Purpose : GnRH analogues(GnRHa) are used to treat central precocious puberty(CPP). However, in some patients, the GV decrease is so remarkable that it impairs predicted adult height(PAH); and there fore, the addition of growth hormone(GH) is suggested. We analysed the growth changes during two years and final adult height(FAH) in girls with idiopathic CPP treated with combined therapy, compared with those of girls treated with GnRHa alone. Methods : For the analysis, we classified the patients, who was treated for longer than two years, into three groups depending on the initial PAH and combination of GH; PAH_L, treated with GnRHa and PAH less than midparental height(MPH) - 5 cm. PAH_H, treated with GnRHa and PAH greater than MPH - 5 cm. GnRHa+GH, combined GH treatment, regardless of PAH before treatment. We analysed the GV and PAH change during the first two years and FAH. Results : In PAH_L, the PAH(SDS) at first year of therapy was significantly increased to $153.5{\pm}6.5cm(-1.4{\pm}1.3)$ from $149.7{\pm}6.4cm(-2.1{\pm}1.3)$ before treatment(P=0.004). In PAH_H, there was no significant increase in PAH during the two years of treatment. During the first year of combination of GH and GnRHa, GV and PAH increased significantly. We observed significant increases in FAH, comparing to the initial PAH in the PAH_L and GnRHa+GH groups. The height gains(FAH - initial PAH) were significantly higher in the PAH_L and GnRHa+GH groups than that in the PAH_H group. Conclusion : This study suggests the FAH and height gains are improved in patients, whose predicted adult height before treatment was shorter than those with higher predicted adult height, with the treatment of GnRHa alone or in combination with GH. GH could not improve the final adult height, but compensated the growth in patients whose growth velocity was decelerated by GnRHa alone.

This paper presents a new design algorithm for piece-removing dynamical system, based on 6-Sigma DMADOV technique using ARIZ and Brainstorming. Our design target is the piece-removing system installed on a mobile platform of bead-grinding equipment. The 6-Sigma DMADOV technique guides us design process according to 6 steps, i.e., Define - Measure - Analyze - Design - Optimize - Verify. A Design strategy to reduce the weight of piece-removing dynamical system will be explored by using ARIZ, i.e.,(the abbreviation of Algorithm for Inventive Problem Solving in Russian). The ARIZ will result in a final solution that the height and angle control parts for a cutting tool should be replaced by a kinematical approach, rather than complicated mechatronic approach(using motors). The Optimize step is composed of two sub-steps: (i) Generating process for obtaining several ideas of piece-removing system by using Brainstorming technique, satisfying the final solution derived from the Design step using ARIZ, and (ii) Optimizing process for selecting the most optimal idea of piece-removing system by using Pugh's matrix from the viewpoints of weight, cost and accuracy. The laststep of Verify has shown that the final design obtained by the 6-Sigma DMADOV technique with ARIZ & Brainstormingcan improve an initial design with design requirements satisfied. In this paper, we have shown that ARIZ and Brainstorming can be cooperatively merged into 6-Sigma DMADOV to give us both a formulatedproblem-solving approach and diverse candidate solutions(or ideas) without trial-and-error efforts.

Objective : To evaluate radiographic results of anterior fusion methods in two-level cervical disc disease : tricortical autograft and plate fixation (ACDF-AP), cage and plate fixation (ACDF-CP), stand-alone cage (ACDF-CA), and corpectomy and plate fixation (ACCF). Methods: The numbers of patients were 70 with a minimum 6 month follow-up (ACDF-AP : 12, ACDF-CP : 27, ACDF-CA : 15, and ACCF : 16). Dynamic simple X-ray and computed tomography were evaluated preoperatively, postoperatively, 6 month, and at the final follow-up. The fusion and subsidence rates at the final were determined, and global cervical lordosis (GCL), cervical range of motion, fused segment angle (FSA), and fused segment height (FSH) were analyzed. Results: Nonunion was observed in 4 (25%) patients with ACDF-CA, 1 (8%) patient with ACDF-AP, 1 (4%) patient with ACDF-CP. The number of loss of FSH (%) more than 3 mm were 2 patients (16%) in ACDF-AP, 3 patients (11%) in ACDF-CP, 5 patients (33%) in ACDF-CA, and 3 patients (20%) in ACCF. The GCL was decreased with ACDF-CA and increased with others. The FSA was increased with ACDF-AP, ACDF-CP, and ACCF, but ACDF-CA was decreased. At the final follow-up, the FSH was slightly decreased in ACDF-CP, ACDF-AP, and ACCF, but ACDF-CA was more decreased. Graft related complication were minimal. Screw loosening, plate fracture, cage subsidence and migration were not identified. Conclusion: ACDF-CP demonstrated a higher fusion rate and less minimal FSH loss than the other fusions in two-level cervical disc disease. The ACDF-AP and ACCF methods had a better outcome than the ACDF-CA with respect to GCL, FSA, and FSH.
The purpose of this study is to investigate the negative effects of cervical pull headgear and to compare the differences between the two groups of growers-vertical grower and horizontal grower group-which are classified by the posterior-anterior facial height ratio. Initial and final lateral cephalograms were taken for 26 patients including 15 vertical growers and 11 horizontal growers ; also, 3 angular measurements and 4 linear measurements were evaluated. The following results were found. 1. The palatal plane was tipped anteroinferiorly in the vertical grower group. 2. The posterior facial height/anterior facial height ratio was increased in the horizontal grower group. 3. The Mandibular plane angle remained stable on both groups. 4. There was no significant difference between the two groups in the amount of maxillary molar extrusion.
Currently, starting with electric vehicles, the application of ultra-high-strength steel sheets and light metals has expanded to improve mileage by reducing vehicle weight. At a time when internal combustion engine vehicles are rapidly changing to electric vehicles, the application of ultra-high-strength steel is expanding to satisfy both weight reductions and the performance safety of the chassis parts. There is an urgent need to improve the quality of parts without defects. It is particularly difficult to estimate the part formability through the finite element method (FEM) in the burring operation, so product design has been based on the hole expansion ratio (HER) and experience. In this study, design of experiment (DOE), analysis of variance (ANOVA), and regression analysis were combined to optimize the formability by adjusting the process variables affecting the burring formability of ultra-high-strength steel parts. The optimal variables were derived by analyzing the influence of variables and the correlation between the variables through FE analysis. Finally, the optimized process parameters were verified by comparing experiment with simulation. As for the main influence of each process variable, the initial hole diameter of the piercing process and the shape height of the preforming process had the greatest effects on burring formability, while the effect of a lower round of punching in the burring process was the least. Moreover, as the diameter of the initial hole increased, the thickness reduction rate in the burring part decreased, and the final burring height increased as the shape height during preforming increased.
Purpose: This study aimed to investigate the effect of smoking duration, smoked cigarettes per day and smoking cessation period on pulmonary function among ex-smokers: based on the 6th Korea National Health and Nutrition Examination Survey (KNHANES). Methods: This study was analyzed using the 6th KNHANES data. Pulmonary function tests were performed on a total of 4,214 adults (> 40 years old). A total of 770 adults ex-smokers were eligible for inclusion in the final analysis. Forced vital capacity (FVC), forced expiratory volume in one second ($FEV_1$) and $FEV_1/FEV$ were measured to evaluate pulmonary function. Results: This study showed that there were significant differences in both $FEV_1$ and FVC values based on gender, age and height, among ex-smokers. $FEV_1/FVC$ significantly differed by age, height and the smoking duration prior to smoking cessation. Multiple regression analysis revealed that, $FEV_1/FVC$ accounted for 26.0% of the variance by age, height and smoking duration. There was a difference in the mean value of $FEV_1/FVC$ with or without smoking for more than 10 years. Conclusion: This study's findings show that smoking for over 10 years in an ex-smoker can lead to problems with the respiratory system. The long-term cigarette has progressive ill effects on the respiratory system.
Background Although the initial projection after primary nipple reconstruction is excellent, nipple projection gradually flattens in most cases due to multiple causes. Although various methods have been reported to rebuild the nipple after nipple flattening, the most effective method of secondary nipple reconstruction remains unknown. The aim of this study was to review our institution's experiences with secondary nipple reconstruction. Methods We conducted a retrospective review from March 2012 to January 2019. We performed secondary nipple reconstruction if the primary reconstructed nipple height differed by more than 6 mm from the normal nipple height. We chose the method of nipple revision according to the degree of tissue scarring and the remaining nipple projection. Results We performed secondary nipple reconstruction on a total of 27 nipples, using purse-string sutures for 19 nipples and star flaps in eight nipples. The median follow-up period was 8 months (range, 6-19 months) after the final nipple reconstruction. Among the 19 nipples reconstructed using purse-string sutures, 10 (53%) demonstrated acceptable projection of more than 5 mm. Among the eight nipples reconstructed using star flaps, six (75%) showed acceptable projection of more than 5 mm. Most of the patients (73%) were satisfied (scores of 4 or 5) with the nipple reconstruction overall. Conclusions Few studies have presented favorable outcomes of secondary nipple reconstruction. When the star flap and purse-string suture methods were used depending on the remaining nipple height and scarring, appropriate projection could be achieved.
Accidental anchor drop can cause disturbances to seabed materials and pose significant threats to the safety and serviceability of submarine structures such as pipelines. In this study, a series of anchor drop tests was carried out to investigate the penetration mechanism of a Hall anchor in sand and clay. A special anchor drop apparatus was designed to model the inflight drop of a Hall anchor. Results indicate that Coriolis acceleration was the primary cause of large horizontal offsets in sand, and earth gravity had negligible impact on the lateral movement of dropped anchors. The indued final horizontal offset was shown to increase with the elevated drop height of an anchor, and the existence of water can slow down the landing velocity of an anchor. It is also observed that water conditions had a significant effect on the influence zone caused by anchors. The vertical influence depth was over 5 m, and the influence radius was more than 3 m if the anchor had a drop height of 25 m in dry sand. In comparison, the vertical influence depth and radius reduced to less than 3 m and 2 m, respectively, when the anchor was released from 10 m height and fell into the seabed with a water depth of 15 m. It is also found that the dynamically penetrating anchors could significantly influence the earth pressure in clay. There is a non-linear increase in the measured penetration depth with kinematic energy, and the resulted maximum earth pressure increased dramatically with an increase in kinematic energy. Results from centrifuge model tests in this study provide useful insights into the penetration mechanism of a dropped anchor, which provides valuable data for design and planning of future submarine structures.

Purpose : The aim of study was to determine the effect of lower trapezius muscle strengthening exercises on the height of rounded shoulder posture and muscle activity in patients with rounded shoulder posture. Methods : Thirty-one subjects with rounded shoulders were selected and assigned, with the use of a random number table, to an experimental group (EG, n=16) that received resistance exercises of the trapezius (lower) muscles or a control group (CG, n=15) that received isometric exercises of the lower trapezius muscles. In the EG, with the subject in a prone position, both arms were abducted at the angles suggested for each posture (145 °, 45 ° abduction), with the hands holding dumbbells to assume a functional posture. The CG performed only isometric exercises without dumbbells in the same conditions. At the start signal, with the thumb raised, the arm was raised to the level of the ear. Both groups were assessed by the height of the rounded shoulder posture (RSP) and muscle activity (EMG). The intervention was performed three times a week. Results : The RSP significantly decreased during the intervention period in both the EG and CG (p<.05), and a significant difference was determined between the RSP of the two groups (p<.05). In the EG, the EMG significantly increased during the intervention period (p<.05), but the CG did not show any significant increase (p>.05). The final EMG was significantly higher in the EG than in the CG (p<.05). Conclusion : Both the EG and CG showed positive improvement in rounded shoulder posture during the intervention period. In particular, lower trapezius resistance exercise appears to reduce shoulder height and positively affects the activity of muscles around the shoulder.

The landscape of Seoul was dynamically changed and developed with the rapid post-war economic growth. Seoul city designated a height regulation district to preserve and manage the city landscape and protect it from haphazard construction. The designation of a maximum height regulation district has obvious purpose and simple regulations which makes the implementation and management easy to apply yet the altitude restriction lacks an objective basis for its enforcement. Many studies have been done and the current uniform height regulation requires more objective and logical guidelines. This study selected the Bukhan Mountain area, a National Park designated to protect the environment, to present a new landscape height guideline to minimize environmental degradation and to harmonize the artificial and natural landscapes of the area. Document research was done to identify the regulation types(absolute height regulation, view line regulation, oblique line restriction regulation) and principles for height regulation district establishment, acknowledge the current status and issues of the Bukhan Mountain area's maximum height regulation district. Gangbuk-Gu was chosen as the site and survey was conducted on outstanding view points and view corridors of residents. From document research, an appropriate landscape height guideline was selected and applied to Gangbuk-Gu. According to the guideline, suitable heights for buildings were suggested. These were then applied to three-dimensional simulations and a final guideline was suggested.
