COMPARATIVE STUDY ON THE BOND STRENGTH OF CEMENTS BETWEEN PFM COPING AND VARIOUS CORES (도재전장관용 Coping과 수종 Core간의 시멘트 결합력에 관한 비교 연구)

  • An in vitro study was conducted to compare the bond strength of cements between Verabond coping and various cores. Fifty-four idential cores simulating maxillary central incisor prepared for PFM crowns were made. Eighteen samples were made with 20K cast gold, eighteen with Verabond, and eighteen with Adaptic. Samples were randomly divided into three groups, each consisting of six 20K cast gold, six verabond, and six Adaptic samples. The first group was cemented with zinc phosphate cement, the second group with poly-carboxylate cement, and the third group with glass ionomer cement. Constant finger pressure was applied for cementation. The sample were then stored at $37^{\circ}C$ in distilled water bath for 24 hours. The tensile strength test was performed on an Instron Universal test machine with crosshead speed of 0.05cm/min and the results compared statistically. Results of the study showed that: 1. A significant difference of bond strength was observed with different types of dental cements and core materials. 2. With gold core, zinc phosphate cement was stronger than both the polycarboxylate cement and glass ionomer cement, which did not differ in bond strength. 3. With base-metal core, zinc phosphate cement showed the highest bond strength and was followed by polycarboxylate cement and glass ionomer cement. 4. With composite resin core, zinc phosphate cement showed the highest bond strength and was followed by glass ionomer cement and polycarboxylate cement. 5. The base-metal core (Verabond core) privided the highest retention of all core materials.

A STUDY ON THE TENSILE BOND STRENGTH OF ETCHED BASE METALS (식각된 비 귀금속 합금과 법랑질간의 접착 인장강도에 대한 연구)

  • The purpose f this study was to evaluate the effect of resin film thickness on the tensile bond strength and to compare the tensile bond strengths of 4 differently treated metal surfaces. For the experiment, seventy metal specimens were cast with Verabond and divided into I, II, III, groups. The metal specimens in group I were electrolytically etched and cemented with Panavia under finger pressure. Cement film thickness was regulated with metal spacers. The metal specimens in Group II were treated by 4 methods, such as electrolytic etching method, salt-roughened method, EZ-oxisor method , chemical etching method and cemented with Panavia. In group III, electrolytically etched metal specimens were cemented with Hy-Bond. The etched surface of metal specimens and the cement film thickness were examined under the scanning electron microscope. Results were as follows; 1. The tensile bond strength showed no significant difference between $30{\mu}m,\;80{\mu}m,\;130{\mu}m$ film thicknessspecimens. 2. There was no significant difference in the tensile bond strength between the 4 differently treated metal specimens. 3. The tensile bond strength showed significant difference between Panavia and Hy-Bond. 4. Scanning electron microscope photograph revealed that tile interdendritic eutectic was removed in electrolytically etched metal surfaces hilt even dendritic arm was removed in Chemically etched metal surfaces. 5. The metal surfaces which were air-abraded with $50{\mu}m$ aluminum oxide showed roughness and small crack on scanning electron microscope photograph.

Respiratory Effort Monitoring Using Pulse Transit Time in Human (인체에서 맥파전달시간을 이용한 호흡노력 모니터링)

  • In this study. respiratory efforts were monitored by the change of pulse transit time (PTT) which is related with the arterial pressure PTT is the time interval between the peak of R wave in ECG and the maximal slope point of photoplethysmogram(PPG). Biosignals, ECG and finger photoplethysmogram(PPG), were converted to digital data, and PTT was evaluated in personal computer with every heart beat. Results were presented as a graph using spline interpolation. The software was implemented in C$\^$++/ as a window-based application program. PTT was periodically changed according to airflow in resting respiration. In the resting respiration, PTT was changed according to the respiratory cycle. The amplitude of PTT fluctuation was increased by deep respiration, and increased by partial airway obstruction. These results suggest that PTT is responsible to respiratory effort which could be evaluated by the pattern of PTT change. And it is expected that PTT could be applied in the monitoring of respiratory effort by noninvasive methods, and is very useful method for the evaluation of respiratory distress.

Clinical Evaluation of Thoracoscopic Sympathectomy in Hyperhidrosis (흉강경하 흉부 교감신경간 절제술을 시행한 본태성 다한증 환자의 임상적 고찰)

  • Background: Essential hyperhidrosis is a condition with excessive sweating, which may be localized in any parts of the body. Thoracic sympathectomy has been a surgical procedure for the management of hyperhidrosis. Methods: We studied 30 ASA I and II patients suffering from severe hyperhidrosis. Bilateral upper thoracoscopic sympathectomy of $T_{2-4}$ was performed in 30 patients under general anesthesia. Anesthesia was induced with 2.5% thiopental sodium 5 mg/kg and succinylcholine chloride 1 mg/kg and was maintained with enflurane 1~2 Vol% and $N_2O-O_2$ mixture adjusted to maintain $SpO_2$ greater than 96%. During anesthesia, invasive arterial pressure, heart rate, EKG, $SpO_2$ and capnography were monitored. Skin temperature was measured with thermister probes attached to the index finger of each hand. An increase in temperature after cautery confirmed success of the sympathectomy. Results: There were 14 men and 16 women whose ages ranged from 16 to 46 years old (mean age 22.2). Of these patients, 13 patients had complained of palm-sole hyperhidrosis, 9 of palm-sole-axilla hyperhidrosis, 4 of palm-sole-face hyperhidrosis and 4 of palm-sole-axilla-face hyperhidrosis. The provocative factors of excessive sweating were tension and stress from interpersonal relationships. There was positive familial history in 37%. The most common complication was compensatory hyperhidrosis in 23 patients comprising 76%. Other complication included peumothorax (4 patients), hemothorax (1 patient), ipsilateral Horner's syndrome (1 patient) and paresthesia of right arm (1 patient). The degree of satisfaction was graded as good, fair and poor with 15, 12 and 3 patients, respectively. Conclusions: Thoracoscopic sympathectomy with VATS is an efficient, safe and minimally invasive surgical procedure for essential hyperhidrosis.

A Study on Community Health Practitioners' Knowledge of, Usee Patterns of, and Attitudes toward Complementary and Alternative Medicine(CAM) (보건진료원의 보완대체요법에 대한 지식정도, 사용경험, 태도에 관한 연구)

  • Purpose: This study was conducted to investigate community health practitioners' knowledge of, use patterns of, and attitudes toward CAM. Method: The subjects of this study were 817 community health practitioners (CHPs) from 9 Provinces in Korea selected through convenient sampling. They completed and submitted self-report questionnaires at the annual meeting of CHPs or sent completed questionnaires to us by mail. Result: Of the CHPs, 8.1% had a certificate of CAM-related education program, and 39.9% worried about the efficacy of CAM. They knew about massage (74.0%), finger pressure (68.1%), hand acupuncture (67.8%), diet (65.5%) and herb (64.7%). They had used herb (43.3%), massage (36.8%), hand acupuncture (32.7%) and vacuum (31.2%) with positive effects and recommended residents to use them. Attitude toward CAM tended to be positive in those with certificates of CAM, those with high academic qualification, those aged 40 or below, and those working in Chungcheong-do. Conclusion: This study showed that CHPs were interested in CAM while they worried about the therapeutic effects of it. Further studies to draw cross-sectional and national data on the use patterns of CAM from the various population and standardized instruments to investigate the types of CAM were suggested.

Sinking and fit of abutment of locking taper implant system

  • STATEMENT OF PROBLEM. Unlike screw-retention type, fixture-abutment retention in Locking taper connection depends on frictional force so it has possibility of abutment to sink. PURPOSE. In this study, Bicon$^{(R)}$ Implant System, one of the conical internal connection implant system, was used with applying loading force to the abutments connected to the fixture. Then the amount of sinking was measured. MATERIAL AND METHODS. 10 Bicon$^{(R)}$ implant fixtures were used. First, the abutment was connected to the fixture with finger force. Then it was tapped with a mallet for 3 times and loads of 20 kg corresponding to masticatory force using loading application instrument were applied successively. The abutment state, slightly connected to the fixture without pressure was considered as a reference length, and every new abutment length was measured after each load's step was added. The amount of abutment sinking (mm) was gained by subtracting the length of abutment-fixture under each loading condition from reference length. RESULTS. It was evident, that the amount of abutment sinking in Bicon$^{(R)}$ Implant System increased as loads were added. When loads of 20 kg were applied more than 5 - 7 times, sinking stopped at $0.45{\pm}0.09\;mm$. CONCLUSION. Even though locking taper connection type implant shows good adaption to occlusal force, it has potential for abutment sinking as loads are given. When locking taper connection type implant is used, satisfactory loads are recommended for precise abutment location.

A Review of Recent Studies on Acupuncture and Moxibustion Treatment Associated with Pregnancy (임신과 관련된 침구치료의 최근 연구 고찰)

  • Purpose : Most pregnant women avoid laking medicine and acupuncture treatment because of its affect on the unborn baby. However, for a healthy pregnancy, appropriate treatments are needed. Until now, there hasn't been much research about acupuncture therapy on pregnant women in Korea. so we decided to review the kinds of research that have been done in the recent 10 years. Methods : We researched for papers associated with pregnancy and acupuncture. Type of research was limited to randomized controlled trials in the recent 10 years. Results : 58 papers were found. Researches associated with pregnancy and acupuncture could be divided into symptoms of pregnancy, childbirth, and infertility. General acupuncture treatment, acupressure band, and finger pressure therapy was used for morning sickness. General acupuncture treatment was most commonly used for pregnancy-related back pain. Acupuncture and moxibustion treatment were used for breech presentation. Conclusion : Further research needed for more acupuncture and moxibustion treatment in pregnant women.

  • The retract actuator makes the landing gear retract or extend during take-off and landing of an aircraft. To prevent folding of landing gear that has remained in the extended state because of an unexpected external disturbance, an internal locking device is applied to the retract actuator. The locking device is restrained with another internal component by oil pressure supplied to the retract actuator, and this restraint makes the locking of the actuator possible. Because locking and unlocking are repeated during retraction and extension of the landing gear, the locking device takes repeated identical loads, and the possibility of fatigue failure exists. In this study, the process and results of fatigue analysis for the locking device are presented, and the appropriateness of the analysis result is verified using a fatigue test.

  • Background : Qigong is an exercise therapy based on the principles of Traditional East Asian Medicine. The exercises combine the practice of motion and breathing, both guided by mental imagery. Beneficial effects of qigong have been reported on a variety of complaints in chronically ill patients and on gait imbalance in the elderly. Parkinson's disease is a progressive neurodegenerative disorder that affects neurophysiological function, movement abilities, and quality of life. Objectives : We developed a qigong program based on the Traditional East Asian medical theory regarding the improvement of clinical symptoms of Parkinson's disease. Methods and Results : Our qigong program consists of three parts. The first one includes movements which stimulate the meridians that run from the toes to the top of the head. The second one is composed of breathing and qigong movements that create harmony and balance with the circulation of Qi and blood on the Meridians. The third is a stage of finger pressure therapy and massage from Daoyin medical qigong to maintain meridian stability. Conclusions : These qigong program would help relieve the clinical symptoms of Parkinson's disease patients.

Effect of suboccipital muscle inhibition and combination technique on the flexibility of hamstring in individuals with shortened hamstring (뒤통수밑근 억제기법과 조합기법이 넙다리뒤근 단축 대상자의 넙다리뒤근 유연성에 미치는 영향)

  • Purpose: The purpose of this study is to compare changes in the flexibility of hamstring muscles which are relaxed on suboccipital muscle through suboccipital muscle inhibition and combination technique. Methods: Thirty sample subjects (16 male and 14 female) were randomly divided into an experimental group (n=15) and a control group (n=15). 1. Suboccipital muscle inhibition: Suboccipital muscles of the patients were placed on a bed. The occipital region was placed on a therapist's fingers, and the posture was maintained for three minutes before remeasurement. 2. Treatment with combination technique (body bolster and wooden pillow): Wooden pillows were placed where the participants could relax the posterior arch of the atlas; additional body bolsters were also placed to avoid high pressure, and the treatment was maintained for three minutes before remeasurement. Results: There was a statistical difference in finger floor distance (FFD) and straight leg raise (SLR) results for those treated with SMI (P<0.05). There was no statistical difference in FFD and SLR results for those treated with the combination technique (P>0.05). Conclusion: Between the two intervention methods; the suboccipital muscle inhibition technique was more effective in increasing the flexibility of subjects with a shortened hamstring than was the combination technique.
