자가 산부식 접착제는 산부식 과정 후 수세없이 와동을 충전할 수 있다. 움직임이 많고 행동 조절이 어려운 환자의 경우 시술의 절차를 줄일 수 있는 자가 산부식 접착제가 치면열구전색제의 사용을 보다 용이하게 할 수 있다. 그러나 법랑질에 대한 산부식 능력이 인산에 비해 낮아서 충분한 부식이 일어나는지에 대해 논란이 되고 있다. 이에 대해 자가 산부식 접착제의 약한 산부식능을 감안하여 열구성형술과 acidic primer의 도포 시간을 증가시키는 등의 임상 술식을 시행하고 자가 산부식 접착제의 단점이 보완되는지를 알아보고자 하였다. 35% 인산으로 산부식 후 전색술을 시행한 경우와 자가 산부식 접착제를 사용하여 도포한 경우의 미세 누출도를 염색액의 침투도를 비교하고 주사전자현미경을 이용한 산부식상 관찰을 통하여 다음과 같은 결론을 얻었다. 1. 열구 성형술을 시행 후 자가산부식접착제를 도포한 군은 인산 부식 20초를 시행한 군과 자가산부식접착제를 20초, 40초 도포한 군보다 미세누출도가 더 낮았다. 2. 자가산부식접착제를 제조 회사의 지시에 따라 20초간 도포한 군과 40초로 늘려 도포한 군 간에 미세누출도의 유의차가 없었다. 3. 주사전자현미경 관찰 시 전형적인 산부식 양상은 인산부식 20초군에서만 관찰되었으며 열구성형술을 시행하고 자가산부식접착제를 도포한 군이 자가산부식접착제만 20, 40초 도포한 군보다는 표면 거칠기가 증가하였다.
치면열구전색제의 물성은 개선되어 왔고 결합강도, 침투도, 미세누출 등에 관련된 연구가 활발히 이루어지고 있으나 국내에서의 치면열구전색제 사용실태와 치과의사의 인식에 관한 연구는 거의 없는 실정이다. 그래서 본 연구에서는 설문조사를 통하여 이를 알아보고자 하였다. 소아치과 전공 개원의와 비소아치과 개원의를 대상으로 직접 방문을 통해 조사를 시행한 결과 다음과 같은 결론을 얻었다. 1. 치면열구전색의 치아우식 예방효과에 대해 소아치과의에서는 영구치와 유치에 모두 효과가 있다는 응답이 96.7%로 높게 나타났고, 반면 비소아치과의에서는 모두에 효과가 없다고 생각하는 응답이 13.5%로 나타났다. 2. 치면열구전색 시 방습법으로 소아치과의의 100%, 비소아치과의의 27%가 러버댐을 사용하는 것으로 나타났고, bonding agent를 사용하는 비율은 소아치과의 83.3%, 비소아치과의 40.5%로 나타났다. 3. 법랑질 성형술의 시행비율은 비소아치과의에서 약간 더 높게 나타났고, 시행방법에서는 두 군 간에 큰 차이가 없었다. 4. 치면열구전색 실패의 원인으로는 부적절한 방습으로 인한 오염이 가장 높게 나타났고 다음으로 우식 상방에 도포, 낮은 강도, 낮은 흐름성, 과충전 순으로 나타났다. 5. 소아치과의의 경우 예방적 레진수복을 바람직하게 생각하는 비율이 90%로 높았고, 치면열구전색보다 예방적 레진수복을 하는 비율이 더 높은 것으로 나타났다.
본 연구의 목적은 유동성 복합레진을 치면열구전색제로 사용할 수 있는지 여부를 평가하는 것이다. 세 종류의 유동성 복합레진(Filtek Flow, Tetric Flow, Charmfil Flow)과 한 종류의 filled sealant(Ultraseal XT plus)가 사용되었다 주사전자 현미경을 이용하여 세 종류의 유동성 복합레진 및 filled sealant의 resin tag형성 양상을 비교하였다. 미세누출 평가를 위해 발거된 사람의 소구치 54개를 각각 18개씩 무작위로 세 군으로 배분하였다. 각 군마다 통상의 filled sealant와 한 가지씩의 유동성 복합레진을 교합면 열구에 도포하였다. 치아들을 thermocycling($5^{\circ}{\pm}2^{\circ}C$와 $55^{\circ}{\pm}2^{\circ}C$ 사이에서 30초씩 1200회 시행) 후 48시간 동안 1% methylene blue 용액에 보관하였다. 각각의 치아를 절단하여 미세누출 정도를 관찰하였다. 세 종류의 유동성 복합레진 및 filled sealant는 유사한 resin tag 형성양상을 보였다 세 종류의 유동성 복합레진은 각각 filled sealant보다 현저히 더 많은 미세누출을 보였다. 세 종류의 유동성 복합레진 간의 미세누출 정도는 유사하였다. 유동성 복합레진은 통상의 filled sealant와 비교해볼 때 보다 많은 미세누출을 야기할 수 있으므로 치면열구전색제로 적절하지 않다.
The purpose of this study was to evaluate the sealing ability of five obturation methods in conjunction with sealer. Fifty extracted upper and lower anterior teeth were selected and the access cavities were prepared lingually with a round bur. The working length was determined with a #15K file, and the root canals were instrumented with a #40K file 1mm short of the apical foramen. The apical third of root canal was flared by step-back technique and the coronal two thirds of root canal using #2 - 3 Gates Glidden drills. And then, the teeth were randomly assigned to five groups of 10 teeth each. A thin coat of Tubliseal was placed into the canal into the canal using the # 35 reamer and the canals were filled by lateral condensation, vertical condensation, ultrasonic activated lateral condensation, ULTRAFIL injecting, and McSpadden compaction methods. All teeth were stored in 100 % relative humidity at 31c for 48 hours. The roots were suspended in 2 % methylene blue solution for 48 hours, left to dry for 24 hours and then, half side of root was removed longitudinally using the fissure bur. A Boley gauge wes used to measure the distance, to the nearest 0.1mm, from the apical foramen to the most coronal level of dye penetration. The measurement of dye penetration was statistically compared by Duncan's multiple range test The results were as follows 1. Ultrasonic group showed the best sealing ability among the all experimental groups, but there were statistically no significant difference in the sealing ability between ultrasonic group and vertical condensation group. 2. There were no statistically significant difference in the sealing ability among the lateral, vertical, and ultrasonic groups. 3. McSpadden group showed the worst sealing ability among the all exprimental groups.
The purpose of this study was to evaluate the microleakage of class II composite resin inlays and compare them with the conventional light-cured resin filling restorations. Class II cavities were prepared in 60 extracted human molars with which cervical margins were located below 1.0mm at the cemento-enamel junction using No. 701 tapered fissure carbide bur. All of the prepared cavities were restored as follows and divided into 6 groups. Group I and 2 were restored using direct filling technique and group 3,4,5 and 6 were restored using direct inlay technique that was cemented with dual-cured resin cements. group I: Cavities were restored with light-curing composite resin, Brilliant Lux. group 2. Cavities were restored with light-curing composite resin, Clearfil PhotoPosterior. group 3: Cavities were restored with Clearfil CR Inlay and heat treated at $125^{\circ}C$ for 7 minutes. group 4: Cavities were restored with same material as group 3 and heat treated at $100^{\circ}C$ for 15 minutes. group 5: Cavities were restored with Brilliant (Indirect esthetic system) and heat treated at $125^{\circ}C$ for 7 minutes. group 6: Cavities were restored with same material as group 5 and heat treated at $100^{\circ}C$ for 15 minutes. All specimens were polished with same method and thermocycled between $6^{\circ}C$ and $60^{\circ}C$, then immersed in a bath of 2.0% aqueous solution of basic fuchsin dye for 24 hours. Dyed specimens were sectioned longitudinally and dye penetration degree was read on a scale of 0 to 4 by Tani and Buonocore's method 45). The results were as follows: 1. Microleakage was observed rather at the cervical margins than at the occlusal margins in all groups. 2. Composite resin inlay groups showed significantly less leakage than direct filling groups at the cervical margins (p < 0.001). 3. In composite resin inlay groups, there was no significant difference in microleakage between specimens by heat treating temperature and time (p > 0.05). 4. There was no significant difference in leakage between each groups at the occlusal margins (p > 0.05).
