Philosophical Counseling in the Clinical Field of Korean Medicine: Applicaition and Case Analysis (한의학 임상현장에서의 철학 상담의 적용: 제언과 사례분석)

  • Objectives: This study suggests the application of philosophical counseling in the clinical field of Korean medicine and analyze the specific case. Methods: Through the analysis of coexisting factors of Korean medicine and philosophical counseling, we establish the area of philosophical counseling that Korean medicine practitioner can apply in the medical field. We described effects of philosophical counseling on the patient's symptom and the progress after discharge through a counseling approach based on philosophical questions. Results: During philosophical counseling, the Korean medical doctor asks questions about the nature of suffering and encourages patients to gain insight into their problems. In answering the vital philosophical questions, the patient leads to a creative interpretation of future life directions. We described the method of specific approaches through the description of the two cases. Conclusions: The philosophical counseling is compatible with the concept of Korean medicine, therefore, a Korean medicine practitioner can induce the recovery of mind and body through the creative interpretation of the patient by asking the patient the appropriate question which causes reflective thinking.

Clinical Outcome of Cranial Neuropathy in Patients with Pituitary Apoplexy

  • Objective : Pituitary apoplexy (PA) is described as a clinical syndrome characterized by sudden headache, vomiting, visual impairment, and meningismus caused by rapid enlargement of a pituitary adenoma, We retrospectively analyzed the clinical presentation and surgical outcome in PA presenting with cranial neuropathy. Methods : Twelve cases (33%) of PA were retrospectively reviewed among 359 patients diagnosed with pituitary adenoma, The study included 6 males and 6 females, Mean age of patients was 49,0 years, with a range of 16 to 74 years, Follow-up duration ranged from 3 to 20 months, with an average of 12 months, All patients were submitted to surgery, using the transsphenoidal approach (TSA). Results : Symptoms included abrupt headache (11/12), decreased visual acuity (12/12), visual field defect (11/12), and cranial nerve palsy of the third (5/12) and sixth (2/12) Mean height of the mass was 29.0 mm (range 15-46) Duration between the ictus and operation ranged from 1 to 15 days (mean 7.0) The symptom duration before operation and the recovery period of cranial neuropathy correlated significantly (p = 0.0286) TSA resulted in improvement of decreased visual acuity in 91.6%, visual field defect in 54.5%, and cranial neuropathy in 100% at 3 months after surgery. Conclusion : PA is a rare event, complicating 3.3% in our series, Even in blindness following pituitary apoplexy cases, improvement of cranial neuropathy is possible if adequate management is initiated in time, Surgical decompression must be considered as soon as possible in cases with severe visual impairment or cranial neuropathy.

  • Monitoring of impact loads is a very important technique in the field of structural health monitoring (SHM). However, in most cases it is not possible to measure impact events directly, so they need to be reconstructed. Impact load reconstruction refers to the problem of estimating an input to a dynamic system when the system output and the impulse response function are usually known. Generally this leads to a so called ill-posed inverse problem. It is reasonable to use prior knowledge of the force in order to develop more suitable reconstruction strategies and to increase accuracy. An impact event is characterized by a short time duration and a spatial concentration. Moreover the force time history of an impact has a specific shape, which also can be taken into account. In this contribution these properties of the external force are employed to create a sample-force-dictionary and thus to transform the ill-posed problem into a sparse recovery task. The sparse solution is acquired by solving a minimization problem known as basis pursuit denoising (BPDN). The reconstruction approach shown here is capable to estimate simultaneously the magnitude of the impact and the impact location, with a minimum number of accelerometers. The possibility of reconstructing the impact based on a noisy output signal is first demonstrated with simulated measurements of a simple beam structure. Then an experimental investigation of a real beam is performed.

  • The potential induced degradation (PID) phenomenon shows severe power loss within several years from the initial installation of solar power system. The accumulated power loss by PID is inevitable because the PID is suspected only if the power loss exceeds several percent. In this paper, we analyzed the cases of PID diagnosis and recovery by visiting the suspected PID site about 17 months after the installation of the 100kW PV system. The power difference of the two 50kW inverters was more than 20kWh, and the PR difference was more than 8%. From the beginning of the installation, the difference in power gradually increased. The recovery was observed for about 7 months by applying 1000V in the reverse bias at night. As a result, the power difference of the two inverters was recovered to within 2kWh. In the case of a power station suffering from PID in the field, it will be helpful for stable development operation by quick diagnosis and problem solving.

Free Flap Reconstruction in Patients with Traumatic Injury of the Forefoot

  • Many techniques have been developed for reconstruction of the hand; however, less attention has been paid to foot reconstruction techniques. In particular, reconstruction of the forefoot and big toe has been considered a minor procedure despite the importance of these body parts for standing and walking. Most of the weight load on the foot is concentrated on the forefoot and big toe, whereas the other toes have a minor role in weight bearing. Moreover, the forefoot and big toe are important for maintaining balance and supporting the body when changing directions. Recently, attention has been focused on the aesthetic appearance and functional aspects of the body, which are important considerations in the field of reconstructive surgery. In patients for whom flap reconstruction in the forefoot and big toe is planned, clinicians should pay close attention to flap survival as well as functional and cosmetic outcomes of surgery. In particular, it is important to assess the ability of the flap to withstand functional weight bearing and maintain sufficient durability under shearing force. Recovery of protective sensation in the forefoot area can reduce the risk of flap loss and promote rapid rehabilitation and functional recovery. Here, we report our experience with two cases of successful reconstruction of the forefoot and big toe with a sensate anterolateral thigh flap, with a review of the relevant literature.

Study on the Turbulent Boundary Layer Disturbed by a Triangular Prism near the Wall (벽근방의 3각주에 의하여 교란받는 난류경계층에 관한 연구)

  • This paper presents the results of some measurement of the fluctuating velocity field in the turbulent boundary layer disturbed by a triangular prism and discusses the discovery of the disturbed boundary layer. A prism of height 8mm was used for experiments. The streamwise location of the prism was fixed at 1200mm downstream from the leading edge and the space between the prism center and the wall was set at three different values, 6, 15 and 33.5mm. The results show that the near-wall region of the disturbed boundary layer recovers original state much more quickly than the outer region. In the case h=6mm the recovery is faster than the other cases. Moreover, it was found that peak of fluctuating velocities moves outwards somewhat rapidly with increasing ${\times}$ mainly due to the turbulent diffusion of the fluctuating velocity.

  • The flow and combustion patterns have been investigated inside the gasoline engine cylinder with the swirl or tumble flow, whereas the air flow characteristics, which are generated in the part of intake system before entering into the intake manifold, have not been known completely. It is necessary to analyze the flow field in the intake system consisting of air rater, throttle valve and intake manifold. The throttle valve, used to control the intake air flow rate, is important because it makes various mass flow rate and flow patterns. Three-dimen-sional How characteristics such as velocities, turbulent intensities and Reynolds shear stresses are measured by the hot wire anemometer at the exit of the throttle valve with the variation in the valve opening angle($15^{\circ}$, $45^{\circ}$, $75^{\circ}$ and $90^{\circ}$) and the Reynolds numbers (45000, 70000 and 140000). There are a lot of changes in flow characteristics at $75^{\circ}$ due to the large recirculation flow comparing with those of the other cases, and the streamwise velocity is especially enforced strongly below the valve shaft. The other component velocities are relatively large near the centerline parallel to the valve shaft. The effects of the Reynolds number on the flow field are not severe.

  • Physiotherapy may be defined as the use of physical techniques for the treatment of injuries and movement dysfunction. In the world of human medicine, physiotherapy has been proven as an indispensable aid in the recovery of many musculoskeletal conditions, as evidenced by the extensive physiotherapy departments within most hospitals. Nowadays, this important branch of medicine is also rapidly becoming a recognised tool in the prevention, cure, and rehabilitation of many equine, canine and feline injuries. In 1978, canine physical therapy techniques were described by Ann Downer, a physical therapist on faculty at Ohio State University. Animal physical therapy is a new and rapidly developing field of health care for animals. The benefits of physical therapy have long been recognized in humans. More recently, work in the veterinary field has shown the same benefits of physical therapy to be true for animal patients. Performing orthopaedic or neurological surgery, or fitting a human patient with a cast or splint, and then discharging the patient is an outdated approach. In such cases, physical therapy is clearly warranted. Similarly, recent research has shown that post-surgical rehabilitation and therapy after injuries significantly improves the functional outcomes for animals. The goals of physiotherapy are to relieve pain, restore range of motion/movement, improve function, prevent injuries and expand the physical potential of the patient. Once in the field, physical therapists actively continue their education to keep up to date on the latest treatments and technologies. Via continuing education courses, physiotherapists can learn how to apply their unique and specialized knowledge to other animal species.

  • Disaster management system can prepare enough for a social disaster. In addition, it is measures to reduce damage. When, where and how to prepare for a disaster will happen we can not expect an appropriate sense of crisis, disaster response and is the right choice early. In this paper, the next national disaster safety sector effective use SNS will find ways. To do this successfully in the field of international disaster safety use cases and measures are derived that can be applied to our country. As a result, comprehensive and systematic introduction of disaster management system to minimize the damage will contribute effectively to the rapid disaster recovery.

The Application of CISG to International Commercial Arbitration (聯合國國際貨物銷售合同公約在國際商事仲裁中的适用(국제물품매매계약에 관한 유엔협약이 국제상사중재에서의 적용))

  • International arbitration is the important field of applying CISG and the backbone of uniform law developed by CISG. Now CIETAC tribunals like courts of contracting states apply CISG precisely, which is beneficial to improving the quality and the credit of arbitral cases. Arbitration has the characters of independence and the non-government. the legal foundation of arbitral tribunal's applying CISG are the national arbitral law, the applicable arbitral procedures and usages of arbitration, not for performing international obligations under the CISG. CIETAC mainly use China Contract Law and CISG over the cases of sale of goods. Because of no provisions on recovery of differential price loss (equal to article 75 and 76 of CISG) Chinese tribunals have more discretion in determining the sum of damages under the China Contract Law. Applying China Contract Law may not beneficial to aggrieved party.
