Chopart 관절 골절 및 탈구: 3예 보고 (Chopart Joint Fracture and Dislocation: A Report of Three Cases)

  • Chopart joint fracture and dislocation are rare injuries compared with other joint injuries with various clinical manifestations. Moreover, there is a lack of knowledge of the radiological findings of the joints, and thus, the extent of joint ligament damage may be underestimated, leading to improper treatment. This paper reports three cases of Chopart joint injury and seeks to reconsider the importance of Chopart joint evaluation and treatment.

Biomechanical Differences of Lower Extremity Joints at the Frontal Plane during Sidestep Cutting in Male and Female Judo Athletes

  • The purpose of this study was to analyze the biomechanical differences of lower extremity joints of the frontal plane during sidestep cutting in male and female Judo athletes. In the knee and hip joint, the female group showed a smaller angle than the male group at the time of IC(initial contact). But peak knee joint adduction moment of female group was greater than male group(p<.05). Therefore, female Judo athletes were more likely to injure their knees at the point where their initial foot contacted the ground than male athletes during sidestep cutting.

  • Control of a biped robot which has compliant ankle joints is dealt in this paper. Simulated version of a human ankle joint is built using springs and mechanical constraints, which gives a flexibility of joint and compliance against the touching ground. The biped robot with compliant ankle joints proposed here gives a good contact between its sole and the ground and makes foot landing soft. As a result, installing force sensors for measuring the center of gravity of the biped becomes easier. A motor to drive an ankle joint is not needed which makes legs light. However, the control problem becomes more difficult because the torque of the ankle joint to put the biped in a desired walking gait cannot be provided from the compliant ankle joint. To solve this problem, we proposed a dynamic gait modification method by adjusting the position of a hip joint. Simulation results for the mathematical model of the SD-2 biped in the Ohio State University are given to show the validity of the proposed controller.

The Relationship Between the Range of Hip Rotation and the Quadriceps Angle in Subjects With and Without Flat Foot

  • Background: Alignment of the lower limb is an important factor, influencing balance and gait in kinematics and kinetics, in patients with and without a flat arched foot. Flat arched foot are associated with the range of motion (ROM) of the hip and alignments of the knee joints, is strongly influenced. Objects: The purpose of this research was to investigate the relationship between hip joint ROM and quadriceps angle (Q-angle), by dividing them into two groups according to the presence or absence of flat feet, using a navicular drop test (NDT) and resting calcaneal stance position (RCSP). Methods: Forty elderly patients were allocated to the experimental group (flat foot group, n1=20) or the control group (non-flat group, n2=20). Universal and digital goniometer, tractograph and tape measure were used to determine the related changes in the hip ROM, Q-angle, NDT and RCSP. Results: Data were analyzed using the Pearson correlation coefficients. Active internal ROM of the hip joint (right, r=.803, p<.001), (left, r=.951, p<.001) were highly correlated with NDT, and also, was moderately correlated with Q-angle (right, r=.562, p=.019), (left, r=.757, p<.001). Passive internal ROM of the hip joint (right, r=.742, p=.001), (left, r=.922, p<.001) were highly correlated with NDT, and also, was moderately correlated with RCSP (right, r=-.530, p=.029) and with Q-angle (right, r=.710, p=.001), (left, r=.698, p=.002) in the flat foot group. However, no strong correlation among the hip ROM, NDT, RCSP and Q-angle were found in the non-flat foot group. Conclusion: This research may provide evidence of the correlations between hip internal ROM and flat foot.

  • Ankle arthrodesis has been used frequently for end-stage ankle arthritis that does not respond to conservative treatment. On the other hand, there are concerns regarding the degenerative changes to the adjacent joint, such as the subtalar or talonavicular joint, due to the altered biomechanics after the loss of ankle motion. Because the arthrodesis for these midtarsal joints may overload stress on another contiguous joint, a salvage procedure should be considered rather than joint sacrificing. This paper reports a case of talonavicular arthritis after malunited ankle arthrodesis that was treated with interpositional arthroplasty using the tibialis anterior tendon.

보행 시 신발 굴 높이어 따른 발/발목 복합체의 Roll-Over특성 (Foot/Ankle Roll-Over Characteristics According to Different Heel Heights of Shoe during Walking)

  • 본 연구에서는 여러 굽 높이의 신발을 신고 보행하는 동안 발/발목 복합체의 roll-over 모양을 분석하였다. 본 연구를 위해서 근골격계 및 발 질환이 없는 15명의 건강한 성인 여성들을 대상으로 보행분석을 수행하였고, 실험에는 굽 높이가 다른 4종의 신발을 사용하였다. 보행분석 시 roll-over의 특성을 구하기 위해 초기접지기부터 반대발 초기접지기까지의 발목 및 무릎의 반사마커 궤적과 지면반발력의 압력중심 데이터를 이용하였다. 연구결과로 굽 높이가 증가할수록 roll-over의 모양은 원위방향으로 이동하고 그 특성은 굽 높이에 상관없이 일정한 형태로 관찰되었다. 또한, 6cm이하의 신발에서는 roll-over의 곡률중심이 수직방향으로 이동함으로써 일정한 roll-over 특성을 유지하도록 발/발목 복합체에서 자동으로 적응함을 알 수 있었다. 그러나 그 이상의 굽 높이의 신발에서는 발목관절만으로 충분한 균형을 보장할 수 없으므로 무릎 및 엉덩관절의 보상작용이 동시에 일어남을 알 수 있었다. Roll-over 특성에 대한 본 연구결과는 인간 보행의 보다 간단하고 넓은 이해를 제공할 것이고, 나아가 의지 또는 보조기와 같은 재활기기의 보다 넓은 이해를 제공할 수 있을 것으로 기대된다.

기능성 인솔유형들의 생체역학적 평가 (The Biomechanical Evaluation of Functional Insoles)

  • The purpose of this study was to compare and biomechanical evaluate the effects of three varying functional insoles on the kinematics of the lower extremities and foot pressure distribution during gait. For this 12 subjects participated in this study and each worn the 3 functional insoles during gait which kinematics, kinetics, electromyography and foot pressures were recorded. The function on the first insole was to absorb shock and increase the dynamic stability, the second was a gel type to absorb shock, and the third was to massage the center regions of the foot sole. the results were as follows; the first insole reduced the joints range of motion and reduced muscular fatigue, the second insole reduce the maximum, total and average foot pressures. Finally, the third insole produced larger values for the contact times and contact area.

Changes in lower extremity alignment in standing position using a foot plate

  • Objective: Eversion of the foot is created with internal rotation of the shank, and inversion of the foot is created with external rotation of the shank. The purpose of the study was to investigate the effect of continuous changes in the angle of the subtalar joint on lower extremity alignments. Design: Cross-sectional study. Methods: Seventeen healthy young adult subjects recruited. The subjects were asked to stand up in a natural standing position on a footplate with eye open and equal weight on each foot for 10s in two different conditions: The right subtalar joint was everted continuously $0^{\circ}-20^{\circ}$ and in separate segments of $0^{\circ}$, $5^{\circ}$, $10^{\circ}$, $15^{\circ}$, $20^{\circ}$. The averages of three trials were used. The observation of the changes in the lower extremity was performed with the use of 3-dimensional motion analysis. For data analysis, the SPSS 18.0 software using paired t-test and repeated measures analysis of variance (ANOVA) was applied. Results: The angle was significantly increased at the horizontal rotation angle of the shank, thigh, and ankle without anterior rotation of the pelvis (p<0.05). The maximum horizontal rotation angle at the thigh on $20^{\circ}$ was $-4.52^{\circ}$ in static, and $-3.10^{\circ}$ in the dynamic conditions compared to $0^{\circ}$. Conclusions: Increased unilateral foot pronation, thigh, shank, ankle horizontal rotation variance was significantly effective. The observation of the changes in foot abduction with the use of a 3-dimensional motion analysis augmented in predicting the angle values of each segment of the lower extremity. In further studies, a comparison of the right and left subtalar joints need to be investigated.

Muscle Activation of Soleus and Tibialis Anterior according to Landing Strategy of Initial Contact during Descending Stairs: Comparison of Forefoot and Whole Foot

  • Objective: To control the rate at which body weight drops, forefoot initiates floor contact with the limb relatively extended at each joint. However, when the knee joints could not extend enough with going down the stairs, the forefoot cannot be contact initially. The purpose of this study was to investigate the differences between forefoot and whole foot in initial contract on soleus and tibialis anterior for health young adults when descending stairs. Design: A cross-sectional observational study design. Methods: Fifteen healthy young adults participated in this study. To compare between forefoot and whole foot in initial contact when going down the stairs, this study measured muscle activation on soleus and tibialis anterior. This study used the paired t-test to analyze the collected data and compare the supporting conditions. Results: After analyzing, the muscle activation of soleus was not statistically significant difference as 25.16% at forefoot initial contact and 24.37% at whole foot initial contact when descending stairs (p>0.05). However, the muscle activation of tibialis anterior muscle was significantly difference was 49.19% at forefoot contact and 71.55% at whole foot contact. Conclusions: The results of this study was that the muscle activation of the tibialis anterior was a higher at whole foot contact than that at fore foot contact when descending stairs. This study suggests that the landing strategy of the initial contact is a beneficial effect at the forefoot contact to maintain the postural balance and the muscle performance effectively when descending stairs in individuals with healthy young adults.

뮐러-와이스 병의 방사선학적 특징 및 수술적 치료의 결과 (Radiographic Characteristics and the Clinical Results of the Operative Treatment of M$\ddot{u}$ller-Weiss Disease)

  • Purpose: To present radiographic characteristics and report the clinical results of the operative treatment of M$\ddot{u}$ller-Weiss disease. Materials and Methods: This is a retrospective study including 13 patients, 14 feet who had been operated for M$\ddot{u}$ller-Weiss disease between April 2006 and December 2011. Osteoarthritis of the peri-navicular joints were radiographically evaluated. Various range of peri-navicular fusion and joint-preserving surgeries according to patients' symptoms and radiographic findings were done. The clinical results were evaluated by American Orthopaedics Foot and Ankle Society (AOFAS) midfoot scale and visual analogue scale (VAS). Results: On radiographs, osteoarthritic changes were presented at talonavicular joint in 11 cases, calcaneocuboid joint in 7 cases, subtalar joint in 6 cases, naviculo-cuneiform joint in 1 case. The mean anteroposterior talocalcaneal angle was 16.6 degrees. On hindfoot alignment view, 6 cases were varus, 5 cases were neutral and 3 cases were valgus alignment. Fusion comprised 6 cases in triple fusion, 1 case in talonavicular-cuneiform fusion, 2 cases in talonavicualr fusion and 1 case in talonavicular & calcaneocuboid fusion. Joint-preserving surgeries were bony fragment excision of the lateral part of navicular & medial displacement calcaneal osteotomy in 1 case, bony spur excision of talonavicular joint in 1 case and medial displacement calcaneal osteotomy in 2 cases. The postoperative AOFAS and VAS score were improved significantly (p=0.000, p=0.000). Conclusion: In cases of M$\ddot{u}$ller-Weiss disease without osteoarthritic changes at peri-navicular joints, fragment excision of navicular, bony spur excision with or without medial displacement calcaneal osteotomy were effective operative treatments.
