean life expectancy of each type of dental prosthesis was calculated. The results were as follows : 1. The life expectancies were 10.5 years in gold crown and bridge, 8.5 years in porcelain fused to metal crown and bridge, 8.3 years in nonprecious metal crown and bridge, 8.1 years in removal partial denture, and 7.7 years in full denture. 2. The causes of mortality were in the order of dental caries(24.6%), fracture of dental prosthesis(19.2%), periodontal problems(18.6%), chronic chewing difficulty and dysfunction due to dental prosthesis(15.0%), excessive exposure of abutments due to the marginal defect of dental prosthesis(14.4%), abnormal occlusion due to severe attrition of artificial teeth in dentures(3.0%), periapical problems(2.4%), perforation of dental prosthesis(1.8%), and loose contacts with neighboring tooth(1.2%). 3. Among survival cases, 66.5% showed normal chewing ability and 31.9% showed partial chewing ability. However, 1.6% of them complained loss of chewing ability. 4. Among failure cases, 6.6% showed normal chewing ability and 38.9% showed partial chewing ability. However, 54.5% of them complained loss of chewing ability.

Locator attachment를 이용한 하악 임플란트 유지 피개의치의 후향적 평가 (Retrospective Evaluation of Implant Retained Overdentures Using Two Implants and Locator Attachments in the Edentulous Mandibles)

  • 이번 연구는 2개의 임플란트 식립 후 로케이터를 이용한 하악 임플란트 유지 피개의치를 한 20명의 환자를 대상으로 임플란트 생존율, 변연골 흡수량, 보철적 합병증을 알아보았다. 진료 기록부를 조사하고 방사선 사진을 계측하여 후향적 임상연구를 하였으며, 임플란트는 95%의 누적 생존율을 보였고, 평균 94주 후 방사선 사진에서 임플란트는 평균 1.21 mm($SD{\pm}0.20mm$)의 골소실을 보였다. 보철적 합병증으로는 피개의치 파절, 의치상 이장 또는 개상, 메일의 탈락 또는 유지력 감소로 인한 메일 교체 등이 있었다. 그러나 의치상 이장 및 개상, 메일 인써트의 간단한 교체로 인해 장기적으로 임플란트 및 임플란트 유지 피개의치가 환자에게 유지되고 있었다. Locator를 이용한 하악 임플란트 유지 피개의치는 최소침습적인 수술, 간단한 보철 과정 및 수리 과정의 장점으로 인해 장기적으로 안정적인 치료법으로 사료된다.

Influence of various metal oxides on mechanical and physical properties of heat-cured polymethyl methacrylate denture base resins

  • PURPOSE. To evaluate the effect of various metal oxides on impact strength (IS), fracture toughness (FT), water sorption (WSP) and solubility (WSL) of heat-cured acrylic resin. MATERIALS AND METHODS. Fifty acrylic resin specimens were fabricated for each test and divided into five groups. Group 1 was the control group and Group 2, 3, 4 and 5 (test groups) included a mixture of 1% $TiO_2$ and 1% $ZrO_2$, 2% $Al_2O_3$, 2% $TiO_2$, and 2% $ZrO_2$ by volume, respectively. Rectangular unnotched specimens ($50mm{\times}6.0mm{\times}4.0mm$) were fabricated and drop-tower impact testing machine was used to determine IS. For FT, compact test specimens were fabricated and tests were done with a universal testing machine with a cross-head speed of 5 mm/min. For WSP and WSL, disc-shaped specimens were fabricated and tests were performed in accordance to ISO 1567. ANOVA and Kruskal-Wallis tests were used for statistical analyses. RESULTS. IS and FT values were significantly higher and WSP and WSL values were significantly lower in test groups than in control group (P<.05). Group 5 had significantly higher IS and FT values and significantly lower WSP values than other groups (P<.05) and provided 40% and 30% increase in IS and FT, respectively, compared to control group. Significantly lower WSL values were detected for Group 2 and 5 (P<.05). CONCLUSION. Modification of heat-cured acrylic resin with metal oxides, especially with $ZrO_2$, may be useful in preventing denture fractures and undesirable physical changes resulting from oral fluids clinically.

Polyetherketoneketone (PEKK)과 다양한 의치상용 전장 레진 간의 인장결합강도에 관한 연구 (A study of the tensile bond strength between Polyetherketoneketone (PEKK) and various veneered denture base resin)

  • 목적: 본 연구는 의치상용 레진의 전장방법이 polyetherketoneketone (PEKK)과의 인장결합강도에 미치는 영향을 알아보고자 하였다. 재료 및 방법: 총 80개의 PEKK 시편을 T자 형태로 절삭 가공하고, airborne-particle abrasion 처리 후 전처리제(Visio.link)를 도포하였다. 전장 방법에 따라 4개의 군(n = 20)으로 나누어, 충전형 광중합형 복합레진(SR Adoro), 유동형 광중합형 복합레진(Crea.lign), 열중합형 의치상용 레진(Vertex RS), 자가중합형 의치상용 레진(ProBase Cold)으로 전장하였다. 각 군은 인공시효처리 여부에 따라 2가지 하위 군(n = 10)으로 나누었다. 만능시험기를 이용해 인장결합강도 측정하고 현미경으로 파절단면을 관찰하였다. 통계적 검증을 위해 이원 분산분석 및 Tukey's HSD 사후 검정을 시행하였다(α = .05). 결과: 인공시효처리와 레진 전장방법은 인장결합강도에 통계적으로 유의한 영향을 주었다(P < .001). 인공시효처리 전과 후 모두 유동형 광중합형 복합레진 군에서 가장 높은 값을 보였고, 열중합형 레진 군에서 가장 낮은 값을 보였다(P < .05). 파절 단면은 군에 따라 혼합 파절과 계면 파절이 혼재되어 나타났다. 결론: 레진의 전장 방식은 PEKK과의 인장결합강도에 영향을 줄 수 있으며, 인공시효처리는 인장결합강도를 감소시킨다.

Holographic Interferometry를 이용한 하악 구치부 도재소부 전장관용 금속 구조물의 굴곡성향에 대한 연구 (FLEXION EFFECTS OF HEAT TREATMENT AND POST-SOLDRING OF CERAMO-METAL FIXED PARTIAL DENTURE FRAMEWORKS USING HOLOGRAPHIC INTERFEROMETRY)

  • Flexion of a metal/ceramic fixed partial denture(EPD) frameworks under function can cause fracture of porcelain or deterioration of the cement seal. This study evaluated the flexion characteristics of three-unit mandibular FPD frameworks, repacing the second pre-molar under compressive load(200g, 400g). Testing was accompished with real-time holographic interferometry, using 6 porcelain fused-to metal frameworks. Tested alloys were non-precious alloy(Heracles, Holland), semi-precious alloy(Degudent U, Germany) and precious alloy(Degudent H, Germany). Changes of the fringe patterns according to the heat treatment(porcelain firing cycle), various loads(200g, 400g), occlusal forms(occlusal porcelain veneering, facial porcelain veneering), various alloys and post-soldering units were compared. Dental study model(Nissan dental products, Inc. D51DP-500A, Japan) and six 3-unit metal/ceramic fixed partial denture frameworks were used as experimental materials. 36 holograms were taken on fixed dental study model by using the 10mW He-Ne laser and real-time holographic interferometry. On the basis of this study, the following conclusions can be drawn : 1. In the frameworks for facial porcelain veneering, the semi-precious alloy framework was least deformed and precious alloy framework, non-precious alloy framework orderly before heat treatment, and the deformation was not shown great difference among three alloys after heat treatment and post-soldering. 2. In the frameworks for occlusal porcelain veneering, the precious alloy framework was greatest deformed and the deformation was not difference between semi-precious alloy framework and non-precious alloy framework before, after heat treatment, and the deformation was not shown great difference among three alloys after post soldering. 3. In the non-precious alloy frameworks for facial porcelain veneering and occlusal porcelain veneering, the deformation was greatly decreased after heat treatment and conversely increased after post-soldering. 4. In the semi-precious alloy framework for facial porcelain veneering, the deformation was not detectable after heat treatment and increased after post-solder. And in the frame-work for occlusal porcelain veneering, the deformation was slightly decreased after heat treatment and increased after post-soldering. 5. In the precious alloy framework for facial porcelain veneering, the deformation was greatly decreased after heat treatment and increased after post-soldering, And in the framework for occlusal porcelain veneering, the deformation was greatly decreased after heat treatment and decreased after post-soldering.

EFFECTS OF CHOPPED GLASS FIBER ON THE STRENGTH OF HEAT-CURED PMMA RESIN

The aim of the present study was to investigate the effect of short glass fibers treated with silane coupling agent on the transverse strength of heat-polymerized PMMA denture base resin. To avoid fiber bunching and achieve even fiber distribution, glass fiber bundles were mixed with PMMA powder in conventional mixer whose blade was modified to be blunt. Composite of glass fiber($11{\mu}m$ diameter, 3mm & 6mm length, silane treated) and PMMA resin was made. Transverse strength and Young's modulus were estimated. Glass fibers were incorporated with 1%, 3%, 6% and 9% by weight. Plasticity and workability of dough was evaluated. Fracture surface of specimens was investigated by SEM. The results of this study were as follows 1. 6% and 9% incorporation of 3mm glass fibers in the PMMA resin enhanced the transverse strength of the test specimens(p<0.05). 2. 6% incorporation of 6mm glass fibers in the PMMA resin increased transverse strength, but 9% incorporation of it decreased transverse strength(p<0.05). 3. When more than 3% of 3mm glass fibers and more than 6% of 6mm glass fibers were incorporated, Young's modulus increased significantly(p<0.05). 4. Workability decreased gradually as the percentage of the fibers increased. 5. Workability decreased gradually as the length of the fibers increased. 6. In SEM and LM, there was no bunching of fibers and no shortening of fibers.

레이저 융합된 Co-Cr 합금의 물리적 성질에 관한 연구 (MECHANICAL PROPERTIES OF LASER WELDED Co-Cr ALLOY)

  • Statement of problem : The joints of removable partial denture alloys have failed frequently after routine usage. Purpose : The purpose of this study was to evaluate the mechanical properties of the laser welded Co-Cr alloys. Material and method : For this study 20 Co-Cr specimens were casted and 10 of them were seperated on the middle area and laser welded with Alpha laser welding machine(Siro Lasertec, Pforzheim, Germany). Rest of them which were as cast, were used as a control group. For the section of the experimental specimens, wire cutting machine was used to make a even gap of the all specimens. Laser welding was done with manufacturer's instrunction and tested each specimen by Instron Machine. Tensile strength, 0.2% yield strength and % elongations were recorded. Fractured surfaces were investigated with SEM. Results : The results were as follows : 1. The tensile strength of the laser welded group(617.7MPa) was about 75% of the as cast group(820.4MPa). It had stastically significant differences(p<0.05). 2. The % elongation of the experimental group was 6.6 which was lower than the control group(14.3). 3. Fracture of the experimental group occured in the welded surface and showed many voids. In contrast, the fracture surface of the control group was showed rough surfaces without any voids. Conclusion : The tensile strengths of the as-cast joints were higher than those for the laser welded joints, and the % elongation of the experimental group was lower than the control group. Porosity was found in laser-welded joints.
