Fistulas between the Esophagus and Adjacent Vital Organs in Esophageal Cancer

  • Esophageal fistulas may occur in an advanced stage or as a potentially life-threatening complication of treatment. They can be divided into esophageal-respiratory and esophageal-aorta fistulas. The diagnosis is confirmed with fluoroscopy using dilute barium oral contrast, followed by thin-section computed tomography, which defines the precise location and extent of the fistula. Flexible esophagoscopy and bronchoscopy are required for confirmation and anatomic assessment of the suspected fistula and provide additional information for treatment planning. Contamination is traditionally controlled by surgical exclusion, along with a jejunal feeding tube. Currently, fully covered self-expanding metal stents are the primary treatment option.

  • Since the introduction of CRT(Cathode Ray Tube) in the 1950s, display technologies have been developed continuously. Flat panel displays such as PDP(Plasma Display Panel) and LCD(Liquid Crystal Display) were commercialized in the late 1990s, and OLED(Organic Light Emitting Diodes) and Micro-LED(Micro-Light Emitting Diodes) are now being developed and are becoming widespread. In the future, we expect to develop ultra-realistic, flexible, embedded sensor displays. Ultra-realistic display can be applied to AR/VR(Augmented Reality/Virtual Reality) devices and spatial light modulators for holography. The sensor-embedded display can be applied to robots; electronic skin; and security devices, including iris recognition sensors, fingerprint recognition sensors, and tactile sensors. AR/VR technology must be developed to meet technical requirements such as viewing angle, resolution, and refresh rate. Holography requires optical modulation technology that can significantly improve resolution, viewing angle, and modulation method to enable wide-view and high-quality hologram stereoscopic images. For electronic skin, stable mass production technology, large-area arrays, and system integration technologies should be developed.

Indications and findings of flexible bronchoscopy in trauma field in Korea: a case series

  • Purpose: Since its implementation, flexible fiberoptic bronchoscopy (FBS) has played an important role in the diagnosis and treatment of tracheobronchial tree and pulmonary disease. Although FBS is often performed by endoscopists, it has also been performed by surgeons, albeit rarely. This study investigated FBS from the surgeon's perspective. Methods: This retrospective study included patients who underwent FBS performed by a single thoracic surgeon between March 2017 and December 2021. Accordingly, the epidemiology, purpose, results, and complications of FBS were analyzed. Results: A total of 47 patients received FBS, whereas 13 patients underwent repeat FBS. Their mean age was 60.7 years. The main organs injured involved the chest (n=22), brain (n=9), abdominal organ (n=7), cervical spine (n=4), extremities (n=4), and face (n=1). The average Injury Severity Score was 22.5. Indications for FBS included atelectasis or haziness on chest x-ray (n=34), pneumonia (n=17), difficult ventilator management (n=7), percutaneous dilatory tracheostomy (n=3), blood aspiration (n=2), foreign body removal (n=2), and intubation due to a difficult airway (n=1). The findings of FBS were mucous plugs (n=36), blood and blood clots (n=16), percutaneous dilatory tracheostomy (n=3), foreign bodies (n=2), granulation tissue at the tracheostomy site (n=2), tracheostomy tube malposition (n=1), bronchus spasm (n=1), difficult airway intubation (n=1), and negative findings (n=5). None of the patients developed complications. Conclusions: FBS is an important modality in the trauma field that allows for the possibility of diagnosis and therapy. With sufficient practice, surgeons may safely perform FBS at the bedside with relative ease.

Optimization of Design Parameters for Lock-Claws of Pneumatic Fitting Using Taguchi Method (다구찌기법을 이용한 공압피팅용 원형 판스프링의 설계변수 최적화)

  • The stress concentration of lock-claws, which are one of the important parts for pneumatic fitting for a flexible tube connection, was investigated by finite element simulation. In this study, the generation of the local plastic deformation was predicted when the tube was hooked up to a pneumatic fitting in order to disperse the stress concentration, and design optimization was carried out using the Taguchi method. For the optimization, the outer width, bending angle, and inner radius of the lock-claws are used as main variables. As a result, their respective contribution ratios are revealed as 81.3%, 10.9%, and 1.5%. The ratio of the total stress distribution was improved by 4% compared with the initial design of the lock-claws.

  • Recently, the space truss has been attracted by many designers because of their ability to support significant loads with a minimum material. And it is relatively flexible to design the configuration of structures. This paper presents a volume optimization for the space truss on the basis of result evaluated from nonlinear analysis. The optimization of the truss is done by nonlinear optimum GINO(General Interactive Nonlinear Optimizer) program. The objective function considered is the volume of the steel bars. The constraints for optimum design are the design limits, such as the axial force strength, maximum slenderness, minimum thickness, allowable deflection and ratio of the external diameter to thickness of the circular tube bars.

Dielectric/Magnetic Nanowires Synthesized by the Electrospinning Method for Use as High Frequency Electromagnetic Wave Absorber

  • High frequency electromagnetic(EM) waves are increasingly being applied in industries because of saturationat lower frequency bands as a result of huge demand. However, electromagneticinterference (EMI) has become a serious problem, and as a result, highfrequency EM absorbers are now being extensively studied. Also, recentdevelopments in absorber technology have focused on producing absorbers thatare thin, flexible, and strong. Hence, one-dimension ferrous nano-materials area potential research field, because of their interesting electronic andmagnetic properties. Commercially, EM wave absorbing products are made ofcomposites, which blend the insulating polymer with magnetic fillers. Inparticular, the shape of the magnetic fillers, such flaky, acicular, or fibrousmagnetic metal particles, rather than spherical, is essential for synthesizingthin and lightweight EM wave absorbers with higher permeability. High aspectratio materials exhibit a higher permeability value and therefore betterabsorption of the EM wave, because of electromagnetic anisotropy. Nanowires areusually fabricated by drawing, template synthesis, phase separation, selfassembly, and electrospinning with a thermal treatment and reduction process.Producing nanowires by the electrospinning method involves a conventionalsol-gel process that is simple, unique, and cost-effective. In thispresentation, Magnetic nanowire and dielectric materials coated magneticnanowire with a high aspect ratio were successfully synthesized by theelectrospinning process with heat treatment and reduction. In addition toestimating the EM wave absorption ability of the synthesized magnetic anddielectric materials coated magnetic nanowire with a network analyzer, weinvestigated the possibility of using these nanowires as high-frequency EM waveabsorbers. Furthermore, a wide variety of topics will be discussed such as thetransparent conducting nanowire and semiconducting nanowire/tube with theelectrospinning process.

Pharyngeal flap damage caused by nasotracheal intubation in a patient who underwent palatoplasty and pharyngoplasty

  • Patients with cleft lip and palate (CLP) must undergo corrective surgeries during infancy and early childhood. Many patients with CLP undergo orthognathic surgery during their childhood for correction of skeletal asymmetries or pharyngoplasty with a pharyngeal flap to improve the quality of speech and velopharyngeal function. During orthognathic surgeries, nasotracheal intubation is performed under general anesthesia. In our case report, the patient had undergone palatoplasty and pharygoplasty previously. During the orthognathic surgery, a flexible fiberoptic bronchoscope-guided nasotracheal tube was inserted through the pharyngeal flap ostium; however, active bleeding occurred in the nasopharynx. Bleeding occurred because the flap was torn. After achieving hemostasis, the surgery was completed successfully. Thus, if a patient may show the potential for velopharyngeal port obstruction, nasotracheal intubation should be performed with utmost care.

Process Development of Constant Curvature Extrusion for Aluminum Alloy (알루미늄 합금의 일정 곡률 압출공정 개발)

  • This paper shows some achievements at bending of extruded aluminum profiles during the extrusion process. The conventional process for the production of bent profiles involves a successive extrusion, stretching, and bending of the profiles. Conventional bending methods can not meet demands far precision and cost-effective production in some cases, due to cross sectional deformation, irregular decrease of tube wall thickness and a complication of the process design. An estimation of spring-back required for precision of the bending radius can not always be achieved by the over bending of the profile. Since the profile is hot during the bending process, the spring-back phenomenon can be avoided. This means that an additional bending process is not necessary. Consequently, flexible bending can be achieved with cost reduction and quality improvement. Experimental tests were completed to study the relationship between curvature radius of profile and position of guide on the extrusion for vehicle bumper. A7108 is applied as a billet material in order to increase strength. The overall correlation between the experimental and numerical results is good. It is therefore concluded that the present method provides an efficient means for the constant curvature extrusion process.

The Solution Structure of 18 residue YH motif Peptide within the Second fas-1 domain of ${\beta}ig-h3$

  • [ ${\beta}ig-h3$ ] is an extracellular matrix protein that mediates cell adhesion through interaction with integrins. The 18 residue YH motifs within each fas-1 domain are known to be responsible for the interaction with the ${\alpha}_v{\beta}_5$ integrin, and the synthetic YH motif peptides are known to inhibit endothelial tube formation and reduces the number of blood vessels, and so expected to be an effective inhibitor of angiogenesis. In this study, we solved the 3D structure of the 18 residue YH motif peptide (EALRDLLNNHILKSAMCA; D2 peptide) within the second fas-1 domain of ${\beta}ig-h3$ using NMR. The Peptide has ${\alpha}-helix$ structure at the C terminal region but the N terminal region is flexible. The present structural information may be helpful for developing more effective peptide drug candidate for the treatment of diseases dependent on angiogenesis.

Static Modeling of a Miniaturized Continuum Robot for Surgical Interventions and Displacement Analysis under Lateral External Loads (중재 시술 적용을 위한 소형 연속체 로봇의 정역학 모델링 및 외부 측면 하중에 의한 변위 분석)

  • In this paper, we deal with the static modeling of a continuum robot that can perform surgical interventions. The proposed continuum robot is made of stainless steel wires and a multi lumen flexible tube using a thermoplastic elastomer. This continuum robot could be most severely deformed in physical contact with narrow external environments, when a lateral external force acts at the distal tip of the continuum robot. In order to predict the shape and displacement under the lateral external force loading, the forward kinematics, the statics modeling, the force-moment equilibrium equation, and the virtual work-energy method of the continuum robot are described. The deflection displacements were calculated using the virtual work-energy method, and the results were compared with the displacement obtained by the conventional cantilever beam theories. In conclusion, the proposed static modeling and the virtual work-energy method can be used in arrhythmia procedure simulations.
