아킬레스건 파열 이후의 능동적인 관절 범위 운동이 없는 간단한 재활 운동 방법 (Simple Postoperative Exercise of Acute Achilles Tendon Rupture without Active Range of Motion Exercise)

  • Purpose: Postoperative exercise for acute Achilles tendon rupture is important for a patient's return to daily life and sports. On the other hand, the protocol requires considerable effort to educate patients and continuous checking. This study evaluated the outcome of a new simple and delayed rehabilitation protocol after Achilles tendon rupture repair. Materials and Methods: From July 2014 to November 2020, one hundred eighty-three patients were operated on by one surgeon. The exercise protocol was classified into two methods. One group (immediate protocol, control group) started immediate full weight bearing with a 20° plantar flexion range of motion from two days postoperatively. Ankle dorsiflexion was restricted to 0°. The other group (delayed protocol, case group) started full weight bearing with a controlled ankle motion boot from two weeks postoperatively. No range of motion exercise was allowed until six weeks postoperatively. Age, sex, body mass index, ankle range of motion, muscle power, time to return to previous physical activity, functional score, and complication rate were evaluated. The results of the two groups were compared using a Mann-Whitney test. Statistical significance was set as p<0.05. Results: The range of motion, double heel rising, and one-leg standing were achieved faster in the control group (p<0.05). However, single-heel rising, repeated single-heel rising, return to previous activity (work, run, and sport), and functional scores showed no statistical difference (p>0.05). Conclusion: Simple and delayed postoperative rehabilitation of acute Achilles tendon rupture without active range of motion exercises showed satisfactory functional results and a low complication rate.

Is Hiding Foot and Mouth Disease Sensitive Behavior for Farmers? A Survey Study in Sri Lanka

  • Foot and mouth disease (FMD) has a long history in Sri Lanka and was found to be endemic in various parts of the country and constitutes a constant threat to farmers. In Sri Lanka, currently there is no regular, nationwide vaccination programme devised to control FMD. Therefore, improving farmers' knowledge regarding distinguishing FMD from other diseases and ensuring prompt reporting of any suspicion of FMD as well as restricting movement of animals are critical activities for an effective FMD response effort. Therefore, the main purpose of this study was to clarify the relationship between farmers' knowledge levels and their behaviors to establish a strategy to control FMD. In our study, item count technique was applied to estimate the number of farmers that under-report and sell FMD-infected animals, although to do so is prohibited by law. The following findings were observed: about 63% of farmers have very poor knowledge of routes of FMD transmission; 'under-reporting' was found to be a sensitive behavior and nearly 23% of the farmers were reluctant to report FMD-infected animals; and 'selling FMD-infected animals' is a sensitive behavior among high-level knowledge group while it is a non-sensitive behavior among the low-level knowledge group. If farmers would understand the importance of prompt reporting, they may report any suspected cases of FMD to veterinary officials. However, even if farmers report honestly, they do not want to cull FMD-infected animals. Thus, education programs should be conducted not only on FMD introduction and transmission, but also its impact. Furthermore, consumers may criticize the farmers for culling their infected animals. Hence, not only farmers, but also consumers need to be educated on the economic impact of FMD and the importance of controlling an outbreak. If farmers have a high knowledge of FMD transmission, they consider selling FMD-infected animals as a sensitive behavior. Therefore, severe punishment should be levied for selling FMD-infected animals.

흰쥐의 신경병증성(神經病症性) 통증(痛症) 모델에서 양로(養老) 자침(刺鍼)의 진통효과(鎭痛效果) (Analgesic effect of acupuncture applied to $SI_6$ in a rat model of neuropathic pain)

  • Objectives : The usage of acupuncture has gained popularity for certain chronic pain conditions. However, the efficacy of acupuncture in various diseases has not been fully established and the underlying mechanism is not clearly understood. In the present study, the effect of electroacupuncture (EA) applied to yangno$(SI_6)$ on the neuropathic pain was examined. Methods : A common source of persistent pain in human is a neuropathic pain. Neuropathic pain was induced by tight ligation of L5 spinal nerve. When rats developed pain behaviors, EA was applied for 30 min. under enflurane anesthesia with repeated train stimuli at the intensity of 10X of muscle twitch threshold. The foot withdraw latency of the hind limb was measured for an indicator of pain level after each manipulation. Results : EA increased the mechanical threshold of the foot in the rat model of neuropathic pain significantly for the duration of 1 hr. suggesting a partial alleviation of pain. EA applied to SI6 point produced a significant improvement of mechanical sensitivity of the foot lasting for at least 1 h. However, $ST_{36}$ point did not produce any significant increase of mechanical sensitivity. The improvement of mechanical threshold was interpreted as an analgesic effect. The analgesic effort was specific to the acupuncture point since the analgesic effect on the neuropathic pain model could not be mimicked by EA applied to a point, $ST_{36}$. In addition, this analgesic effect of EA is mediated by a adrenergic mechanism of descending control of spinal cord from the brain. Conclusions : The data suggest that EA produces a potent analgesic effect on the neuropathic pain model in the rat; and 2) that EA-induced analgesia is mediated by a adrenergic mechanism of descending control in a point specific manner.

트레드밀 달리기시 신발 내부의 부하에 관한 연구 (In-shoe Loads during Treadmill Running)

  • To enhance our understanding of the loads on the foot during treadmill running, we have used a pressure-sensitive insole system to determine pressure, rate of loading and impulse distributions on the plantar surface during treadmill running, both in minimally cushioned footwear and in cushioned shoes. This report includes pressure, rate of loading, impulse and contact time data from a study of ten subjects running on a treadmill at 4.0m/s. Among heel-toe runners, the highest peak pressures and highest rates of loading were observed under the centre of the heel and in the medial forefoot. The arch regions were only lightly loaded. Contact time was greater in the forefoot than in the heel. Two-thirds of the impulse recorded during the step was the result of forces applied through the forefoot, mostly in the region of the metatarsal heads. The distribution of loads in the shoe suggests that the load distributing properties of the cushioning system are most important in the centre of the heel, under the metatarsal heads and great toe. Shock attenuation is primarily required under the centre of the heel and to lesser extent under the metatarsal heads. Some energy dissipation may be desirable in the heel region because it causes shock to be absorbed with less force. All the 'propulsive' effort is applied through the forefoot. Therefore, this region should as resilient as possible.

Development of Evaluation Checklist for Personal Office Furniture and Apparatus and Fact-Finding Survey

  • Objective: The objective of this research is to develop evaluation checklist for personal office furniture and apparatus to shape comfortable and efficient worksite for workers' welfare improvement, productivity enhancement and labor force preservation, and to identify office work environment by applying the checklist. Background: Because most office workers work using computers in a sitting posture, the ratio of office workers among total musculoskeletal disorders patients is forecast to increase. In this regard, an effort to prevent and manage such musculoskeletal disorders is required. Method: This research developed evaluation checklist for personal office furniture and apparatus by examining 25 domestic and international ergonomic literature and anthropometric data. This research carried out a fact-finding survey targeting the A Office and B Office of one public agency using the checklist. Results: Although, the checklist items on desks, chairs, computers and other goods conformed to the checklist standards, the following items did not conformed: desk height adjustment, seat board depth adjustment, lumbar support depth, foot rest, wrist rest, mouse tray, headset, speaker phone and Bluetooth. Conclusion: The evaluation checklist for personal office furniture and apparatus and fact-finding survey results are considered to be used as basic data for office work environment and workers' welfare improvement. Application: The information drawn from this research can be helpful to manufacturers' design and manufacture of ergonomic furniture and apparatus.

지하철 안전사고 방지를 위한 기관사의 적성관리 연구 (Study on aptitude test on engineers for preventing subway safety accidents)

  • The total number of passengers for Line $1{\sim}4$ operated by Seoul Metro among subways that act as foot of Seoul residents was 1.43116 billion passengers in 2006 and average of 3.921 million passengers per day. The results of analysis on safety accidents that occurred recently in subways show that various complaints on subways were reduced through the expansion of safety related facilities and training but accidents caused by negligent handling by employees including engineers are increasing on the contrary. At present time where various man-made accidents break out such as wrong handling and habits of engineers in charge of driving trains and momentary mistakes, there needs effort for active management and improvement to prevent and minimize accidents. Therefore, this study has drawn out problems through surveys on aptitude test system to engineers and compared with case studies in overseas. Thus, it attempted to minimize safety accidents in subways, and derived methods for improving safety management by focusing on improving the current aptitude test system for nurturing engineers suitable to handle railway driving in the future.

테니스화겉창과 테니스 스포츠바닥재간의 마찰관계상관 분석 (A analysis of friction relation between tennis outsole and tennis playing surfaces)

  • 마찰력(friction)은 급정지나 급출발, 또는 두 동작이 동시에 이루어질 때, 미끄럼방지는 효과적으로 이루어져야 하며, 특히 테니스, 농구, 배구 등 코트 스포츠에 있어서 최적의 마찰력은 필수적이다. 이러한 마찰력은 무게가 많이 나갈수록, 다른 물체와의 접촉면이 넓을수록, 장력이 클수록 커진다. 또한 표면이 매끄러울 때보다는 거칠수록 커지는 특성을 가지며, 본 연구의 목적은 테니스화겉창과 테니스코트에 설치되어 있는 스토츠바닥재사이에 마찰력을 대해서 고찰해 보고자 하였으며, 마찰력운동 및 이의 관련 선행 국외 연구활동이 어떻게 이루어졌는지를 종단적 국외문헌조사를 통해 어떤 상관관계가 있는지를 구명해 보고자 하였다. 국내 테니스화의 연구와 테니스스포츠바닥재간의 국외 연구활동의 종단적 연구지표로서 스포츠와 스포츠바닥재간의 마찰력 관련주요연구연표를 작성해 봄으로서 연구활동의 경과추이를 조사하였다. 테니스 바닥재 현황으로는 테니스코트의 표층재는 일반적으로 천연재료를 사용한 클레이계코트와 합성재료를 사용한 전 천후계 하드코트로 구별된다. 본 연구를 통하여 테니스화 겉창과 테니스 스포츠바닥재간의 마찰력의 국외선행연구현황 이해를 돕고, 향후 국내에서 스포츠화 겉창과 마찰력, 스포츠바닥재와 마찰력의 평가에 대한 연구시 국외에서 기 실시된 연구를 중복연구하는 시행착오 방지 및 국외 선행연구에 대한 기초자료 및 연구현황을 자세히 파악할 수 있을 것으로 사료된다.

바지락 (Ruditapes philippinarum)의 번식량 측정 및 유생발달에 관한 연구 (Quantification of Reproductive Effort and Microscopic Observation on the Larval Development of Manila clam Ruditapes philippinarum (Adams and Reeve, 1850))

  • 이 연구에서는 바지락 (Ruditapes philippinarum) 의 번식량 및 유생발생을 관찰하였으며, 이를 위하여 수정란 및 유생을 21일 간 실내사육수조를 이용하여 배양하였다. 바지락은 2011년 8월 제주시 김녕에서 채집 실험 수조로 옮겨졌으며, 다음날 자연적인 방란 방정이 관측되었다. 바지락 수정란은 19시간의 배 발생과정을 거쳐 21일 후 포복피면자유생(pediveliger) 으로 발달하였다. 조직학적 방법을 이용하여 바지락의 생식소를 관찰한 결과, 방란, 방정 전 모든 개체는 성적으로 완숙한 상태였고, 산란 후 모든 개체에서 부분산란이 관찰되었다. 효소면역학적 방법 (ELISA) 에 의한 번식량 측정 결과, 암컷 바지락의 생식소 지수는 (GSI) 산란 전 28.6%에서 산란 후 17.3%로 11.3% 감소하였으며, 추정된 포란수는 6,998,658 eggs였다. 이 연구서 광학현미경 및 전자현미경을 이용하여 관찰된 바지락 유생 발달 및 번식량은 향후 바지락 인공종묘 산업 및 바지락 자원관리에 있어 중요한 자료로 활용될 것으로 사료된다.

한국전통마을에 있어서 갈림길의 유형과 형태에 관한 연구 (A Study of Types and Forms of Branch-road in Traditional Korean Villages)

  • The purpose of this study is to analyse types and forms of branch-road in traditional korean villages. To do this, this study has selected and measured 87 branch-road cases of five traditional villages. An analysis of the case study has revealed the following results : 1. Branch-roads have four formations : 3 way, 4 way, multi-way and others. 2. Branch-roads comprise 64 percent of 3 way, 18 percent of 4 way, 8 percent of multi-way and 9 percent of others, respectively. 3. Topographic conditions have mostly affected the formation of branch-roads. 3 way of branch - roads are easily found in flat village. 4 way are multi-way of branch-road are found in semi-hilly and hilly village 4. 3 way of branch-roads have three types: T-type, Y-type, and y-type. 5. T-types are easily found in flat village, but Y-type and y-type are found in semi-hilly and hilly village. 6. Each angle of T-type is 171,99,90, respectively. It has turned out 8 degree is slightly deviated form the square. 7. Y-type of branch-road has an asymmetric form, comprising 145, 128, 87 degree of the angle, compared to 150, 150, 60 degree of symmetric one. 8. Average angles of y-type that are mostly found in hilly village are 175, 113, 72. Angle differences from each spot are 62 and 41 degree. It is assumed that 39 degree is a slope of least effort to walk. 9. 4 way of branch-roads have not only "+" type but various types of K, Y and Y, Y and y, T and y and a foot of bird. 10. It is assumed that multi-way of branch-roads, mostly found on the middle and bottom of the hilly slope, has naturally formed to connect up and down, left and right.and right.

성장기 태권도 선수의 만성 통증과 손상 유형 (Chronic Injuries and Types of Injuries Related to Adolescent Taekwondo Athletes)

  • 목적: 성장기 태권도 선수가 호소하는 만성 통증의 유형과 원인을 분석하고 이에 대한 선수 및 지도자의 인식 조사와 함께 만성 통증을 줄일 수 있는 방법을 모색해 보고자 하였다. 대상 및 방법: 2006년 3월부터 2007년 6월까지 1년 4개월 동안 광주광역시의 초, 중, 고교 태권도 선수 210명과 지도자 12명을 조사하였다. 진단은 이학적 검사와 단순 방사선 촬영, MRI 등을 통해 이루어졌다. 결과: 조사 대상 선수 210명 중 만성 통증을 겪은 선수는 162명(77.1%)이였으며 세 부위의 만성 통증을 겪은 선수는 19명(11.7%), 두 부위는 74명(45.7%), 한 부위는 69명(42.6%)이였다. 162명의 만성 통증 274예에서 손상 부위로는 족부, 족관절부가 145예(52.9%)로 가장 많았고 수부, 수근관절부 47예(17.2%), 슬관절부 38예(13.9%), 고관절부 25예(9.1%), 요추부 19예(6.9%)였다. 만성 통증의 손상 유형으로는 족부의 좌상(타박상)이 103예로 가장 많았고 다음으로 족관절부 염좌 40예, 수부, 수근관절부 좌상 28예 등이었으며 이러한 만성 통증을 유발하는 손상 원인으로는 겨루기 연습 손상이 가장 많았고, 다음으로 무리한 훈련이었으며, 경기 손상 등이 있었다. 결론: 성장기 태권도 선수들에게 만성 통증이 흔히 있었고 족부, 족관절부 통증이 많았으며 손상 유형으로는 좌상과 염좌가 많았다. 손상 원인으로는 겨루기 연습 손상이 가장 많았으며 만성 통증으로의 이행은 무리한 훈련과 잦은 경기가 큰 원인이 되고 있었다. 이러한 선수들의 손상을 예방하고 치료하기 위한 다각적인 노력이 필요할 것으로 사료된다.
