Pontomedullary Laceration, a Fatal Consequence of Skull Base Ring Fracture

  • Due to improvements in emergency resuscitation provided by rescue teams, more trauma victims who could have died due to sudden heart failure at the scene are brought to the hospital following resuscitation. Most of these patients present with major organ injuries and hypovolemic shock at the time of trauma. However, head trauma associated with sudden heart arrest is rare. Here, we report a case of ring fracture with pontomedullary laceration that led to sudden heart arrest.

  • This paper proposes a fuzzy tuning PID controller for straight line type inverted pendulum. The conventional PID controller which is used widely in industrial field has fatal drawback on determining control gains for practical system. The proposed controller tunes the gains automatically based on fuzzy urle derived from the experience of expert operator. The results of simulation and experiment show the efficiency of the proposed control method comparing with conventional PID control method in terms of rising time, overshoot, and overall errors.

  • Myxoma is a benign growth constituting nearly 50% of all primary cardiac tumors. It is important because in can be abolished by surgical therapy and is usually fatal if unrecognized and untreated. Primary cardiac tumors are uncommon in all age group. In contrast, tumors metastatic to the heart are significantly more common. The diagnosis is made by echocardiography and cardiac angiography. Recently a wider use of echocardiogram as the screening test of valve lesions can be expected to increase the number of myxomas found preoperatively. We report the two cases of left atrial myxomas which were removed successfully by surgery in the thoracic and cardiovascular surgery department, Marryknoll Hospital.

  • Reexpansion pulmonary edema following treatment of pneumothorax and pleural effusion is a rare complication. However, because of possibility of its fatal outcome, physicians must be aware of this complication and every effort must be made to prevent its occurrence. We experienced 5 cases of reexpansion of pulmonary edema. One was complete tension pneumothorax and became death despite of intensive management. Remained four were 3 pneumothoraces and 1 pleural effusion and discharged without event, fortunately.

  • Fatal anaphylactic reactions to protamine sulfate during cardiac surgery are very rare. We report a case of catastrophic bronchial spasm due to an anaphylactic reaction to protamine. The patient was managed successfully using a bronchodilator, steroid treatment, and extracorporeal membrane oxygenation.

  • The present paper deals with the casualties of bulk carriers, many of which resulted in catastrophic and fatal consequencies - losses of ships and lives. In fact the fatality and the alarming statistics of bulk carrier casualty have ling been criticized since 1980's by several seafarers. In the paper, the features of hull structure and operation of bulk carrier are, firstly, examined and, then, casualties and major causes are speculated, Secondly, the issues of circumstances around shipping and naval architectural circles related to the casualties are raised and discussed. Finally, it highlights the need and the way for a higher safety standard for the shipping industy.

  • Fracture of tracheostomy tube with subsequent migration into the tracheobronchial tree is rare, but tracheobronchial foreign body in child carries the potentially fatal risk of respiratory obstruction, We report a case of a 5-year-old girl who had aspirated a fractured tracheostomy tube which was removed under rigid bronchoscope.
