노인의 발 인체 측정 및 형태분류에 관한 연구 (A study on Anthropometric measurement and Type classification of Foot for the Elderly.)

  • 인간의 발 형태와 보행특성은 보행습관과 질병에 의해 변형되어진다. 특히, 노인의 발은 장단기의 신발 착용이나 퇴행성관절염으로 인해 심한 발의 변형이 나타나, 청 장년층과는 발의 형태나 유형이 다르다. 그러므로 노인신발 설계 및 화형개발을 하기 위해서는 노인발의 정확한 인체 측정치와 발 형태분석 및 유형분류가 필요한 실정이다. 그럼에도 불구하고 노인신발 제작에 노인 발의 인체 측정치의 부재로, 청·장년충의 형을 그대로 사용하고 있는 것이 업계의 현실이다. 본 연구에서는 과학적 노인신발 제작을 위하여 발의 49개 인체 측정부위를 남녀노인 총 252명을 대상으로 하여 측정하였고, 발 너비 대 길이지수와 족선각을 이용하여 형태 및 유형 을 분류, 분석하였다. 연구결과 노인의 발은 젊은 청년에 비하여 가늘고 긴, 즉 세장화 경향을 보인다. 이는 고령 남성보다도 여성에게 더 두드러진다. 본 연구는 노인신발 설계를 위해 화형개발에 필요한 자료를 제공하고, 인간공학적 노인신발을 디자인하는데 활용되리라 생각된다.

체중 감량 중에 발생한 편측 족하수: 증례보고 (Unilateral Foot Drop During Weight Reduction:A Case Report)

  • Peroneal neuropathy scarcely can develop after massive weight reduction. A 21-year-old man complained left foot drop after 28% weight reduction (from 94 kgs to 67.5 kgs). During previous seven months, he played PC games with sitting cross-legged more than seven hours a day. In addition, he started a heavily restricted diet three months ago. Except for those, he had neither any medical history nor trauma to his knee. Electrophysiologic study showed the partial conduction block of left peroneal neuropathy at the fibular head. Four-week well balanced diet and physical therapy improved his foot drop. For the prevention of peroneal neuropathy related to weight reduction, well balanced diet and lifestyle modification are needed.

Antiviral Activity of Hederasaponin B from Hedera helix against Enterovirus 71 Subgenotypes C3 and C4a

  • Enterovirus 71 (EV71) is the predominant cause of hand, foot and mouth disease (HFMD). The antiviral activity of hederasaponin B from Hedera helix against EV71 subgenotypes C3 and C4a was evaluated in vero cells. In the current study, the antiviral activity of hederasaponin B against EV71 C3 and C4a was determined by cytopathic effect (CPE) reduction method and western blot assay. Our results demonstrated that hederasaponin B and 30% ethanol extract of Hedera helix containing hederasaponin B showed significant antiviral activity against EV71 subgenotypes C3 and C4a by reducing the formation of a visible CPE. Hederasaponin B also inhibited the viral VP2 protein expression, suggesting the inhibition of viral capsid protein synthesis.These results suggest that hederasaponin B and Hedera helix extract containing hederasaponin B can be novel drug candidates with broad-spectrum antiviral activity against various subgenotypes of EV71.

외측 뒤꿈치 당뇨발 환자의 보행 중 발바닥 전단응력 및 압력분포 (Plantar Shear Stress and Normal Pressure in Lateral Heel Diabetic Foot Patients During Walking)

  • In this study, we analyzed the plantar shear stress and normal pressure in lateral heel diabetic foot patients during walking by using in-shoe local shear stress and plantar pressure measurement systems. The shear force transducer based on the magnetic-resistive principle, was a rigid 3-layer circular disc. Shear transducers were mounted on the locations of four metatarsal heads and heel in the insole. Twelve normal subjects and three diabetic foot patients with diabetic neuropathy in the lateral heel participated in this study. The center of pressure in lateral heel diabetic foot patients moved more medially and directed toward the first, medial to the second metatarsal heads, and the hallux during late stance, making pressure at the medial heel and the second metatarsal head significantly larger than in the normal. Shear stress at the heel changed significantly in early stance and the magnitude of shear stresses in each metatarsal head also changed. Further studies would be very helpful to design foot orthoses in patients with diabetic neuropathy or other diseases.

토구 운동이 자세균형과 족저압 분포에 미치는 영향 (The Effect of TOGU Exercise on the Postural Balance and Foot Pressure Distribution)

  • Background : The purpose of this study to find the Effect of TOGU Exercise on the Postural Balance and Foot Pressure Distribution. Methods : Four male and five female subjects last one year and no damage at all, even unique Foot skin diseases or orthopedic surgical disease who are not selected and applied before and after exercise TOGU and Foot Pressure Distribution of static equilibrium and measured using were compared. Results : After exercise conducted TOGU. right and life the static balance measurements of the height difference between the two sides of the acromion before exercise to 0.8cm and 1.4cm reduction in the movement, and also before and after the static balance measured sagittal the lateral tibial line drawn straight down from the earlobe and vertical slit in the distance before exercise from 5.8cm to 3.4cm decreased after exercise before and after the balance has also improved significantly(p<.05). Static balance of the body, according to the pressure of the foot in the exercise before/after there was no significant difference(p>.05). Conclusion : Dynamic shaking of the body have a positive impact on the static equilibrium could know that.

노숙인 거주 시설의 족부 족관절 실태에 대한 조사 (A Study on the Foot and Ankle Disease of Shelter-based Homeless People)

  • Purpose: Foot and ankle disease (FAD) is a frequent cause of morbidity among the homeless population. Various conditions, exacerbated by malnutrition, poor lifestyle habits, psychiatric disorders, physical injuries, poor hygiene, and limited access to healthcare, have been described in this population. The purpose of this study was to investigate the cognition and management status of FAD in shelter based homeless people. Materials and Methods: Fifty-two male and twenty-five female volunteer homeless individuals were recruited from two homeless shelters. Each person completed a questionnaire assessing any presence of pain, pain management, as well as foot and ankle care status. A foot and ankle surgeon examined the physical status of the individual's foot and ankle, including tenderness and instability. A radiologic evaluation was done for 18 male and 11 female homeless people who agreed to participate in this test for the existence osteophyte or joint space narrowing representing osteoarthritis and some reference angles for hallux valgus, flatfoot and cavus foot. Results: Homeless people had higher prevalence of body mass index, diabetes, and smoking than the general population. The most prevalent infectious disease was fungal infection (male 78%, female 68%), with a low compliance of management for FAD. Conclusion: Although most of shelter-based homeless people showed an acceptable foot and ankle management status, the potential risk for FAD development and exacerbation of mild FAD was high.

보행시 과도한 회내와 관련된 발질환을 위한 맞춤형 발보조기의 교정적 기능 (Correctional Function of Custom Foot Orthotics for Foot Diseases related to Excessive Pronation during Gait)

  • The purpose of this study was firstly to investigate correctional function of custom semi-rigid foot orthotics for excessively pronated people during gait by observing comfort, navicular movement and leg muscles' activity according to short-term and mid-term wearing duration and secondly to understand positive and/or negative point of view of a recently proposed paradigm related to foot orthotics more profoundly. Sixteen subjects who showed excessive pronation at navicular drop test were recruited for this study. Custom semi-rigid foot orthotics were made fitting for foot characteristics of the subjects by podiatry division of Otto Bock Korea company. While wearing the foot orthotics for two months, comfort of wearing were questioned and vertical navicular movement and electromyography of leg muscles during gait were measured at the condition of both immediately after and 2 months after including a control condition, respectively. The subjects were required to walk on a treadmill at the speed of 1.5m/s and four digital video camera filmed the movement of navicular process at the speed of 60 frames/s. In conclusion, in excessively pronated group continuous increase of comfort from short-term to mid-term wearing of custom foot orthotics is assumed to be closely related with short-term and mid term correctional action, of which are consisted the decrease of the range of navicular drop and navicular raisins- the faster timing of minimum navicular position occurring, and the decrease of leg muscles' activities. This conclusion could lead to positively accept new paradigm related to foot orthotics suggested by Nigg and the author suggest that in the future study the variable which could observe navicular movement would be one of major variables to study preferred path of skeleton in the paradigm.

평지 보행 시 발 각도 형태에 따른 발바닥 압력 비교 연구 (A Comparative Study of the Plantar Foot Pressure according to the Form of Foot Angle during Level Walking)

  • Purpose: The purpose of this study was to examine the influence of foot angles on plantar pressure and the center of pressure (COP) trajectory length during level walking. Methods: The study subjects were 30 female university students without orthopedic diseases in the foot. The foot angle was divided into three forms (out-toeing, normal, in-toeing). The subjects practiced each type of gait, and then performed each of level walking, three times, and their averages were calculated. A plantar pressure measurement instrument was used, and the maximum force was obtained by dividing the foot into nine regions covering the anterior medial-lateral, middle medial-lateral, and posterior medial-lateral. The COP trajectory length was statistically processed by obtaining medial-lateral, anterior-posterior, and entire travel distance. Results: During normal walking, the maximum force was significantly higher in the anterior lateral than in the other areas, and the COP trajectory length was significantly shorter in the front-back and entire travel distances (p<0.05). During stair climbing. Conclusion: Walking at abnormal foot angles does not cause appreciable problems in the short term as pressure is concentrated on a specific plantar part. However, it becomes the cause of deformed foot structures and can result in musculoskeletal disabilities in the long term. Therefore, a kinesiatrics-based intervention is required to maintain normal foot angles.

가축매몰지 환경관리에 있어 차단방역의 필요 및 절차 (Needs of Biosecurity and Protocols for the Environmental Management of Carcasses Burial)

  • Avian influenza (AI) and foot and mouth disease (FMD) are two main contagious pathogenic viruses causing massive mortality burial, as burial is a primary measure to quaranteen the causative viruse(s). Biosecurity is a set of preventive measures designed to reduce the risk of propagation of infectious diseases. Main objectives of this paper were to discuss the needs of biosecurity and develop protocol outlines for environmental management of burial sites. Pathological characteristics of contagious viruses should be considered during environmental management practices. Current practice prescribes to minimize the potential for on-farm pollution and the spread of infectious diseases, policy makers should understand robust knowledge regarding biosecurity to make informed decisions on future legislation.

  • Tarsal tunnel syndrome (TTS) is an entrapment neuropathy of the posterior tibial nerve or one of its branches within the tarsal tunnel, and is often caused by ganglia, lipoma, accessory muscles, varicosities, neural tumours, trauma and systemic diseases. We have successfully treated a patient with tarsal tunnel syndrome which was associated with os sustentaculi.
