복부와 경부 관통상 환자에 대한 임상적 고찰 (Clinical Analysis of Patients with Abdomen or Neck-penetrating Trauma)

  • Purpose: Recently, the change to a more complex social structure has led to an increased frequency of traumas due to violence, accident and so on. In addition, the severity of the traumas and the frequency of penetrating injuries have also increased. Traumas to cervical and abdominal areas, what are commonly seen by general surgeons, can have mild to fatal consequences because in these areas, various organs that are vital to sustaining life are located. The exact location and characteristics of the injury are vital to treating patients with the trauma to these areas. Thus, with this background in mind, we studied, compared, and analyzed clinical manifestations of patients who were admitted to Wonju Christian hospital for penetrating injuries inflicted by themselves or others. Methods: We selected and performed a retrospective study of 64 patients who had been admitted to Wonju Christian Hospital from January 2005 to December 2009 and who had cervical or abdominal penetrating injuries clearly inflicted by themselves or others. Results: There were 51 male (79.7%) and 13 female (20.3%) patients, and the number of male patients was more dominant in this study, having a sex ratio of 3.9 to 1. The range of ages was between 20 and 86 years, and mean age was 43.2 years. There were 5 self-inflicted cervical injuries, and 19 self-inflicted abdominal injuries, making the total number of self-inflicted injury 24. Cervical and abdominal injuries caused by others were found in 11 and 29 patients, respectively. The most common area involved in self-inflicted injuries to the abdomen was the epigastric area, nine cases, and the right-side zone II was the most commonly involved area. On the other hand, in injuries inflicted by others, the left upper quadrant of the abdomen was the most common site of the injury, 14 cases. In the neck, the left-side zone II was the most injured site. In cases of self-inflicted neck injury, jugular vein damage and cervical muscle damage without deep organ injury were observed in two cases each, making them the most common. In cases with abdominal injuries, seven cases had limited abdominal wall injury, making it the most common injury. The most common deep organ injury was small bowel wounds, five cases. In patients with injuries caused by others, six had cervical muscle damage, making it the most common injury found in that area. In the abdomen, small bowel injury was found to be the most common injury, being evidenced in 13 cases. In self-inflicted injuries, a statistical analysis discovered that the total duration of admission and the number of patients admitted to the intensive care unit were significantly shorter and smaller, retrospectively, than in the patient group that had injuries caused by others. No statistically significant difference was found when the injury sequels were compared between the self-inflicted-injury and the injury-inflicted-by-others groups. Conclusion: This study revealed that, in self-inflicted abdominal injuries, injuries limited to the abdominal wall were found to be the most common, and in injuries to the cervical area inflicted by others, injuries restricted to the cervical muscle were found to be the most common. As a whole, the total duration of admission and the ICU admission time were significantly shorter in cases of self-inflicted injury. Especially, in cases of self inflicted injuries, abdominal injuries generally had a limited degree of injury. Thus, in our consideration, accurate injury assessment and an ideal treatment plan are necessary to treat these patients, and minimally invasive equipment, such as laparoscope, should be used. Also, further studies that persistently utilize aggressive surgical observations, such as abdominal ultrasound and computed tomography, for patients with penetrating injuries are needed.

Modern Cause and Effect Model by Factors of Root Cause for Accident Prevention in Small to Medium Sized Enterprises

  • Background: Factors related to root causes can cause commonly occurring accidents such as falls, slips, and jammed injuries. An important means of reducing the frequency of occupational accidents in small- to medium-sized enterprises (SMSEs) of South Korea is to perform intensity analysis of the root cause factors for accident prevention in the cause and effect model like decision models, epidemiological models, system models, human factors models, LCU (life change unit) models, and the domino theory. Especially intensity analysis in a robot system and smart technology as Industry 4.0 is very important in order to minimize the occupational accidents and fatal accident because of the complexity of accident factors. Methods: We have developed the modern cause and effect model that includes factors of root cause through statistical testing to minimize commonly occurring accidents and fatal accidents in SMSEs of South Korea and systematically proposed educational policies for accident prevention. Results: As a result, the consciousness factors among factors of root cause such as unconsciousness, disregard, ignorance, recklessness, and misjudgment had strong relationships with occupational accidents in South Korean SMSEs. Conclusion: We conclude that the educational policies necessary for minimizing these consciousness factors include continuous training procedures followed by periodic hands-on experience, along with perceptual and cognitive education related to occupational health and safety.

외상성 흉부대동맥 파열 수술 (Surgical Treatment of Traumatic Rupture of Thoracic Aorta)

  • 외상성 흉부대동맥 파열은 수술적 치료가 요구되는 치명적 손상이며 이외에 다른 부위에 복합손상이 동반되면 심폐 바이패스에 의한 위험도를 크게 증가시킬 수 있다. 여기서 저자들은 심한 동반손상을 먼저 치료한 후 대동맥수술을 하고 그 결과를 관찰하였다. 대상 및 방법: 1997년부터 2003년까지 외상성 흉부 대동맥질환으로 수술을 받은 24명의 환자를 대상으로 의무기록을 후향적으로 검토하여, 동반손상 여부, 수술방법, 수술 후 경과, 합병증 등을 분석하고 국내외 문헌들과 비교하였다. 수술은 초저온 심정지법하에서 심폐 바이패스를 이용한 개흉술로 하였는데 근위부 연결 후 곁가지를 통해 뇌관류를 시행하였고 요추 카테터를 통한 뇌척수액 배액으로 하반신 마비를 예방하였다. 결과: 대상환자 전원에서 83예의 동반손상이 발생하였는데, 흉부손상이 49예, 근골격계 손상 18예, 복부 손상 13예였고, 수상 후 12명의 환자에서 7.6$\pm$12.6일에 16예의 동반손상에 대한 수술이 행해졌다. 수상당시 18예에서만 대동맥 손상이 진단되었다. 중환자실에서 혈압을 약물투여로 조절하면서 안정시키고 수상 후 693 $\pm$ 1350일에 지연수술을 시행하였는데, 관찰기간 중 사망이나 대동맥 파열의 진행은 없었다. 수술 사망은 없었으며, 술 후 큰 합병증은 없었다. 결론: 외상성 흉부대동맥 파열에 있어서 다른 부위 손상이 동반되면 심한 동반손상을 먼저 치료한 후 수술하는 것이 수술 사망률이나 이병률 면에서 좋은 결과를 가져온다.

산업용 로봇의 사용실태에 관한 조사 연구 (Site Survey on the Safe use of the Industrial Robots)

  • Robot related injuries in industrial accidents statistics during 2008~2010 have accounted for a total of 109 cases equivalent to 30~40 cases for each of those years. The number of injured compared to the dissemination of industrial robots(51,302 units/2004) can be regarded as quite low. However, the fatal injuries sustained by 7 (6.4%) out of 109 cases paints the stark reality of robot-related accident fatalities. It is a sad probability that as the automation process expands its use of industrial robots which have increased significantly in demand, the incidence of workplace accidents will also increase. Therefore, the incidence of accidents throughout the period of 2008~2010 has been analysed to prevent the injuries due to the increased use of industrial robots. In the analysis, the injuries occurred during the industrial robot operation accounted for 45.9% of the entire accidents. Thus, we examined the present status of the industrial robot operation to analyze the root cause of accidents occurred in our studied time period. We looked at a total of 469 workplaces. 456 workplaces responded in the year 2009 and survey studies were implemented at 13 of the 29 workplaces where work injuries were sustained in the year 2010. Even where protective measures and interlock devices were in place, our studies indicated that workers could access the robot area to perform the tasks in 188 sites(40.1%). Also, the 143 sites(30.5%) had control measures and equipment located in the safety fence. In addition, the robots found at 164 sites(35.0%) could be restarted without additional restarting operation. These three causes accounted for most of the workplace injuries during the industrial robot operations. Futhermore, we confirmed the fact that the protective measures of the current safety regulations were not strictly enforced. Based upon our studies and the investigation of the present status of the industrial robot operation, higher standards in training and supervision of workers in the robot operation must quickly be met in order to prevent these industrial injuries.

충돌 해석을 이용한 능동형 보행자 보호 시스템의 성능 분석 (Performance Analysis of an Active System for Pedestrian Protection Using Impact Analysis)

  • Although automotive safety technologies have been developed steadily, the efforts for pedestrian protection still seems to be insufficient. In a car-pedestrian accident, the structures such as the engine under a hood, the lower part of a windshield and the A-pillar are the major causes of fatal pedestrian injuries. Recently, there have been several studies on the active safety system to reduce the pedestrian injuries. The safety system consists of an active hood lift system and a pedestrian airbag. In this research, the safety performance of the active hood lift system and the pedestrian airbag is investigated by using the finite element method. The finite element model of the system is set up based on the head impact test, and the impact analyses are performed. The necessity and the usefulness of the safety system are verified.

황산 손상 환자의 임상적 고찰 (A Clinical Analysis of Patient Exposure to Sulfuric Acid Injured)

  • Purpose: This study was conducted to identify the characteristics associated with sulfuric acid injury in the emergency department. Methods: Data were collected retrospectively from January 2007 to December 2015 on all sulfuric acid injuries presenting to the emergency department in Gu-mi Soonchunhyung University Hospital. Patients injured by sulfuric acid were recorded over a nine year study period and collected data included demographics, injury mechanism, injured body part, hospital care and final diagnosis. Results: A total of 26 cases were identified. Most patients were male (88.5%) and the face was the most commonly injured body part. The most common mechanism of injury was splashing injury. A total of 16 (61.5%) patients were identified as having lesions worse than second degree burns. Conclusion: Sulfuric acid can cause severe and fatal skin burn. When working with sulfuric acid, acid proof protect clothing, goggles and glove should be worn. Furthermore, safety education and workplace environment improvement are necessary to reduce sulfuric acid injury.

전로사고 예방을 위한 인적오류 분석 (A Case Study on the Human Error Analysis for the Prevention of Converter Furnace Accidents)

  • Occupational fatal injury rate per 10,000 population of Korea is still higher among the OECD member countries. To prevent fatal injuries, the causes of accidents including human error should be analyzed and then appropriate countermeasures should be established. There was an severe converter furnace accident resulting in five people death by chocking in 2013. Although the accident type of the furnace accident was suffocation, many safety problems were included before reaching the death of suffocation. If the safety problems are reviewed throughly, the alternative measures based on the review would be very useful in preventing similar accidents. In this study, we investigated the converter furnace accident by using human error analysis and accident scenario analysis. As a result, it was found that the accident was caused by some human errors, inappropriate task sequence and lack of control in coordinating work by several subordinating companies. From the review of this case, the followings are suggested: First, systematic human error analysis should be included in the investigation of fatal injury accidents. Second, multi man-machine accident scenario analyis is useful in most of coordinating work. Third, the more provision of information on system state will lessen human errors. Fourth, the coordinating control in safety should be performed in the work conducting by several different companies.

  • Comparatively, the construction industry has, on average, a higher rate of fatal and major injuries, work induced ill-health and damage to properties than other industrial sectors; and this is a source of concern to industry stakeholders. The study showed that although subcontracting could be contributory to an increase in workplace accidents in the construction industry, contractual aspects of subcontracting arrangements (such as the power imbalance that exist along the client-customer interface) also present opportunities for improvements in health and safety management (HSM) practices in organisations. This conclusion was reached after an analysis of a questionnaire survey (with a 27% response rate) that assessed the attitudes and perception to health and safety issues.

Abdominal Impalement Injury Caused by Scaffolding Pipe following a Traffic Accident - A Case Report

  • Traffic accidents involving agricultural machinery have been decreasing, but mortality is still high due to a lack of safety devices such as seat belts. Furthermore, secondary damage, such as abdominal impalement injury caused by loaded materials, is more likely to occur, and this type of injury leads to a poor prognosis. Impalement with pipes is often more fatal than other penetrating injuries because the diameter of the pipe is usually larger in size than other loaded materials. We report a case of a 72-year-old man with secondary abdominal impalement injury caused by a scaffolding pipe following a traffic accident.

래들에 의한 재해발생 실태와 예방대책 (The Status of Accident and Prevention Measures for Ladles)

  • This study is aimed to improve safety of ladle in the metal products manufacturing industry. Burns may occur from spills, spatters or eruptions of hot metal from ladles during pouring or transporting. According to the statistics of occupational accidents about the ladle, many victims were exposed to or contacted with extreme temperatures. The many fatal injuries occurred as a result of unexpected discharging of molten metal due to tipping over ladles. To prevent ladles from turning over, the trunnion axis shall be located below the center of gravity of the ladle at all operating condition. For this purpose, the equation is proposed to calculate the minimum location of a trunnion axis for definite static stability when tilting.
