Surgical Repair of the Traumatic Urethral Transection with Urethrocutaneous Fistula in a Pung-san Dog (풍산 개에서 발생한 외상성 요도 절단과 요도 피부 누공의 수술적 교정)

  • A five-year-old, castrated male, Pung-san dog was referred to Konkuk University Veterinary Teaching Hospital with trauma and persistent hemorrhage in the inguinal region. The dog had a history of being wounded by a wild boar at 5 days prior to presentation. Rupture of the membranous urethra and urethrocutaneous fistula were demonstrated by the retrograde positive contrast urethrography. A urinary catheter was placed to identify the urethra and urethrocutaneous fistula. The necrotic tissues and damaged tissues by urine leakage around urethrocutaneous fistula were debrided. An urethral anastomosis over an indwelling catheter was performed. The dog maintained normal urination without other complications including dysuria and hematouria at the follow-up evaluation after 1 month postoperatively. A retrograde urethrogram repeated at 2 months after surgery showed no urethral stricture.

Oculocardiac Reflex Caused by Retrobulbar Block in a Dog with Scoliosis (척추측만증이 있는 개에서 안구후방마취 후 발생된 안구심장반사 1례)

  • Enucleation of a 9-year-old, spayed female Pekingese's right eye was scheduled because of recurrent eyeball rupture caused by chronic corneal ulcer and descemetocele. Scoliosis of the thoracic vertebra was observed on thoracic radiography. Complete blood count, electrolytes, serum chemistry profiles were within normal ranges except of alkaline phosphatase, which was markedly improved, compared with that of five months ago. Severe respiratory sinus arrhythmia was observed before induction of anesthesia, it disappeared after induction. Retrobulbar block was performed with 0.5% bupivacaine, 2% lidocaine, 0.1% epinephrine combination (4 : 1 : 0.2 ratio of volume) before start of surgery. After retrobulbar block, heart rate decreased from 110 to 76 beats/min and sinus arrhythmia recurred. It was considered as oculocardiac reflex caused by increase of intraorbital pressure from retrobulbar block, atropine (0.025 mg/kg, IV) was administered and intermittent positive pressure ventilation was started. Three minutes after atropine administration, abnormal waveform of the electrocardiograph was appeared, it suspected as ventricular or supraventricular tachycardia, so lidocaine (2 mg/kg, IV) was administered. Then, heart rate was maintained around 130 beats/min, and the surgery was finished without the other problems. Although, retrobulbar block is performed to provide analgesia and to prevent oculocardiac reflex in ophthalmic surgery, occasionally it could induce oculocardiac reflex by infiltrated volume.

A Case Study on the Effect of the Art Therapy through the Self Expression and the Interpersonal Relations by a Schizophrenic Adolescent I (정신분열증 청년의 자기표현과 상호작용의 효과에 관한 미술치료 사례연구 I)

  • Objectives : This article aims to find how effective the art therapy is on the improvement of the self expression and the interpersonal relations with others, as medium for verbal communication of schizophrenic patients. Methods : The subject of this study was one schizophrenic patient who had been treated with drugs for five years in W university hospital, as well as counseled by an art therapy oo counsel office in J city. The art therapy had been performed the total 25 sessions, fifty minute per session, one session per week, from November 28 in 2008 to April 27 in 2009. The test was by HTP. Results : The results of this study are as follows: First, in the pretest of HTP, the subject showed a little lethargy, painting very simple house-shaped figures and too simple rectangular figures corresponding to those painted by 4 or 5-year-old children. In the post-test, however, the structures of houses were described concretely and three-dimensionally, and the descriptions of people were changed into concrete figure, by which we could see the improvement effect on the self expression and the interpersonal relations. Second, in the earlier stage, there was little verbal self expression, and little self respect, staring into space and showing passive attitude without positive assertion and expression. In the later stage, however, there were positive assertion and expression, even showing sense of humor. Lastly, in the earlier stage, there were few interpersonal relations. In the later stage, however, eye contacts and the interpersonal relations were increased during conversation and in the art work process, respectively. Conclusions : These results suggest that the art therapy can be effective on improvement of the self expression and the interpersonal relations with others, as medium for verbal communication of schizophrenic patients.

  • Thymic carcinoid tumor associated with Cushing's syndrome is a rare disease with a poor prognosis. Thymic carcinoid with Cushing's syndrome caused by CRH (corticotropin-releasing hormone) production is even rarer. We report a 58-year-old woman with a huge anterior mediastinal mass. Five months after thymectomy the patient was readmitted with symptoms of generalized edema and dyspnea. Recurrence and metastases were discovered and Cushing's syndrome diagnosed.

Case of Deep Vein Thrombosis Patient Treated by Korean Medicine (한의학적 치료로 호전된 심부 정맥혈전증 치험 1례)

  • This is a case report on a deep vein thrombosis(DVT) patient whose pain, numbness, and swelling in the left leg have been decreased with Korean Medicine. The 59 year-old male patient who was diagnosed with DVT and showed the classic symptoms of DVT(pain, numbness and swelling in the left leg). Although he was treated with medication(100mg of aspirin, 20mg of rivaroxaban) for five weeks, his symptoms did not improved. Then, he was gaven Sunkihwalhyul-tang three times a day and Jungsongouhyul pharmacopuncture once a day for two months. Conservative therapy including leg elevation and an elastic stocking was co-administered. During our treatment period, the size of the thrombus in the left external iliac vein was decreased according to sonography compared with the pre-hospital's sonography, and the patient felt his symptoms had improved. This case report suggests Korean Medicine may play a role in improving the deep vein thrombosis symptoms.

The effects that massage has on a hypertensive's blood pressure (마사지 요법이 고혈압 환자의 혈압에 미치는 영향)

  • To closely examine the effects of the massage treatment as one method of curing hypertension by exploring the effects that one-time circulation-centered massage has on a hypertensive's blood pressure, this study selected as the subject ten among fifty-year-old essential hypertensives visiting N clinic, and measured and compared their blood pressures before and after I gave them massage. After stabilizing them, I measured their blood pressures. In their prone and supine postures, I gave them rubbing massage for twenty minutes from positions proximal to their hearts to ones distal from their hearts to help their artery circulation and circulation-centered stroking massage for ten minutes from positions distal from their hearts to ones proximal to their hearts to help their vein circulation. After the massage, I had them rest for five minutes and measured their blood pressures. Regarding each item analyzed, I calculated the mean and standard deviation before and after the massage. The difference before and after the massage was tested by corresponding t-test and the level of significance was made p<0.05. The findings are as follows. First, systolic pressure was $157.70{\pm}7.48$ before the massage and $143.00{\pm}9.39$, which is less than before the massage, after the massage, showing statistically significant difference (p<.05). Secondly, diastolic pressure was $97.80{\pm}6.56$before the massage and $95.20{\pm}5.18$, which is less than before the massage, after the massage, not showing statistically significant difference (p<.05).

The Effects of a Green Based Growth Education Program on Young Children's Environmental Preservation Knowledge, Attitude and Behavior (녹색성장교육에 기초한 프로그램이 유아의 환경보전지식, 태도 및 행동에 미치는 효과)

  • This study started as an effort to solve the serious environmental program that the Korean government and internationally focused on. For the purpose of the study, a green based growth education program was composed that young children could participate in. With this program, the study participants could form attitudes, behavior, and knowledge of environmental preservation. To see the effectiveness of the green growth educational program, the study set up a control and an experimental group. The program was composed with four major content 'ecosystem', 'energy', 'resource', and 'green citizenship'. The study planned 20 sessions including fairy tales, storytelling, art, game, and science activities. The study's participants were a total of 42 five-year-old children. The result of the research showed that the experimental group's environmental preservation knowledge, attitude, and behavior score was significantly higher than the control group. The green growth educational program had a positive effect on young children's knowledge, attitude, and behavior of environmental preservation. This study included the realities of the difficulties applying green growth education in kindergarten, the result provided practical information regarding the green growth educational program to kindergarten teachers.

A Study on the Relationship between Father's Verbal Control Modes, Children's Self-efficacy, Social Competence, and Interactive Peer Play (아버지 언어통제유형과 유아의 자아효능감, 사회적 유능성 및 상호작용적 또래놀이의 관계)

  • The purpose of this study was to examine the relationship of father's verbal control modes, children's self efficacy, social competence and interactive peer play. The subjects were pairs of 195 three, four and five year-old-children and their fathers. Data were analyzed with mean, standard deviation, and bivariate correlation. Results indicated that father's verbal control modes were significantly correlated with children's self efficacy, social competence and interactive peer play. Father's person-oriented control verbal mode was positively related to children's self-recognition, self-emotion, social competence, leadership and interactive peer play while imperative control verbal mode was negatively related to children's instability, interrupted play behavior, and disturbed play behavior.

A Case Report of Cervical Myelopathy Caused by Ossification of Posterior Longitudinal Ligament: Focusing on Warm Needling Therapy (후종인대 골화증으로 유발된 경추 척수병증 1례에 대한 증례보고: 온침요법을 중심으로)

  • Objectives : The purpose of this study is to investigate the effects of warm needling therapy in a patient with cervical myelopathy caused by ossification of posterior longitudinal ligament. Methods : A 53-year-old Korean male patient diagnosed with cervical myelopathy due to ossification of posterior longitudinal ligament was treated with warm needling therapy on posterior neck aside from the routine inpatient treatment from June 26th to July 16th. Clinical improvement was evaluated with visual analogue scale(VAS), Japanese orthopedic score(JOA), grip and release test, finger escape sign and patient satisfaction was measured using a five-point Likert scale. Results : After 21 days of treatment, the patient showed improvement in VAS, JOA, grip and release test and finger escape sign. The patient also expressed satisfaction with the treatment. Conclusions : The results suggest that warm needling therapy may be an effective additional treatment for cervical myelopathy and other symptoms related to ossification of posterior longitudinal ligament(OPLL). Further studies should be carried out regarding possible long-lasting effects.

Multiple nonmelanocytic skin cancers in multiple regions

  • The most common forms of nonmelanocytic skin cancer (NMSC) are basal cell carcinoma (BCC) and squamous cell carcinoma (SCC). The growing incidence of skin cancer in the Republic of Korea has sparked increasing scientific interest in these types of tumors. In the case described herein, multiple NMSCs occurred asynchronously in various areas of the body in a single patient. A 67-year-old man presented with an ulcerative lesion on the right retro-auricle and multiple keratinized masses on the back and face. The right retro-auricular lesion was diagnosed as BCC, and the keratinized masses on the patient's back and face were diagnosed as SCC. He subsequently presented with numerous pigmented skin lesions on the forehead, temple, pre-auricle, neck, right forearm, right hand, and both thighs. One lesion on the neck was diagnosed as BCC, and five lesions on the right hand and forearm were diagnosed as SCC. The patient was also diagnosed with supraglottic SCC and external auditory canal SCC. An otolaryngologist performed radical excision of the primary SCC. Suspected skin cancer lesions observed on the face and both ears were diagnosed as SCC. Patients with multiple NMSCs are at an elevated risk for additional skin cancers, making periodic follow-up important; furthermore, all suspicious lesions should be biopsied.
