  • Background: It is very important to obtain vascular access that resists repeated punctures and maintains an adequate blood flow for performing hemodialysis in patients with ESRD. This study was designed to identify the risk factors that may influence the patency rate of arteriovenous fistula (AVF) using perforating vein on antecubital fossa. Material and Method: We analyzed 205 cases of AVF in 195 patients who underwent hemodialysis access surgery on antecubital fossa in our hospital from May 2006 to December 2009. Result: The patency rate of AVF from 6 months after surgery using perforating vein was 75.91%. The risk factors that influence the patency rate was age. There was no statistic difference between used vessels. Conclusion: The patency of the AVF using perforating vein on antecubital fossa was comparable. The condition of sex and location and presence or absence of diabetes and hypertension and other cardio-neurovascular disease did not make statistically significant effect on the AVF patency rate. The age was an independent risk factor for patency rate.

  • An important research challenge about the traditional Internet environment is to enable open networking architecture on which end users are able to innovate the Internet based on the technologies of network programmability, virtualization, and federation. The SDN (Software Defined Network) technology that includes OpenFlow protocol specifications, is suggested as a major driver for the open networking architecture, and is closely coupled with the classical Internet (non-SDN). Therefore, it is very important to keep the integrated SDN and non-SDN network infrastructure reliable from the view point of network operators and engineers. Under this background, this paper proposes an operations and management framework for the combined software defined network environment across not only a single-domain network, but also multi-domain networks. The suggested framework is designed to allow SDN controllers and DvNOC systems to interact with each other to achieve sustainable end-to-end user-oriented SDN and non-SDN integrated network environment. Plus, the proposed scheme is designed to apply enhanced functionalities on DvNOC to support four major network failure scenarios over the combined network infrastructure, mainly derived from SDN controllers, SDN devices, and the connected network paths.

  • This study has obtained Soil Water Retention Curve (SWRC) of the unsaturated soil from the volumetric pressure plate extractor test and the triaxial compression tests was also conducted. By using the rainfall data measured in the site the seepage analysis of unsteady flow was performed with the program of SEEP/W in Geostudio 2007 and stability of the slope was analyzed with SLOPE/W program. Results of analyses show that shear strength of the unsaturated soil increases with the increase of matric suction. And it was also found that the net volumetric stress and the apparent cohesion increased with the matric suction. The seepage analysis of rainfall represents that the increasing rate of negative pore pressure at the zone of large negative pore pressure is appeared to be high even though lower rainfall intensity, but this tendency declines with ground depth. The stability analysis of slope was carried out for the actual plane of failure with the data representing the field condition. The factor of safety thus calculated was about unity (1.0) or just below, which means that the adopted method of analysis is in good agreement with the field condition.

  • In vascular smooth muscle, K+ channels, such as voltage-gated K+ channels (Kv), inward-rectifier K+ channels (Kir), and big-conductance Ca2+-activated K+ channels (BKCa), establish a hyperpolarized membrane potential and counterbalance the depolarizing vasoactive stimuli. Additionally, Kir mediates endothelium-dependent hyperpolarization and the active hyperemia response in various vessels, including the coronary artery. Pulmonary arterial hypertension (PAH) induces right ventricular hypertrophy (RVH), thereby elevating the risk of ischemia and right heart failure. Here, using the whole-cell patch-clamp technique, we compared Kv and Kir current densities (IKv and IKir) in the left (LCSMCs), right (RCSMCs), and septal branches of coronary smooth muscle cells (SCSMCs) from control and monocrotaline (MCT)-induced PAH rats exhibiting RVH. In control rats, (1) IKv was larger in RCSMCs than that in SCSMCs and LCSMCs, (2) IKv inactivation occurred at more negative voltages in SCSMCs than those in RCSMCs and LCSMCs, (3) IKir was smaller in SCSMCs than that in RCSMCs and LCSMCs, and (4) IBKCa did not differ between branches. Moreover, in PAH rats, IKir and IKv decreased in SCSMCs, but not in RCSMCs or LCSMCs, and IBKCa did not change in any of the branches. These results demonstrated that SCSMC-specific decreases in IKv and IKir occur in an MCT-induced PAH model, thereby offering insights into the potential pathophysiological implications of coronary blood flow regulation in right heart disease. Furthermore, the relatively smaller IKir in SCSMCs suggested a less effective vasodilatory response in the septal region to the moderate increase in extracellular K+ concentration under increased activity of the myocardium.

  • In situ rock mass is generally heterogeneous and discontinuous, with varying degrees of strength along the planes of weakness. The planes of weakness such as joints, faults, cracks and bedding planes, control the strength and deformation characteristics of the rock mass. Subsequently, the stability of underground opening depends upon the spatial distribution of discontinuities and their mechanical properties in relation with geometrical shape of openins as well as the mechanical properties of intact rock materials. Understanding the behaviour of a discontinuous rock mass remains a key issue for improving excavation design in hiかy stressed environments. Although recent advances in rock mechanics have provided guidelines for the design of underground opening in isotropic rock mass, prediction and control of deformation in discontinuous rock masses are still unclear. In this study, parametric study was performed to investigate the plastic zone size, stress distribution and deformation behavior around underground opening in a discontinuous rock mass using a continuum joint model. The solutions were obtained by an elasto-plastic finite difference analysis, employing the Mohr-Coulomb failure criteria. Non-associated flow rule and perfectly plastic material behavior are also assumed.

  • At the initial stage of the underground reservoir design one should thoroughly consider surface and subsurface hydrology, hydrogeologic characteristics of aquifer system, and the function of cut - off wall because it is linked to the effective management. In this study, three dimensional finite difference model was applied to analyse the function of Ian underground reservoir at Kyungbuk Province. The steady and unsteady state conditions after construction of the underground dam were simulated through the model, and from these results the groundwater budget and the safe yield were determined. The model simulation indicates the infiltration of irrigation water to be one of the major factors of seasonal fluctuation of groundwater level. The recharge rates of irrigation water were estimated as 4.3mm/d during May and June, and 1.7mm/d during July and Agust. Groundwater recharge from the watershed area estimated to about $0.04m^3/s$, almost consistent through the year. In 1984, groundwater discharge through the transverse section of the dam was $0.002m^3/s$ and the optimum yield for two momths(July and Aguest)was $254000m^3$, however, the discharge became $0.013m^3/s$ in1993, implying the failure of cut -off function. without appropaiate of the cut - off wall, optiumum yield during the irrigaton period would be $93, 000m^3$.

  • Atrial septal defect is one of the most frequently encountered congenital heart disease. Up to December 31, 1976, 1682 cardiac patients received cardiac catheterization in the cardiac department of Yonsei university medical college. Out of the 1682 cardiac patients 723 cases had congenital heart disease and only 116 cases had congetial atrial septal defect. This amounted to 16.04% of all those with congenital heart disease. 58 cases of congenital atrial septal defect operated in the chest surgery department were presented. Of these 58 cases of atrial septal defect, 27 cases were male and 31 cases were female. Their ages ranged from 5 years to 54 years. The systolic pressure of the main pulmonary artery of 40 out of the 58 cases of atrial septal defect was below 40% of that of the systemic blood pressure: in 6 cases, the range of the systolic pressure of the main pulmonary artery was 50-90mmHg; in 12 cases, the range of the systolic pressure of the main pulmonary artery was 40-50mmHg. Average age of these was 30. 1 years. This study tends to show that Korean patients with atrial septal defect even though younger have a slight higher systolic pressure of the main pulmonary artery than Western patients have. The pulmonary blood is 1.5-2.5 times of systemic blood flow in 52 cases out of 58 cases of atrial septal defect. In only one of the 58 cases of atrial septal defect, the Rp was found to be as high as 45% of Rs. All other cases were below this level.51 cases had ostium secundum defect, 4 out of these cases had ostium secundum defect combined with mitral incompetence and 6 out of them had double ostium secundum defect. The remaining 7 cases had ostium primum defect. Their atrial defects were repaired under direct vision utilizing extracorporeal circulation, by hemodilution technic combined with moderate hypothermia. 44 cases [2nd atrial septal defect] were repaired by direct sutures while 14 cases, including the 7 cases ostium primum defects needed patches [1 pericardium and 13 teflon patch]. In 4 cases there were single defects while showed two defects. However the associated septal defect was so small that it could be closed by direct sutures. The size of the defect ranged between 6.0cm2and 10.0cm2 in 19 cases[33.7%]: the smallest being 0. 5cm2 and the largest 24cm2. The surgical mortality was 2 cases [3.4%]. These one case with ostium primum defect, could not be resuscitated on operation table. The cause of death in this case was myocardial failure and MI. The other, a case of ostium primum defect had a second operation on the first operative day due to massive bleeding from LV vent-line insertion site. The patient died on 26th post-operative day due to sepsis.

  • Backgrouds: It is almost universally accepted that occlusive vascular diseases are best managed by anatomical reconstruction. However, the mortality and the morbidity have limited this operation for patients with high operation risks. In these patients, palliative operations such as extra-anatomic bypass and lumbar sympathectomy, are accepted as useful treatment. Material and Method: A retrospective study was conducted in 38 patients who underwent palliative operations for occlusive vascular disease at Korea University Guro Hospital between 1996 and 2000. Mean age of the patients was 60.37 $\pm$ 17.65 years, and preoperative diagnoses were atherosclerosis in 32 patients, Buerger's disease in 4 patients, Raynaud's syndrome in 1 patient and SVC syndrome in 1 patient. Result: Extra-anatomic bypass(40procedures), lumbar sympathectomy(17), thromboembolectomy(7) and femoral artery graft interposition(1) were performed. Six patients were required reoperation due to graft flow failure or fistula. Three year primary patency rate of entire operations was 78.29 $\pm$ 8.81%, and the correlation between type of operation and patency rate was not statistically significant. Conclusion: Palliative operations for occlusive vascular disease are useful treatment in limited patients with high operation risks or limited life expectancy.

  • Dispersed generation (DG) such as wind power (WP) and Photovoltaic systems (PV) that has been promoted at the national level recently is mainly being introduced into distribution systems adjacent to consumers because it is generation on a small scale when compared to current generation. Due to its characteristics, DG can be operated by interconnection with distribution systems to present security of more stable power and efficient use of power facilities and resources. Problems on protection coordination of distribution systems by reverse flow of DG can roughly be divided into three possibilities: excess in rated breaking capacity (12.5KA) of protective devices by a fault in DG current supply, failure to operate protective devices by an apparent effect that can occur by reduction in impedance parallel circuit fault current due to interconnection of DG, and malfunction of protective devices by interconnection transformer connection type. The purpose of this study is to analyze problems in protection coordination that can occur when DG is operated by interconnection with distribution systems by conducting modeling and simulations by using theoretical symmetrical components and MATLAB/SIMULINK to present methods to improve such problems.
