수부 및 족부 동종이식에 대한 위험 승인도와 기대치 조사 (Investigation of Risk Acceptance and Expectations in Hand and Foot Allotransplantations)

  • Purpose: Composite tissue allotransplantation is a new therapeutic modality to reconstruct major tissue defects of the head and neck region and extremities. However, there is a serious ethical debate about whether the risks posed by the life-long immunosuppression that a recipient would have to take justify the benefits of receiving this non-life saving procedure. The purpose of this study is to examine differences between expert and non-expert groups in risk acceptance and expectations regarding hand and foot allotransplantations. Methods: The author conducted a survey of 345 subjects in total (lay public n=110; medical students, n=120; doctors, n=115), using a questionnaire-based instrument, the Louisville Instrument for Transplantation (translated to Korean). Results: Of the three groups studied, risk acceptance was found to be lowest in the doctor group and highest in the non-expert group, and the difference was significant (p < 0.05). The expectations of aesthetic and functional improvement from the procedure, however, were found to be highest in the non-expert group and lowest in the doctor group, and the differences were also significant (p < 0.05). Conclusion: The results of this study shows that the three populations have noticeable differences in risk acceptance and expectations regarding hand and foot allotransplantations. Therefore, accurate and sufficient information on these procedures should be provided to patients from both medical and ethical perspectives.

스마트 슈즈의 에너지 하베스팅 기능향상을 위한 복합재료 프레임 특성평가 (Characterization of Composite Frame for Enhancing Energy Harvesting Function of a Smart Shoes)

  • 본 연구에서는 인장 시 에너지 하베스팅을 하는 Polyvinylidene Fluoride(PVDF) 리본 하베스터를 신발에 접목한 스마트 슈즈의 에너지 하베스팅 효율을 증가시키기 위한 복합재료 프레임을 설계하였다. 프레임의 하중방향 변형량을 최소화하기 위해 이방성 재료인 탄소 연속 섬유를 사용하여 설계하고 3D 프린터를 이용하여 복잡한 형상을 제작하였다. 보행 시 발생하는 하중에 의한 안창과 중창의 변형량을 계산하기 위해 스프링 요소를 이용하여 안창과 중창을 모델링 하였다. 유한요소 해석을 사용하여 보행 시 스마트 슈즈에 장착된 리본형 하베스터의 인장량을 계산하였다. 예측된 하베스터의 최종 인장 길이 정보는 스마트 슈즈의 에너지 하베스팅 효율 증대에 활용할 수 있을 것으로 기대된다.

노인의 낙상과 연관된 위험요소와 균형 측정 방법 (Risk Factors and Methods in Balance Assessment Associated with Fall in Older Adults)

  • Purpose : The purpose of this study was to determine risk factors and methods in balance assessment associated with fall in older adults. Methods : This article describes many of the tools that can be used to evaluate the physical parameters associated with fall risk in older adults. Results : Composite ratings of performance(Tinetti balance assessment, Guralnik test battery, Berg balance scale, modified-physical performance test) measures the score compounding the balance measure to determine fall risk. Static balance instruments are composed of FICSIT-4 that measures the ability of maintaining foot positions and CTSIB that measures postural stability. Dynamic balance instrument is composed of functional reach test. To measure walking velocity and mobility, 8-foot up-and-go test and walking around two cones are used. We can use 1-RM and to measure muscular strength, isokinetic dynamometery, and 30-second chair stand to measure lower extremity muscle strength. Conclusion : The described instruments are easy to use and widespread. To select and use these tool kits carefully is considered to be helpful in identifying those who are most likely to fall. The final part of the article includes a brief discussion of the potential role of exercise training interventions to improve these physical parameters and prevent falls.

Effect of FRP composites on buckling capacity of anchored steel tanks

  • Enhancement in the seismic buckling capacity of steel tanks caused by the addition of fiber reinforced polymers (FRP) retrofit layers attached to the outer walls of the steel tank is investigated. Three-dimensional non-linear finite element modeling is utilized to perform such analysis considering non linear material properties and non-linear large deformation large strain analysis. FRP composites which possess high stiffness and high failure strength are used to reduce the steel hoop stress and consequently improve the tank capacity. A number of tanks with varying dimensions and shell thicknesses are examined using FRP composites added in symmetric layers attached to the outer surface of the steel shell. The FRP shows its effectiveness in carrying part of the hoop stresses along with the steel before steel yielding. Following steel yielding, the FRP restrains the outward bulging of the tank and continues to resist higher hoop stresses. The percentage improvement in the ultimate base moment capacity of the tank due to the addition of more FRP layers is shown to be as high as 60% for some tanks. The percentage of increase in the tank moment capacity is shown to be dependent on the ratio of the shell thickness to the tank radius (t/R). Finally a new methodology has been explained to calculate the location of Elephant foot buckling and consequently the best location of FRP application.

A survey on the incidence rate of foot diseases in dairy cattle

  • This study was carried out to investigate the status of hoof diseases and to develope treatment method for cattle with hoof diseases during the period of January to December in 2000. Out of 435 heads, 34 heads(7.8%) had hoof diseases. In respect to season, incidence rate was higher in August to September than that of other seasons. The incidence rates of hoof rot, pododermatitis verrucosae, laminitis, other cases and trauma were 14 cases(41%), 10 cases (29%), 7 cases(21%), 2 cases(6%) and 1 case(3%), respectively. In respect to age and milk production, Incidence rate was higher in the cattle with high milk production. Incidence rate of hoof diseases on the hind-limbs was higher than that of the fore-limbs. E. coli(8 strains), Clostridium spp(8 strains), Staphylococcus spp(6 strains), Fusobacterium spp(6 strains), and Bacteroides spp(6 strains) were isolated from the hoof lesions. All isolates were sensitive to composite preparation made of mainly formalin with picric acid and phenol. (Treatment against each strains isolated was sensitive to composite preparation made out of the main constitutions of formalin with picric acid and phenol).

족배부 복합 피부-건 유리피판을 이용한 Achilles건의 일단계 재건술 (One-Stage Achilles Tendon Reconstruction Using the Free Composite Dorsalis Pedis Flap in Complex Wound)

  • The soft tissue defects including the Achilles tendon are complex and very difficult to reconstruct. Recently, several free composite flaps including the tendon have been used to reconstruct large defects in this area in an one-stage effort. Our case presents a patient reconstructed with free composite dorsalis pedis flap along with the extensor digitorum longus and superficial peroneal nerve for extensive defects of the Achilles tendon and surrounding soft tissue. A 36-year-old-man sustained an open injury to the Achilles tendon. He was referred to our department with gross infection of the wound and complete rupture of the tendon associated with loss of skin following reduction of distal tibial bone fracture. After extensive debridement, $6{\times}8cm$ of skin loss and 8cm of tendon defect was noted. Corresponding to the size of the defect, the composite dorsalis pedis flap was raised as a neurosensory unit including the extensor digitorum longus to provide tendon repair and sensate skin for an one-stage reconstruction. One tendon slip was sutured to the soleus musculotendinous portion, the other two were sutured to the gastrocnemius musculotendinous portion with 2-0 Prolene. The superficial peroneal nerve was then coaptated to the medial sural cutaneous nerve. The anterior tibial artery and vein were anastomosed to the posterior tibial artery and accompanying vein in an end to end fashion. After 12 months of follow-up, 5 degrees of dorsiflexion due to the checkrein deformity and 58 degrees of plantar flexion was achieved. The patient was able to walk without crutches. Twopoint discrimination and moving two-point discrimination were more than 1mm at the transferred flap site. The donor site healed uneventfully. Of the various free composite flaps for the Achilles tendon reconstruction when skin coverage is also needed, we recommand the composite dorsalis pedis flap. The advantages such as to control infection, adequate restoration of ankle contour for normal foot wear, transfer of the long tendinous portion, and protective sensation makes this flap our first choice for reconstruction of soft tissue defect including the Achilles tendon.

한글판 Louisville Instrument for Transplantation 설문지의 신뢰도 및 타당도 평가 (Evaluation of Reliability and Validity of the Louisville Instrument for Transplantation (LIFT) in Korean Population)

  • Purpose: Composite tissue allotransplantation has emerged as a new therapeutic modality to reconstruct major tissue defects of the head, neck and extremities. A questionnaire-based instrument, the Louisville Instrument for Transplantation (LIFT), has been developed to objectively assess the risk-versus-benefit ratio for composite tissue allotransplantation procedures. The objective of this study is to assess if the LIFT is a useful, reliable and valid tool to apply to the Korean population. Methods: Seventy-three medical students and 60 lay public completed the LIFT questionnaire (translated to Korean) over the period from February 2010 to April 2010. Internal consistency was assessed using Cronbach's alpha. Test-retest reliability was analyzed using Pearson's correlation coefficient. Construct validity was assessed by comparing Pearson's correlation coefficients between perceived improvements in quality of life and responses to risk tolerance questions concerning organ transplants. Results: Measurements of the test-retest reliability showed that Pearson's correlation coefficients ranged from 0.241 to 0.902, and Cronbach's alphas ranged from 0.52 to 0.80 for medical students and from 0.63 to 0.83 for the lay public. Pearson's correlation coefficients showed significant correlations between perceived improvements in quality of life and responses to risk tolerance questions concerning organ transplants. Hand transplant showed a significant correlation in medical students. Foot, hand, two hands, larynx, partial face transplants showed significant correlations for the lay public. Conclusion: The applicability of the LIFT to the Korean population was found to be reliable and valid. The LIFT may serve as a useful tool for clinical application in the Korean population.

  • This paper is concerned with the numerical investigation of the transfer characteristics of the landing impact force exerted on court sport shoes to the sport surface condition. The reaction force occurred by the impact between court sport shoes and sport surface is absorbed by shoes to some extent, but the remaining impact force is to transfer the human body from the sole of a foot. We consider four surface conditions, asphalt, urethane, clay and wood court surfaces. For the dynamic response analysis, we construct a coupled leg-shoes FEM model and create the multi-layered composite surface model. The numerical simulations are performed by an explicit nonlinear finite element method. Through the numerical experiments, we examine the transfer characteristics of the landing impact force to the surface condition.

  • Symptoms of foot-and-mouth disease include fever and drooling a lot around the hoof, blisters in the mouth, poor appetite, blisters around the hoof, and blisters around the hoof. Research is underway on smart barns that remotely manage these symptoms through cameras. Visible light cameras can measure the condition of livestock such as blisters, but cannot measure body temperature. On the other hand, infrared thermal imaging cameras can measure body temperature, but it is difficult to measure the condition of livestock. 족배부 유리편을 이용한 생건 피부편이식 (Tendocutaneous Free Flap Transfer from Dorsum of the Foot)

  • Tendocutaneous free flap transfer has been usually used to treat troublesome wounds, which had extensive defect of skin and tendons, since Daniel and Taylor had reported successful free flap transfer in 1973. Among the numerous types of free flap, the dorsalis pedis flap, which could include superficial peroneal nerve, extensor tendon and second metatarsus, was widely used as composite free flap. The authors analysed 13 cases of tendocutaneous free flap transfer from dorsum of the foot which were operated at Korea University Hospital from March 1981 to August 1991. The results were as follows: 1. The average size of these flaps was $53.7cm^2$(mazimum $82cm^2$, minimum $30cm^2)$, the average number and length of tendons were 2.9(maximum, 5, minimum 1), and 9.2cm (maximum 17cm, minimum 5cm). 2. The survival rate of flaps was 100%, and functional results by Dargan's criteria were 4 in excellent, 4 good, 3 fair and 2 poor. 3. The delayed healing on donor site could prevented by the meticulous skin graft and repair of extensor retinaculum. 4. The cases of electrical burn were more worse than the traumatic cases in functional results.
