폐에 발생한 염증성 가성종양의 세침흡인 세포학적 소견 - 선암종으로 오진한 1예 보고 - (Fine Needle Aspiration Cytology of Inflammatory Pseudotumor of the Lung - Report of A Case Misdiagnosed as Adenocarcinoma -)

  • Cytologic features of inflammatory pseudotumor of the lung have not been described frequently. We report fine needle aspiration cytologic(FNAC) finding of a case of inflammatory pseudotumor misdiagnosed as adenocarcinoma in a 63-year-old man. The FNAC displayed a mixture of histiocytes, myofibroblasts, pneumocytes, and plasma cells. Some histiocytes and myofibroblasts had large nuclei with irregular nuclear membrane and prominent nucleoli, which mislead the diagnosis on adenocarcinoma on FNAC. The heterogeneous cell population is the unique cytologic features of inflammatory pseudotumor, which are helpful to distinguish it from other circumscribed benign and malignant lesions. Familiarity with these features is essential to avoid misdiagnosis and possible overtreatment.

  • Fine needle aspiration cytology (FNAC) is a very useful tool in the preoperative diagnosis of lesions of the salivary gland. Surgical therapy of high-grade malignancies (salivary duct carcinoma, mucoepidermoid carcinoma, squamous cellcarcinoma, carcinoma ex pleomorphic adenoma, small cell carcinoma, and sebaceous carcinoma) is different from that of benign lesions or low-grade malignancies. Therefore, the recognition of high-grade malignancies is important in salivary gland FNAC. Although recognition of high-grade malignancies of the salivary gland by FNAC is not difficult, precise classification of these malignancies is often impossible. Additionally, because of its rarity, FNAC of spindle cells and mesenchymal lesions of the salivary glands is a tool that is not familiar to many cytopathologists. The characteristic cytomorphologic features of these lesions are reviewed here with a discussion of specific diagnostic problems.

  • Fine needle aspiration cytology (FNAC) is an easy convenient non-invasive method in the diagnosis of superficial palpable masses. The cytologic findings by FNAC of reactive and neoplastic lesions in various organs including breast, lymph node, thyroid, salivary gland, etc., have been described, but, those of soft tissue lesions including proliferative fasciitis are relatively rare to find. We recently experienced a case of FNAC of proliferative fasciitis in the left back of a 72-year-old male. The FNAC smears were scant in cellularity and contained large cells with abundant basophilic cytoplasm, one to two nuclei lying at the periphery, and prominent nucleoli that resemble ganglion cells.

  • Actinomycosis involving the female genital tract has increased since the advent of the intra-uterine contraceptive devices (IUD) : the incidence of actinomyces in IUD users with pelvic inflammatory disease (PID) was reported to be between 17% and 30%. However a definte relationship between actinomyces and PID has not been demonstrated as yet. We present a case of pelvic actinomycotic infection that was initially diagnosed by means of fine needle aspiration. A 57-year-old female presented with urinary frequency and difficulty for 100 days. Computerized tomographic findings showed a mixed heterogenous soft tissue mass in the left superior aspect of urinary bladder. Fine needle aspiration was done under the impression of urinary bladder cancer. Microscopically, smears disclosed many sulfur granules in necrotic background with many neutrophils, histiocytes, multinucleated giant cells and granulation tissue. These findings were confirmed by surgical removal of the mass.

  • Fine needle aspiration cytologic findings in 14 cases of gynecomastia are described. General cytomorphologic features resemble those of fibrocystic disease in women than those of fibroadenoma. Among the cytologic parameters, three-dimensional structure of epithelial cell clusters, presence of micronucleoli and irregularities of nuclear size and shape are suggestive of epithelial proliferative activity. In audition, 4 cases are proliferative breast disease without atypia and 10 cases are nonproliferative breast disease depending on cytologic criteria grading system.

  • Alveolar soft part sarcoma (ASPS) is a rare soft tissue sarcoma, which occurs predominantly in adolescents and young adults. The cytological characteristics of this condition have been described only rarely in the literature. Here, we report a case of alveolar soft part sarcoma. A 28-year-old man presented with a mass in his right buttock, which had persisted for three years. The mass was subjected to a fine needle aspiration cytology (FNAC). The smears were cellular. The observed tumor cells were round or polygonal, and exhibited vesicular nuclei with prominent nucleoli and finely granular cytoplasm. Naked nuclei were frequently detected. Tumor cells were arranged singularly, but occasionally in a pseudoalveolar pattern.

  • Utilization of fine needle aspiration cytology (FNAC) in the diagnosis of odontogenic tumors seems to have little attention. Ameloblastoma is the most common epithelial odontogenic tumor. However, the cytologic findings of ameloblastoma are described recently. We report a case of ameloblastoma diagnosed by FNAC. The aspirated blood-tinged mucoid fluid from maxillary mass showed a few clusters of tumor cells with distinct two-cell population: basaloid cells with palisading arrangement and squamous cells with more abundant and eosinophilic cytoplasm. Nuclear atypism, pleomorphism or mitosis were absent. A few keratinized bodies in the tumor cell clusters and the third component, stellate cells were also characteristic.

  • Lymphoepithelial carcinoma, also known as malignant lymphoepithelial lesion or lymphoepithelioma, is a rare tumor in salivary glands. Lymphoepithelial carcinoma has a characteristic histological findings comprising irregularly-shaped nests of malignant epithelial cells within a lymphocyte-rich stroma, occasionally forming lymphoid follicles. We recently experienced a case of fine needle aspiration cytology (FNAC) of lymphoepithelial carcinoma of parotid gland in a 61-year-old male. The FNAC yielded a hypercellular smear of many irregular clusters of malignant epithelial cells in the background of lymphoid stroma.

  • Tubulolobular carcinoma is a tumor of the breast that shares histoiogic features of both tubular and lobular carcinoma of the breast. The cytologic features of this neoplasm have not been reported in Korea. We described a case of tubulolobular carcinoma diagnosed by fine needle aspiration cytology in a 46-year-old female. The tumor contained several cytologic features of both tubular and lobuar carcinoma, including tubular structures, single filing of cells, intracytoplasmic vacuoles, and low nuclear grade.

Infection of Thyroid Cyst Occurring 1 Month after Fine-Needle Aspiration in an Immunocompetent Patient

  • Fine-needle aspiration (FNA) with ultrasonography is considered a minimally invasive and safe procedure. Complications of it are infrequent and occur immediately or within a few days after FNA. Such complications may occur mainly in patients with underlying problems. We here report a rare case of thyroid cystic nodule infection occurring 1 month after FNA in an immunocompetent patient and serial sonographic findings in this patient. A 33-year-old woman with a cystic nodule including partially isoechoic solid areas on the right thyroid gland complained of difficulty swallowing and painful sensations in the right neck 1 month after FNA. On follow-up examination, the cystic nodule and perithyroidal soft tissue were suspicious of infection. The possibility of infection after FNA should be considered even if the patient is immunocompetent in order to prompt evaluation and immediate management with empirical antibiotic therapy to avoid life-threatening complications.
