EMS Helicopter Operation and Flight Safety in Korea (한국에서의 응급의료헬기 운영과 운항안전)

  • EMSS is the organic system offering quick and efficient transfer and treatment for those who need an emergency service. For EMS aviation operation is the important transportation for the common benefits. There are several types of aviation operation. Among the different types of the operations, helicopter operation is very common because it is free from any locations. Notwithstanding the benefit, various types of EMS helicopter accidents and incidents are occurred because of its poor operation condition. In South Korea, even though so much effort was put to decrease the accidents rates, there are still many accidents happening by fire helicopters and the forest service helicopters. Therefore, in order to regularize the EMSH operation, measures for the aviation safety have to be prepared. This research is, related to the EMSH, focused on the suggestion of safety measure for the EMSH in Korea based on the accidents analysis in various countries.

A Study on Prevention Measure Establishment through Cause Analysis of Chemical-Accidents (화학사고 원인분석을 통한 예방대책 수립에 관한 연구)

  • Even if several chemical accident prevention systems such as PSM(Process Safety Management), RMP(Risk Management Plan), etc. have been carried out, many chemical accidents have still occurred at industrial plants in Korea. We describe the status of chemical industry and the trend of occurrence of chemical accidents in Korea. And this paper analyzes the recent chemical accidents in eight ways. These ways include chemical accident forms, ignition sources, sources of chemical equipment, human vs equipment/material causes, worker's working situation, employee scale, hazardous substances, week & time, fatalities of manufacture & contractor's workers. Finally we proposes the four representative prevention measures brought to result of cause analysis by accident statistics.

  • Almost all companies have paid much attention to the safety management ranging from maintenance to operation even at the stage of designing in order to prevent accidents, but fatal accidents continue to increase throughout the world. In particular, it is essential to systematically prevent such fatal accidents sa fire, explosion or leakage of toxic gas at factories in order to not only protect economic losses and environmental pollution. In addition, HRA may be used to detect the human errors which may cause accidents or trace back to any mistake on the part of workers. This work focuses on the coincidence of human error and mechanical failure for management of human error, and on some important performance shaping factors to propose a method for improving safety effectively of the process industries.

In particular, it is essential to systematically prevent such fatal accidents as fire, explosion or leakage of toxic gas at factories in order to not only protect the workers and neighbors but also prevent economic losses and environmental pollution. In addition, HRA may be used to detect the human errors which may cause accidents or trace back to any mistake on the part of workers. Usually, HRA technique is used in association with other risk assessment techniques. Moreover, it can serve to enumerate the human errors which may occur during operation or down-time or correct the existing system to reduce the mistakes. This work focuses on the coincidence of human error and mechanical failure for management of human error, and on some important performance shaping factors to propose a method for improving safety effectively of the process industries.

A Study on Asphyxiation Accidents occurred in the confined space, and their Prevention (밀폐공간 질식재해 발생현황 고찰 및 예방에 관한 연구)

  • A confined space means places where the risk of damage to health caused by lack of oxygen or hazardous gases in inadequate ventilation conditions and the risk of fire and explosion caused by flammable substances. Asphyxiation accidents in a confined space occured every year and also occurred more than two people at the same time. In this study, we surveyed the domestic statistical data occurred the lack of oxygen in confined space for the last 10 years(2006-2015) and, analyzed the accident by industries sector, workplace size etc. 17 fatal work accidents that occurred in confined spaces in Korea between 2013 and 2015 were investigated and analyzed using the database of the KOSHA and suggested interventions to minimize asphyxiation accidents in confined spaces. This paper is expected to be used to establish interventions planning and training as a preventive measures in workplace having confined spaces.

Evaluation of Human Factors in Ship Accidents in the Domestic Sea (국내 해양선박사고의 인적 오류의 요인 평가)

  • In this study, we investigated and identified criterial human factors(errors), most of which lead to terrible ship accidents such as collisions, sinking, fire and explosions resulting both in human lives and physical damages to ships as well as surrounding environments. To this end, we went through the accident reports of 413 cases over 2005~2009 period and classified the human factors into 6 major factors with 19 sub ones which were constructed in hierarchical order. The relative importance of major factors was calculated and among others the lack of awareness turned out to be the most important factor with the weight of 0.391. The contributions of the results in the research are two fold: it will help (i) identify the root causes of ship accidents and prevent further potential ship related incidents, (ii) analyze the degree of the risk associated with the ship accidents, when risk analysis is performed.

  • This study is a preparation for the application of FSA (Formal Safety Assessment) to the fire safety of ships. FSA is the new-fashioned methodology proposed to prevent ships from the accidents. To make a base of the fire safety assessment about ship's fire protection design and Classification Society rule, statistical informations for the fire safety engineering are investigated. From results, the necessity of new criterion for ship's fire safety design, the need to study the human behavior in the evacuation from fire, and the development of new fire progress model considering special situations in ships are acknowledged.

  • Aircraft fires due to accident at an airport may be under a special situation. An outstanding characteristic of aircraft fires is their tendency to reach lethal intensity within a very short time. In domestic aviation act, there are regulations related to services coping with the aircraft accidents within an airport. To cope aircraft fires, it is necessary to define clearly the regulations and standards about the services and related activities of rescue and fire fighting, and to keep the regulations and standards. The study has been performed on the basis of International Civil Aviation Organization (ICAO) Airport Services Manual of rescue and fire fighting. The objective of study is to suggest improvement methods for standards applicable to actual service practices of rescue and fire fighting by analyzing through the comparison of domestic and international regulations and standards.

  • In this paper, The infrastructure case for reproduction or analysis of the mechanism on the fire or the electrics shock of the electrical appliances is proposed. The various electrical appliances(Washing machine, Ballast, Electric heaters, Electronic switches, Mobile phone chargers) used in the home can be tested on the high voltage and over current test through the implementation of the infrastructure. The electrical tests of fire and electric shock consists of the high voltage of maximum 5000 V and over current test of 3 steps(90 A, 60 A, 40 A) and noise test. The mechanism of the fire and the electric shock tests reproduced are analyzed through the monitoring system and the oscilloscope. The electronics manufacturers can prevent accidents through the tests of the electronics factor reproduced and the analysis of the infrastructure designed.

  • This paper estimates electrical fire damage cost-effect to analyze electrical safety efficiency by applying a cost-benefit analysis method for analyzing the efficiency of a business public interest. Electrical fire loss statistics data was presented as direct costs and casualties, including deaths due to an electrical fire, this paper adds overhead "Incidental Cost of Accidents to the Employer" by W. H. Heinrich was applied to estimate the cost of economic loss. Also wounded, including the loss of human deaths by referring to the car accident insurance claims costs and human development estimated the losses caused by an electrical fire. And to perform a cost-benefit analysis of the electrical safety as a result of future work. In this paper, the economic effect of the electric field of safety and public interest work to systematically presented.
