Transcutaneous Oxygen Pressure to Predict Wound Healing in Mild Diabetic Feet (경증의 당뇨발에서 창상치유 예측인자로서의 경피산소분압치)

  • Purpose: Microcirculation of diabetic patients is commonly comporomised, regardless of the condition of the macrocirculation. Therefore, direct tissue oxygenation measurement is recommended in determining tissue viability and predicting wound healing potential. This study was designed to determine cut-off value of the tissue oxygenation in predicting wound healing in diabetic foot patients. Methods: This study included 41 feet of 41 diabetic foot patients who were treated in the Diabetic Wound Center of author's institution between January and June, 2009. Main inclusion criteria were type 1 or 2 diabetes and a foot ulcer (duration > 3 weeks) and ulcer area (from 1 $cm^2$ to 4 $cm^2$). Measurements of the area of diabetic foot ulcer were carried out before treatment. Transcutaneous oxygen pressure ($TcpO_2$) was measured at adjacent site of ulcer. The healing wound was defined as complete wound closure within 12 weeks. Results: Average diabetic foot ulcer areas with healing and nonhealing wounds were $2.67{\pm}0.76$ and $2.59{\pm}0.75\;cm^2$, respectively. There was no significant difference in the wound area between the groups. Average foot $TcpO_2$ in healing and nonhealing wounds were $68.56{\pm}23.07$ and $30.98{\pm}16.66$ mmHg, respectively ($p$ <0.01). The rate of healing wound increased as $TcpO_2$ increased. In particular, $TcpO_2$ lower than 40 mmHg and higher than 40 mmHg showed the most significant difference (wound healing rates of 25% and 71%, respectively). Conclusion: Based on the results of the study, the minimal $TcpO_2$ value thought to be required for adequate wound healing in diabetic wounds (cut-off value) is 40 mmHg.

Recurred Macrodystrophia Lipomatosa of the Foot: A Case Report (족부에 재발한 지방종 거대증: 증례 보고)

  • We reported on a rare case of recurred macrodystrophia lipomatosa of the foot, and reviewed the literature. A 62-year-old male patient presented with right foot second toe pain; preoperative magnetic resonance imaging and radiograph examination was performed. After surgery the biopsy confirmed the diagnosis. American Orthopaedic Foot and Ankle Society score was checked before and after surgery. Wide excision of the affected area including ray amputation is an effective way to prevent recurrence and relieve the pain after surgery. The 2nd toe ray amputation was performed in the treatment of recurred macrodystrophia lipomatosa of the foot, and is thought to be an effective way to relieve pain and prevent recurrence. After minimally invasive surgery with complete excision surgery, additional data on recurrence and pain relief rate are needed.

The effects of Aroma foot reflexology and foot reflexology on blood pressure, pulse rate and blood lipid level of elderly essential hypertensive patients in a rural area (아로마발반사요법과 발반사요법이 농촌의 본태성고혈압 노인환자의 혈압, 맥박, 혈중지질농도에 미치는 효과)

  • The purpose of this study was to examine the effects of aroma foot reflexology and foot reflexology on blood pressure, pulse rate and blood lipid level of elderly essential hypertensive patients in a rural area. The research method was a non-equivalent control group pretest-posttest quasi-experimental design and the participants were consisted of 71 elderly patients with essential hypertension who were being registered at C health Clinic(G gun, Chungnam province). Here, we compared experimental group I(aroma foot reflexology) 24 persons, experimental group II(foot reflexology) 27 persons and control group(conventional treatment) 20 persons to measure the effect. The data were analyzed with SPSS/WIN 12.0. The systolic blood pressure(p<.05), the diastolic blood pressure(p<.05), and the pulse rates(p<.01) of experimental group I and II after intervening respectively with aroma foot reflexology, and foot reflexology for 6 weeks were significantly decreased than the control group, but the blood lipid level was no significant difference among 3 groups(p>.05). In conclusion, both of aroma foot reflexology and foot reflexology had positive effects to decrease the blood pressure and pulse rates of the elderly essential hypertensive patients in a rural area and would be utilized as a nursing intervention for them.

A Study on the Property of the Foot Form of Early Childhood Children from Three to Six Years old(I) -On the Difference of the Age and the Sex- (유아의 발 형태에 관한 연구(I) -연령과 성에 따른 발 형태 분석을 중심으로-)

  • For design of early childhood children's footwear, this study was to analyze the properties of their foot. This study was performed in 200 male and 200 female early childhood children from 3 and 6 years old lived in susan urban area. There were measured 17 items of the foot and calculated 4 indexs for analysis. The result was as follows; 1 The growth rate differs from the sites of a foot. According to the growth of age, early childhood children's foot are more slender. 2. The average of foot print angle is $15~27^{\circ}$ and it is flat. Among the age groups, the numbers of 4 kinds of foot print angle are different siginificantly. 3. The average of metatarso phalanx angle is $174~178^{\circ}$ that is higher than adults'. 4. There are no significant difference between male children's foot and female childen's one except tarsal circumference and tarsal height. Tarsal part of male children is higher and ticket than female children's. 5. Because some of 3~6 age groups belong to the one foot length group, we should consider the age properties of foot in order to design footwears. When the size of footwear is divided by only foot length, the ranges of another measurements are larger than the range of foot length.

The sample size for the study was 200 college men between age 19 and 24 in Pusan urban area. There were measured 17 items of the foot for factor analysis and cluster analysis. The result was as follows: 1. The size of If items of men's foot is larger than women's foot. 2. There were 9 items selected by factor analysis. 3. The cluster analysis of the direct measurement: Cluster 1: The items of the direct measurement is all much the same to mean value of this age group. Cluster 2: The foot size is relatively small to other clusters. Cluster 3: The foot size is relatively large to other clusters. 4. The cluster analysis of indirect measurement: Cluster 1: The foot print angle is the most value and Metatarso-Phalanx angle is nomral Cluster 2: The foot print angle is middle and Metatarso-Phalanx angle is normal. Cluster 3: The foot print angle is high and Metatarso-Phalanx angle is the smallest. Cluster 4: The foot print angle is low and Metatarso-Phalanx angle is all the much same to mean value of this age group.

The Analysis of Foot Shape of Elementary School Boys (학령기 남아의 발 형태 분석)

  • The purposes of this study were to investigate the relationship between anthropometric data of foot and other body sizes. to categorize the foot shape of elementary school boys and to find out determinant factors related the foot that enable us to deduce the foot shape and size for the design of more comfortable shoes. Subjects of this study were 249 elementary school boys of age ranged from 6 to 11 residing Seoul and lncheon area. Anthropometric sizes were measured with the direct measurement method using Martin scales and the indirect measurement method using digital photos. Pearson's correlation, factor analysis. cluster analysis. analysis of variance, post-hoc test, and cross tabs were performed for statistical analysis of the data by SPSS program. From the investigation on the relationship between foot-related items and body items, most items of foot measure were significantly related to body size items. However, angle of the foot did not related to other body sizes although other height items and mass items of the foot did have relationships with other body sizes. Results of ANOVA indicated there were significant differences in foot-related items except for items of foot angle and all body anthropometric items by subjects' age. This implicates big toe angle, little toe angle and foot ratio factors are required in sizing shoes besides foot length. On the basis of cluster analysis using factor scores. three different foot shapes were categorized. Type 1 was large and wide foot, Type 2 was small and narrow foot with large toe angle. and Type 3 was medium foot with no deformity on big toe. These three groups show significant differences in almost all measurement items. However, Rorher index and foot angle didn't show any significant differences among groups. This implicates the foot shape can be a determinant of shoe size.

A Study on the Correlation between Scoliosis and Foot (척추측만과 족부의 상관관계 연구)

  • The object of this thesis is to study the correlation between scoliosis and foot in a theoretical and empirical method. My research process was investigated for 34 students on the forward bending test, foot printer test, feedoscope test, X-ray test and so on. The main results of this study were as follows: 1. There was statistical significance on the correlation between foot length and spine length. 2. Scoliosis angle is the smallest on the pes plannus group and the biggest on the pes cavus group without statistical significance. 3. Left lumbar curve is the smallest, and right thoracic spine curve is the biggest on the left foot pattern group with statistical significance. 4. On the foot weightbearing groups, there was statistical significance both of between left and right foot groups. 5. There was no statistical significance on the t-test analysis between left foot area and right foot area according to scoliosis typology.

A 3D Foot Scanner Using Mirrors and Single Camera (거울 및 단일 카메라를 이용한 3차원 발 스캐너)

  • A structured beam laser is often used to scan object and make 3D model. Multiple cameras are inevitable to see occluded areas, which is the main reason of the high price of the scanner. In this paper, a low cost 3D foot scanner is developed using one camera and two mirrors. The camera and two mirrors are located below and above the foot, respectively. Occluded area, which is the top of the foot, is reflected by the mirrors. Then the camera measures 3D point data of the bottom and top of the foot at the same time. Then, the whole foot model is reconstructed after symmetrical transformation of the data reflected by mirrors. The reliability of the scan data depends on the accuracy of the parameters between the camera and the laser. A calibration method is also proposed and verified by experiments. The results of the experiments show that the worst errors of the system are 2 mm along x, y, and z directions.

Effect of Foot Massage on Sleep, Vital Sign and Fatigue in the Elderly who live in their Home (발마사지가 노인의 수면, 활력징후, 피로에 미치는 영향)

  • Purpose : This study aimed to investigate the effect of foot massage on sleep, vital signs and fatigue in the elderly who live in their Home. Method: Data were collected from 20 elderly who live in the Seoul-Kyunggi province area by convenience sampling and analyzed the change of sleep and sleep satisfaction, vital signs(pulse, respiration, systolic and diastolic blood pressure) and general fatigue between pre and post foot massage using Paired t- test at 0.05 of significance level. Result: There were very significant difference in the sleep and fatigue between pre and post foot massage. Conclusion: Foot massage can improve the sleep and decrease fatigue, therefore we recommend to use foot massage as a effective sleep related nursing intervention in the elderly who live in their home.
