The purpose of this study was to determine the effect of surface modification on the fibrovascular ingrowth into porous polyethylene (PE) spheres ($Medpor^{(R)}$), which are used as an anophthalmic socket implant material. To make the inert, hydrophobic PE surface hydrophilic, nonporous PE film and porous PE spheres were subjected to plasma treatment and in situ acrylic acid (AA) grafting followed by the immobilization of arginine-glycine-aspartic acid (RGD) peptide. The surface-modified PE was evaluated by performing surface analyses and tested for fibroblast adhesion and proliferation in vitro. In addition, the porous PE implants were inserted for up to 3 weeks in the abdominal area of rabbits and, after their retrieval, the level of fibrovascular ingrowth within the implants was assessed in vivo. As compared to the unmodified PE control, a significant increase in the hydrophilicity of both the AA-grafted (PE-g-PAA) and RGD-immobilized PE (PE-g-RGD) was observed by the measurement of the water contact angle. The cell adhesion at 72 h was most notable in the PE-g-RGD, followed by the PE-g-PAA and PE control. There was no significant difference between the two modified surfaces. When the cross-sectional area of tissue ingrowth in vivo was evaluated, the area of fibrovascularization was the largest with PE-g-RGD. The results of immunostaining of CD31, which is indicative of the degree of vascularization, showed that the RGD-immobilized surface could elicit more widespread fibrovascularization within the porous PE implants. This work demonstrates that the present surface modifications, viz. hydrophilic AA grafting and RGD peptide immobilization, can be very effective in inducing fibrovascular ingrowth into porous PE implants.
Hydroxyapatite 안구삽입물내로의 섬유혈관증식 여부를 진단하는데 $^{99m}Tc$-MDP 골신티그라피가 유용한지를 알아보기 위해, 전남대학교병원 안과에서 안구적출술 후 hydoxyaptite 안구삽입물을 삽입하고 골신티그라피 검사를 시행한 13예의 환자의 골신티그라피 17예를 분석하고, 이들에서 의안부착을 위해 안구삽입물에 구멍을 뚫을 때 출혈이 있는지를 관찰하여 섬유혈관증식 여부를 확인하였다. 대상환자의 연령은 21세부터 56세까지 분포하였고(평균연령 38세), 성별로는 남자 10, 여자 3예였다. 이 중 2예에서는 2번씩, 또 1예에서는 3번 검사를 시행하였으므로 시행한 골신티그라피는 17번이었고, 이 17예의 검사소견을 분석하였다. 평면상은 정성적으로 판독하였는 데, 안구삽입물에 섭취가 안면골보다 많은 정도를 4+, 안면골과 같은 정도를 3+, 주변 안면골보다 작은 경우를 2+, 섭취가 정상 안구보다 조금 더 진한 정도를 1+로 정하였다. 단면상의 경우는 정상안과 비교한 섭취율을 구하였으며, 그 공식은 (삽입물의 방사능/반대측 안와의 방사능)으로 하였다. 대상환자 모두의 골신티그라피 평면영상에서 1 이상의 섭취를 보였고, 단면상의 정량분석은 1.18이상의 섭취를 보였다. 골신티그라피를 2번 이상 시행한 3예 중 2예에서는 두번째 시행한 골신티그라피상 $^{99m}Tc$-MDP 섭취의 증가가 관찰되었으나, 나머지 1예에서는 3번 시행한 골신티그라피상 전혀 변화를 관찰할 수 없었다. 또 대상환자에서의 안부착시 출혈 여부를 관찰하였는 바 모든 환자에서 충분한 출혈이 있어 삽입물내로 섬유혈관증식이 있음을 확인하였다. 즉, 골신티그라피상 $^{99m}Tc$-MDP 섭취를 보인 안구삽입물 13예 전예에서 삽입물 내로의 혈관증식을 확인하였다. 결론적으로, hydroxyaptite 안구삽입물내로 섬유혈관증식은 매우 잘 일어나며, 주변 안면골보다 낮은 정도의 섭취만 보이더라도 섬유혈관증식 일어난 상태임을 의미하고, 이 경우 안심하고 2차 천공을 시행할 수 있다는 사실을 확인하였다.
Purpose: A new form of porous polyethylene, characterized by higher porosity and pore interconnectivity, was developed for use as a tissue-integrated implant. This study evaluated the effectiveness of porous polyethylene blocks used as an onlay bone graft in rabbit mandible in terms of tissue reaction, bone ingrowth, fibrovascularization, and graft-bone interfacial integrity. Methods: Twelve New Zealand white rabbits were randomized into 3 treatment groups according to the study period (4, 12, or 24 weeks). Cylindrical specimens measuring 5 mm in diameter and 4.5 mm in thickness were placed directly on the body of the mandible without bone bed decortication, fixed in place with a titanium screw, and covered with a collagen membrane. Histologic and histomorphometric analyses were done using hematoxylin and eosin-stained bone slices. Interfacial shear strength was tested to quantify graft-bone interfacial integrity. Results: The porous polyethylene graft was observed to integrate with the mandibular bone and exhibited tissue-bridge connections. At all postoperative time points, it was noted that the host tissues had grown deep into the pores of the porous polyethylene in the direction from the interface to the center of the graft. Both fibrovascular tissue and bone were found within the pores, but most bone ingrowth was observed at the graft-mandibular bone interface. Bone ingrowth depth and interfacial shear strength were in the range of 2.76-3.89 mm and 1.11-1.43 MPa, respectively. No significant differences among post-implantation time points were found for tissue ingrowth percentage and interfacial shear strength (P>0.05). Conclusions: Within the limits of the study, the present study revealed that the new porous polyethylene did not provoke any adverse systemic reactions. The material promoted fibrovascularization and displayed osteoconductive and osteogenic properties within and outside the contact interface. Stable interfacial integration between the graft and bone also took place.
Purpose: The key points of treatment of cryptotia are the elevation of invaginated ear helix and the correction of deformed cartilage. Prevention of stabilized cartilage contouring from returning to the previous state is also important. The authors carried otoplasty by modified Onizuka's method or Ohmori's method that conchal cartilage graft or high density polyethylene implant(MEDPOR$^{(R)}$) graft served as fixation after spreading posterior aspect of adhered antihelix and a splint for prevention of recurrence of cartilage deformities. The aim of this study is to reveal the availability of the high density polyethylene implant(MEDPOR$^{(R)}$) graft for the correction of cryptotia. Methods: We have repaired 17 cryptotic deformities using cartilage graft from cavum of concha(12 cases) or high density polyethylene implant(5 cases) for correction of deformed cartilage. We investigate the operative time, complications, and satisfaction of postoperative ear shape on both autogenous cartilage graft group and high density polyethylene implant graft group. Results: There was 1 case of reinvagination on autogenous cartilage graft group. Implant exposure was occurred on high density polyethylene implant graft group, as 1 case. These were statistically no differences between autogenous cartilage graft group and high density polyethylene implant graft group to the satisfaction of ear shape. Conclusion: High density polyethylene implant(MEDPOR$^{(R)}$) present an alternative to autogenous material as they allow of fibrovascular ingrowth, leading to stability of the implant and decreased infection rates. The correction of deformed cartilage by using the high density polyethylene implant(MEDPOR$^{(R)}$) is a good option for the treatment of cryptotia.
목적: 안구보충물로 사용된 hydroxyapatite 내로의 섬유혈관증식은 이물질 반응으로 인한 염증의 동반없이 의안의 운동성을 유지시키기 위한 운동성 나사못을 삽입하는데 중요한 역할을 한다. 안구보충물 삽입술 후에 섬유혈관증식의 정도를 알아보는데 Tc-99m MDP를 이용한 평면영상과 단면영상에서 정성적 평가와 정량적 평가의 비교분석을 함으로서 완전한 섬유혈관증식을 예측하는데 단면영상의 유용성을 알아보고자 하였다. 대상 및 방법: 안구 제거술후 hydroxyapatite를 이용하여 안구보충술을 시행받고 $197{\pm}81$일 후에 골신티그라피를 시행받은 17명의 환자(남:녀=12:5, 연령; $50.4{\pm}17.5$세)의 18예를 대상으로 하였다. 골신티그라피는 Tc-99m MDP 740 MBq를 정맥주사 후 4시간 경과뒤에 이중헤드 감마카메라(ECAM, Siemens, Germany)로 안면부 평면영상을 얻은 후 이어서 단면영상을 얻었다. 평면영상과 단면영상에서의 안구보충물의 방사능 섭취가 비교(nasal bridge)보다 약한 경우를 grade 1, 비슷한 경우를 grade 2, 강한 경우를 grade 3로 평가하였고, 정상안구에 대한 안구보충물의 섭취비를 정량적으로 구하여 운동성 나사못 삽입술 후 시행된 세극등 검사에서 염증반응의 동반 없이 완전한 섬유혈관증식을 보인 환자 군과 염증 반응의 동반과 함께 불완전한 섬유혈관증식을 보인 환자 군으로 나누어 비교 분석하였다. 결과: 평면영상에서 grade 1은 9예, grade 2가 1예, grade 3가 2예로 충분한 섬유혈관증식을 의미하는 grade 2와 3를 보인 경우가 9예 (50%) 이었으나 단면영상에서는 grade 1이 6예, grade 2가 4예, grade 3가 8예로 평면영상에 비하여 grade 2와 3를 보이는 경우가 12예(61%) 이었다. 정성적 평가에 의한 완전한 섬유혈관증식을 예측하는 데 있어서 평면영상은 예민도가 60% (9/15),특이도 100% (3/3), 양성예측률 100% (9/9), 음성예측률 33.3% (3/9), 정확도 66.7% (12/18) 이었고 단면영상은 예민도 80% (12/15), 특이도 100% (3/3), 양성예측률 100% (12/12), 음성예측률 50% (3/6), 정확도 83.3% (15/18) 이었다. 정량적으로 구한 안구보충물 내의 방사능 섭취비는 평면영상에서 grade 1은 $1.19{\pm}0.10$, grade 2는 $2.16{\pm}0.43$, grade 3는 $3.57{\pm}0.74$로 grade가 높을수록 정량적 섭취비가 유의하게 증가하였다(p<0.001). 단면영상에서 grade 1은 $3.06{\pm}1.22$, grade 2는 $3.71{\pm}0.83$, grade 3는 $12.49{\pm}4.31$로 grade가 높을수록 정량적 섭취비가 유의하게 증가하였다(p<0.001). 완전한 섬유혈관증식을 보인 환자 군과 불완전한 섬유혈관증식을 보인 환자군으로 나누어 정량적으로 측정한 방사능 섭취비를 비교시 평면영상에서는 $1.96{\pm}0.87$ 대 $1.17{\pm}0.08$로 유의한 차이를 보이지 않았으나 단면영상에서는 $8.44{\pm}5.45$ 대 $2.20{\pm}0.87$로 유의한 차이를 보였다 (p<0.05). 결론: 안구보충물 삽입 후 섬유혈관증식을 평가할 때 평면영상에 비하여 단면영상이 섬유혈관증식 정도를 평가하는데 보다 도움을 주었다.
