A Case Study of a Taeeumin Patient Treated with Cheongsimyeonja-tang Who was Misrecognized as Wiwansuhan-pyohan Disease (태음인(太陰人) 위완한증(胃脘寒證)으로 오인한 청심연자탕증(心蓮子湯證) 환자 치험1례)

  • Objectives It is important to decide if the patient is belong to cold syndrome or heat syndrome in Sasang Constitutional Medicine. The purpose of this study is to report some proof to determine cold and heat syndrome and characteristic of the patient who can be treated with Cheongsimyeonja-tang. Methods To evaluate the results of this treatment, muscular symptom was assessed by Visual Analogue Scale(VAS). The patient's general condition such as urinary and fecal discomfort is assessed by the progress notes. Results The patient did not improved when she was treated with Choweseuncheung-tang. But when she was treated with Cheongsimyeonja-tang, she got improved with not only her main problem such as muscular symptom, but also general condition such as indigestion, urinary & fecal discomfort and menopausal symptom. Conclusion This study suggests that number of defecation per day also can be the clue to decide heat or cold syndrome not only appearance of defecation. And the patients who can be treated by Cheongsimyeonja-tang is different from the patients who can be treated by Yeoldahansotang in the color of their face, personality and edema.

Evaluation of raw wastewater characteristic and effluent quality in Kashan Wastewater Treatment Plant

  • Due to the lack of water in arid and semi-arid areas, reuse of wastewater can be a suitable way to compensate for water scarcity. Therefore, in this research, evaluation of the quality of wastewater of Kashan Treatment Plant to use for irrigation was studied. This descriptive cross-sectional study was conducted in 2016. pH, TSS, TDS, turbidity, COD, BOD5, Total Kjeldahl Nitrogen, Total Phosphorus, Total Coliform, fecal coliform, nematode eggs of inlet and outlet of wastewater treatment plant in Kashan were studied. Mean and standard deviation and wastewater quality parameters before and after treatment were tested with SPSS 22 (2014) software. The mean wastewater output of COD, BOD5, TSS, TDS and turbidity were respectively 86.6, 41.2, 11.11, 1095 mgL-1 and 17.5 NTU and the pH was equal to 7.22. Also, the average of Total Kjeldahl Nitrogen and phosphorus were 22.4 and 2.2 mgL-1 respectively. The mean of Total Coliform and fecal coliform were 225, 161 MPN / 100 ml respectively. In addition, no nematode eggs were found in final effluent. The results indicated that the treatment plants had a significant role in the control of microbial and organic pollution load of wastewater. Also, it is concluded that all parameters were in accordance with the standards of Iran's Department of Environment, so, it can be used for unrestricted irrigation.

  • Swine proliferative enteritis(SPE) caused by inかsoma intracellularis is a common enteric disaese of grower and finisher pig. Swine affected with SPE show variable clinical signs including diarrhea, weight loss, aberrant growth and death. The characteristic lesion of ileitis at necropsy is marked thickening of the last section of the small intestine. The inner lining of the thickened intestine proliferates almost like a cancer and curved rod bacteria(L intracellularis) are always seen inside the intestinal wall. Infected swine shed the organism in the feces. Isolation and growth of pure L intracellularis in vitro requires a suitable cell culture. This procedure is difficult and not a practical means of diagnosis, thus the polymerase chain reaction(PCR) test of feces can be used to determine whether a pig is shedding the infective organism. A sensitive assay based on amplification of a 319bp DffA fragment of the L intracellularis of Swine proliferative enteritis was attempted for the detection of the organism in the 62 feces of swine. L intracellularis was identified on three herds and detected in 6 fecal samples, representing a infection rate of 9.7%. The PCR was very sensitive and specific on the individual level. The PCR technique could be very useful for the diagnosis of this disease.

Effect of Dietary Zeolite Treated on the Performance and Carcass Characteristics in Finishing Pigs (사료내 Zeolite 첨가가 비육돈의 생산성과 도체특성에 미치는 영향)

  • This study was conducted to investigate the effect of dietary supplementation of zeolite on growth performance, carcass characteristics and fecal ammonia gas release in the finishing pigs. A total of 120 crossbred finishing pigs (50.2$\pm$1.4kg, Landrace$\time$Yorkshire$\time$Duroc) were assigned to 4 treatments and randomly divided into 12 groups. They were divided between early finishing period (50~80kg) and late finishing period (80-120kg). Finishing pigs were fed on a basal diet supplemented with 0, 1, 2 and 4% levels of zeolite. Average daily gain and feed conversion were significantly (P<0.05) improved by the dietary supplementation of 4% zeolite compared with other treatments during overall period. In the finishing periods, ammonia gas emission was reduced (P<0.05) in 4% zeolite treatment more than that of the other treatments. Carcass weight and back fat thicknesses were not affected by zeolite treatments. But, the appearance of A grade pork was increased by the dietary supplementation of 4% zeolite. Feed cost was linearly decreased by increasing the supplementation of zeolite. According to this study, 4% supplement of zeolite is suitable for the improvement of carcass quality and feed conversion, the reduction of fecal ammonia gas emission and feed cost per body weight gain.

Genotypes and Infection Status of Human Enterovirus Associated with Enteric and Respiratory Patients in Seoul, Korea (서울시내 소화기 및 호흡기 환자에서 분리한 엔테로바이러스 감염실태 및 유전형분석)

  • Objective: Human enteroviruses (HEVs) are a common causative agent of gastrointestinal or respiratory infections. In this study, to examine the genotypic diversity and characteristics of HEVs associated with patients in Seoul, we collected and analyzed stool and throat swab samples taken from patients with acute gastroenteritis or a common cold from 2011 to 2012. We researched the difference in genetic characteristics of HEVs from gastroenteritis and respiratory patients. Methods: For genetic analysis, we amplified the 5'-noncoding region and partial VP1 region of HEVs by RT-PCR. The genotypes of HEVs were further identified based on nucleotide sequences of the VP1 region. Results: The majority of the HEV infections in Seoul occurred from June to August. The molecular characteristic assay showed that although the majority of HEVs can be propagated by a fecal-oral route, Coxsackievirus A2 (n=13, 19.4%), A4 (n=8, 11.9%), and A5 (n=4, 6.0%) can be preferentially transmitted by a respiratory route. Conclusions: This Enterovirus surveillance system plays an important role in preparing for a severe outbreak. The genotypic characteristics of HEV may provide potentially useful data needed for epidemiological studies.

Establishment for Improving Productivity of Cattle by Fecal Steroid and Milk Urea Nitrogen analysis - II. Application of Early Pregnancy Diagnosis and Monitoring Postpartum Ovarian Activity

  • Progesterone levels in the blood plasma or skim milk of cows are considered to be very useful indicator fur the detection of estrus cycle and early pregnancy diagnosis. During 13 to 14 days after estrus the level of progesterone in plasma or skim milk were not different between the inseminated arid non-inseminated cows. In the pregnant cows the peak level of progesterone reached on 14th day after artificial insemination (AI), but in the absence of conceptus the level declines after the 14th day slowly, and then very rapidly towards the basal level after the 17th day. This low level persists about 4 days, including those of estrus and ovulation a highly characteristic pattern which differs so markedly from that in the pregnant cows. Progesterone levels in blood plasma or skim milk can provided a reliable diagnosis of early pregnancy and monitoring ovarian activity in cows. The mean $\pm$ standard deviation of milk urea nitrogen(MUN) and protein concentration in the cows at 9 herds were 17.7$\pm$2.35 mg/dL and 3.2$\pm$0.17%, respectively. The days of nonpregnant after parturition was shorter in the cows in which the lower level of MUN than higher level of MUN concentration.

Infectious Bursal Disease-A Review (전염성 훼브리셔스낭병)

  • Infectious bursal disease, so called Gumboro disease, is found world-wide in areas of intensive poultry farming. The clinical signs of the disease are very indicative, but most infections occur unnoticed due to the age of infection of chicken as well as the degree of virulence of virus affected. Edematous and hemorrhagic lesions in BF at early course of infection and the complete atrophies of BF in later are the most characteristic. The infection is considered highly contagious by direct contact, by fecal material and by contaminated feed and water. The virus is also highly resistant in environment and belongs to Diploma virus with size of 55 to 60nm of Ribovirus group. IBDV grows in embryos, embryonic cells and BF of susceptible chickens. Immune-diffusion using agar gel is the method of a choice to determine IBDV infection in chickens. Maternal immunity is very effective in protecting chickens of critical age when IBDV infection severely damages the function of BF. Immunosuppressive effect of IBDV causes more production losses than direct effects of clinical disease of IBD. Inclusion body hepatitis, infectious anemia and gangrenous dermatitis syndrome are the disease associated with the immunosuppressive condition of chickens.

The Prevalence of Enteroadherent Escherichia coli(EAEC) and the Study of its Pathogenic Role in Korean Children with Diarrhea (우리나라 소아 설사에 있어서의 Enteroadherent Escherichia coli의 병원적 역학)

  • Adherence to HEp-2 cells has been proposed as a virulence characteristic of enteropathogenic E. coli (EPEC). The role of the HEp-2 adherent E. coli was evaluated in a group of children with endemic diarrhea admitted to Hanyang University Hospital in Seoul, Korea. HEp-2-adherent E. coli was detected in fecal samples of 59 (59%) of 100 cases and ten (22.7%) of 44 concurrent control children (p<0.0005). Adherence was exhibited by 15 serogroups and subgroups, but within these groups more than one adherence pattern was frequently observed. Of 17 strains belonging to traditional infantile EPEC serogroups, 12(70.6%) gave a positive adherence. Of 45 enterotoxin producing strains, 24 (53.3%) gave a positive adherence. HEp-2-adherent strains that did not belong to classic EPEC serogroups and did not produce heat-stable and/or heat-labile enterotoxins(referred as enteroadherent E. coli, EAEC) was found in 29 (29%) of the patients with diarrhea and in six (13.6%) of the well children (p<0.05). From 22 of the 29 cases, no pathogen other than EAEC was isolated. These findings strongly implicate EAEC as the cause of diarrhea in the children. Our study supports the concept that EAEC may be an important cause of endemic diarrhea in Korean children.

Characteristics and Clinical Correlations of Staphylococcus aureus Discovered in Stools from Children Hospitalized at a Secondary Hospital (일개 이차병원에 입원한 소아의 분변에서 발견된 황색포도알균의 특징과 임상적 연관성)

  • Purpose: Research on the clinical role of Staphylococcus aureus as a pathogen in acute gastroenteritis (AGE) in children has been scarce. This study aimed to clarify the prevalence and clinical correlation of S. aureus detection in children with AGE. Methods: Fecal samples were collected from children with symptoms of AGE who visited a secondary hospital between January 2012 and December 2015. The samples were sent to the Seoul Metropolitan Government Research Institute of Public Health and Environment to test for pathogenic organisms. Clinical patterns were analyzed through medical record review. Results: Among the 663 participants, the bacteria detection rate was 26.2% (n=174), the virus detection rate was 29.7% (n=197), and the non-detection rate was 43.1% (n=286). S. aureus was tested positive from 102 cases and was confirmed as a single pathogen in 53 cases. It was the third most common pathogen. The prevalence by age was highest (45.3%) in 0-2 year-olds. Most cases occurred in summer. Symptoms included diarrhea (71.7%), vomiting (67.9%), fever (49.1%), and abdominal pain (37.7%). Only vomiting showed a significant difference between the S. aureus group and the non-detection group (67.9% vs. 43.0%; P=0.001). Among enterotoxins, the higher incidence of vomiting was associated with classical staphylococcal enterotoxins (SEA, SEB, SEC, SED, and SEE) and SEH (P=0.027). Conclusions: S. aureus was the bacteria commonly isolated from children with AGE. Our study identified cases of staphylococcal AGE in children based on fecal samples and confirmed the characteristic symptoms, affected age groups, seasonal distribution, and correlation with enterotoxins.

Diversity of Duodenal and Rectal Microbiota in Biopsy Tissues and Luminal Contents in Healthy Volunteers

  • The diverse microbial communities that colonize distinct segments of the gastrointestinal tract are intimately related to aspects of physiology and the pathology of human health. However, most recent studies have focused on the rectal or fecal microbiota, and the microbial signature of the duodenum is poorly studied. In this study, we compared the microbiota in duodenal and rectal samples to illustrate the characteristic microbial signatures of the duodenum in healthy adults. Nine healthy volunteers donated biopsies and luminal contents from the duodenum and rectum. To determine the composition and diversity of the microbiota, 454-pyrosequencing of bacterial 16S rRNA was performed and multiple bioinformatics analyses were applied. The α-diversity and phylogenetic diversity of the microbiota in the duodenal samples were higher than those of the rectal samples. There was higher biodiversity among the microbiota isolated from rectal biopsies than feces. Proteobacteria were more highly represented in the duodenum than in the rectum, both in the biopsies and in the luminal contents from the healthy volunteers (38.7% versus 12.5%, 33.2% versus 5.0%, respectively). Acinetobacter and Prevotella were dominant in the duodenum, whereas Bacteroides and Prevotella were dominant in the rectum. Additionally, the percentage of OTUs shared in biopsy groups was far higher than in the luminal group (43.0% versus 26.8%) and a greater number of genera was shared among the biopsies than the luminal contents. Duodenal samples demonstrated greater biological diversity and possessed a unique microbial signature compared with the rectum. The mucosa-associated microbiota was more relatively conserved than luminal samples.
