Placental Superoxide Dismutase, Genetic Polymorphism, and Neonatal Birth Weight

  • Background : The roles of antioxidants in the placenta and genetic susceptibility to oxidant chemicals in relation to neonatal birth weight have not been elucidated. We determined whether the level of placental manganese superoxide dismutase (MnSOD) and its genetic polymorphism plays any role in oxidative stress and neonatal birth weight. Methods : We measured placental MnSOD and determined MnSOD genetic polymorphism among 108 pregnant women who were hospitalized for delivery and their singleton live births in Korea. Main outcome measurements are maternal urinary malondialdehyde (MDA) and birth weight. Results : Maternal urinary concentrations of MDA were significantly associated with neonatal birth weight (P=0.04). The enzyme level of placental MnSOD was also significantly associated with MDA concentration (P=0.04) and neonatal birth weight (p<0.01). We observed dose-response relationships between placental MnSOD and maternal urinary MDA, and neonatal birth weight after adjusting for maternal weight, height, age, and neonatal sex. After controlling for covariates, MnSOD variant genotype increased maternal urinary MDA concentrations (p<0.01) and reduced birth weight by 149 gm (P=0.08). Conclusions : This study demonstrates that the placental level of MnSOD during pregnancy significantly affects fetal growth by reducing oxidative stress, and that genetic polymorphism of MnSOD probably modulate the effects of oxidants on fetal growth.

Impact of dental imaging on pregnant women and recommendations for fetal radiation safety: A systematic review

  • Purpose: This study was conducted to investigate the safety of dental imaging in pregnant women with respect to fetal health. Materials and Methods: Searches were conducted of the PubMed, Scopus, and Web of Science databases in May 2023. The inclusion criteria encompassed cross-sectional and longitudinal studies that focused on the analysis of diagnostic dental imaging in pregnant women, as well as studies utilizing phantoms to simulate imaging examinations. The exclusion criteria consisted of reviews, letters to the editor, book chapters, and abstracts from scientific conferences and seminars. Results: A total of 3,913 articles were identified. Based on a review of the titles and abstracts, 3,892 articles were excluded, leaving 21 articles remaining for full-text review. Of these, 18 were excluded, and 4 additional articles were included as cross-references. Ultimately, 7 articles underwent quantitative-qualitative analysis. Three retrospective studies were focused on pregnant women who underwent dental imaging procedures. The remaining 4 studies utilized female phantoms to simulate imaging examinations and represent the radiation doses absorbed by the uterus or thyroid. Conclusion: Few dental radiology studies have been conducted to determine the safe radiation threshold for pregnant women. Additionally, the reviewed articles did not provide numbers of dental examinations, by type, corresponding to this dose. Dental imaging examinations of pregnant women should not be restricted if clinically indicated. Ultimately, practitioners must be able to justify the examination and should adhere to the "as low as diagnostically acceptable, being indication-oriented and patient-specific" (ALADAIP) principle of radioprotection.

Fetal safety of medicinal herbs and food ingredients during pregnancy: Recommendations from traditional Korean medicine based on expert opinions

  • Objectives: This study aimed to establish and provide reliable information for general public, based on expert consensus, on the risks of misuse of medicinal herbs for food and pure food ingredients for the fetus during pregnancy. Methods: A panelist of seven traditional Korean medicine (TKM) gynecologists responded to a questionnaire summarizing the fetal safety literature for twenty-five medicinal herbs for food and pure food ingredients derived from medicated diet (藥膳, Yaksun) recipes during three online Delphi rounds anonymously. Results: Ginkgonis Semen (Ginkgo nut), Illici Veri Fructus (Star anise), lavender, bitter gourd, and parsley were agreed at the level 1 of "Do not consume". These five ingredients were recognized as having significant risks both in the literature evidence and in expert opinion. Rosemary, Citri Unshius Pericarpium, Discoreae Rhizoma, lemongrass, Schisandrae Fructus, Cassiae Semen, Foeniculi Fructus, Mori Fructus, Cinnamomi Cortex, and Astragali Radix were agreed at the level 2 of "consultation with TKM practitioner is required". Conclusion: Based on the consensus of a seven-member expert panel of TKM gynecologists, consumption of Ginkgonis Semen (Ginkgo nut), Illici Veri Fructus (Star anise), lavender, bitter gourd, and parsley should be avoided by pregnant women. For Rosemary, Citri Unshius Pericarpium, Discoreae Rhizoma, lemongrass, Schisandrae Fructus, Cassiae Semen, Foeniculi Fructus, Mori Fructus, Cinnamomi Cortex, and Astragali Radix, the level 2 advisory may be recommended to use with caution and to consult a TKM practitioner for advice on consumption, dose, and duration.

MC3T3-E1 세포의 골기질 단백질 발현에 대한 혈소판유래성장인자-BB의 효과 (The Effects of Platelet- Derived Growth Factor-BB on the Expression of Bone Matrix Protein in the MC3T3-E1 Cells)

  • Bone remodeling results from the combined process of bone resorption and new bone formation which is regulated in part by some of the polypeptide growth factors such as platelet derived growth factor(PDGF), which has been known to be an important local regulator of bone cell activity and participate in normal bone remodeling. This process includes strictly regulated gene expression of several bone matrix proteins such as type I collagen and osteopontin, a 44 kDa phosphorylated glycoprotein, which has important roles in bone formation. The purpose of this study is to evaluate the effecs of PDGF-BB on the mRNA expression of bone matrix protein, type I collagen and osteopontin, in MC3T3- E1 cell culture. Cells were seeded at $5{\times}10^5$ cells in 10 ml of minimum essential medium alpha(${\alpha}-MEM$) containig 10% fetal bovine serum, 10 mM beta glycerophosphate. 0.1, 1, 10 ng/ml PDGF-BB were added to the cells for the day 3, 7, 14, 21, 28 and cultured for 24 hours. Type I collagen cDNA, Hf677, and osteopontin cDNA were used as probes for northern blot analysis. Total cellular RNA was purified at indicated day and northern blot analysis was performed. The results were as follows : Type I collagen mRNA expressions were higher at the day 3 and 7, and lower in the day 14, 21 in the control groups. In the experimental groups, mRNA expressions were increased when 0.1 ng/ml PDGF-BB were added on the day 3, 7, 21, and decreased in dose-dependent manner on the day 14, decreased at all added dose on the day 28. Osteopontin mRNA expressions were highest in the day 21 groups and lowest in the day 14 groups in the control groups. Interesting results were shown in the day 14 and 21 groups. We found that osteopontin mRNA level was increased in dose dependent manner in the day 14 groups, and decreased dose dependent manner in the day 21 groups. In conclusion, PDGF-BB may have various control effects on type I mRNA expression in the growth and differentiation process of MC3T3-E1 cells and may have contrary regulatory effects on osteopontin mRNA expression. For examples, when the baseline level of osteopontin mRNA was low, as in the day 14, PDGF-BB up-regulated osteopontin mRNA expression in dose dependent manner, and when the baseline level was high as in the day 21, PDGF-BB down-regulated dose dependent manner. Thus, it may be useful for clinical application in periodontal regeneration procedure if further study were performed.

불소투여에 따른 태내백서 치아의 생화학적 및 주사전자현미경적 연구 (Biochemical and Scanning Electron Microscopic Study on the Enamel Organ of Fetal Rat following a Ingestion of Fluoride)

  • 임신중에 투여된 불소가 흰쥐태아의 법랑질형성에 미치는 영향을 알아보고자 어미흰쥐에게 불소를 음용 시킨 후, 생후 11일이 경과된 어린 흰쥐를 희생하였다. 이후, 하악절치를 발치한 후, 법랑단백질의 종류 및 양적 변화와 법랑질 표면의 형태학적 변화를 관찰하였다. 분비법랑질과 성숙법랑질에서 추출한 법랑단백질을 전기 영동한 결과, 분비법량질에서는 분자량 $22\sim24kDa$의 amelogenin이 확인되었는데, 대조군에서보다 불소투여 농도가 높아질수록 양이 감소하였다. 성숙법랑질에서는 분자량이 68kDa의 enamelin이 확인되었으며, 대조군에 비해 불소투여 농도가 높을수록 양적인 증가가 확인되었다. 그리고 주사전자현미경을 사용하여 법랑질 표면을 관찰한 결과, 대조군에서는 평탄하고 매끄러운 표면인 반면에 불소투여군은 거친 표면과 균열이 심하게 나타났고, 불규칙한 소공이 관찰되었다. 그리고 일부에서는 균열과 함께 주머니 같은 결절과 법랑질 형성부전을 확인하였다. 따라서 본 연구결과, 태아발생과정 중 투여된 불소가 법랑질형성에 관여하는 법랑모세포에 영향을 줌으로써 단백질의 형성과 분비가 지연 또는 억제되며, 수분과 단백질 제거에 관여하는 법랑모세포의 기능을 저하시키는 것으로 사료된다. 이러한 단백질의 비정상적인 형성과 분비는 법랑질 형성부전 등의 법랑질형성에 전반적으로 영향을 미치는 것으로 확인되었다.

임신 ddy Mice에서 착상전기 방사선 조사에 따른 산전 사망 및 외부 기형 발현 (Prenatal Deaths and External Malformations Caused by X-Irradiation during the Preimplantation Period of ddy Mice)

  • 목적 : 착상전기의 진단 및 치료 영역의 방사선 조사가 임신 ddy mouse에서 산전 사망과 외부 기형을 유발하는지, Compaction전 시기의 반응에 시기별 차이가 있는지 그리고 계통에 따르는 차이가 있는지 여부를 알아 보고자 하였다. 대상 및 방법 : 임신 ddy mouse 대조군 32 마리와 실험군 53 마리를 대상으로 연구를 시행하였으며, 실험군에 대한 방사선 조사는 착상전의 중요한 두 시기인 24 h p.c.와 48 h p.c.에 진단 영역에서도 이용되는 방사선 선량인 0.1, 0.5 Gy를 포함하여 0.75, 1.5, 3 Gy를 조사한 후, 임신 18일에 희생시켜 산전 사망 즉 착상전 사망, 배 사망 및 태아 사망과 외부 기형을 관찰하였다. 결과 : 1) 착상전 사망은 24 h p.c. 및 48 h p.c.에서 대조군에 비해 현저하게 많이 발생하였으며 선량 의존성을 나타냈고, 시기별 한계 선량은 각각 0.05 Gy 및 0.075 Gy이상으로 24 h p.c.가 48 h p.c.보다 방사선에 대한 감수성이 높은 시기임을 알 수 있었다. 2) 배 사망은 48 h p.c.의 0.1 Gy 조사군을 제외한 24 h p.c. 및 48 h p.c.의 모든 조사군에서 대조군에 비해 많이 발생하였고 선량 의존성을 보였으며, 한계 선량은 각각 0.1 Gy 및 0.25 Gy이상으로 24 h p.c.가 48 h p.c.보다 방사선 조사에 의한 배 사망의 감수성이 높았다. 3) 태아 사망은 24 h p.c. 및 48 h p.c.의 실험군 모두에서 발생하지 않았다. 4) 외부 기형은 24 h p.c. 실험군에서 뇌노출 기형 2예, 안검결손 기형 3예, 무안구증 3예, 구개열 2예, 복벽 파열 2예, 꼬리 기형 2예 및 다리 기형 1예, 국소 복벽 결손 1예 등이 발생하였는데, 그 중 진단 영역의 방사선 선량인 0.1 Gy군에서 안검결손 기형 1예, 복벽 파열 1예, 0.5 Gy군에서 뇌노출 기형 1예, 꼬리 기형 2예 및 다리 기형 1예가 발생하여 대조군에 비해 통계적으로 유의한 증가를 나타냈으며 이 시기의 한계선량은 0.2 Gy이상이었다. 48 h p.c.군에서도 안검결손 기형 1예, 꼬리 기형 2예가 발생하였으나 대조군과 통계적인 유의차가 없었다. 결론 : 이상의 결과를 통해 치료 영역뿐만 아니라 진단 영역의 방사선 조사로도 착상전기 임신 ddy mouse에서 착상전 사망 및 배 사망이 발생하고 24 h p.c.에서는 기형도 유발되어 이 시기의 방사선 영향이 "all-or-none" 반응만 일어나는 것이 아님을 알 수 있었으며, 24 h p.c.가 48 h p.c.보다 방사선 감수성이 높은 시기라는 사실과 함께 다른 연구 결과들과 비교하여 계통에 따르는 차이가 있음을 알 수 있었다. 그러므로 가임기 여성의 방사선 진단 및 치료시 Rugh의 10일 법칙을 적용하여 착상전기 방사선 조사로 인한 부작용들을 적극적으로 예방하는 것이 매우 중요하다고 생각한다.

Pregnancy outcomes following the administration of high doses of dexamethasone in early pregnancy

  • Objective: In the present study, we aimed to evaluate the effects of high doses of dexamethasone (DEX) in early pregnancy on pregnancy outcomes. Methods: Pregnant BALB/c mice were treated with high-dose DEX in the experimental group or saline in the control group on gestational days (GDs) 0.5 to 4.5. Pregnant mice were sacrificed on GDs 7.5, 13.5, or 18.5 and their peripheral blood, placentas, fetuses, and uterine tissue were collected. Decidual and placenta cell supernatants were examined to evaluate the effect of DEX on the proliferation of mononuclear cells, the quantity of uterine macrophages and uterine natural killer (uNK) cells, and levels of progesterone and $17{\beta}-estradiol$, as determined by an 3-(4,5-dimethylthiazole-2-yl)-2,5-diphenyltetrazolium bromide assay, immunohistochemistry, and enzyme-linked immunosorbent assay, respectively. We also were measured fetal and placental growth parameters on GD 18.5. Results: We found that high doses of DEX were associated with an increased abortion rate, enhancement of the immunosuppressive effect of the decidua, alterations in placental growth parameters, decreased progesterone and $17{\beta}-estradiol$ levels, and a reduced frequency of macrophages and uNK cells. Conclusion: Our data suggest that the high-dose administration of DEX during early pregnancy negatively affected pregnancy outcomes.

행인(杏仁)이 천식관련 chemokine 분비와 호산구 chemotaxis에 미치는 영향 (Armeniacae Amarum Semen Contributes to the Chemotaxis of Eosinophils and Secretion of Chemokines in A549 Human Epithelial Cells)

  • Objective: Eosinophils are typically characterized by a bilobar nucleus with highly condensed chromatin and cytoplasm containing two major types of granules, specific and primary granules, and lipid bodies. The role of inflammation in asthma and other allergic diseases of the airways is widely appreciated, and airway inflammation is now included as a defining feature of asthma. The importance of the presence of eosinophils in the airways of patients with fetal asthma has long been recognized, but the mechanism by which these cells are recruited and retained in the lungs are only now being elucidated. Eotaxin is a potent and specific eosinophil chemoattractant that is mobilized in the respiratory epithelium after allergic stimulation. Methods : Water extracts of Armeniacae Amarum Semen(AAS) and pulmonary epithelial cell lines A549(alveolar typeII epithelial cells) and human eosinophils were used. Cytotoxic effects of AAS and MIS assay were estimated, as well as the effects of AAS on chemokines from prestimulated A549 cells by sandwich ELISA and RI-PCR. Chemotaxis assay was conducted on prestimulated eosinophils treated with AAS. Results : In this study it is demonstrated that $TGF-{\alpha}$, IL-4 and $IL-1{\beta}$ induced the accumulation of chemokine mRNAs in the alveolar epithelial cell lines A549 in dose-dependent manner. Eotaxin and IL-8 were inhibited by AAS in dose-dependent manner(p<0.05). Eosinophil migration was inhibited at high concentrations of AAS(p<0.05). Conculusions : These findings are indicative of suppression of eotaxin and IL-8, and suggest that this is accomplished through AAS treatment. This raises the possibility that AAS is of therapeutic value in diseases such as asthma.

Eotaxin is a potent and specific eosinophil chemoattractant that is mobilized in the respiratory epithelium after allergic stimulation. Material and Methods: We used water extracts of Liripois Tuber and pulmonary epithelial cell lines A549(human typeII-like epithelial cells) and human eosinophils. We estimated cytotoxic effects of Liripois Tuber via MTS assay, and estimated the effects of Liripois Tuber on chemokines from prestimulated A549 cells by sandwich ELISA and RT-PCR. We conducted chemotaxis assay on prestimulated eosinophils treated with Liripois Tuber. Result: In this study we demonstrated that $TNF-{\alpha}$, IL-4 and $IL-1{\beta}$ induced the accumulation of chemokines mRNA in the pulmonary epithelial cell lines A549 in dose-dependent manner. Chemokines were inhibited by Liripois Tuber in dose-dependent manner. The eosinophil migration was inhibited at high concentration of Liripois Tuber. Conculusion: These findings indicate that the supression of the expression of chemokines can be accomplished by Liripois Tuber treatment, raising the possibility that Liripois Tuber might be of therapeutic value in diseases such as asthma.

Inhibition of Inducible Nitric Oxide Synthase Attenuates Monosodium Urate-induced Inflammation in Mice

  • The present study elucidated the effect of the selective inducible nitric oxide synthase (iNOS) inhibitor $N^6$-(1-iminoethyl)-L-lysine (L-NIL) on monosodium urate (MSU) crystal-induced inflammation and edema in mice feet. L-NIL (5 or 10 mg/kg/day) was administered intraperitoneally 4 h before injection of MSU (4 mg) into the soles of mice hindlimb feet. Twenty-four hours after MSU injection, foot thickness was increased by 160% and L-NIL pretreatment reduced food pad swelling in a dose dependent manner. Pretreatment of 10 mg/kg/day L-NIL significantly suppressed the foot pad swelling by MSU. Plasma level of nitric oxide (NO) metabolites and gene expression and protein level of iNOS in feet were increased by MSU, which was suppressed by L-NIL pretreatment. Similar pattern of change was observed in nitrotyrosine level. MSU increased the gene expression of tumor necrosis factor (TNF)-${\alpha}$ and interleukin (IL)-$1{\beta}$ and L-NIL pretreatment suppressed MSU-induced cytokines expression. The mRNA levels of superoxide dismutase and glutathione peroxidase1 were increased by MSU and L-NIL pretreatment normalized the gene expression. Phosphorylation of extracellular signal-regulated kinase 1/2 and p38 was increased by MSU, which was suppressed by L-NIL pretreatment. The mRNA levels of iNOS, TNF-${\alpha}$, and IL-$1{\beta}$ were increased by MSU in human dermal fibroblasts, C2C12 myoblasts, and human fetal osteoblasts in vitro, which was attenuated by L-NIL in a dose dependent manner. This study shows that L-NIL inhibits MSU-induced inflammation and edema in mice feet suggesting that iNOS might be involved in MSU-induced inflammation.
