Background: For anatomic total arthroscopic repair, cementless humeral fixation has recently gained popularity. However, few studies have compared clinical, radiographic, and patient-reported outcomes between cemented and press-fit humeral fixation, and none have performed follow-up for longer than 5 years. In this study, we compared long-term postoperative outcomes in patients receiving a cemented versus press-fit humeral stem anatomic arthroscopic repair. Methods: This study retrospectively analyzed 169 shoulders that required primary anatomic total shoulder arthroplasty (aTSA). Shoulders were stratified by humeral stem fixation technique: cementation or press-fit. Data were collected pre- and postoperatively. Primary outcome measures included range of motion, patient reported outcomes, and radiographic measures. Results: One hundred thirty-eight cemented humeral stems and 31 press-fit stems were included. Significant improvements in range of motion were seen in all aTSA patients with no significant differences between final cemented and press-fit stems (forward elevation: P=0.12, external rotation: P=0.60, and internal rotation: P=0.77). Patient reported outcome metrics also exhibited sustained improvement through final follow-up. However, at final follow-up, the press-fit stem cohort had significantly better overall scores when compared to the cemented cohort (visual analog score: P=0.04, American Shoulder and Elbow Surgeon Score: P<0.01, Simple Shoulder Test score: P=0.03). Humeral radiolucency was noted in two cemented implants and one press-fit implant. No significant differences in implant survival were observed between the two cohorts (P=0.75). Conclusions: In this series, we found that irrespective of humeral fixation technique, aTSA significantly improves shoulder function. However, within this cohort, press-fit stems provided significantly better outcomes than cemented stems in terms of patient reported outcome scores. Level of evidence: III.
This research applies a pre-trained bidirectional encoder representations from transformers (BERT) handwriting recognition model to predict foreign Korean-language learners' writing scores. A corpus of 586 answers to midterm and final exams written by foreign learners at the Intermediate 1 level was acquired and used for pre-training, resulting in consistent performance, even with small datasets. The test data were pre-processed and fine-tuned, and the results were calculated in the form of a score prediction. The difference between the prediction and actual score was then calculated. An accuracy of 95.8% was demonstrated, indicating that the prediction results were strong overall; hence, the tool is suitable for the automatic scoring of Korean written test answers, including grammatical errors, written by foreigners. These results are particularly meaningful in that the data included written language text produced by foreign learners, not native speakers.

This prospective clinical study evaluates the 5-10 year results of the BiPolar shoulder Arthroplasty in patients with end-stage RotatorCuff Arthropathy. The study group consisted of 48 patients (59 shoulders). Average age was 72 years and average FU time was 73 months. Results showed that the average UCLA score went from 7.9 Pre-op to 23.3 Post-op. Final Constant score averaged $52\% (unadjusted). Pain relief using the VAS was 1.2(were 0=no pain. 15 = excruciating pain). ROM improved by an average of $20^\circ$. There were 2 reoperations because of periprosthetic fractures. Despite rather poor functional results, these patients were satisfied with their pain relief and the functional gains accompanying pain relief would be an added benefit.
Background: Differentiated thyroid cancer (DTC) is a cancer group that shares molecular and cellular origin but shows different clinical courses and prognoses. Several prognostic factors have been reported for predicting recurrence for individual patients. This literature review aimed to evaluate prognostic scores for predicting recurrence of DTC. Materials and Methods: A search of the MEDLINE database for articles published until December 2015 was carried out using the terms "thyroid neoplasms AND (recurrent OR persistent) AND (score OR model OR nomogram)". Studies were eligible for review if they indicated the development of prognostic scoring models, derived from a group of independent prognostic factors, in predicting disease recurrence in DTC patients. Results: Of the 308 articles obtained, five were eligible for evaluation. Two scoring models were developed for DTC including both papillary and follicular carcinoma, one for papillary carcinoma, and the other two for papillary microcarcinoma. The number of patients included in the score development cohort ranged from 59 to 1,669. The number of evaluated potential prognostic factors ranged from 4 to 25. Tumor-related factors were the most common factors included in the final scores, with cervical lymph node metastases being the most common. Only two studies showed internal validation of the derived score. Conclusions: There is a paucity of prognostic scores for predicting disease recurrence in patients with DTC, in particular for follicular thyroid carcinoma. Several limitations of the created scores were found. Performance of the scores has not been adequately studied. Comprehensive validation in multiple cohorts is recommended before widespread use.
Objective : This study was designed to evaluate the effect of treatment of Bell's palsy patients with postauricural pain by Anti-inflammatory pharmacopuncture Methods : The clinical comparison studies were carried out 30 cases of Bell's palsy patient with postauricular pain. Anti-inflammatory pharmacopuncture. We divided into two groups. One was control group that was treated only Acupuncture and Herbal therapy and The other was a Anti-inflammatory pharmacopuncture group and the conclusions for treatment are as follows. Results : 1. Regarding alleviation of pain by VAS, Anti-inflammatory pharmacopuncture Group II showed significant decrease of VAS in the 5days treatment. 2. They were not significant score within two groups after 5th days treatment. 3. As a result of evaluation by using Yanagihara score, they were not significant score within two groups after final treament.

There are many positioning studies utilizing Analytic Hierarchy Process (AHP) in tourism industry. In this study, it was tried to develop new evaluation criteria for positioning and to discuss the importance of evaluation factors, as the development of tourist's spots was pursued in diversified ways. The purpose of this study was to allow the development of tourist spots more objectively and more efficiently utilizing Analytic Hierarchy Process ("AHP" hereunder) to appoint the best place for the tourism complex in Buyeo. According to AHP analysis procedures, 5 places near Baikje Cultural Block connecting with a ferry in Buyeo-gun were selected as candidates. To perform AHP analysis, evaluation criteria and a survey were used and then weight of evaluation criteria was induced. With the conditions on criteria, standard score of each criterion and candidate was calculated. By multiplying standard score and final weight and then summing up, the candidate place with the highest score shall be appointed as the most optimal place for Buyeo-gun Agricultural Tourism Complex.
Purpose: Hwabyung is a culture-bound syndrome in Korea, characterized by emotional stress such as anger, depression, and anxiety. The purpose of the present study was to explore the characteristics of Hwabyung patients, based on constitutional medicine, for their symptoms, duration, and coping styles. Method: Participants were 57 Hwabyung patients selected by Hwabyung criteria. They were also diagnosed for their constitution by doctors specialized in constitutional medicine. Each participant was asked to respond to a variety of items such as cause and duration of stress, symptoms of hwabyung, anger and depression, and coping styles. Eight patients were excluded from the total sample; two male patients and six patients unable to be diagnosed for any particular type of constitution. Thus, final sample was 49 female hwabyung patients, which will be reported in what follow. Result: To begin with, constitutional type revealed significant differences in such coping styles as 'perseverance,' 'positive interpretation,' 'positive comparison,' and 'emotional pacification.' More specifically, post-hoc comparison revealed that Taeeumin(person who have taeeum characteristics) was higher in 'perseverance' compared-to other two types of constitution. Taeumin also revealed higher score than Soeumin(person who have soeum characteristics) in 'positive comparison.' With respect to the marital status, the married compared to the unmarried showed higher score in 'anger-control,' and lower score in 'anger-in.' The married compared to the unmarried, regarding coping styles, also showed higher score in 'accommodation,' but lower score in 'emotional expression.' The duration of hwabyung revealed that the longer duration of hwabyung, the more frequent utilization of 'positive interpretation' and 'emotional pacification. Conclusion: The main purpose, the exploration of hwabyung based on four types of constitution, was not supported, as a whole in the present study. However, marital status and duration of hwabyung appear to influence on emotional stress (e.g., anger) as well as coping styles, suggesting that these variables should be dealt with in future studies on hwabyung.

Two diabetes mellitus patients treated by arthroscopy and associated procedure for an infection after arthroscopic rotator cuff repair were involved. The time interval from rotator cuff repair to symptom development was 18 days in average and arthroscopic debridement and associated procedure for infection applied in average 65 days after symptom development. Patient evaluation was done according to the KSS, ASES, UCLA and Constant Score. We used antibiotics for average 22.5 days after arthroscopic debridement and associated procedure, the infection was treated in average 4 months. At final follow-up, the mean KSS score was 82 points, the mean UCLA score was 33 points, the mean ASES score was 91 points, the mean Constant score was 71 points. All infections following arthroscopic rotator cuff repair were cured by arthroscopy and associated procedure. ROM and functional results were much improved, the pain and satisfaction were also much improved.

Objectives : The purpose of this study is to report the effectiveness of a Korean Medicine Treatment on 89 severe atopic dermatitis patients all through 12 months. Methods : The degree of atopic dermatitis was measured in Objective SCORAD Index (OSI) with photos of patients. Then, the OSI score was estimated according to gender, age and intensity. Results : 1. The ratio of male to female was 41.57 : 58.43. The 24.72% of patients were in 0~11 year-old group, 16.85% were in 12~18 year-old group and 58.43% were in the group above 19 years old. 2. The initial OSI score was 64.43 and the final was 37.03 after 12 months. After 3, 6, 9, 12 months, the OSI score was significantly lower than the initial visit. 3. There was no significant difference between male and female patients on age, OSI scores and OSI improvements. 4. The patients who treated acupuncture were significantly older then who not treated. The OSI improvement had no significant difference whether treated with acupuncture or not. 5. There was no significant difference among 3 different age groups on initial OSI scores. The OSI improvement of 0~11 year-old group was significantly higher than the other groups after 12 month treatment. 6. The OSI score of 97.75% was decreased after 12 month treatment compared to initial visit. 7. After 12 month treatment, the distribution of severe atopic dermatitis was decreased from 100% to 37.08%. 8. There was no significant difference betweent steroid group and non-steroid group on OSI scores at initial visit and after 12 month treatment. Conclusions : The OSI Score of severe atopic dermatitis patients was decreased on the significant rate with Korean medicine treatment for 12 months.
Background: The purpose of the current study was to investigate short- to mid-term outcomes and complications following radial head replacement (RHR) for complex radial head fractures and to identify factors associated with clinical outcomes. Methods: Twenty-four patients with complex radial head fractures were treated by RHR. The mean age of the patients was 49.8 years (range, 19-73 years). Clinical and radiographic outcomes were evaluated for a mean follow-up period of 58.9 months (range, 27-163 months) using the visual analog scale (VAS) score for pain, the Mayo elbow performance score (MEPS), the quick disabilities of the arm, shoulder and hand (Quick-DASH) score, and serial plain radiographs. Complications were also evaluated. Results: At the final follow-up, the mean VAS score, MEPS, and Quick-DASH score were 0.6±1.1, 88.7±11.5, and 19.4±7.8, respectively. The mean range of motion was 132.7° of flexion, 4.7° of extension, 76.2° of pronation, and 77.5° of supination. Periprosthetic lucency was observed in six patients (25%). Heterotopic ossification was observed in four patients (16.7%). Arthritic change of the elbow joint developed in seven patients (29.2%). Capitellar wear was found in five patients (20.8%). Arthritic change of the elbow joint was significantly correlated with MEPS (P=0.047). Four cases of complications (16.6%) were observed, including two cases of major complications (one stiffness with heterotopic ossification and progressive ulnar neuropathy and one stiffness) and two cases of minor complications (two transient ulnar neuropathy). Conclusions: RHR for the treatment of complex radial head fractures yielded satisfactory short- to mid-term clinical outcomes, though radiographic complications were relatively high.
