음식점에서 사용하는 자외선 살균소독기 내 자외선 강도 분포 및 자외선 누출 (Distribution of Ultraviolet Intensity and UV Leaking of Commercial UV Sterilizers Used in Restaurants)

  • 식품제조 및 조리 현장에 널리 보급되어 사용되고 있는 자외선 살균소독기의 유효성과 안전성을 검정하기 위하여 자외선램프의 출력 및 거리에 따른 자외선 강도를 측정하였고, 시판 자외선 살균소독기 5종에 대하여 장치 내 자외선 분포를 조사하였으며, 자외선 누출을 검정하였다. 자외선 강도는 램프 출력에 비례하였으며, 근거리(<25 cm)에서는 램프로부터의 거리에, 원거리(>25 cm)에서는 거리의 제곱에 반비례하여 변화하였다. 자외선 살균소독기 내 자외선 강도는 선반의 중앙부에서 가장 높았고, 선반의 중심에서 전후 또는 좌우 방향으로 멀어짐에 따라 감소하였다. 선반 위치에 따른 자외선 강도는 상단 선반과 하단 선반사이에 큰 차이를 보였으며, 각 선반의 외곽부위에서는 자외선강도가 극히 약화되었다. 문틈 등 틈새가 있을 경우 이를 통한자외선 누출은 상당한 수준에 달하였으며, 누출 자외선 강도는거리에 따라 낮아졌다. 자외선이 누출되는 자외선 살균소독기에 대하여 선진국의 자외선 노출 허용량을 근거로 안전작업거리를 제시하였다.

농촌보건사업지역(農村保健事業地域)의 아동영양(兒童營養) 실태조사(實態調査) (Child Nutrition Survey in Rural Health Project Areas)

  • The main purposes of the nutrition and clinical surveys were to provide baseline information on the nutritional status of pre-school children in rural health demonstration project areas of the Korea Health Development Institute (KHDI) for nutrition guidance services for the MCH target group. The survey covered a total of 222 pre-school children and 135 mothers in Okgu Gun, Cholla Pukto Province from August 10 to August 17, 1979. The survey results are summarized as follows: 1) Family Environment Seventy percent of the households had more than three children, and the mean family sire was 6. Sixty-nine percent of the mothers and 47% of the fathers of the surveyed households were educated at or below the primary school level. The majority, 70% of the mothers, were aged between 20 years and 35 years. 2) Anthropometric Measurements and Hemoglobin Value 4.5% of the children were lower than 80% weight for age of the Korean standard, and 5.4% were lower than 85% arm circumference for age of the Jelliffe's standard resectively, and those were suffering from protein-energy malnutrition. Angular stomatitis were observed on 66.2% of the subjects. Mean hemoglobin value was 11.1g/100m1, and 44.2% of the subjects were categorized as anaemia. 3) Food and Nutrient Intake of animal foods was very low, ranging from 2.9 to 17%. Consumption of eggs was less than 2% of total food intake, and intake of legumes was also very scanty, between 0.8 to 3.7%. These data present evidence of very poor protein intake, quality as well as quantity. Energy intake of children was 60.0 to 64.4% of the recommended allowance, and mean protein intake only met 47.4% of the recommendation. Low intake of vitamins except thiamin were also found. 4) Mother's Nutrition Knowledge Eighty-five percent of the mothers were entirely ignorant regarding the 'five basic food group' which is most important fact on food and nutrition guidance. Mean knowledge score from 14 basic questions about food and nutrition was as low as 5.1. There was a significant positive correlation between mother's educational level and nutrition knowledge score. 5) Family Planning Variable There were significant correlation among maternal, family planning variables, and some of the nutritional and physical measurements. The study revealed that the mother's educational level and nutrition knowledge score are more crucial factors than the family planning variables on effecting food intakes on children. Recommendation : According to the results of the surveys, there were high incident rates of nutritional anaemia and angular stomatitis among pre-school children, and most of rural women had very limited knowledge about food and nutrition. As a main part of the health education activities, the community health workers should provide nutrition education to the village mothers to improve the nutrional status of young children in rural areas. Nutrional promotion at the primary health care level should be mainly based on appropriate nutrition education.

2019년 충남지역 고등학교에서 발생한 다병원체에 의한 집단식중독의 역학적 분석 (Epidemiological Analysis of a Food Poisoning Outbreak Caused by Multiple Pathogens in a High School in Chungnam Korea, 2019)

  • Objectives: This study was performed in order to report the epidemiological features of a food poisoning outbreak caused by multiple pathogens in a high school in Chungcheongnam-do Province, Korea in April 2019 and to suggest measures to prevent a similar incidence. Methods: A total of 39 patients with diarrhea were examined. Environmental samples were obtained from 6 food handlers, 4 food utensils, 72 preserved foods served during the food poisoning outbreak, 9 door handles, 10 drinking water samples from water dispensers, and 6 ground water samples from water taps. These analyzed to detect viruses and bacteria. Results: Among the 39 patients, 21 cases (53.8%) of enteroaggregative E. coli (EAEC), 7 cases of Staphylococcus aureus (17.9%), and 17 cases of norovirus (43.6%) were positive, and in 16 of the cases a co-infection with at least one other pathogen were observed. EAEC was assumed to be transmitted from contaminated drinking water because it was also detected in the water sample from a water dispenser in the dormitory. Staphylococcus aureus was isolated only in the fecal samples of patients, meaning it was not possible to trace its origin. The genotype of norovirus detected in the drinking water and ground water was consistent with that isolated from patients, and it was determined that the norovirus infection originated from the school's water environment. Conclusions: These findings indicate that a lack of environmental hygiene management related to school meals caused the food poisoning incident. In particular, a lack of management of drinking water, water supply, and personal hygiene should be pointed out. This should be urgently addressed and continuous monitoring should be carried out in the future. In addition, students and staff should be educated and trained to improve their personal hygiene.

안전 환경 인식과 사고 경험에 따른 외식산업 주방 종사원의 안전사고에 관한 연구 (Study on the Accident of Kitchen Food Service Industry Employees in Accordance with the Safety and Environment Awareness and Accident Experience)

  • 본 연구의 목적은 외식산업 주방에서 안전 매뉴얼의 방향을 제시하고자 하였다. 외식산업 주방에서의 사고는 87.2%의 매우 높은 사고경험을 가지고 있다. 사고의 유형별로는 칼로 인한 베임(84.7%)이 가장 많았으며 그 뒤를 이어 뜨거운 불과 물, 기름 등에 의한 화상인 데임(74.4%)과 넘어짐(28.1%) 순으로 나타났다. 성별에 대한 주방안전환경에 대한 인식과 개인의 주방안전 이행 정도에서는 남녀간 유의적 차이를 보임으로써 남성이 여성보다 사고 확률이 높다고 볼 수 있다. 근무연수에서 보여주는 주방안전과 안전이행정도는 1~3년차일 때 가장 높게 나타나지만, 7~10년이 되면 가장 낮은 것으로 보여지는 것은 환경에 익숙해지면서 위험도에 둔감해지는 시기로 볼 수 있어, 년수가 지날수록 위험에 대한 환기를 시켜 안전수칙 이행도를 높일 수 있어야 할 것이다. 순환적 구조에서 예방과 사고처리, 사후처리과정을 보다 전문적인 관점에서 심도 있게 검토하여 외식주방의 안전을 지키는 매뉴얼로 제시하는 바이며, 이를 기본으로 하여 각 외식주방별 환경에 적합하도록 수정 보완하여, 향후 사고 예방과 대처에 필요한 매뉴얼을 개발, 배치할 수 있는 방안을 향후 과제로 제언하는 바이다.

Reflections on the US FDA's Warning on Direct-to-Consumer Genetic Testing

  • In November 2013, the US Food and Drug Administration (FDA) sent a warning letter to 23andMe, Inc. and ordered the company to discontinue marketing of the 23andMe Personal Genome Service (PGS) until it receives FDA marketing authorization for the device. The FDA considers the PGS as an unclassified medical device, which requires premarket approval or de novo classification. Opponents of the FDA's action expressed their concerns, saying that the FDA is overcautious and paternalistic, which violates consumers' rights and might stifle the consumer genomics field itself, and insisted that the agency should not restrict direct-to-consumer (DTC) genomic testing without empirical evidence of harm. Proponents support the agency's action as protection of consumers from potentially invalid and almost useless information. This action was also significant, since it reflected the FDA's attitude towards medical application of next-generation sequencing techniques. In this review, we followed up on the FDA-23andMe incident and evaluated the problems and prospects for DTC genetic testing.

Mate and Tea Intake, Dietary Antioxidants and Risk of Breast Cancer: a Case-Control Study

  • Recently, we reported an inverse association between high 'mate' intake (infusion of Ilex paraguariensis herb, a staple beverage in temperate South America) and breast cancer (BC) risk. Stronger inverse associations were found in high strata of tea, vegetable, fruit and energy intakes, and in overweight/obese women, suggesting possible roles for 'mate' mainly from its antioxidant contribution. The present study attempted to thoroughly explore possible associations among 'mate' and tea intake, dietary antioxidants and BC risk. Combining two databases of previous studies, 572 BC incident cases and 889 controls were interviewed with a specific questionnaire featuring socio-demographic, reproductive and lifestyle variables, and a food frequency questionnaire (64 items), focusing on 'mate' intake (consumer status, daily intake, age at start, age at quit, duration of habit). Food-derived nutrients were calculated from available databases. Odds ratios (OR) and their 95% confidence intervals were calculated through unconditional logistic regression, adjusting for relevant potential confounders. The highest 'mate' intake was significantly inversely associated with BC risk for both low and high carotenoids (OR=0.40 vs. 0.41), vitamin C (OR=0.33 vs. 0.50), vitamin E (OR=0.37 vs. 0.45), flavonols (OR=0.38 vs. 0.48) and reduced glutathione (OR=0.48 vs. 0.46) strata. High tea intake showed significant inverse risk associations only with high carotenoids (OR=0.41), vitamin E (OR=0.48) and reduced glutathione (OR=0.43) strata. In conclusion, a strong and inverse association for 'mate' intake and BC was found, independent of dietary antioxidant levels. Also strong inverse associations with tea intake were more evident only at high levels of certain dietary antioxidants.

단풍마 단백질 추출물의 스트레스로 인한 면역력 저하 개선 효과 (Immunopotentiating Effect of Protein Extract from Dioscorea quinqueloba in Stressed Mice)

  • It is noted that Dioscorea quinqueloba is a medicinal herb that is widely used to treat cardiovascular disease and is assessed as useful to treat other various medical conditions. The immunopotentiating effects of the protein extract (DQP-1) from Dioscorea quinqueloba were thus formally investigated in vivo under incident of cold stress. In this case study, the spleen and thymus weight in mice was shown to have decreased after a measured exposure to cold stress, while the adrenal gland weight in the mice was shown to have increased. The systematic oral administration of DQP-1 significantly recovered the weight loss of the spleen and suppressed the adrenal gland hypertrophy during the association with cold stress. Additionally, the DQP-1 also restored the ascorbic acid level in the adrenal gland reduced after cold stress. The cold stress exposure lowered the percentage of $CD4^+$ and $CD8^+$ cells in the mouse thymus as determined by the flow cytometric analysis, as well as the levels of some serum immunological cytokines(interleukin-2, interleukin-12, and interferon-${\gamma}$) in the studied mice. The resulting identified weakened immunity caused by cold stress was also recovered by a treatment with DQP-1. The DQP-1 significantly suppressed the formation of serum enzymes of alanine aminotransferase, aspartate aminotransferase, and lactate dehydrogenase, which were systematically elevated during the cold stress episode. These results indicate that DQP-1 can improve immunity in mice that are characteristically weakened under stress.

40세 이상 성인 한국인에서 한국형 식사염증지표 수준에 따른 당뇨병 발생률 및 당화혈색소 수준 변화 연구 (Effects of Korean Food-based Dietary Inflammatory Index Potential on the incidence of diabetes and HbA1c level in Korean adults aged 40 years and older)

  • 본 연구 결과 우리나라 40세 이상 성인에서 FBDI와 당뇨병 발생률 간의 상관성은 없었으나 당화혈색소 수준과 상관관계를 가지고, 이러한 경향은 여성이거나 운동을 하지 않는 사람, 그리고 비만인 경우 영향을 더 많이 받는다는 결과를 도출해냈다. 이는 앞으로 우리나라 당뇨병 관리 측면에서 염증을 고려한 다각화된 임상영양치료의 발판이 되리라 사료된다. 또한 추후 장기간의 추적 연구를 통해 식사염증지표와 당뇨병의 발생에 대한 추가적인 연구가 필요한 것으로 보이며, 당뇨병의 심각성에 관한 연구는 거의 없는 실정이기 때문에 추후 연구가 반드시 필요하다고 사료된다. 본 연구를 발판 삼아 추가적인 연구를 기반으로 임상현장에서 식사염증지표가 당뇨병 관리 도구로 활용될 수 있기를 기대하는 바이다.

가축 사료 중 방사성 물질 허용 기준 설정에 관한 연구 (Studies on the Establishment of Tolerance Level of Radioactive Compounds in Livestock Feeds)

  • In order to provide an effective preparedness for a nuclear or radiological emergency happening in the domestic or neighborhood countries and to solve the vague fear of the people for the ingestion of radioactive livestock products, the establishment of national guideline level for radionuclides in feed is urgently necessary. This is because it is important to secure the safety and to manage the crisis in the agricultural, fishery and food sector by performing the effective safety control during and after nuclear incident. This study was performed to investigate the report cases of international organizations and foreign countries to set up a domestic control standard for managing radioactive substances that may be contaminated in animal feeds due to the nuclear power plant incident. In addition, an attempt was made to provide a useful reference that can help prepare a domestic control standard, using a coefficient that can consider the transfer into livestock through the intake of radioactive contaminated animal feeds. The standard radioisotopes investigated were confined to radioactive cesium ($^{137+134}Cs$) and iodine ($^{131}I$). Guideline level for the radionuclides was calculated by using the transfer coefficient factor and the maximum daily intake of animal feed provided by IAEA. For example, the maximum daily intake of animal feed was set as $25kg\;d^{-1}$ for dairy cows, $10kg\;d^{-1}$ for beef cattle, $3.0kg\;d^{-1}$ for pigs and $0.15kg\;d^{-1}$ for chickens. The result values for radioactive cesium were calculated as $8,696Bq\;kg^{-1}$, $4,545Bq\;kg^{-1}$, $1,667Bq\;kg^{-1}$ and $2,469Bq\;kg^{-1}$, respectively. The results for radioactive iodine showed the ranges between $741Bq\;kg^{-1}$ and $76,628Bq\;kg^{-1}$. These data can be utilized as a scientific reference for the preparation of a crisis management manual for the emergency control due to nuclear power plant accident in Korea and neighboring country. These results will contribute to establish the safe feed management system at national level as manual for responding the radioactive exposure of agricultural products and animal feeds, which are currently not established.

레스토랑 유형별 서비스 인카운터 청사진 설계 및 비교 (The Blueprint of Service Encounter by Types of Restaurants)

  • 본 연구에서는 서비스 청사진의 개념과 작성 방법 및 그 활용 의의를 문헌 연구를 통해 고찰하고, 서비스 청사진 기법을 활용하여 고급 레스토랑, 패밀리 레스토랑, 패스트푸드점의 서비스 청사진을 설계하고 각 레스토랑 유형별 차이점을 분석하고자 하였다. 레스토랑 유형에 따른 분석한 결과, 고급 레스토랑과 패밀리 레스토랑은 유사한 서비스 청사진을 나타낸 반면 패스트푸드점은 다른 형태로 나타났는데, 이는 고급 레스토랑과 패밀리 레스토랑은 풀서비스 레스토랑(full-service restaurant)이지만 패스트푸드점은 셀프 서비스 시스템(self-service system)이므로 차이를 나타낸 것으로 조사되었다. 고급 레스토랑과 패밀리 레스토랑에는 전화 예약과 주차 관련 항목이 있고, 그 중 일부 패밀리 레스토랑에서는 테이크아웃을 하고 있는 점이 차이가 있었다. 패스트푸드점은 전화 예약, 주차 등이 서비스 청사진에서 생략되어 상대적으로 간단한 도면으로 나타났으며, 전화 주문 및 배달 서비스가 있는 점이 다른 두 유형의 레스토랑과 다르게 나타났다. 각 레스토랑 유형별로 상호작용선, 가시선, 내부적 상호작용선, 실행선을 중심으로 5개 구간으로 나뉘었다. 상호작용선을 중심으로 고객의 행동과 서비스 인카운터 접점 종업원의 행동으로 나뉘었으며, 가시선을 중심으로 눈에 보이는 부분과 눈에 보이지 않는 부분으로 나뉘었다. 내부적 상호작용선을 중심으로 일선과 후방으로 나뉘었고, 실행선을 중심으로 지원 부문과 관리 부문으로 나뉘었다. 본 연구에서는 질적 연구 방법인 서비스 청사진 기법을 활용하여 중점적으로 관리해야 할 서비스 인카운터를 확인하고자 하였으나, 레스토랑 서비스 인카운터에 청사진 기법을 활용한 초기 연구였기 때문에 가시적으로 레스토랑 유형간 차이를 나타내는 정도의 성과를 나타냈다. 앞으로 서비스 청사진에 대해 연구할 때 각 스텝 간의 시간을 측정한다면, 어떤 지점에서 서비스 지연이 일어나는지 확인할 수 있을 것이라 생각된다. 또한 서비스 청사진과 CIT(Critical Incident Technique) 방법을 병행하면 서비스 실패를 사전에 예방하기 위해 중점 관리해야 할 지점에 대해 더 많은 정보를 얻을 수 있으리라고 사료된다.
