알코올 섭취가 임신부와 태아의 건강상태에 미치는 영향 (The Effect of Alcohol on health status of pregnant women and fetus)

  • In modern society, the amount of alcohol ingestion is increasing at a dangerous level, especially among women. One of reason for increased alcohol consumption is stress caused by social pressures. Alcohol is a kind of depressant of centric nervous system, so it can induce relaxation of body and decrease the stress. The evidence on the effects of alcohol on the fetus is somewhat hazy, whereas that of smoking is quite clear. The literature on the ingestion of alcohol strongly suggests that drinking during pregnancy is associated with teratogenic effect and low birth weight. Therefore, the adverse effect of alcohol ingestion during pregnancy must be informed to public. More remarkable warning sign about alcohol ingestion must be attached on the top of bottles. This can be an effective measure for public education. Also legal sanction or tax imposition for the production of liquor be required. First of all, drinking habit or drinking culture must be changed. In fact, the strongest motivation of drinking in adolescent is a peer pressure which is related to drinking habit or culture. Secondly, early detection and treatment must be required to prevent from fetal alcohol syndrome. Accordingly, drinking history of pregnant women must be assessed as early as possible and health professional should give a warning about the abstinence of alcohol to drinking women. Thirdly, to minimize the adverse effects for mother, withdrawal syndrome by alcohol ingestion must be treated. to correct the malformation by fetal alcohol syndrome(FAS) can be corrected. Sometimes surgical intervention may be required for this purpose.

  • Alcohol drinking during pregnancy can result in abnormal fetal development including fetal alcohol syndrome (FAS). The molecular mechanisms of FAS, however, is not completely elucidated. Association of Fatty Acid Ethyl Esters in Meconium of Neonates with Growth Deficits at Birth: a Prospective, Single-Centre Cohort Study

  • Background: In this prospective cohort study, we investigated the association between fatty acid ethyl esters (FAEEs) in meconium as biomarkers of prenatal ethanol exposure and growth deficits, as birth outcomes, that constitute several of the key cardinal features of fetal alcohol syndrome. Methods: A total of 157 meconium samples were collected from enrolled infants within 24 hours of birth, and nine FAEEs were quantified using liquid chromatography/tandem mass spectrometry. The relationships between cumulative concentrations of nine species of FAEEs in meconium and birth parameters of growth (age-sex-specific centiles of head circumference [HC], weight, and length) and respective and combined birth outcomes of growth deficits (HC ${\leq}10th$ centile, weight ${\leq}10th$ centile, and length ${\leq}10th$ centile) were determined. Results: Multivariate logistic regression analysis demonstrated that higher cumulative concentrations of meconium FAEEs correlated with elevated risks for HC and length, both, 10th percentile or less (adjusted odds ratio [aOR], 2.94; 95% confidence interval [CI], 1.12-7.74; P = 0.029) and HC and weight and length, all of them, 10th percentile or less (aOR, 3.27; 95% CI, 1.12-9.59; P = 0.031). Conclusion: The elevated cumulative FAEEs in meconium were associated with combined growth deficits at birth, specifically HC and length, both, 10th percentile or less, which might be correlated with detrimental alcohol effects on fetal brain and bone development, suggesting a plausible alcohol-specific pattern of intrauterine growth restriction.

운동기술훈련이 태아알코올증후군 흰쥐 모델의 소뇌 발달과 운동기능에 미치는 영향 (The Effect of Motor Skill Training on Motor Function and Cerebellar Development After Alcohol Exposure in Neonatal Rats)

  • The purpose of this study was to test that motor skill training enhance motor function and cerebellar development. Using an animal model of fetal alcohol syndrome-which equates peak blood alcohol concentrations across developmental period-critifical periods for the effect of alcohol on body and cerebellar weigh was examined. The effect of motor skill training on motor function and cerebellar development of rat exposed alcohol on postnatal days 4 through 10 were studied. Newborn rats were assigned to one of two groups: (1) Control group (CG), via artificial rearing to milk formula and (2) experimental groups (EG), via 4.5g/kg/day of ethanol in a milk solution. After completion of the treatments, the pups were fostered back to lactating dams, and wearing they were raised in standard caged until they were postnatal 48 days. Rats from experimental group of postnatal treatment then spent 10 days in one of two groups: Experimental group II (EGII) was had got motor skill training (training traverse a set of 6 elevated obstacles) for 4 weeks. Experimental group I (EGI) was not trained. Before sacrificing, the rat got examined two behavioral test, body weigh and cerebellar weigh, then coronal sections were processed. The section was investigated the Purkije cell in the cerebellum using light microscope. The results of this study were as follows. 1. In body weight test, the outcome of alcohol groups were significantly lower than the normal group. 2. In cerebellar weight test, the outcome of EGI were significantly lower than CG and EGII. 3. In motor behavioral test, the outcome of EGI was significantly lower than NG and EGII. 4. In Purkinje cells counting test, the outcome of EGI was significantly lower than the NG and EGII. These result suggest that improved motor function induced by motor skill training after postnatal exposure is associated with dynamically altered expression of Purkinje cells and that is related with cerebellar function. Also, these data can potentially serve as a model for therapeutic intervention.

Using an animal model of fetal alcohol syndrome - which equates peak blood alcohol concentrations across developmental period - the effects of alcohol on body weight during periods were examined. The effect of complex motor training on motor function and synaptic plasticity of rat exposed alcohol on postnatal days 4 through 10 were studied. Newborn rats were assigned to one of two groups: (1) normal group (NG), via artificial rearing to milk formula and (2) alcohol groups (AG), via 4.5 g/kg/day of ethanol in a milk solution. After completion of the treatments, the pups were fostered back to lactating dams, where they were raised in standard cages (two-and three animals per cage) until they were postnatal 48 days. Rats from alcohol group of postnatal treatment then spent 10 days in one of two groups: Alcohol-experimental group was had got complex motor training (learning traverse a set of 6 elevated obstacles) for 4 weeks. The alcohol-control group was not trained. Before consider replacing with "the experiment/study", (avoid using "got" in writing) the rats were examined during four behavioral tests and their body weights were measured, then their coronal sections were processed in rabbit polyclonal antibody synaptophysin. The synaptophysin expression in the cerebellar cortex was investigated using a light microscope. The results of this study were as follows: 1. The alcohol groups contained significantly higher alcohol concentrations than the normal group. 2. The alcohol groups had significantly lower body weights than the normal group. 3. In alcohol groups performed significantly lower than the normal group on the motor behavioral test. 4. In alcohol-control group showed significantly decreased immunohistochemistric response of the synaptophysin in the cerebellar cortex compared to the nomal group. These results suggest that improved motor function induced by complex motor training after postnatal exposure is associated with dynamically altered expression of synaptophysin in cerebellar cortex and that is related with synaptic plasticity. Also, these data can potentially serve as a model for therapeutic intervention.

임신경과에 영향을 미치는 건강위험행위 (Health Risk Behaviors Affecting the Process of Pregnancy)

  • Comprehensive review of the literature was conducted to determine 1) selected risk factors and its impact that affect pregnancy outcome such as smoking, alcohol consumption, and substance abuse 2) these factors can facilitate future strategies for health promotion and prevention for both pregnant women and fetus. Review of literature were extracted from searching MEDLINE(1966 - Oct. 2000). CINAHL (1982 - Oct. 2000) and the domestic literature. The following factors were identified: 1. The effects of risk behaviors on pregnancy. ${\cdot}$Maternal smoking was associated with the occurrence of premature or LBW delivery, fetal growth retardation, extremities defects, heart defects and sudden infant death syndrome. ${\cdot}$Maternal alcohol consumption was associated with spontaneous abortion, premature or LBW delivery, morphologic/neurologic problems, especially fetal alcohol syndrome. ${\cdot}$Heroin was associated with withdrawal after birth in which were born to heroine addicts for gestational age and lung maturation in animal studies. ${\cdot}$Cocaine was associated with spontaneous abortion, abruptio placenta and a poor response to environmental stimuli. ${\cdot}$So far, the effects of caffeine on pregnancy was controversial, but severe caffeine consumption was associated with premature or LBW delivery, spontaneous abortion, still birth and dystocia. 2. Intervention methods and its effects identified were as follows ${\cdot}$Conducted intervention for smoking, alcohol and drug consumption were single or combined. ${\cdot}$Intervention methods were counseling, phone contact, mailing, use of educational videotape, booklet, support person and alternatives such as nicotine patch. ${\cdot}$The interventions increased the rates of smoking cessation during pregnancy and awareness of the risk of drug consumption, and decreased amount of alcohol consumption. ${\cdot}$The intervention outcome found positive effect on birth weight and length. 3. Our recommendations were as follows ${\cdot}$The personal and social cognition should be enhanced through education and the mass media. ${\cdot}$It's necessary to educate and give information of preconceptional care, planned pregnancy and early prenatal care for optimal pregnancy outcome. ${\cdot}$It's necessary to develop comprehensive assessment tool which is reliable and valid on smoking, alcohol consumption and substance abuse to identify supportive or interventional program.

2H-Gel 이미지의 정렬 및 클러스터링 (Clustering of 2D-Gel images)

  • 2D-Gel 이미지간의 유사성을 기준으로 생물학적인 시료가 프로테옴 수준에서 유사성의 정도와 서로 다른 단백질 스팟을 파악해 낼 수 있다. 그러나 생물학적인 시료는 개체간 변화가 크고 2차원 전기영동장치의 재현성의 한계로 인하여 비교가 어려운 경우가 많고 의미 없는 차이점만 발견되는 경우 또한 비일비재하다. 이를 극복하기 위해서는 프로테옴 이미지간의 정렬을 통하여 정확한 비교가 가능하게 하여야한다. 본 연구에서는 이미지상의 단백질 스팟을 일일이 찾지 않고 여러 개의 원시 이미지를 동시에 정렬시키는 multiresolution-multilevel algorithm을 활용하여 소프트웨어를 개발하였다. 또 이렇게 정렬된 이미지들이 서로 얼마나 유사한지 보여주는 Phylogenetic tree를 자동으로 생성시키는 소프트웨어를 개발하였다. 이 방법을 이용하여 Fetal Alcohol Syndrome의 case와 control의 10개의 프로테옴 이미지에 대하여 클러스터링을 시도하였다. 이와 같이 2D-Gel 프로테옴 전체의 이미지를 비교하여 유사한 정도에 따라 모으는 클러스터링은 FAS 시료의 경우 case와 control 보다는 시료원의 외연적인 특징인 나이 혹은 성별에 더 의하여 의존하는 것으로 나타났다.

  • 1. 이 증례는 Pierre Robin Sequence로 진단받은 14개월 남아로 PRS의 3요소 중 하악왜소증, 설하수증의 소견을 보이고 있었으며 기도확보는 측와위를 통하여, 섭식은 PEG 도관삽입술을 시행하여 유지하고 있었다. 2. PRS는 해부학적 폐쇄 질환이기도 하지만 성장 장애이기도 하므로 여러 분야의 다원적 접근 및 치료계획이 필요하며, 다른 증후군들과 연관되어 심장질환, 신경질환, 안과질환, 청력문제 등이 동반될 수 있으므로 치과 진료실 에서 PRS 환아의 진료시 이 점을 유의해야할 것이다.
