소아 99mTc-DMSA renal scan에서 방광차폐유무와 방사성동위원소 주입방법에 따른 콩팥섭취율 차이에 관한 연구 (Study on the Difference in Intake Rate by Kidney in Accordance with whether the Bladder is Shielded and Injection method in 99mTc-DMSA Renal Scan for Infants)

  • DMSA 방사성의약품은 몸쪽 세뇨관과 주위 콩팥겉질 세포에 섭취되어 콩팥 겉질의 평가 및 영상화에 유용하게 사용되는 의약품으로 신우신염의 진단에 매우 예민도가 높은 검사여서 소아에게 많이 사용되고 있는 검사이다. 소아에게 투여되는 방사성 동위원소의 양은 미량이 되고 소아의 신체가 Field of View (FOV)에 대부분이 포함이 되는 만큼 방광에 소변이 차있게 된다면 그만큼 콩팥을 영상화하는데 영향을 미치게 됨을 연구를 통해 확인하고자 하였다. 본 연구에서는 총 계수 설정법과 시간 설정법 중에 시간 설정법으로 연구를 진행하였다. 2015년 10월에서 12월까지 요로감염 및 신우신염이 의심되어 본원을 내원 및 입원하여 시행한 생후 1개월부터 12개월까지의 소아 34명을 대상으로 하였으며 환자에게는 동일한 선량 18.5 MBq (0.5 mCi)를 각각의 환자에게 동일한 양을 주입 후 2~3시간 후 검사를 진행하였다. 이때 사용된 장비는 Siemens사의 Symiba E (Siemens Medical solution USA, Inc.) 장비를 사용하였고 영상의 분석하기 위하여 Syngo MI Applications VA60C 소프트웨어를 사용하였다. 통계학적 분석은 IBM SPSS Statistics Ver. 21를 이용하여 분석하였으며 Paired t-test를 이용하여 비교 분석하였다. 검사는 한번의 검사에 7분의 시간으로 후면상을 획득하였으며 이후 자체 제작된 납을 이용하여 방광을 가린 후 추가로 동일한 시간으로 영상을 획득하였다. 영상 분석 시에 동일한 크기의 (가로 55.2 mm ${\times}$ 세로 70.0 mm)의 ROI (Region of Interest)를 설정하여 분석하였다. 콩팥의 계수는 (Lt. Kidney counts + Rt. Kidney counts) / Total counts의 백분율로 나타내어 계산하여 평가하였고. Background 수치는 같은 영상을 비교하기에 배제하고 연구를 진행하였다. 방광을 차폐시킨 후의 콩팥 계수는 $79.40{\pm}5.19%$ 방광을 차폐시키기 전의 콩팥 계수는 $70.87{\pm}3.18%$으로 나타났으며 (차폐시킨 후 - 차폐 전)의 콩팥 계수는 $8.52{\pm}3.29%$로 차폐시킨 후와 차폐시키기 전을 비교 분석하였을 때 유의한 것으로 나타났다. 주사 방법 중 3way stopcock를 이용하여 주사하였을 경우 차폐 후 콩팥 계수는 $78.10{\pm}4.61%$ 차폐 전 콩팥계수는 $68.92{\pm}2.80%$로 (차폐시킨 후 - 차폐 전)의 콩팥 계수는 $9.18{\pm}3.53%$로 나타났으며 Heparin cap을 이용하였을 경우 차폐 후 $79.84{\pm}3.26%$, 차폐 전 $71.33{\pm}5.14%$로 (차폐시킨 후 - 차폐 전)의 콩팥 계수는 $8.51{\pm}2.92%$로 나타났으며 마지막으로 직접 주사했을 경우 차폐 후 콩팥 계수는 $82.07{\pm}2.35%$, 차폐 전 콩팥 계수는 $75.11{\pm}4.30%$로 (차폐시킨 후 - 차폐 전)의 콩팥 계수는 $6.96{\pm}2.78%$로 세 가지 방법 모두 차폐시킨 후와 차폐시키기 전을 비교 분석하였을 때 유의한 것으로 나타났다. 그리고 직접 주사, Heparin cap, 3way stopcock 순의 콩팥 계수율을 보임을 확인 할 수 있었다. 소아의 Renal DMSA scan검사 시에 방광의 방사능을 제거하여 방광을 차폐하였을 때 차폐하지 않았을 때보다 개선된 콩팥섭취율을 보였고 소아의 경우에 혈관 확보에 어려움이 있지만 직접 주입하거나 환자의 몸에 근접하도록 방사성 동위원소를 주입한다면 더 나은 영상 획득에 도움이 될 것이다.

도항리목관묘(道項里木棺墓) 안사국(安邪國) (A Study of Dohang-Ri wooden coffin and Anya-Kuk)

  • 삼한시대(三韓時代)의 성격과 성립시기(成立時期)에 대하여서는 다양한 견해가 제시되어 있는 상황이나 모두 나름대로의 한계를 가지고 있다고 볼 수 있다. 문헌자료의 기록과 고고자료상에서 서로 공통적으로 관련지어 볼 수 있는 것은 모두 전국시대(戰國時代)의 철기문화(鐵器文化)를 근간으로 하고 있다는 점이며, 연(燕)의 철기문화(鐵器文化)를 갖춘 위만조선(衛滿朝鮮)의 성립은 근본적으로 한반도 북부지역 뿐 만 아니라 중 남부지역에 이르기까지 큰 파장을 미쳤던 것으로 볼 수 있다. 철기문화(鐵器文化)의 도입단계에는 철기(鐵器)의 실용적이고 희소적인 가치에 의해서 매장유구(埋葬遺構)에는 쉽게 부장되지 않았을 것으로 보이나 기원전 2세기 전반경부터는 일부 한정된 철기(鐵器)만이 매장유구(埋葬遺構)에 부장되는 경향이다. 따라서 삼한시대(三韓時代)의 성립시기는 철기의 도입과 보편적인 활용으로 보아야하며 목관(木棺)을 사용한 서북한지방(西北韓地方)의 토광묘문화(土壙墓文化)를 포함하여 중남부지역(中南部地域)에 있어서 목관(木棺)의 흔적이 확인되고 있는 적석목관묘(積石木棺墓) 단계, 즉 기원전 3세기전반대의 어느 시점으로 파악되어야 한다. 한편 함안(咸安) 도항리고분군(道項里古墳群)에서 확인된 목관묘(木棺墓)는 목관(木棺)의 형태에 있어서 판재조립식목관(板材組立式木棺)과 통나무목관(木棺)의 2종류가 확인되며, 판재목관(板材木棺)의 사용 비율이 3배 이상 높은 편이다. 출토유물(出土遺物)은 비교적 단순한 조합상(組合相)을 보이고 있는데, 이는 영남지역(嶺南地域)을 중심으로 한 목관묘유적(木棺墓遺蹟)에서도 모두 확인되고 있는 특징이므로 전반적으로 변(弁) 진한사회(辰韓社會)의 매장문화는 동일한 흐름 속에서 전개된 것으로 볼 수 있다. 특히 통나무로 제작한 목관(木棺)은 적석목관묘(積石木棺墓)에서부터 보이기 시작하므로 한국식동검문화(韓國式銅劍文化)와 함께 목관(木棺)의 전통이 이어져 내려온 것으로 생각되며, 다호리유적(茶戶里遺蹟)과 도항리목관묘(道項里木棺墓)에서만 이러한 목관(木棺)의 존재가 확인되고 있는 것은 변한사회내(弁韓社會內)에서 양지역간(兩地域間)의 유기적인 관계를 살피는데 있어서 주목되어야 한다. 도항리목관묘(道項里木棺墓)에서 출토되고 있는 철기류((鐵器類)도 거의 다호리유적(茶戶里遺蹟)의 그것들과 유사한 경향을 보이고 있다. 도항리(道項里) 67호묘(號墓) 출토 주조철부(鑄造鐵斧)는 다호리 1호분에서 출토된 주조철부(鑄造鐵斧)와 같은 지금(地金)의 기능을 했던 것으로 보이며, 소형철부(小形鐵斧)의 구성비율이 다른 철기류에 비해서 월등히 높은 것과 기경구(起耕具)로 이용되는 철제따비의 출토가 거의 없는 것으로 보아 도항리목관묘(道項里木棺墓)단계에는 목제농기구(木製農器具)의 제작과 사용이 활발하였던 것으로 생각된다. 결국 다호리유적(茶戶里遺蹟)에 비해 절대적으로 빈약한 철기유물은 안사국(安邪國)의 내적(內的)인 한계(限界)를 보여주는 것이며 소국(小國) 수장(首長)의 신분적(身分的)인 상징(象徵)으로서 사용하였던 동경(銅鏡)과 의기류(儀器類)의 출토가 전혀 확인되지 않고 있는 것으로 보아 도항리목관묘(道項里木棺墓)를 조영한 집단(集團)은 당시 대외문화교류(對外文化交流)의 중심적인 선진지역(先進地域)에 비해서 사회문화적(社會文化的)인 변동(變動)이 덜한 주변 지역적인 성향이 강한 집단(集團)일 것으로 생각된다. 기원후 2세기 후반경 영남지역의 목관묘(木棺墓)는 목곽묘단계(木槨墓段階)로 변화되었을 것으로 생각되는데, 도항리(道項里)와 다호리유적(茶戶里遺蹟)에서도 목관묘(木棺墓)는 2세기대를 종점(終點)으로 사라진다. 고고자료상으로도 3세기에서 4세기대에 해당되는 적극적인 유구가 확인되지 않고 있으므로 이에 대한 근본적인 해석이 요구된다. 따라서 이러한 현상을 3세기 전반 이후 한반도 남부지역에서 일어났던 포상팔국(浦上八國)의 전쟁과 관련지어 합리적으로 추론하고자 한다. 즉 변한사회내(弁韓社會內)에서 도항리(道項里)와 다호리유적(茶戶里遺蹟)의 양집단(兩集團)은 낙동강(洛東江)과 남강수계(南江水系)를 장악한 교역(交易)의 중심지(中心地)로서 서로 경제적(經濟的) 정치적(政治的)인 공조체제(共助體制)가 형성되었을 것으로 생각된다. 기록에 보이는 포상팔국(浦上八國)의 대상국은 가나(加羅)와 아라국(砑羅國)이며, 가나(加羅)는 다호리집단(茶戶里集團)이 중심을 이루고 있는 것으로 파악된다. 그리고 포상팔국(浦上八國)의 전쟁시기도 기원 3세기 전반대의 상황을 비교적 자세하게 다루고 있는 "삼국지(三國志)" 위서(魏書) 동이전(東夷傳)에 이 전쟁에 대한 언급이 없는 것으로 보아 3세기 전반대 이후의 사건으로 파악될 수 있으며, 4세기 후반에 이르기까지 양지역(兩地域)에서 뚜렷한 목곽묘(木槨墓)의 축조가 이루어지지 않고 있는 점도 두 집단(集團)이 전쟁(戰爭)의 피해를 직접 받은 당사자였음을 암시하는 것으로 생각된다.

동북아시아 계단식 논의 명승지정 현황 및 보전방안 (The Present Status and the Preservation Method of the Rice Terrace as Scenic Sites Resources in Northeast Asia)

  • 본 연구는 한국, 중국, 일본의 계단식 논의 현황 및 보전관리방안을 중점적으로 살펴보고, 한국 계단식 논이 명승자원으로 보전되고, 지속적으로 연구될 수 있는 기초자료 제시를 목적으로 하며, 본 연구에서 도출한 결과는 다음과 같다. 첫째, 우리 선조들은 경사지를 개간하면서도 경관의 훼손을 최소화 하며 다랑이 논을 축조하는 방법을 전수하였으며, 다랑이 논은 한국 고유의 전통 경관으로 가치를 지니고 있다. 그러나 경작의 운용면에서 불리한 환경에 놓여있어, 휴경(休耕)되거나 폐경(廢耕)되어 사라져가는 다랑이 논의 면적이 증가하고 있는 추세이다. 이러한 문제에 대하여 정부는 소실되어가는 전통 경관인 다랑이 논의 보전을 위해서 '한국 다랑이 논 데이터베이스 구축'을 실시하여 우수 경관자원 발굴의 토대를 마련해야 할 것이다. 문화 경관적 가치가 높다고 판단되는 다랑이 논은 명승지나 기념물로 지정 관리되어 법적 보호를 받을 필요가 있다. 둘째, 한국 계단식 논의 효율적 관리와 지속적인 연구를 위한 국 내외 문헌조사 분석의 결과는 다음과 같다. 관련 법규 부분에서 계단식 논은 문화재보호법에 의해 명승으로 지정 관리되고 있을 뿐, 그 외에 경관법 및 농어촌관련법은 계단식 논 경관계획 수립을 위한 참고사항으로 법적 구속력 및 실효성이 없는 상태이다. 이에 계단식 논의 효율적 보전과 관리를 위해서는 계단식 논 관련법의 제정, 제도의 보완과 확립이 반드시 이뤄져야 할 것으로 판단된다. 셋째, 본 연구에서 한국, 중국, 일본 계단식 논의 조사분석 결과가 시사하는 바는 다음과 같다. 계단식 논의 보전을 위한 관련 법규는 조금씩 국가별 차이를 보이고 있으나, 우수한 경관자원의 보전과 보호에 바탕을 두고 지정 관리하고 있다는 점에서 유사하다. 국가별 계단식 논 관련 법규의 조사결과로 일본의 문화재보호법과 경관법의 연동, 중국의 자치지구 법규 등의 제도 도입을 통한 한국 계단식 논 경관의 보전 방안이 보완되어야 할 것이다. 넷째, 한국 계단식 논의 효율적 보전을 위해 주민 자체의 교육을 통한 인식 향상과 함께 계단식 논 관리를 위한 마을 단위 조직을 운영해야 한다. 한국 계단식 논 마을주민들의 자발적 참여를 위한 대책들을 도입할 경우, 각 지역의 마을에서 주민들의 합의나 논의 관리, 주변 환경미화 등의 부분에서 서로 일체감을 유발하여 마을의 활성화가 가능할 것으로 사료된다. 또한 마을단위 주민자치 운영회는 문화재청 관계자, 대학교수 등 전문가 집단의 자문 기구를 두어 운영에 대한 자문 및 지원을 쉽게 받을 수 있는 시스템을 구축해야 하며, 도시 주민과의 교류를 통한 계단식 논 지역의 활성화 또한 중요하다고 사료된다.

High Resolution HC$_3$N Observations toward the Central Region of Sagittarius B2

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500,550,570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500,550 and 570nm. The correlation between glucose concentration and parameters was the best at 610nm. 30535 T00401030535 ^x Radiation-induced fibrosarcoma tumors were grown on the flanks of C3H mice. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100,300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. Results show the superiority of the proposed method to the conventional method using power spectrum and also its usefulness for the tissue characterization. 30541 T00401030541 ^x This paper describes the implementation of a computerized radial pulse diagnosis by aids of a clinical expert. Then, we have got the characteristic parameters of radial pulse wave and also quantified that according to the method of Inyoung-Cheongu Comparison Radial Pulse Diagnosis. To synthesize microspheres for biomedical application, "two stage shot growth method" was developed. The uniformity ratio of synthesized microspheres was always smaller than 1.05. And the surface charge density (or the number of ionizable functional groups) of the microspheres synthesized by "two stage shot growth method" was 6~13 times higher than that of the microspheres synthesized by conventional seeded batch copolymerization. As a previous step for biomedical application, adsorption experiments of bovine albumin on microspheres were carried out under various conditions. The maximum adsorbed amount was obtained in the neighborhood of pH 4.5. Isoelectric point of bovine albumin is pH 5.0, so experimental result shows that it shifted to acid area. In this BEAM, instantaneous change and average energy distribution over any arbitrary time interval of brain electrical activity could be observed and analyzed easily. Our department has developed an information system for routine hematology using down-sized computer system. We have used an IBM 486 compatible PC with 16MB main memory, 210 MB hard disk drive, 9 RS-232C port and 24 pin dot printer. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Stimulus patterns of our stimulator were designed to simulate electromyographic activity monitored during locomotion of normal subjects. Muscle types of the lower extremity were classified according to their mechanical property of contraction, which are slow muscle (msoleus m.) and fast muscle (medial gastrocneminus m., rectus femoris m., vastus lateralis m.). Optimal parameters of electrical stimulation for slow muscles were 20 Hz, 0.2 ms square pulse. For fast muscle, 40 Hz, 0.3 ms square pulse was optimal to produce repeated contraction. Higher stimulus intensity was required when synergistic muscles were stimulated simultaneously than when they were stimulated individually. Electrical stimulation for each muscle was designed to generate bipedal locomotion, so that individual muscles alternate contraction and relaxation to simulate stance and swing phases. Portable electrical stimulator with 16 channels built in microprocessor was constructed and applied to paraplegic patients due to lumbar cord injury.
