The Effect of Polycystic Ovarian Follicular Fluid on Sperm Motility in Human in vitro Fertilization (인간체외수정시술시 다낭성난포종 난포액이 정자의 운동성에 미치는 영향)

  • Objective: The purpose of this study was to evaluate the effect of polycystic ovarian follicular fluid on sperm motility in human in vitro fertilization (IVF). Methods: From May, 1998 to July, 1999, 55 patients who complained of infertility were involved in this study. We obtained ovarian follicular fluids from the patients by ultrasono-guided aspiration. Subjects were divided into two groups. 20 patients who had polycystic ovarian disease were belong to study group, and 25 patients who had normal ovarian follicular fluid were belong to control group. The follicular fluid dilution was done with Ham's fluid as 10%, 20%, 50%, 100%. The sperm motility was analyzed by CASA at 6hr and 12hr after incubation in follicular fluids. Results: The levels of average path velocity (VAP) in all concentration fluid didn't show significant difference between study and control group. The other parameters including curvilinear velocity (VCL), amplitude of lateral head displacement (ALH), and linerity (LIN) were didn't show any significant difference between both groups. Conclusion: PCOD fluid had seemed to have an adverse effect on the sperm biological function. But, this study showed that PCOD fluid had no different effect on sperm motility with normal follicular fluid.

Efficacy of ablation and sclerotherapy for the management of ovarian endometrioma: A narrative review

  • Ovarian cystectomy is the preferred technique for the surgical management of ovarian endometrioma. However, other techniques such as ablation or sclerotherapy are also commonly used. The aim of this review is to summarize information regarding the efficacy of ablation and sclerotherapy compared to cystectomy in terms of ovarian reserve, the recurrence rate, and the pregnancy rate. Several studies comparing ablation versus cystectomy or sclerotherapy versus cystectomy in terms of the serum anti-Müllerian hormone (AMH) decrement, endometrioma recurrence, or the pregnancy rate were identified and summarized. Both ablation and cystectomy have a negative impact on ovarian reserve, but ablation results in a smaller serum AMH decrement than cystectomy. Nonetheless, the recurrence rate is higher after ablation than after cystectomy. More studies are needed to demonstrate whether the pregnancy rate is different according to whether patients undergo ablation or cystectomy. The evidence remains inconclusive regarding whether sclerotherapy is better than cystectomy in terms of ovarian reserve. The recurrence rates appear to be similar between sclerotherapy and cystectomy. There is not yet concrete evidence that sclerotherapy helps to improve the pregnancy rate via in vitro fertilization in comparison to cystectomy or no sclerotherapy.

Studies on the Improvement of Fertilization and Development Rates Using ICSI with In Vitro Matured Bovine Oocytes (소 체외성숙 난자에 ICSI에 의한 수정율의 향상에 관한 연구)

  • This study was carried out to investigate on the improvement of fertilizing and developing ability of in vitro matured oocytes from sperm density, motility and PVP(polyvinylpyrrolidone), HA(hyaluronic acid) concentrations, sperm capacitation and intact, free-zona of bovine oocytes obtained by intracytoplasmic sperm injection(ICSI). 1. The fertilization and cleavage rates of bovine oocytes obtained by ICSI treated 0.01, 0.02, 0.03, 0.05% of PVP concentrations were 72.7∼90.9% and 38.5∼54.5%, respectively and these values of 0.02% addition of PVP were higher than other concentrations of PVP 2. The fertilization and cleavage rates of bovine oocytes obtained by ICSI treated 0.01, 0.02% HA and 0.02% PVP + HA concentrations were 72.7%, 40.9% and 81.8%, 54.5% and 83.3%, 37.5%, respectively. and these values of 0.02% addition of PVP + HA were lowe than other concentrations of HA. 3. The fertilization and cleavage rates of bovine oocytes obtained by ICSI treated fresh and frozen sperm were 93.3%, 85.7% and 60.0%, 46.7%, respectively and these values of fresh sperm injection were higher than frozen sperm. 4. The fertilization and cleavage rates of bovine oocytes obtained by ICSI capacitated sperm of heparin, BFF and His methods were 86.7%, 78.9%, 65.0% and 61.9%, 52.6%, 50.0%, respectively. 5. The fertilization and cleavage rates of zona-intact and zona-free bovine oocytes obtained by ICSI were 84.2%, 78.3% and 57.9%, 34.8%, respectively. 6. The fertilization and cleavage rates of bovine oocytes obtained by IVF or ICSI were 63.3∼64.6%, 88.2∼90.0% and 26.7∼29.2%, 52.9∼67.5%, respectively. This ICSI method improved high fertilization rates of bovine oocytes.

  • A high molecular ar weight water-soluble chitosan (WSC) with an average molecular weight of 300 kD and a deacethylation level of over 90% was produced using a simple multi-step-membrane separation process. It is known that WSC prevents obesity induced by a high-fat diet. Consequently, this study investigated whether or not WSC improved the ovarian dysfunction caused by obesity in mice. The mice were fed a high density protein and lipid diet for weeks, followed by the administration of WSC at 480 mg/kg body weight per day for 4 days. Thereafter, the changes in body weight, ovulation rate, in vivo and in vitro fertilization and emboryonic development were measured . WSC markedly reduced the body weight of obese mice fed with a high-fat diet, but not in mice fed with a normal diet. WSC had siginificant effects on the ovulation rate, both the in vivo and in vitro fertilization rates and embryonic development. These results indicate an improvement in the ovarian and oviduct dysfunction caused by obesity, and suggest an adjustment in the internal secretions and metabolic functions.

Lived Experience of IVF-ET Program (시험관 아기 시술 체험)

  • Purpose: This study was to identify what experience meant for the wives that attempted IVF-ET(In Vitro Fertilization-Embryo Transfer) program due to the spouse's infertility and provide fundamental materials to improve nursing interventions. Method: Giorgi's phenomenological method was employed, the subjects were five wives who had ever attempted IVF-ET program due to the infertility of their spouses. In-depth interview and observation method were utilized to gather information from April to December 2003. Result: The significant results from analyzing the interviews can be grouped into 34 themes, 8 categories. The essential themes for the experiences of the wives were 'shocked by the unbelievable reality', 'can't give up the connection to the blood', 'Lack of social education on pregnancy, and childbirth', 'self-pity', 'feelings of both families', 'Being afraid of the unexpected result', 'physical and mental agony', 'Positively coping with the reality. Conclusion: The results show that infertility is not a mere personal matter, and infertile people, their families and society should team up with in tackling it. The physical, psychological and social problems triggered by infertility could be ironed out by making both personal and collaborative approaches to that.

Effect of EGF on In Vitro Oocyte Maturation and Embryo Development and Expression of EGF mRNA in Bovine Oocytes and Embryo I. Influence of Cumulus Expression and Maturation and Embryo Development during Bovine Oocyte Maturation In vitro by Addition EGF

  • The objective of this study was to the effect on subsequent development of EGF present in defined medium during bovine 1)oocyte maturation or 2)embryo culture. The presence of EGF during IVM, irrespective of concentration(1, 10, 100ng/$m\ell$), stimulated cumulus expansion and significantly increased the proportion of oocytes attaining metaphaseII, the rate of cleavage, and develop to blastocyst. 1. In the group of EGF-added medium(1, 10, 100ng/$m\ell$), nuclear maturation rate for in vitro maturation was 91% to 92% but was not significantly higher than control group(87%). 2. For in vitro maturation, in the group of EGF-added medium(1, 10, 100ng/$m\ell$)the rate of cumulus cell expansion degree 2 ranged from 81% to 87%, which was significantly higher than the control group(medium with EGF not added). The rate of in vitro fertilization, developing to 2-to 4- cell stage, was 76% to 80%, which was also significantly higher(p<0.05)than control group(62%). 3. For in vitro maturation, in the group of EGF added in medium(1, 10, 100ng/$m\ell$)the development rate to blastocyst was 24.3% to 27%, which was significantly higher than control group(13.7%). The total cleavage rate in the group of EGF-added medium was 77% to 82%, which was higher than control group. 4. The development rate to blastocyst for 6 days of cultivation and the hatching blastocyst were 30.6% and 59.1%, respectively, in the group of 100ng/$m\ell$ of EGF, which were significantly higher(p<0.05)than control group(14.0% and 24%, respectively), The numbers of cells in blastocyst were 140.2 and 148, respectively, in 10ng/$m\ell$ and 100ng/$m\ell$ of EGF-added medium, which were higher than 108.5 in control group. 5. The development rate of in vitro fertilized embryos to blastocyst in 10ng/$m\ell$ of EGF-added medium co-cultured with somatic cell was 28%, which was significantly higher(p<0.05)than control group(11.8%). The numbers of cells in blastocyst were 141.6 for EGF-added medium and 145 for EGF+co-culture group, which were higher than control(101.6)and medium co-cultured with somatic cells(110.6). These results showed that in vitro maturation and fertilization, EGF was found a significant effect of increase of development rate to blastocyst and cell number.
