Noninvasive fetal RHD genotyping using cell-free fetal DNA incorporating fetal RASSF1A marker in RhD-negative pregnant women in Korea

  • Purpose: Conventional methods for the prenatal detection of fetal RhD status involve invasive procedures such as fetal blood sampling and amniocentesis. The identification of cell-free fetal DNA (cffDNA) in maternal plasma creates the possibility of determining fetal RhD status by analyzing maternal plasma DNA. However, some technical problems still exist, especially the lack of a positive control marker for the presence of fetal DNA. Therefore, we assessed the feasibility and accuracy of fetal RHD genotyping incorporating the RASSF1A epigenetic fetal DNA marker from cffDNA in the maternal plasma of RhD-negative pregnant women in Korea. Materials and Methods: We analyzed maternal plasma from 41 pregnant women identified as RhD-negative by serological testing. Multiplex real-time PCR was performed by amplifying RHD exons 5 and 7 and the SRY gene, with RASSF1A being used as a gender-independent fetal epigenetic marker. The results were compared with those obtained by postnatal serological analysis of cord blood and gender identification. Results: Among the 41 fetuses, 37 were RhD-positive and 4 were RhD-negative according to the serological analysis of cord blood. There was 100% concordance between fetal RHD genotyping and serological cord blood results. Detection of the RASSF1A gene verified the presence of cffDNA, and the fetal SRY status was correctly detected in all 41 cases. Conclusion: Noninvasive fetal RHD genotyping with cffDNA incorporating RASSF1A is a feasible, reliable, and accurate method of determining fetal RhD status. It is an alternative to amniocentesis for the management of RhD-negative women and reduces the need for unnecessary RhIG prophylaxis.

  • 현재 사용되고 있는 침습적 산전진단법(양수천자, 융모막샘플링)은 1-2%의 태아 손실이 초래되어, 비침습적 산전진단법이 산전진단의 궁극적인 목표로 대두되어 왔다. 1997년 Dr. Lo에 의해서 임신부 혈장 내에 세포 유리 태아 DNA (cffDNA)의 존재가 발견된 후 비침습적 산전진단의 새로운 가능성이 열렸으며, 과거 10년간 이에 대한 연구의 많은 진전을 보여주고 있다. 최근에 cffDNA를 이용한 Hemophilia A와 듀센형 근이영양증 등 반성 유전병(X-linked disorders) 진단에 필수적인 산전태아의 성 판정과 RhD-음성 임신부에서 태아의 RhD유전자 핵형 분석 등이 이미 외국에서 임상적으로 적용되고 있으나, 한국에서는 아직 실용화되지 않고 있다. CffDNA의 임상 사용에는 여전히 많은 제약점이 있으며, 이는 임신부 혈장 내 cffDNA 양에 비해 많은 양의 모태 DNA가 존재하고, 종래에 사용되었던 특이적인 Y염색체 유전자(Y-specific gene)는 남아 태아 임신 시에만 적용된다는 것에 기인한 다. 따라서 모든 태아에 적용할 수 있는 태아 성과 무관한 마커(sex-independent universal fetal marker as internal positive controls)가 요구되며, 이를 이용하여 정확한 태아 DNA를 검출할 수 있다. 본 연구진은 국내 처음으로 임신부 혈장 내에 cffDNA를 이용하여 SRY 유전자, RhD-exon 7, 태아 성과 무관한 DNA마커(universal fetal DNA marker)로써 RASSF1A 유전자를 실시간 중합효소연쇄반응(RT- PCR)을 사용하여 뛰어난 결과를 얻었다. 이는 한국에서 처음으로 성공적으로 시도된 것이다. 연구결과에서 산전 태아 성 판별과 산후 태아의 성이 100% 일치하였으며, 임신 주기별 SRY 수치는 임신이 진행할수록 증가함을 확인할 수 있었다. 따라서 이러한 방법은 혈우병 A, 듀센형 근이영양증, 선천성 부신증식증과 연골 무형성증의 진단과 치료 상담에 이용할 수 있으며 50%에서 침습적인 방법을 줄일 수가 있다. 또한, RhD-음성 임신부 대상으로 태아의 성 판정과 RhD 태아 유전자형을 분석한 결과 RhD-음성 태아를 정확히 검출함으로써 앞으로 기존 양수천자 등 침습적 검사를 대체할 수 있을 것이다. 특히 이는 치료가 필요 없는 RhD-음성 태아에서 RhD-면역글로불린의 예방적 치료를 사전에 막을 수 있어, 임신부 건강을 보호하고 의료 비용을 줄일 수 있는 큰 장점을 가진다. 한국에서 최초로 시도된 임신부 혈장 내 cffDNA를 이용한 본 연구의 성공은 비침습적 산전진단 임상 적용의 새 길을 제시하였다. 따라서 이를 각 유전질환의 산전진단에 유용하게 활용하는 것은 태아와 임신부의 건강 증진과 의료비용 절약 등 개인과 국가에 많은 기여를 할 것으로 사료된다.

Determination of the Genital Structures using Ultrasound in Canine Prenatal Fetuses

  • The objective of this study was to evaluate the initial detection time and development of the fetal genital structures using ultrasound in twelve pregnant small bitches. The initial detection time of the fetal genital structures was as follows: genital tubercle at days 32.6; os penis at days 45.2; labia at days 45.7; scrotum at days 47.5. Ultrasonograms of fetal genital structure according to gestational stage were as follows: Undifferentiated stage (before day 35), the genital tubercle was observed to have a small elevation and just a hyper-echogenic structure in the midline between the umbilical cord and the tail in male and female fetus. Migration stage (between day 35~45), the genital tubercle was observed as a hyper-echogenic, bilobular, oval shaped and the genital tubercle began to migrate from the initial position toward the umbilical cord in males, and toward the tail in females. Differentiated stage (after day 46), the penis and os penis were observed to stand out in the abdominal wall and the scrotum was observed toward the perineal region in male fetuses. The labia was detected at the base of the tail in female fetuses. These results indicate that ultrasound of fetal genital structures could be useful for fetal gender determination and a completely prepartum evaluation of the canine fetus.

Effective Method for Extraction of Cell-Free DNA from Maternal Plasma for Non-Invasive First-Trimester Fetal Gender Determination: A Preliminary Study

  • 목 적: 모체 혈장으로부터 가장 효과적으로 세포 유리 DNA(cell free DNA, cf-DNA)를 추출하는 방법을 찾기 위해 우리는 viral DNA 추출 방법과 일반 혈액DNA 추출 방법을 이용하여 비침습적 임신 초기 태아 성별 확인 결과를 비교하였다. 대상 및 방법: 임신 초기 44명의 임산부로부터 모여진 모체 혈장을 통한 전향적 연구가 구성되었다. Cf-DNA는 viral DNA 추출 방법과 일반 혈액 DNA 추출 방법을 이용하여 각각 추출되었다. 정량 형광-중합효소 연쇄 반응(QF-PCR)을 이용하여SRY 와AMXY 유전자를 검출하였다. QF-PCR의 진단 정확도는 최종 분만 기록을 토대로 결정하였다. 결 과: 전체 44명의 여성이 실험에 참여하였지만, 최종 분만 기록은 단지 36명의 여성에서 획득하였다. 이들 중 16명은 남아를 20명은 여아를 임신하였다. 두 추출 방법에서 태아 성별의 진단적 정확도는 일반 혈액 DNA 추출 방법에 경우 63.9% (23/26)였으며 viral DNA 추출 방법에 경우 97.2% (35/36) 였다. 결 론: QF-PCR을 이용한 비침습적 임신초기 태아 성별 확인에 있어 viral kit를 사용하는 것이 높은 진단적 정확도를 이끌 수 있을 것으로 사료된다.

초산모와 경산모의 모성역할 자신감에 대한 영향요인 비교 (Comparative Study on Predictors of Maternal Confidence between Primipara and Multipara)

  • Purpose: This study was done to investigate the predictors of maternal confidence between primipara and multipara mothers. Methods: The participants enrolled in this study were 145 mothers between 2 and 5 days postpartum. Data were collected using a questionnaire and analyzed using descriptive analysis, t-test, $X^2$ test, ANOVA, Pearson correlation coefficients, and stepwise multiple regression. Results: The predictors of maternal confidence among primipara mothers were social support, self-esteem, mode of delivery ($R^2$=.287, p<.001). Whereas, gender of baby, antenatal fetal attachment, and educational status explained maternal confidence among multipara mothers ($R^2$=.270, p<.001). Conclusion: The results indicate that when nurses develop programs to encourage maternal confidence for new mothers, they should take into account the differences between predictors of maternal confidence according to the mothers' experience of birth.

에스트로겐이 진피섬유아세포의 증식 및 교원질합성에 미치는 영향의 다양성 (Variable Effect of Estrogen on Fibroblast Proliferation and Collagen Synthesis by Gender and Age)

  • It was assumed that the effect of estrogen on wound healing would be variable according to patient's gender and age since estrogen is a sex steroid. This study was designed to determine the variability of the effect of estrogen on proliferation of human dermal fibroblasts and collagen synthesis which are most important in wound healing considering patient's gender and age. Fibroblasts were isolated from the dermis of female patients in premenstrual, menstrual, or postmenopausal age group and that of male patients. The isolated fibroblasts were cultivated in the presence of estrogen($1.0{\mu}g/ml$). The cells were seeded at $5.0{\times}10^3cell/well$ in Dulbecco's Modified Eagle's Medium/Ham's F-12 nutrient including 5% fetal bovine serum in 96-well plates. The cells were incubated for 3 days. For fibroblast proliferation MTT assay method was used. To measure the production of collagen, the collagen type I carboxy- terminal propeptide enzyme immunoassay was carried out. Estrogen stimulated the proliferation of fibroblasts in female patients, but not in male patients. The greatest cell proliferation and collagen synthesis was seen at women in menstrual and postmenopausal age. These results demonstrated that effects of estrogen on dermal fibroblast proliferation and collagen synthesis were variable with gender and age.

제대혈 Adiponectin과 IGF-I이 태아성장에 미치는 영향 (Correlation of Cord Serum Adiponectin and IGF-I with Fetal Growth in Healthy Term Neonates)

  • 목 적 : 태아 시기의 성장이 출생 이후 성인기에 이르기까지 대사 질환 및 심혈관 질환, 제2형 당뇨병 등의 발현에 영향을 미친다는 보고들이 있어왔다. 이에, 정상 체중 만삭아의 제대혈 내 APN 과 IGF-I 농도와 태아 성장 간의 관계에 대해서 알아보고자 하였다. 방 법 : 2003년 10월부터 2005년 3월 사이에 이대목동병원에서 출생한 정상 체중 만삭아 72명을 대상으로 하였다. 대상아들의 제대혈 내 APN과 IGF-I 농도를 ELISA 방법을 이용하여 측정하였고 측정치와 출생체중, 신장, 머리둘레, 성별, ponderal index, 태반 무게, 태아-태반 무게 비, 산모의 임신 중 체중 증가, 산모의 임신 전 BMI와의 관계를 분석하였다. 결 과: 제대혈 평균 APN 농도는 29.2${\pm}$10.46 $\mu$g/mL 였으며, 제대혈 APN 농도와 체중의 관계는 체중이 증가할수록 APN 농도가 증가하다가 일정 체중에 도달한 이후에는 APN이 감소하는 종모양의 관계를 보였다. 제대혈 APN 농도는 여아에서 남아 보다 높았으며(P=0.001), 제대혈 APN 농도와 산모의 BMI는 유의한 음의 상관관계를 보였다(r=-0.301, P=0.027). 평균제대혈 IGF-I 농도는 51.26${\pm}$21.54 ng/mL 였다. 제대혈 IGF-I 농도는 출생체중과 양의 상관관계를 보였다(r=0.312, P=0.009). 결 론: 건강한 만삭아에서 제대혈 APN 농도는 출생 체중에 따라 증가하다가 일정 체중에서부터 감소하는 패턴의 종모양의 관계를 보였다. IGF-I 은 태아 성장, 특히 출생체중과 밀접한 관계가 있었다.

Characterization of gender-specific bovine serum

  • Animal cell cultures generally require a nutrient-rich medium supplemented with animal serum. Adult bovine serum contains a variety of nutrients including inorganic minerals, vitamins, salts, proteins and lipids as well as growth factors that promote animal cell growth. To evaluate the potential use of gender-specific bovine serum (GSBS) for cell culture, the biochemical properties of male serum (MS), female serum (FS) and castrated-male serum (CMS) were investigated. Overall, the chemical profile of GSBS was similar to that of bovine references except for glucose, creatine kinase, lactate dehydrogenase and potassium. FS showed elevated total protein and sodium concentrations compared to MS and CMS. Proteins present in MS, FS and CMS but absent in fetal bovine serum (FBS) were selected by two-dimensional gel electrophoresis and identified by peptide mass fingerprinting. Some of the identified proteins are known to be involved in immune responses and the others have unknown physiological roles. Moreover, it was found that some proteins such as alpha-2-macroglobulin appeared to be gender-specific with higher contents in FS. Insulin and testosterone was significantly higher in MS, and $17{\beta}$-estradiol and estrone were higher in FS, as compared to the other sera. Taken together, the results indicate that each GSBS has a different ratio of components. Differences in serum constituents may affect cell cultures in a different manner and could be beneficial, depending on the specific aim of cell cultures.

치위생(학)과와 보건계열 대학생들의 생명 의료윤리 의식에 관한 융합연구 (A Convergence Study on the Consciousness of Bio-medical Ethics of Dental Hygiene and Health Science related Majoring Students)

  • 본 연구는 치위생(학)과와 보건계열 학생들을 대상으로 2018년 6월에서 7월까지 생명윤리의식을 파악하여 생명윤리 교육과정 개발에 필요한 기초자료를 제공하고자 설문조사하였다. 생명권과 인공수정 인식도의 관한 상관관계는 성별(r=0.241), 종교(r=-0.176), 학과(r=-0.160)에서 유의하였고, 장기기증 및 장기이식에 관한 인식도에 관한 상관관계는 학년(r=0.129), 종교(r=-0.098), 생명윤리 교육여부(r=0.235)에서 유의하였다. 안락사의 인식도에 관한 상관관계는 종교(r=0.139), 생명윤리교육 여부(r=-0.157)에서 유의하였다. 결과적으로 보건계열 대학생들에게 바람직한 윤리관을 확립하기 위해서는 교육과정을 개발하고 생명의료 윤리의식에 영향을 미치는 요인을 다각적으로 분석하여 체계적으로 교육 프로그램을 운영해야 할 것이다.

Stem Cells and Cell-Cell Communication in the Understanding of the Role of Diet and Nutrients in Human Diseases

  • The term, "food safety", has traditionally been viewed as a practical science aimed at assuring the prevention acute illnesses caused by biological microorganisms, and only to a minor extent, chronic diseases cause by chronic low level exposures to natural and synthetic chemicals or pollutants. "food safety" meant to prevent microbiological agents/toxins in/on foods, due to contamination any where from "farm to Fork", from causing acute health effects, especially to the young, immune-compromised, genetically-predisposed and elderly. However, today a broader view must also include the fact that diet, perse (nutrients, vitamins/minerals, calories), as well as low level toxins and pollutant or supplemented synthetic chemicals, can alter gene expressions of stem/progenitor/terminally-differentiated cells, leading to chronic inflammation and other mal-functions that could lead to diseases such as cancer, diabetes, atherogenesis and possibly reproductive and neurological disorders. Understanding of the mechanisms by which natural or synthetic chemical toxins/toxicants, in/on food, interact with the pathogenesis of acute and chronic diseases, should lead to a "systems" approach to "food safety". Clearly, the interactions of diet/food with the genetic background, gender, and developmental state of the individual, together with (a) interactions of other endogenous/exogenous chemicals/drugs; (b) the specific biology of the cells being affected; (c) the mechanisms by which the presence or absence of toxins/toxicants and nutrients work to cause toxicities; and (d) how those mechanisms affect the pathogenesis of acute and/or chronic diseases, must be integrated into a "system" approach. Mechanisms of how toxins/toxicants cause cellular toxicities, such as mutagenesis; cytotoxicity and altered gene expression, must take into account (a) irreversible or reversal changes caused by these toxins or toxicants; (b)concepts of thresholds or no-thresholds of action; and (c) concepts of differential effects on stem cells, progenitor cells and terminally differentiated cells in different organs. This brief Commentary tries to illustrate this complex interaction between what is on/in foods with one disease, namely cancer. Since the understanding of cancer, while still incomplete, can shed light on the multiple ways that toxins/toxicants, as well as dietary modulation of nutrients/vitamins/metals/ calories, can either enhance or reduce the risk to cancer. In particular, diets that alter the embryo-fetal micro-environment might dramatically alter disease formation later in life. In effect "food safety" can not be assessed without understanding how food could be 'toxic', or how that mechanism of toxicity interacts with the pathogenesis of any disease.
