Delayed Foreign Body Reaction around the Absorbable Implants in Facial Bone Fracture: A Case Report (안면부 골절에 사용된 흡수성 보형물의 지연성 이물반응: 증례보고)

  • Purpose: Absorbable implants are frequently used to fix facial bone fractures, because they are radiolucent and compatible with magnetic resonance imaging (MRI). Despite their increasing usage, however, there have been few reports about their long-term side-effects. In this paper, a case in which absorbable implants led to a foreign body reaction 17 months after their insertion is presented. Methods: A previously healthy 19-year-old male fell from a flight of stairs and visited the authors' hospital with right periorbital pain. Zygomaticomaxillary fracture involving right orbital floor was detected via a facial bone computed tomography (CT). Internal fixation with absorbable implants was followed by open reduction. 17 months after the surgery, the patient complained of localized periorbital swelling. Removal of the granulomatous lesion including the absorbable implants along with their biopsy and culture was performed. Results: The granulomatous lesion around the implants was firm and extended into the maxillary sinus. The histologic finding showed a microabscess with a foreign body reaction. Methicillin-sensitive Staphylococcus aureus growth was confirmed in the culture. No definite abnormal symptoms ensued after the complete removal. Conclusion: A microabscess-forming granulomatous lesion around the absorbable implant can cause delayed symptomatic foreign body reaction despite its rare occurrence. Complete removal of the lesion including implants is expected to have a successful outcome if it is encountered.

A Case of Massive Hemoptysis & Pneumonia Caused by Metallic Foreign Body (금속성 이물의 기관지 내 장기 체류로 발생한 대량 객혈 및 폐렴 1예)

  • Endobronchial foreign bodies are difficult to diagnose as the cause of obstructive pneumonia and atelectasis, However, once discovered, they can generally be removed, leading to an immediate and dramatic resolution of the symptoms. Occasionally, small foreign bodies that lodge in the peripheral airway are often initially asymptomatic but become symptomatic several years later. We reported a case of obstructive pneumonia and massive hemoptysis caused by a foreign metallic body. The patient knew that the foreign body was lodged in the peripheral airway on the chest X-ray, but did not want treatment. Several years later, he had a massive hemoptysis and obstructive pneumonia. Removal with a flexible bronchoscope failed, but the metallic foreign body was self-expectorated by coughing after the procedure. The pneumonia was resolved after removing the foreign body. The patient improved and was discharged without any sequela.

  • Taking history and ultrasonography is important to detect foreign body which might be located in the soft tissue. We encountered a case of soft tissue foreign body (transobturator tape) in a patient who had been treated with the impression of adductor myositis resulting from vagina fistula. The foreign body was found through ultrasonographic examination and the patient's symptom was relieved after removal of the foreign body from the ultrasonography-guided operation. Clinicians should keep in mind the fact that foreign bodies may exist when the inflammation persists without appreciable cause and ultrasonography can be useful tool in case of small fibrous band not detected with magnetic resonance imaging due to inflammatory change of the surrounding tissues.

A Study on Factors Affecting Cultural Competency of General Hospital Nurses (종합병원 간호사의 문화적 역량수준과 영향요인)

  • Purpose: As Korea has been rapidly transformed to be a multicultural society, it becomes essential for nurses to develop cultural competency. The purpose of this descriptive study was to assess the cultural competency level of general hospital nurses and to examine its contributing factors adopting ecological model. Method: A convenience sample of 327 nurses from six general hospitals in Seoul metropolitan area was recruited between November 14 to 28, 2011. Cultural competency was measured using the Korean version of the Caffrey Cultural Competence in Healthcare Scale (CCCHS) and Cultural Competence Assessment(CCA). Hierarchical multiple regression analysis was performed to assess the influence of intrapersonal, cultural-experience, and organizational factors on cultural competency. Results: Mean cultural competency in this study was 2.57(${\pm}0.43$). Cultural competency was associated with job position (p=.044) at the intrapersonal level. At the cultural experience level, foreign language classes experience (p=.005), fluency in a foreign language (p=.000), experience of working with foreign health care professionals (p=.000) were associated cultural competency. At the organizational level, training program (p=.000), job description (p=.002), health education materials (p=.016), insufficient job control (p=.000), and interpersonal conflicts (p=.011) showed significant difference. Foreign language class experience, foreign language fluency, experience of working with foreign health care professionals, and job control were the most consistent predictors of cultural competence and explained 20.4% of the total variance in the proposed ecological model. Conclusions: The findings suggest that to enhance cultural competency in general hospital nurses, cultural experience and organizational factors need to be more fully considered than intrapersonal factors.

  • Bronchial foreign body is not a rare disease in children and it is urgently necessary to remove this foreign body from the airway to relive life or to prevent further damages and complications. But the innate small size of airways in infants makes it difficult to access by interventional methods such as intubation or bronchoscopy and etc. Laryngeal mask airway is a new way of method of airway management which is relatively recently introduced into medical practice. It gives way to access to airways without reducing the size of airway or incresing airway pressure during procedure through it and have many other advantages compared to the previous traditional endotracheal intubation, especially in infants. We successfully removed a case of bronchial foreign body, peanut, via laryngeal mask airway during fiberoptic bronchoscopy and by this method we can avoid the unnecessary tracheostomy in this 1 year old infant.

Wound healing effect of regenerated oxidized cellulose versus fibrin sealant patch: An in vivo study

  • Background: Topical hemostatic agents are used when ligation, electrocauterization, or other conventional hemostatic methods are impractical. Because a hemostatic agent is a foreign body, it can cause foreign body reactions, inflammation, and infections that can interfere with the wound healing process. Therefore, we should select hemostatic agents after considering their effects on wound healing. Here, we compared the effects of hemostatic agents on wound healing in a rectus abdominis muscle defect in rats. Methods: Twelve Sprague Dawley rats were subjected to creation of a $6{\times}6mm$ defect in the rectus abdominis muscle and divided into four groups: control group; group A, Tachosil fibrin sealant patch; group B, Surgicel Fibrillar oxidized regenerated cellulose; and group C, Surgicel Snow oxidized regenerated cellulose. For the histologic analysis, biopsies were performed on the 3rd, 7th, and 27th days. Results: The foreign body reaction was the weakest in group A and most significant in group C. The inflammatory cell infiltration was the weakest in group A and similar in groups B and C. Muscle regeneration differed among periods. The rats in group A were the most active initially, while those in group C showed prolonged activity. Conclusion: Tachosil and Surgicel administration increased inflammation via foreign body reactions, but the overall wound healing process was not significantly affected. The increased inflammation in the Surgicel groups was due to a low pH. We recommend using Tachosil, because it results in less intense foreign body reactions than Surgicel and faster wound healing due to the fibrin action.

Airway foreign body occurs unintentionally during anesthetic management of patient with asthma

  • Intraoperative delivery of salbutamol (${\beta}_2$ agonist) through a breathing circuit may be performed in asthma patient. A 28-year-old woman with a history of asthma was diagnosed with chronic sinusitis and bilateral nasal polyps, and an endoscopic sinus surgery was performed. The patient was recommended salbutamol nebulization every 4 hours during the perioperative period because of the risk of asthma attack. At the end of the operation, when salbutamol was sprayed through the tube before extubation and the connector tip went inside the tube during injection. The patient was immediately referred to the pulmonary medicine department for bronchoscopy, where the foreign body was removed safely without any complications. When general anesthesia is performed on a patient who usually uses an inhaler for asthma, caution is required because the tip that connects the inhaler and the breathing circuit can aspirate into the endotracheal tube and enter the lungs when applying the inhaler before waking up the patient.

  • The purpose of this study is to analyze and find out key success factors and marketing strategies of Bumrungrad Hospital in Bangkok, Thailand. The major success factors of Bumrungrad Hospital are as follows; First, Bumrungrad Hospital had professional medical team and board of directors who had the international career. Second, Bumrungrad was supported by Thai government and they were in a cooperative relationship with each other for the development of the medical industry. Third, Bumrungrad appropriately handled the internal and external changes including Asia Financial Crises in 1997 and others. Fourth, Bumrungrad diversified and broaden its business field such as global medical investing and management, medical technology, anti-age medicine and wellness. Marketing strategies of Bumrungrad analyzed are the following four factors. First, Bumrungrad focused on the quality of services by employing professional medical staffs, who have the international certification, and by constructing IT system for hospital management. Second, Bumrungrad has maintained an equalized price policy to attract the customers bothin the domestic and foreign markets. The cost for care has appealed the foreign customers for its comparatively low price, but it focused mainly on the upper middle class in Thailand. Third, it established, managed, and consulted hospitals in the foreign countries including the Philippines and the Arab Emirates. Fourth, it adopted differentiated promotion strategies suitable for the special needs of domestic and foreign customers, and put emphasis on the buzz marketing.

Analysis of Domestic and Foreign Research Trends on the Korean Medical Treatment of Avascular Necrosis of the Femoral Head (대퇴골두 무혈성 괴사의 한의학적 치료에 대한 국내·외 연구동향 분석)

  • Objectives This paper is an analysis of domestic and foreign trends for the Korean medical treatment of avascular necrosis of the femoral head. Methods Domestic and foreign databases were searched and analyzed for Korean medical treatment interventions, research types, and treatment effects for avascular necrosis of the femoral head. Results There were 10 case reports in Korea, 2 case reports and 3 randomized controlled studies abroad. A total of 12 treatment interventions were used, and the types include acupuncture, herbal medicine, phamacopunture, electroacupuncture, cupping, physical therapy, chuna, acupotomy, moxibustion, warm acupuncture, laser acupuncture, and motion style treatment. As for the evaluation index, visual analogue scale, range of motion and harris hip score were the most common with 7 cases each. Conclusions Through this study, it was possible to understand the trends of domestic and foreign research on the treatment of avascular necrosis of the femoral head with Korean medicine. However, due to the small number of literature and no randomized control studies in Korea, treatment evidence is insufficient. It is considered that further research is needed in the future.

Foreign Nurse Missionaries Starting Yonsei Nursing School (연세간호를 태동 시킨 외국 선교사들)

  • Purpose: Nursing in Korea was started by foreign nurse missionaries. This study was intended to recognize the foreign nurse missionaries contribution with the development of Yonsei nursing education in Korea. Method: This study used an historical study approach. The study target was foreign nurse missionary contributions during 1895~1917. The data was collected from the existing historical nursing studies and Korean Mission Field. Result: Since A.P. Jacobson, the first nurse missionary from the North Mission branch in America in 1895, there were 16 missionaries sent to Korea. Among them, 14 missionaries worked for Severance Hospital Nurse Training School. 7 missionaries served less than 10 years and 5 missionaries stayed in Korea more than 20 years. E.L. Shields served the longest time with 42 years in Korea. They contributed not only in nursing education and services in the hospital, but also public health in the community. Conclusion: Foreign nurse missionaries were a cornerstone for nursing development in Korea. They contributed by not only starting nursing education, but also giving women freedom into Korean society. The historical research regarding foreign nurse missionaries during early 1900s helped to recognize their effort for Korean nursing and society.
