만성간질환 위험요인에 대한 코호트연구: 잠재적 발병자 집단을 감안한 분석전략 (A Cohort Study on Risk Factors for Chronic Liver Disease: Analytic Strategies Excluding Potentially Incident Subjects)

  • 연구자들은 만성간질환의 코호트연구에서 잠재적 발병자 집단이 포함되어 있을 때 분석 결과의 비뚤림을 평가하고 적절한 분석전략을 제안하기 위하여 본 연구를 수행하였다. 1993년 현재 서울코호트로 구축된 14,529명을 대상으로 4년 6개월 동안 만성간질환 발생을 파악하였다. 의료보험공단의 수진내역 및 해당 의무기록 조사, 암등록사업자료 및 통계청 사망원인통계자료와의 연계를 통하여 총 102명에서 간질환이 발생하였다. 대상 코호트를 간질환의 잠재적 발병자 집단은 기초조사 자료에서 B형간염 표면할원 양성인 경우, 혈청 간효소인 GPT(ALT)가 40 이상으로 증가된 경우, 또는 설문조사 결과 간질환을 앓았거나 현재 앓고 있는 대상자로 정의하였으며 총 2,217명이었다. Cox의 회귀분석모형으로 전체 코호트 대상자, 잠재적 발병자 집단 및 기타 대상자 7,305명에 대하여 각각 위험요인을 파악하였으며, 각 분석결과를 비교하였다. 전체 대상자 및 잠재적 발병자 집단에 대한 분석결과는 대체적으로 일치하며, 표면항원 양성, 높은 간효소치, 급성간염 과거력, 최근 금연, 금주 등이 유의한 위험요인이었으며, 식이 측면에서는 돼지고기, 커피 등이 보호요인으로 관찰되었다. 한편 잠재군에서는 비만도가 높을 경우 위험도가 감소하는 경향이 있었다. 잠재적 발병자 집단을 제외한 코호트 대상자 분석에서는 상이한 양상이 관찰되었는데 체질량지표의 경우 비만도가 높은 집단에서 발생 위험도가 4배 정도로 증가하였다(p<0.10). 그러나 금주나 금연 등은 무관하였으며, 생간(生肝) 섭취, 가공생선 및 된장찌개의 섭취 빈도가 위험도를 증가시켰다. 한편 표면항원 양성, 높은 간효소치, 급성간염 과거력 등은 다른 대상자의 분석결과가 유사하였다. 이상의 결과에서 잠재적 발병자 집단을 포함할 경우, 질병의 결과로 변화하는 생활습관이 해당 질병의 위험요인으로 파악될 가능성이 있었으며, 특히 금연, 금주 및 비만도 등이었다. 즉 연구자들은 만성적 경과를 밟아 진행되며 임상적으로 확인되는 질병에 대한 코호트 연구에서 잠재적 발병자 집단을 감안하지 않을 경우 발생하는 비뚤림의 가능성을 실증적으로 평가하였다. 4년 6개월간의 추적조사기간 동안 102명의 만성간질환 발병 예를 확인하였지만 잠재적 발병자 집단을 제외한 분석이 타당하며, 이들에서는 19명의 환자만이 발생하였다. 만성간질환의 위험요인에 대한 유용하고도 정밀한 연구결과를 도출하기 위해서는 향후 추적조사기간을 연장하여 충분한 발병 예를 확보할 필요가 있다.

Taurine supplementation in diet for olive flounder at low water temperature

  • The objective of this study was to examine the effect of dietary supplementation of taurine for juvenile olive flounder (Paralichthys olivaceus) at low water temperature ($16.4{\pm}0.36^{\circ}C$). Fish meal (FM)-based diet was used as the control diet. Four other experimental diets were prepared by adding taurine to FM-based diet at 0.25, 0.50, 1.00, and 1. 50% (T1, T2, T3, and T4, respectively). Each experimental diet was fed to triplicate groups of fish (initial mean body weight, 19.5 g) for 10 weeks. At the end of the feeding trial, growth performance and feed utilization, hematological parameters, non-specific immune responses, whole-body proximate composition, and liver mRNA expression of insulin-like growth factor-1 (IGF-1) were investigated. Feed conversion ratio was significantly reduced while protein efficiency ratio was significantly increased in taurine-supplemented groups. Hematocrit and hemoglobin were also significantly increased while plasma cholesterol levels were decreased in taurine-supplemented groups than those in the control group. Nitroblue-tetrazolium, myeloperoxidase and lysozyme activities, and plasma immunoglobulin level were significantly increased by taurine supplementation. These results suggest that dietary taurine supplementation is effective in improving growth performances, feed utilization, and innate immunity of olive flounder in low water temperature season.

Morphological and Molecular Characteristics of Anisakis typica Larvae in Two Species of Threadfin Bream, Nemipterus hexodon and N. japonicus, from the Gulf of Thailand

  • The third stage larvae (L3) of Anisakis typica were detected in 2 species of threadfin bream, Nemipterus hexodon and N. japonicus, from the Gulf of Thailand, and were morphologically and molecularly characterized. Total 100 threadfin breams, 50 Nemipterus hexodon and 50 N. japonicus, were examined with naked eyes after the opening of abdominal cavity with scissors. Almost all infected larvae remained alive and active even the fish were transported for 1-2 days. Anisakid larvae were exclusively distributed in the body cavity and rarely in the liver. The prevalence of A. typica L3 were 68.0% and 60.0% in N. hexodon and N. japonicus and their infection intensities were 3.5 and 4.2 per fish infected each. Morphological and morphometric analysis were performed by viewing specimens under both a light microscope and a scanning electron microscope. Interestingly, the protruded mucron of Anisakis typica under SEM showed a distinct cylindrical shape that differed from the cone shape of A. simplex. The protruded mucron could be used to identify A. typica L3 larvae in the future. A comparison of the ITS1-5.8S-ITS2 rDNA nucleotide sequences of these species revealed high blast scores with A. typica. Conclusively, it was confirmed that A. typica L3 are prevalent in threadfin breams from the Gulf of Thailand, and their morphological and molecular characters are something different from those of other anisakid larvae, including A. simplex and A. pegreffii.

Prevalence of the Intestinal Flukes Haplorchis taichui and H. yokogawai in a Mountainous Area of Phongsaly Province, Lao PDR

  • Phongsaly Province, located in the northernmost area of Lao PDR, was previously suggested to be endemic for the liver fluke Opisthorchis viverrini infection. To confirm, or rule out, this suggestion, the Phonxay village in the Khoua District, Phongsaly Province, was selected for a survey. Ten volunteers (8 men and 2 women aged 31-57 years) who consumed raw freshwater fish and had gastrointestinal troubles were treated with a single dose of praziquantel (40 mg/kg) and pyrantel pamoate (10 mglkg) and purged with magnesium sulfate to recover any worm parasites. Eight of the 10 volunteers expelled 1 or more species of trematodes, nematodes, or cestodes (worm positive rate; 80%). The worms were morphologically identified as H. taichui (861 worms from 8 people), H. yokogawai (59 from 6 people), Phaneropsolus bonnei (1 from 1 person), Trichostrongylus sp. (2 from 2 people), Ascaris lumbricoides (2 from 1 person), Enterobius vermicularis (11 from 3 people), and Taenia saginata (1 strobila with scolex from 1 person). The results indicate that the mountainous area of Phongsaly Province, Lao PDR, is not endemic for the liver fluke but endemic for intestinal flukes, in particular, Haplorchis taichuiand H. yokogawai.

  • Astaxanthin을 산란계에 경구 투여하여 얻은 AEY에는 총 2.88 mg%의 carotenoid가 함유되어 있었다. Carotenoid 성분을 HPLC로 분석한 결과, AEY에는 존재하지 않는 $\alpha$- cryptoxanthin, canthaxanthin, cynthiaxanthin, triol 등이 AEY 에서 확인되었다. CEY에서는 존재하지 않기 때문에 이들은 astaxanthin의 대사생성물로 예상할 수 있다 AEY는 LPS로 유발한 mouse의 catabolic response overcome의 효과 시험에서 control구에 비해 유의성 있는 체중 감소 억제 효과를 보였다. 또한 AEY는 LPS 처리에 의한 면역기관에 미치는 영 향에서는 췌장과 간의 체중에 대한 상대적 비율을 증가시켰다. 따라서 AEY 처리가 면역 작용을 하는 세포의 활성에 관여하는 것으로 생각되고 이것은 AEY 처리구에서 관찰되는 $\alpha$-cryptoxanthin, canthaxanthin, cynthiaxanthin, triol 등에 의한 것으로 추측된다.

쥐에서 임신기, 수유기 및 이유후에 식이로 섭취한 어유가 뇌조직의 DHA 분포량에 미치는 영향 (Influence of Dietary Supplementation of Fish Oil at Different Life Cycle on the Incorporation of DHA into Brain in Rats)

  • The incorporation of docosahexaenoic acid(DHA) and arachidonic acid(AA) into brain and liver lipid has been compared in male pups from binth to 10 weeks old by feeding DHA-rich experimental diets or chow diets to dams from pregnancy in rats. The experimental DHA-rich diets contained 7g fish oil and 3g corn oil per 100g diet. There were three experimental groups, FO-I : Dams were fed DHA-rich diet during pregnancy and lactation, and their it pups fed the same diet until 10 weeks old. FO-II Dams fed chow diet during pregnancy and DHA-diet during lactation, and their pups fed the same DHA-diet until 10 weeks. FO-III : Dams fed chow diet during gestation and lactation, and then the pups fed DHA-diet after weaning. The relative % of DHA in hepatic lipid was about 12% with chow diets, but increased rapidly to 20-25% level when DHA-rich diets were supplied after weaning. The AA(%) of FO-III group was relatively high when a chow diet containing higher amount of linoleic acid was given, but there was no significant difference between the groups after feeding on a DHA-rich diet. When the DHA-rich diet was supplied from pregnancy(FO-I), the relative % of DHA in brain lipid was 13.7% at birth and continuously increased to a maximum level(17.2%) at 3-weeks and then was sustained until 5 weeks old. Similar levels of DHA incorporation were observed when DHA-rich diet was supplied from lactation(FO-II). However, the pups of FO-III group showed significantly lower levels of DHA incorporation(72%) at birth. These livels slowly increased and reached an 87% level of FO-I at 10 weeks when the pups ate DHA-rich diets after weaning. The relative % of AA in brain lipid was 10.4% in the FO-I group at birth, which was significantly lower than those of other groups, but there was no significant difference between groups after feeding DHA-rich diets in all groups. The Ah(%) level increased to maximum(11-12%) at 3-weeks and then was slightly reduced and was sustained at about 10% after S-weeks. Total amounts of DNA in the whole brain rapidly reached maximum level at 3-weeks and then was sustained at a constant level after S-weeks. DNA content was not significantly different between groups at birth, but it was significantly higher in FO-I and FO-II groups than in FO-III group at 3-weeks. However, DNA content in FO-III group was continuously increased to 80% level of FO-I at 10-weeks after feeding DHA-rich diet since weaning. In conclusion, the DHA(%) in whole brain was most effectively deposited when DHA-rich diet had been supplied during pregnancy and lactation in rats. However, DHA supplementation after weaning also improved the incorporaton of DHA into brain and content of DNA even though brain development was almost completed, which suggests that DHA supplementation might be necessary to improve brain development in humans during infancy as well as pregnancy and lactation. People Participation Towards Opisthorchis viverrini Prevention and Control in Chaiyaphum Province, Northeastern Thailand

  • This cross-sectional descriptive and qualitative study was aimed to study the people participation and their approaches toward the human carcinogenic liver fluke, Opisthorchis viverrini, prevention and control in Ban Chaun sub-district administrative organization (BCSAO) and Bamnet Narong sub-district municipality (BNSM), Bamnet Narong district, Chaiyaphum Province, Thailand between June 2013 and February 2014. Participants were purposive selected, included chiefs of sub-district administrative organizations, sub-district municipalities, sub-district health promotion hospitals (SHPHs), heads of village, and a further sample was selected with a multi-stage random sampling for public health volunteers, and villagers. The pre-designed questionnaire contained items for individualized status and the participatory steps of sharing ideas, decision-making, and planning, procession, evaluation, and mutual benefit, for the project O. viverrini prevention and control (POPC). In-depth interviews were used for collection of need approaches to POPC. With 375 participants who completed the questionnaire, it was found that people had a high level regarding to participate in the POPC, particularly in the process stage ($\bar{X}=3.78$, S.D. = 0.56), but the lowest level was found in sharing ideas, decision making, and planning step ($\bar{X}=3.65$, S.D. = 0.63). By comparison, participant status and organization did not significantly differ with people participation. In each step, Ban Chaun sub-district had a high level of participation in the step of sharing ideas, decision making, and planning toward POPC, more than Bamnet Narong sub-district municipality (t=2.20, p=0.028). Approaches for POPC in Ban Chaun sub-district and Bamnet Narong sub-district municipality included requirements for budget support, annual campaigns for liver fluke prevention and control, campaign promotion, risk group observation, home visiting, community rules regarding reducing raw fish consumption in their communities, and a professional public health officer for working in their communities, BCSAO, BNSM, and SHPH, for O. viverrini prevention and control. This study indicates that people realize that eradication of the liver fluke needs a continuous people participation for O. viverrini prevention and control in their communities.

  • 본 연구는 2002년 6월부터 2003년 11월까지 복수증을 나타내는 양식넙치의 세균상과 virus를 동정한 것이다. 생화학적 성상을 기초로 한 표현형으로, 병어체의 복수, 간, 신장, 내장으로부터 Vibrio, Edwardsiella, 및 stonella 등 3개 속이 동정되었다. Vibrio속은 5종으로 우점이었고 V. harveyi (32균주), V. parahaemolyticus (23균주), V. alginolyticus (10균주), V. carcariae (2균주), V. metschnikovii (5균주)였다. 그밖에 Photobacterium damselae (6균주)와 장내세균인 Edwardsiella tarda (15균주)가 동정되었다. 감염어의 내부 장기로부터 해산어 병원성 바이러스로 알려진 marine birnavirus (MABV)가 분리되었다.

  • 양식 넙치, Paralichthys olivaceus를 실험어로 하여 동물 성장인자중의 하나인 insulin-like growth factor-I (IGF-I)의 함량을 검토하였다. 넙치의 혈액중에 존재하는 IGF-I의 양은 47ng/m1이고, 절식을 할 경우 식이를 섭취한 것에 비해 약 $15\%$ 감소하였으며, 운동에 의해서는 약 $13\%$의 감소를 보였다. 혈액중 IGF-I의 농도는 어체의 크기에 따라서도 영향이 있음을 알수 있었다.

Characterization and expression profiles of aquaporins (AQPs) 1a and 3a in mud loach Misgurnus mizolepis after experimental challenges

  • Two distinct cDNAs encoding aquaporins (mmAQPs 1a and 3a) were isolated and characterized from mud loach Misgurnus mizolepis. The identified mud loach AQP cDNAs encode for polypeptides of 260 and 302 amino acids. Topology predictions confirmed six putative membrane-spanning domains connected by five loops and the N- and C-terminal domains being cytoplasmic. The mud loach AQPs 1a and 3a showed broad distribution in multiple tissues including immune-responsive tissues as well as osmoregulatory tissues. Hence, the diversity of AQP distribution and expression possibly indicated its differential functions in the regulation of fluid movement in response to environmental stimuli. The transcription of mmAQP genes was differentially modulated by immune challenges. In particular, the mmAQP3a expression level in the liver was more responsive to immune challenges than that of mmAQP1a. Taken together, fish stimulation or infection resulted in significant modulation of mud loach AQP genes, suggesting potential functional roles of these proteins in piscine pathophysiological process.
