Is fetal nuchal cord associated with autism spectrum disorder? A meta-analysis

  • Purpose: There is inconsistent evidence about the association between fetal nuchal cord and the risk of autism spectrum disorder (ASD). We performed a meta-analysis to investigate whether fetal nuchal cord is associated with an increased risk of ASD in offspring. Methods: Three main English databases were searched until January 2021. The Newcastle-Ottawa Scale was used to assess study quality. Study heterogeneity was determined using the I 2 statistic, while publication bias was assessed using Begg and Egger tests. Results are presented as odds ratios (ORs) and relative ratios with 95% confidence intervals (CI) and were determined by a random-effects model. Results: Five articles (1 cohort, 4 case-control; 3,088 total children) were included in the present meta-analysis. Fetal nuchal cord was not a risk factor for ASD (OR, 1.11; 95% CI, 0.66-1.57). There was homogeneity among studies that reported a risk of ASD (I2=0.0%). Conclusion: Our results showed that fetal nuchal cord is not a risk factor for ASD. Future large cohort studies should confirm this finding.

The Effects of MRI on Mouse Embryos During Fetal Stage

  • The effects of Magnetic resonance imaging (MRI) on mouse embryos at the early stage of organogenesis were investigated. Pregnant ICR mice were exposed on day 8 of gestation to MRI at 0.5 T for 0.5 hour to 3 hours. The mortality rates of embryos or fetuses, the incidence of external malformations, fetal body weight and sex ratio were observed at day 18 of gestation. A significant increase in embryonic mortality was observed after exposure to either 0.5 T MRI for 0.5 hour or 2 hours. However, the exposure to MRI for 1 hour or 3 hours did not induce any significant increase in embryonic mortality when compared with control. External malformations such as exencephaly, cleft palate and anomalies of tail were observed in all experimental groups exposed to each MRI. A statistically significant increase of external malformations was observed in all groups treated with 0.5 T MRI for 0.5 hour and 3 hours. The incidence of external malformations in the mice group exposed to 0.5 T MRI for 0.5-hour was found to be higher than those of mice group exposed to 0.5 T MRI for 2 hours. The effects of MRI on the external malformations might not to be dose-dependent. There was no statistically significant difference in fetal body weight and sex ratio among each MRI exposure groups.

임신말 태아의 아미노산영양에 대한 연구 (A Study on the Fetal Amino Acids Nutrition at Term of Human Pregnancy)

  • This study was performed to investigate the placental transfer and the human fetal utilization of amino acids at term of pregnancy. The plasma levels of 23 free amino acids in both the umbilical circulation ( umbilical vein and artery ) and the uterine circulation ( uterine vein and iliac artery ) of 34 pregnant women were measured at delivery by the cesarean section. In the umbilical circulation, 9 amino acids ( alanine, lysine, valine, leucine, arginine, isoleucine, ornithine, cystine, ${\alpha}-aminobutyrate$ ) were significantly higher and 2 amino acids ( glutamate, aspartate ) were significantly lower in the umbilical vein than in the umbilical artery. In the uterine circulation, alanine, tyrosine and methionine were significantly lower in the uterine vein than in the iliac artery. Glutamate was significantly lower in the uterine vein than in the iliac artery. According to these results, the origin of fetal plasma amino acids was discussed in terms of the metabolic conversions which would occur in the Placenta and the fetal utilization of amino acids was estimated.

산모 집중치료실에 입원한 고위험 임부의 건강관리 요구: 혼합적 연구방법 적용 (Health-care Needs of High-risk Pregnant Women Hospitalized in Maternal-Fetal Intensive Care Units: A Mixed-methods Design)

  • Purpose: To identify the characteristics and health-care needs of high-risk pregnant women in maternal-fetal intensive care units (MFICU). Methods: A mixed-methods design was adopted. Data were collected from 78 high-risk pregnant women admitted to the MFICU. Qualitative data included ten participants' experiences with hospitalization and childbirth, which were analyzed using mixed content analysis. Quantitative data were analyzed using at-test and one-way ANOVA testing. Results: The average score for pregnancy and childbirth health-care needs was 3.54 points. Average score by area was before-admission health care (3.70), health care of baby (3.67), health of childbirth (3.61), postpartum health (3.51), and pregnancy health care during hospitalization (3.48). Qualitative results showed diverse feelings and experiences of high-risk pregnant women and their need for health care, which was expressed in three themes and 11 sub-themes. Conclusion: Nurses should recognize high-risk mothers' feelings and needs for pregnancy and childbirth-focused health care to help patients accept their vulnerability and cope positively.

임신흰쥐에서 모체와 태자의 장기에 축적되는 수은에 대한 마늘의 저감효과에 대한 연구 (Decreasing Effects of Korean Garlic against the Accumulation of Mercury Levels in Maternal and Fetal Organs in Pregnant Fischer-344 Rats)

  • The purpose of this study was to evaluate the decreasing effects of Korean garlic against the accumulation of mercury levels in maternal and fetal organs in pregnant Fischer 344 rats, based on the theory and information that neutral amino acids have protective effects against mercury poisoning and garlic contains a large of neutral amino acids. The results obtained are as follows: 1. On the 20th day of gestation, the maternal body weight in 20 mg/wt$\cdot$kg methyl mercuric chloride groups was 76.1% of those in control group, but those recovered to be 81.2% and 93.6% by treating with garlic (0.5 g/wt$\cdot$kg and 1.0 g/wt$\cdot$kg). 2. The mercury levels in maternal organs were reduced 6.2% and 47.2% (p<0.05) in kidney, 8.2% and 42.1% (p<0.05) in spleen, 9.7% and 40.9% (p<0.05) in blood, 35.6% (p<0.05) and 67.2% (p<0.05) in liver, 38.0% (p<0.05) and 57.6% (p<0.05) in brain, by treating with garlic (0.5 g/wt$\cdot$kg and 1.0 g/wt$\cdot$kg). 3. The mercury levels were reduced 22.4% and 44.3% (p<0.05) in placenta, and 34.7% (p<0.05) and 54.9% (p<0.05) in fetal body, by treating with garlic (0.5 g/wt$\cdot$kg and 1.0 g/wt$\cdot$kg). 4. The mercury levels in fetal organs were reduced 17.5% and 46.7% (p<0.05) in kidney, 15.1% and 37.0% (p<0.05) in brain, 30.2% (p<0.05) and 46.7% (p<0.05) in liver, by treating with garlic (0.5 g/wt$\cdot$kg and 1.0 g/wt$\cdot$kg). 5. Mercury levels in maternal kidney were 6.73~7.71 times higher than those in fetal kidney, but those in fetal liver and brain were 1.67~2.25 times and 1.98~2.93 times higher than those in maternal liver and brain, respectively. In conclusion, Korean garlic decrease the accumulation of mercury levels in maternal and fetal organs in pregnant Fischer 344 rats as increasing the dose.

Characterization of the Ryanodine Receptor and SERCA in Fetal, Neonatal, and Adult Rat Hearts

  • The mammalian heart is known to undergo significant mechanical changes during fetal and neonatal development. The objective of this study was to define the ontogeny of the ryanodine receptor/$Ca^{2+}$ release channel and SERCA that play the major roles in excitation-contraction coupling. Whole ventricular homogenates of fetal (F) (19 and 22 days in gestation), postnatal (N) (1 and 7 days postnatal), and adult (A) (5 weeks postnatal) Sprague-Dawley rat hearts were used to study [$^3H$]ryanodine binding and oxalate-supported $^{45}Ca^{2+}$ uptake. For the ryanodine receptor, the major findings were: (1) The ryanodine receptor density, as determined by maximal [$^3H$]ryanodine binding ($B_{max}$), increased 3 fold between the F22 and A periods ($0.26{\pm}0.1$ vs. $0.73{\pm}0.07$ pmoles/mg protein, p<0.01), whereas there was no significant change during the F22 and N1 development phases ($0.26{\pm}0.1$ vs. $0.34{\pm}0.01$). (2) Affinity of the ryanodine receptor to ryanodine did not significantly change, as suggested by the lack of change in the $K_d$ during the development and maturation. For SERCA, changes started early with an increased rate of $Ca^{2+}$ uptake in the fetal periods (F19: $8.1{\pm}1.1$ vs. F22: $19.3{\pm}2.2$ nmoles/g protein/min; p<0.05) and peaked by 7 days (N7) of the postnatal age ($34.9{\pm}2.1$). Thus, we conclude that the quantitative changes occur in the ryanodine receptor during myocardial development. Also, the maturation of the $Ca^{2+}$ uptake appears to start earlier than that of the $Ca^{2+}$ release.

몬테카를로 방법을 이용한 임신한 여성 핵의학 종사자의 모체 장기 및 태아선량 평가 (Assessment of Maternal Organs and Fetal Doses in Pregnant Female Nuclear Medicine Practitioners Using the Monte Carlo Method)

  • The purpose of this study was to evaluate maternal organ and fetal doses by week of pregnancy for pregnant women nuclear medicine practitioners in the nuclear medicine field. In addition, we intend to present basic data for the management of exposure doses of female nuclear medicine practitioners. In this study, phantoms of childbearing women, 3, 6, 9 months pregnant women were simulated using MCNPX(Monte Carlo N-Particle Extended) among the Monte Carlo methods. First, volume source was constructed based on 10 cm of the anterior part of the lower abdomen of the phantom, and the organ and fetal doses were evaluated for each week of the pregnant woman according to the type of radioactive isotope. Second, the organ and fetal dose of pregnant women were evaluated by increasing the distance between the source and the abdominal surface by 50 and 100 cm. As a result, 18F sources showed high organ and fetal doses in pregnant women 0 to 3 months, and the dose distribution gradually decreased in 6 to 9 months pregnant women. The distribution of organ and fetal doses for 99mTc and 123I sources showed the same tendency as that of 18F, and the overall absorbed dose distribution was relatively lower than that of 18F. Through this study, it is considered that workers in the early stages of pregnancy within 3 months will need appropriate management to minimize occupational exposure dose.

  • The present report describes a case that showed a normal fetal karyotype in an antenatal genetic study but an abnormal placental karyotype of 46,XX,r (15) on postnatal examination. The pregnancy was complicated by fetal nuchal translucency in the first trimester and intrauterine growth restriction in the second and third trimesters. A 1780 gm female baby was born after 40 weeks of gestation, but died of respiratory distress and sepsis on the 10th day of life. Our case was unique in that the placental chromosomal aberration was a structural abnormality instead of a numerical aberration that is seen in most reported cases of confined placental mosaicism.

Teratological Studies of Ginkgo biloba Extract(EGb 761) in Rabbits

  • A teratological study was performed using New Zealand White rabbits to examine the teratological potential of Ginkgo biloba extract(EGb 761), which is a known strong platelet activating factor antagonist. Ginkgo biloba extract(EGb 761) was administered per intravenously during the organogenesis period (day 6th to 18th of gestation) of rabbits at dose levels of 7.5, 15, and 30 mg/kg/day. All pregnant females were sacrificed on day 29 of gestation and teratological abnormalities of their fetuses was examined. No statistically significant difference of body weight change between control and treated groups during experimental periods was noted. There was no statistically signifiant difference of numbers of corpus lutes and implantations, fetal death ratio, fetal sex ratio, and placental weight between control and rabbits exposed to three different concentration ranges of Ginkgo biloba extract (EGb 761). No marked external, visceral and skeletal abnormalities related to Ginkgo biloba extract(EGb 761) were observed in the fetuses. In conclusion Ginkgo biloba extract(EGb 761) does not show any effect on implantation or embryonic development.
