Clinical Study of Duromedics Bileaflet Valve (Duromedics 판막의 임상적 연구)

  • From June, 15th, 1987 to June, 14th, 1989, 105 Duromedics bileaflet cardiac valve prostheses were implanted in 81 patients. Mitral valve replacement was done in 42 patients, 7 underwent aortic valve replacement, 28 underwent double valve replacement, & 4 patients underwent triple valve replacement. Concomitantly used valves were 13 cases; 11 cases were St. Jude Medical valves[M: SJM #29 X4, #27 X5, #25 X 1, T: SJM #33] & two cases were Carpentier Edwards bioprostheses[T: C - E #31X2]. The early mortality rate[within 30 days] was 3.7%[2 patients] & the late mortality rate was 7.4%[3 patients]. Follow-up was done on 72 surviving patients; mean follow-up period was 21.17$\pm$5.36 months. Anticoagulant-related hemorrhage was observed in two patients, possible prosthetic valvular endocarditis was observed in one patient and other specific valve-related complications were none. We concluded, therefore, that good clinical results & a low complication rate could be achieved with Duromedics bileaflet valve in short-term follow-up, & long-term follow-up was also necessary.

Supernovae Follow-up Observations and the Korean Neutrino Telescope

  • Massive stars (${\geq}8M_{\odot}$) are believed to experience core-collapse and finish their lives as supernova (SN) explosions. Astronomers operating the current SN survey facilities try to catch the first moments of SN explosions. Since neutrinos are emitted first from the SNe before the electromagnetic lights, any neutrino detections from more than two sites within around 10 seconds could be useful alert for early follow-up observations, especially for optical SN follow-up telescopes. In this talk, I will brief the current SN follow-up observation projects, what they want to find out and contribute to SN sciences. Focus will be on the early detection and early sciences on SNe, which is what the Korean Neutrino Telescope can contribute most importantly.

Long-term follow-up study and long-term care of childhood cancer survivors

  • The number of long-term survivors is increasing in the western countries due to remarkable improvements in the treatment of childhood cancer. The long-term complications of childhood cancer survivors in these countries were brought to light by the childhood cancer survivor studies. In Korea, the 5-year survival rate of childhood cancer patients is approaching 70%; therefore, it is extremely important to undertake similar long-term follow-up studies and comprehensive long-term care for our population. On the basis of the experiences of childhood cancer survivorship care of the western countries and the current Korean status of childhood cancer survivors, long-term follow-up study and long-term care systems need to be established in Korea in the near future. This system might contribute to the improvement of the quality of life of childhood cancer survivors through effective intervention strategies.

Clinical Results of Mitral Valve Replacement (승모판 치환술의 임상 성적)

  • From August, 1986 to December, 1993, mitral valve replacement was performed in 178 patients. Of the valve implanted, 114 were St.Jude Medical, 47 Duromedics, 16 Carpenter-Edward and 1 Ionesc-Shiley. The hospital mortality rate was 2.8%[5 patients and the late mortality rate was 7.5%[13 patients . The causes of hospital death were LV rupture in 1, renal failure in 1, cardiac tamponade in 1, valve malfunction in 1 and hypoxic brain damage in 1. The causes of late death were sudden death in 6, congestive heart failure in 4, brain ischemic injury in 3. Follow-up was done on 155 surviving patients : mean follow-up period was 50.94$\pm$8.04 months. The actual survival rate was 88.2% at 8 years. We concluded, therefore, that good clinical results could be achieved with mitral valve replacement in mid-term follow-up, and long-term follow-up is also necessary.

Arthroscopic Decompression in Stage II Subacromial Impingement - Five to Twelve Years Follow up - (제 2단계 견봉하 충돌증후군에서 관절경적 감압술(5년에서 12년의 장기추시관찰 결과))

  • We investigated the effectiveness of arthroscopic decompression in stage Ⅱ subacromial impingement after long term follow up. Arthroscopic subacromial decompression was done in 104 consecutive patients who had stage Ⅱ sub­acromial impingement. After average of 8.4 years follow up, the final results were as following; 57 shoulders(55%) in excellent, 25(24%) in good, 16(15%) in fair and 6(6%) in poor. All parameters-pain, function, muscle strength and motion-were improved significantly(p<0.00l). Rotator cuff tear was developed in 10 shoulders after decompression. Among them, 8 shoulders had unfavorable results including two poor. In 6 failures, two had rotator cuff tear, three had recurrence of impingement with degenerative change and reflex sympathetic dystrophy was developed in one. Reoperations were done in 4 shoulders. Improvement of impingement symptoms was maintained in the most of patients(79%) after long term follow up. Arthroscopic decompression surgery was very effective means for stage Ⅱ impingement syndrome.

A Study of Optical Follow-up Pipeline for Gravitational-Wave transients using QUEST data

  • The direct detection of gravitational wave has a very important meaning as a basis for verification of the theory of relativity. Several laser interferometer detectors have attempted to detect GW directly (e.g. LIGO, VIRGO), but positional accuracy of GW detector is too wide (about 10~100sq deg) to find which objects emit GW. One of the main sources of GW is gamma-ray burst which can be detected even in electromagnetic wave. Then to verify Gamma-ray burst object as a GW source, we proceed EM follow-up observation with wide field of view. A first program initiating EM follow-ups to possible transients GW events has been developed and exercised by LIGO and VIRGO community in association with several partners. Using QUEST optical data, we tested the method of cross-convolution recommended by EM follow-up community. We will describe the results of that test.

A Causal Relation of Employment and Depression among Low-Income Young Mothers in New Chance Demonstration Study (저소득층 여성의 취업과 우울 증세간의 인과관계 연구: 미국의 New Chance Demonstration Study 참여자를 대상으로)

  • This study examined a causal relation between employment and depression among low-income young mothers who participated in New Chance Demonstration Study(NC). Using baseline and first follow-up interview data from NC study, this study selected 756 young mothers who were at risk of depression and unemployed at baseline. About 18% of them were employed and 45% of them were no longer at risk of depression at the first follow-up interview. The young mothers grade level and their race/ethnicity were significantly related to the number of weeks employed between the two time points and employment at the 1st follow-up interview. It was also found that employment was negatively associated with depression at the 1st follow-up interview. These findings imply that employment can ameliorate depressive symptoms of low-income young mothers.

Clinical Study of St.Jude Medical Cardiac Valve (St.Jude Medical 판막의 임상성적)

  • St. Jude Medical cardiac valve replacement was performed in 135 consecutive patients from Aug.1986 to Dec. 1991.72 had mitral, 28 had aortic, 1 had tricuspid and 34 had double valve replacement. The hospital mortality rate was 4.4% & the late mortality rate was 3.7 %. Follow-up was done on 115 surviving patients:mean follow-up period was 29.78 $\pm$ 18.32 months. Paravalvular leakage was observed in two patients, possible prosthetic valvular endocarditis wasobserved in one patient and other specific valve-related complications were none. The overall actuarial survival rate at 6 years were 91.6% in total, 96.4% in aortic, 95.5 % in mitral and 81.9 % in double valve replacement.We concluded, therefore that good clinical results and a low complication rate could be achieved with St. Jude Medical valve in short-term follow-up & long-term follow-up was also necessary.

The fate of overfilling in root canal treatments with long-term follow-up: a case series

  • This study describes 6 cases of endodontic overfilling with successful clinical outcomes during long-term (up to 35 years) radiographic follow-up. Successful endodontic treatment depends on proper shaping, disinfection, and obturation of root canals. Filling materials should completely fill the root canal space without exceeding the anatomical apex. Overfilling may occur when the filling material extrudes into the periapical tissues beyond the apex. The present case series describes 6 root canal treatments in which overfilling of root canal sealer and gutta-percha accidentally occurred. Patients' teeth were periodically checked with periapical radiographs in order to evaluate the outcomes during long-term follow-up. All cases showed healing and progressive resorption of the extruded materials in the periapex. The present cases showed that if a 3-dimensional seal was present at the apical level, overfilling did not negatively affect the long-term outcomes of root canal treatment.

Long Term Outcomes of Gamma Knife Radiosurgery for Typical Trigeminal Neuralgia-Minimum 5-Year Follow-Up

  • Objective : Gamma knife radiosurgery (GKRS) is the least invasive surgical option for patients with trigeminal neuralgia (TN). However, the indications and long term outcomes of GKRS are still controversial. Additionally, a series with uniform long-term follow-up data for all patients has been lacking. In the present study, the authors analyzed long-term outcomes in a series of patients with TN who underwent a single GKRS treatment followed by a minimum follow-up of 60 months. Methods : From 1994 to 2009, 40 consecutive patients with typical, intractable TN received GKRS. Among these, 22 patients were followed for >60 months. The mean maximum radiation dose was 77.1 Gy (65.2-83.6 Gy), and the 4 mm collimator was used to target the radiation to the root entry zone. Results : The mean age was 61.5 years (25-84 years). The mean follow-up period was 92.2 months (60-144 months). According to the pain intensity scale in the last follow-up, 6 cases were grades I-II (pain-free with or without medication; 27.3%) and 7 cases were grade IV-V (<50% pain relief with medication or no pain relief; 31.8%). There was 1 case (facial dysesthesia) with post-operative complications (4.54%). Conclusion : The long-term results of GKRS for TN are not as satisfactory as those of microvascular decompression and other conventional modalities, but GKRS is a safe, effective and minimally invasive technique which might be considered a first-line therapy for a limited group of patients for whom a more invasive kind of treatment is unsuitable.
