Korean industry has achieved dramatic growth in a short time through continuous technical innovations. However, safety consciousness and management have not kept pace with industry growth. The characteristics of industrial accidents in socio-technical systems have become so complicated that it has been challenging for safety professionals to implement effective preventive measures because it is difficult to determine what is happening in the complex workplace. Thus far, they have focused on technical improvements and the requirements of OSH regulations to avoid legal responsibility. Accordingly, this study has used the systemic analysis method to explain the emergence of something unexpected in complex and multi-layered business structures, which is usually related to the variabilities of humans and organizations. This study chose a shipyard where a few fatal accidents had occurred because the shipbuilding industry includes numerous variabilities, including obtaining orders, manpower supply, procurement, etc. Systemic analysis progressed using FRAM based on two accident cases related to the truss platform used in LNG vessels. The outcome shows that each function within the system has its variabilities and has become coupled or dependent on other functions, increasing the possibility of accidents. This analysis method can provide OSH professionals with practical techniques for explaining what is happening in a complex socio-technical system and how to take proper measures.

Recently, the criteria for assessing industrial accidents have been replaced by the mortality rate. It was found that the number of deaths from excavation work was the highest among construction machinery. The risk assessment is being conducted, however the industrial accident mortality rate has not decreased. Accordingly, this study aims to provide the basic for the create of a risk assessment model specialized in construction work at excavator. It provides absolute value from the risk model which is capable of delivery the probability of a disaster. In addition, we provide a relative risk model that compares the risk through scores between detailed works. The relative risk model is combined by likelihood and severity; the likelihood indicates the frequency of accidents and the severity indicates seriousness of fatal accidents. A variable that reflects the conditions of the construction site was added to the risk assessment model based on past disaster cases. And using the concepts of probability and average, the risk assessment process was quantified and used as an objective indicator. Therefore, the model is expected to reduce disasters by raising the awareness of disasters.

Frequent accidents occur when workers at construction sites leave the safety zone, and particularly in the past 5 years, 9 fatal accidents occurred at the Korea Railroad Corporation due to train accidents on other tracks during track work. With the Severe Accident Punishment Act taking effect in January 2022, it is a priority to secure a safe work environment for workers at industrial (construction) sites. Therefore, there is a need to manage workers' departure from the safety zone (construction zone) and to facilitate communication within the construction zone. In this study, a mobile edge computing CCTV system is proposed that uses geofencing to determine whether workers are working in the danger zone, which can judge and respond in real-time to the ever-changing field environment. The proposed system is mobile and flexible, rather than server-based fixed CCTV. However, since it is designed mainly based on images, it has limitations in recognition rate depending on the environment such as distance, viewing angle, and illumination. As a way to compensate for this, it is required to develop more reliable equipment by combining technologies such as LiDAR and Radar.
Fatal injuries and deaths have occurred resulting from the operation or disassembling of a powder extinguishers in Korea. If a rusted powder extinguisher is exploded during operation or disassembling, it rises to the air like a rocket and hits a head, chest, or neck part of an operator, resulting in his (her) death or injury. In this study examines the explosion accident cases of a powder fire extinguisher and analyzes their causes, reactions to explosion, and precautionary measures. And in this paper suggests 1) no using of old fire extinguishers in fire drill or education, 2) education and public information on danger of old fire extinguishers, 3) indication of explosion danger to cartridge-type dry chemical powder fire extinguishers had already placed to buildings etc., 4) obligatory indication of durable years on fire extinguishers, 5) examination for introduction of system that do hydraulic pressure test about fire extinguishers that given period past, 6) construction of system that reclaim old fire extinguishers, 7) construction of system that recycle or reuse old fire extinguishers, and 8) operation of help desk related to old fire extinguishers as precautionary measure of fire extinguisher explosion accidents.

Fatal accidents in the construction industry account for a higher proportion than other industries, and in particular, the collapse accident of formwork is likely to lead to a serious accident like death. This study aim to derive the importance ranking of formwork collapse factors using AHP technique for preventing fatal accident. The AHP survey was conducted on field construction engineers, construction project managers, safety managers, and formwork specialist foreman with 10 years on site experience. The results of AHP analysis is that the most importance factor of formwork collapse accident is 'non-compliance with the formwork shoring assembly drawing'. Next it is important in the order of 'poor installation of formwork shoring and accessories', 'formwork shoring is not installed vertically', 'non-compliance with the concrete curing period of the formwork shoring', 'safety supervisor not designated and negligent'. It is necessary to preferentially and intensively manage the high importance factors presented as a result of this research for reducing formwork collapse accident. In addition, it will contribute to reducing construction safety accidents if the factors of the formwork collapse accident suggested in this research are included in the formwork inspection check list and checked step by step in formwork construction.
Elevators are the main means of transport in buildings. A malfunction of an elevator in operation may cause in convenience to users. Furthermore, fatal accidents, such as injuries and death, may occur to the passengers also. Therefore, it is important to prevent failure before accidents happen. In related studies, preventive measures are proposed through analyzing failures, and the lifespan of elevator components. However, these methods are limited to existing an elevator model and its surroundings, including operating conditions and installed environments. Vibration occurs when the elevator is operated. Experts have classified types of faults, which are symptoms for malfunctions (failures), via analyzing vibration. This study proposes an artificial intelligent model for classifying faults automatically with deep learning algorithms through elevator vibration data, hereby preventing failures before they occur. In this study, the vibration data of six elevators are collected. The proposed methodology in this paper removes "the measurement error data" with incorrect measurements and extracts operating sections from the input datasets for proceeding deep learning models. As a result of comparing the performance of training five deep learning models, the maximum performance indicates Accuracy 97% and F1 Score 97%, respectively. This paper presents an artificial intelligent model for detecting elevator fault automatically. The users' safety and convenience may increase by detecting fault prior to the fatal malfunctions. In addition, it is possible to reduce manpower and time by assisting experts who have previously classified faults.

For a holistic and precise assessment of safety benchmarks within a construction venture, it's paramount to delineate between the intrinsic features of the construction and its real-time, on-site performance metrics. In this study, we delved into genuine accident instances to discern the interplay between these construction attributes and on-ground performance determinants in relation to safety mishaps, employing the binomial logit analytical framework. Our scrutiny underscored that construction expenditure profoundly modulates the likelihood of fatal occurrences. Notably, variables pertinent to on-site safety protocols wielded considerable influence over both fatal mishaps and accidents implicating multiple personnel. These revelations intimate that while ascertaining the safety quotient of a construction initiative, a mere classification and recalibration based on fiscal dimensions can elucidate much. Yet, a comprehensive safety appraisal necessitates transcending quantitative indices, such as frequency of mishaps or casualty rates, to encapsulate the multifaceted interventions and strategies adopted at the construction locale.
There are ongoing debates on the effectiveness of the Serious Accidents Punishment Act (SAPA) on ensuring safety management and accident prevention, as the annual number of fatal injuries did not decrease even after its enforcement. However, for right appraisal of the effectiveness of SAPA, it must not only be rated based on direct outcomes such as the number of fatal injuries, but also on the indirect effects related to the improvement of the safety management of firms or organizations. A construction superintendent is one of crucial persons who ensure worker safety in construction sites. They must have a high safety consciousness for effective and appropriate safety management in construction sites. In this, the impact of the enforcement of SAPA on the safety consciousness of the construction superintendents is investigated to add a new dimension in the appraisal of the effectiveness of SAPA. DAGMAR-based safety consciousness assessment model is used to measure four safety consciousness components, i.e. awareness, comprehension, conviction, and action. Three hundred and five responses obtained from a survey conducted among construction superintendents are used to analyze the shift of the safety consciousness of the construction superintendents due to the enforcement of SAPA. The results reveal that awareness and comprehension components significantly improved after the SAPA enforcement. Conversely, conviction and action components slightly improved. They also reveal that the changes are more significant for construction superintendents affiliated to principal contractors, compared to those affiliated to subcontractors.
Almost all companies have paid much attention to the safety management ranging from maintenance to operation even at the stage of designing in order to prevent accidents, but fatal accidents continue to increase throughout the world. In particular, it is essential to systematically prevent such fatal accidents as fire, explosion or leakage of toxic gas at factories in order to not only protect the workers and neighbors but also prevent economic losses and environmental pollution. Though it is well known that accident probability is very low in NPP(Nuclear Power Plants), the reason why many researches are still being performed about the accidents is the results may be so severe. HRA is the main process to make preparation for possibility of human error in designing of the NPP. But those techniques have some problems and limitation as follows; the evaluation sensitivity of those techniques are out of date. And the evaluation of human error is not coupled with the design process. Additionally, the scope of the human error which has to be included in reliability assessment should be expanded. This work focuses on the coincidence of human error and mechanical failure for some important performance shaping factors to propose a method for improving safety effectively of the process industries. In order to apply in these purposes into the thesis, I found 63 critical Performance Shaping Factors of the eight dimensions throughout studies that I executed earlier. In this study, various analysis of opinion of specialists(Personal Factors, Training, Knowledge or Experience, Procedures and Documentation, Information, Communications, HMI, Workplace Design, Quality of Environment, Team Factors) and the guideline for construction of PSF were accomplished. The selected method was AHP which simplifies objective conclusions by maintaining consistency. This research focused on the implementation process of PSF to evaluate the process of PSF at each phase. As a result, we propose an evaluation model of PSF as a tool to find critical problem at each phase and improve on how to resolve the problems found at each phase. This evaluation model makes it possible to extraction of PSF succesfully by presenting the basis of assessment which will be used by enterprises to minimize the trial and error of construction process of PSF.

Expresway traffic accident is fatal accident by high speed, especially human injury is a great social issue. This paper aims to identify characteristic differences of highway accidents that involve human injuries or not. To analysis the elements that affect the two types of accidents used the logistic regression model. The analysis showed that human injury accident rate is increased in case of straight road, flat, or cut-slope areas, barriers, male driver, and older driver. These results provide the ground for actions to counter the problems. By discovering factors for accidents leading to fatality, this study can provide important implications for authorities that establish highway safety measures and policies in preventing human injuries or deaths from car accidents.
