Treatment of Deformity in Polyostotic Fibrous Dysplasia Using Interlocking Intramedullary Nailing (다발성 섬유성 골이형성증 변형에 대한 나사못 맞물림 골수정을 이용한 치료)

  • The fibrous dysplasia is a progressive and disabling condition that lead to deformity, especially weight bearing bones. The morbidity that is associated with the polyostotic fibrous dysplasia is the recurrent fracture and deformity. Various methods of treatment had been failed to control this problem. We used osteotomy and reconstruction nailing for polyostotic fibrous dysplasia occured in the proximal part of right femur with varus deformity and reconstruction nailing in left femur without osteotomy, and interlocking intramedullary nailing in right tibia to prevent pathologic fracture. These methods brought a good result of bone union and full weight bearing ambulation after 1 year and 6 month follow up. We think these methods are useful methods to control refracture and deformity, so we reported this case with bibliographic reviews.

Hepatic Vascular Stress Gene Expression in the Liver Response to Trauma

  • Trauma remains one of the important sources leading to systemic inflammatory response anti sub-sequent multiple organ failure. Although hepatic microvascular dysfunction occurs during trauma, the mechanism responsible remains unclear. The aim of this study was to investigate the effect of trauma on hepatic vascular stress gene expression. Femur fracture (EFx) was induced by torsion to the femur at midshaft. Liver samples were taken for RT-PCR analysis of mRNA for gtenes of interest: endothelin-1 (ET-1), its receptors $ET_A$ and $ET_B$, nitric oxide synthases (iNOS and eNOS), cyclooxygenase-2 (COX-2), heme oxygenase-1 (HO-1), and tumor necrosis tactor-${\alpha}$ (TNF-${\alpha}$). The expression of ET-1 mRNA was significantly increased by FFx. Expression of mRNA in FFx group showed no change in $ET_A$, $ET_B$, iNOS and HO-1 and showed a slight increase of 2.2-fold and 2.7-fold for eNOS tll1d COX-2, respectively. The level of TNF-${\alpha}$ mRNA significantly increased in FFx group. In conclusion, mild trauma alone causes little change in expression of vasoactive mediators.

  • Total hip prosthesis is used for the patients who have hip fracture and are unable to recover naturally. To de-sign highly durable prostheses one has to take into account the natural processes occurring in the bone. Finite element analysis is a computer based numerical analysis method which can be used to calculate the response of a model to a set of well-defined boundary conditions. In this paper, the static load analysis is based, by se-lecting the peak load during the stumbling activity. Two different implant materials have been selected to study appropriate material. The results showed the difference of maximum von Misses stress and detected the frac-ture of the femur shaft for different model (Charnley and Osteal) implant with the extended finite element method (XFEM), and after the results of the numerical simulation of XFEM for different was used in deter-mining the stress intensity factors (SIF) to identify the crack behavior implant materials for different crack length. It has been shown that the maximum stress intensity factors were observed in the model of Charnley.

The Roles of Kupffer Cells in Hepatocellular Dysfunction after Femur Fracture Trauma in Rats

  • The aim of this study was to investigate the effects of trauma on alterations in cytochrome P450 (CYP 450)-dependent drug metabolizing function and to determine the role of Kupffer cells in hepatocellular dysfunction. Rats underwent closed femur fracture (FFx) with associated soft-tissue injury under anesthesia, while control animals received only anesthesia. To deplete Kupffer cells in vivo, gadolinium chloride (GdCl$_3$) was injected intravenously via the tail vein at 7.5 mg/kg body wt., 1 and 2 days prior to FFx surgery. At 72 h after FFx, serum alanine aminotransferase (ALT) activity was increased, and this increase was attenuated by GdCl$_3$ pretreatment. Serum aspartate aminotransferase (AST) and lipid peroxidation levels were not changed by FFx. Hepatic microsomal CYP 450 content and aniline p-hydroxylase (CYP 2E1) activity were significantly decreased; decreases that were not prevented by GdC1$_3$. The level of CYP 2B1 activity was decreased by Kupffer cell inactivation, but not by FFx. There were no significant differences in the activities of CYP 1A1, CYP 1A2 and NADPH-CYP 450 reductase among any of the experimental groups. Our findings suggest that FFx trauma causes mild alterations of hepatic CYP 450-dependent drug metabolism, and that Kupffer cells are not essential for the initiation of such injury.

MECHANICAL AND HISTOMORPHOLOGIC CHANGES OF THE RAT FEMUR AFTER ADMINISTRATION OF SODIUM FLUORIDE (불화물에 의한 백서 대퇴골의 강도 및 조직형태학적인 변화)

  • The purposes of the study was to examine changes in thickness and strength of the rat femur after administration of sodium fluoride in the drinking water. 48 female Sprague-Dawley rats were randomly divided into 4 groups, and they were supplied with 0(control), 1, 5 and 50 ppm of fluoride in the drinking water ad libitum for 6 weeks. Rats were killed and both sides of femur were dissected. Bone strength was measured as the stress of failure of femur at the middle point and femoral neck with Instron. Histologic slides were prepared from the femur shaft with routine processing of fixing, demineralizing, embedding and HE staining. Thickness and area of cortical bone and medullary cavity were measured by using Camera Lucida and Image Analyzer. All the collected data were analyzed with one-way ANOVA, Duncan's multiple range test for post-hoc tests and ANCOVA using the SAS 6.12 package at the level of 0.05. Bone strength increased significantly in the animals given 1 and 5 ppm of fluoride in the water, as compared to the control group. There were significant decreases of stress at fracture in 50 ppmF group compared to the 1 and 5 ppmF groups. The similar trends of bone strength at the femoral neck fracture test, but there were no statistical significances. Cortical bone thickness and area of the femur increased in the 1 and 5 ppmF groups compared to the control. However, the thickness of 50 ppmF group also decreased significantly as compared to 1 and 5 ppmF groups. On the other hand, medullary thickness and area increased in all fluoride groups than control group. All of the findings presented support the conclusion that, low fluoride dosage used in water fluoridation could increase the bone strength and might have preventive effect on femur fracture.

Effect of Korean Medicine Treatments for Pain Reduction in Patients with Hip Fracture : A Retrospective Observational Study (고관절을 이루는 골격의 골절 환자의 통증 감소에 대한 한의학적 치료 효과 : 후향적 관찰연구)

  • The aim of this study was to observe the effectiveness of Korean Medicine treatments on pain reduction of patients who were diagnosed hip fracture - Acetabulum and Proximal Femur. We reviewed the 17 patients' records who had admitted to Daejeon Korean Medical hospital and diagnosed fracture of acetabulum or proximal femur bones from January 1, 2011 to April 30, 2020 during the admission retrospectively. Statistical analysis was done using the IBM SPSS statistics 25 program. We measured Numeric Rating Scale to evaluate the treatment's efficacy. Korean Medicine treatments reduced median of Numeric Rating Scale reduced statistically significantly from 6.00 to 3.00(p<0.05). During the admission, patients didn't experience the side effects of Korean Medicine treatments. In conclusion, these results implied that Korean medicine treatments showed a positive effect on pain reduction of hip fracture patients. Further research is required to confirm the effectiveness of Korean Medicine treatments.

  • Falling related injuries are categorized as the most serious and common medical problems experienced by the elderly. Hip joint fracture, one of the most serious consequences of falling in the elderly, occurs in only about 1% of falling. In this study, according to the loading conditions, the analysis is the behavior of the femur. The CT images using the commercial program "Mimics" the bones of three-dimensional CAD data generated, and we will analyze the results of finite element analysis. The boundary conditions on the basis of existing research has been simplified. In this paper, the whole femur was assumed to be isotropic linear elastic material. Predicted the behavior of the femur according to the loading condition, it can be help the development of high-precision artificial bones and joints can be treated with surgery and will be able to perform efficiently.

Relationship between Delirium and Clinical Prognosis among Older Patients underwent Femur Fracture Surgery (대퇴부골절 후 수술환자의 섬망과 임상예후와의 관계)

  • This study was a retrospective examination to identify the association of postoperative delirium of the prognosis on following femur fracture surgery in elderly patients. Data was collected from the medical records of elderly patients (aged 65 years or older), who underwent femur fracture surgery from July 2010 to January 2014, following on 3-years in one university hospital. A total of 68 patients were involved. There were 31 cases (45.6%) with delirium and 37 cases (54.4%) without delirium. The participant's average age was 80.8 (patients with delirium), and 81.8 (delirium without patients) years of age, respectively, and most of them were female. There was no significant difference between the two groups. Taking five or more medications, serum creatinine level, and the total medical costs were significantly different in the delirium group and non-delirium group. In addition, the proportional hazard model of Cox to determine the predictors for the major clinical outcome occurring after surgery revealed delirium, five or more multi-drug use, and an experience of transfusion to be significant predictors. In conclusion, postoperative delirium in the elderly undergoing femur fracture surgery can have a negative clinical outcome in patients and caregivers. Therefore, a preoperative evaluation and management of the risk factors will be necessary.

A Comparative Study on BMD of Lumbar Spine and Proximal Femur in Post-Menopausal Women Using Dual Energy X-ray Absorptiometry (이중에너지 X선 흡수계측법을 이용한 폐경기 여성의 요추 및 근위 대퇴부의 골밀도 비교 연구)

  • Osteoporosis, which causes mainly fracture of the spine, proximal femur and distal radius by minimal trauma, is a major public health problem and its prevalence is steadily increasing in Korea according to the development of public health care. There are reliable methods for diagnosis based on bone densitometry. Early detection and intervention are important for reducing the incidence of fractures. A consensus definition of osteoporosis, based on bone density measurement, has been developed by the World Health Organization(WHO). In this study, bone mineral density(RMD) was measured by dual energy x-ray absorptiometry(DEXA) at the proximal femur and lumbar spine in 132 post-menopausal women. The purpose of this study is to find influential factors on the BMD of the proximal femur and the lumbar spine and to analyze correlation between BMD and the problematic factors. We obtained the following results : 1. Mean BMD score, T-score and Z-score of the proximal femur were $0.81(g/cm^2)$, -2.45(S.D.) and -2.09(S.D.) respectively and in the lumbar spine were $0.83(g/cm^2)$, -2.02(S.D.), -2.43(S.D.) respectively. 2. In correlation analysis between BMD and many factors, correlation coefficients were -0.467, 0.212, -0.321 and 0.241 in age, height, duration after menopause respectively. BMI and the residuals were comparatively small. 3. Correlation coefficients to age matched BMD, in height and body weight were 0.222 and 0.241, in age and duration after menopause were -0.268, -0.282. 4. The fracture threshold of proximal femur BMD to the 90th percentile was $0.845(g/cm^2)$. 5. At the result of multiple regression analysis, age, body weight, $BMI(kg/m^2)$ and duration after menopause described as significant variables.

Multicentric Chondrosarcoma - case report - (다발성 연골육종 1례 보고)

  • Multicentric chondrosarcoma other than the mesenchymal subtype is rare separate entity. We experienced a case with nonmonomelic synchronous multicentric chondrosarcoma without any preexisting lesions of Oilier's disease or Maffucci's syndrome. To our knowledge, there was no report of synchronous nonmonomelic multicentric chondrosarcoma. A thirty-three year old man had right distal thigh pain of one and half year. Bone scan showed hot lesions on medial condyle of right femur and shaft of left femur. Plain X-ray showed osteolytic lesion on right femur and slight cortical thickening and calcific lesion was observed on left femoral shaft. Curettage and bone cement filling was done on both lesions. The pathology reports were grade I chondrosarcoma on both side of femur. At one month from operation, pathologic fracture of left femur occurred on bone cement-host bone junction. Conservative treatment and radiotherapy of 60Gy was done. At 8 months from operation, nonunion was evident. Segmental resection of left femur with contralateral fibula graft and second look operation on right condyle lesion were done. At 6 months from revision, fracture occurred at host-graft bone junction. We removed previous hardware and applied long DCP and massive autogenous bone graft. Afterwards, the patient looks good and union was progressing. But at 4 years from last operation, hypertrophic nonunion occurred. Another revision was done with condylar plate and bone graft and now he is well without any sign of local recurrence or metastasis.
