  • Malignant hyperthermia is a hypermetabolic, fatal syndrome triggered by anesthetic drugs that occurs frequently in genetically susceptible persons. It is characterized by tachycardia, rapidly increasing temperature, skeletal muscle rigidity, respiratory and metabolic acidosis, cyanosis etc. It has been noted that the majority of cases of malignant hyperthermia are fatal unless early diagnosis and treatment are performed. Thus, the accurate prediction of preanesthetic susceptibility and early diagnosis of malignant hyperthermia is necessary to appropriate treatment. Dantrolene sodium has been shown to be effective in the prevention and treatment of malignant hyperthermia. We experienced a case of malignant hyperthermia, which is presented of a 32-year-old healthy male patient in whom a orthognatic surgery was performed under $O_2-N_2O$-enfl-rane anesthesia with induction by pentobarbital and succinylcholine. We discuss this case with reviewing the history, incidence, etiology, pathophysiology, clinical signs & biochemical changes, prevention & treatment.

  • Ventricular septal defect complicating myocardial infarction is rare but fatal condition which requires early surgical intervention before end-organ failure ensues from cardiogenic shock. Since the first successful repair by Cooley et al in 1956, surgical skills and strategies were developed and modified to a great extent, and we adopted the new repair technique in our case which stresses that minimal or no part of the infarcted septum and left ventricular wall be resected. This technique obviates the need to resect the infarcted part of the septum and prevents recurrence of an even larger VSD, and provides adequate size and shape of the left ventricle after of transinfarction left ventriculotomy.

  • The rupture of a renal artery aneurysm is a rare disease that is difficult to diagnose. Although we usually consider the appropriate treatment to be open laparotomy with aortic aneurysm surgery or stenting with graft insertion through intravascular intervention, thus far, there is no general consensus on the treatment protocol for renal artery aneurysm. Notably, ruptured renal artery aneurysm is a true critical emergency that may result in a fatal outcome. We are reporting two renal artery aneurysm patients who had ruptured and underwent emergency laparotomy.

  • The main difference between a linear system and a nonlinear system is the existence of direct interactions between input signals. These interactions will be classified into three types, (1) self-interaction among different order terms of control signals, (2) static mutual interactions between the control signals, and (3) dynamic interactions through the coefficient venctor fields of the control variables. In this paper, we will show that interactions of type (2) and (3) can be avoided by applying an appropriate dynamic compensator, while the interaction of type (1) is fatal.

  • Generally, the concrete constructed during cold weather has the frozen damage which cause the fatal damage so that heat curing and sheet curing was performed to prevent the early freezing of concrete. However, partial refrigeration caused by thermal gradient has many troubles so that the construction hasn`t been done as possible. This paper presents the development of strenth properties and optimal mix design against frozen damage under the cold weather, 1$0^{\circ}C$ below the zero.

  • Crytococcosis is a subacute or chronic, very rare in incidence and very often fatal infection caused by Cryptococcus Neoformans. The organism is of world-wide distribution, affecting both sexes and all ages. The most common portals of entry appear to be respiratory tract. Recently we experienced one case of isolated Cryptococcal Granuloma on the right lower lobe. The patient was 63 year old housewife, who complained only of epigastric pain. On baseline studies of chest X-ray, we incidentally found the abnormal finding of mass density on right lower lobe.

  • Sixteen cases of cardiac valve replacements have been done in this department since 1970. Twelve cases of mitral valve replacement were done with Beall valve, 2 cases of aortic valve replacement with Starr-Edwards and Magoven valve and 2 cases of double valve replacement using Beall valve for mitral and Magovern valve for aortic. Three patients [18.8%] died during operation. Two cases [12.5%] of hospital mortality occurred because of congestive heart failure and asphyxia due to tracheomalacia 3 months after operation. Follow-up studies from two to 27 months showed excellent results except three cases of late mortality [18.8 %]. Thromboembolism occurred in two double valve replacement patients[12.5%]who were fatal.

  • Tracheo-innominate artery fistula[TIF] is the uncommon delayed fatal complication of tracheostomy. The mortality rate of the lesion, if not treated surgically, approaches 100%. A 64-year-old man presenting with a TIF after tracheostomy was treated by lateral repair and muscle interposition between the innominate artery and trachea. Preoperatively, bleeding was controlled by gauze packing around the tude under manual compression and hyperinflation of the balloon cuff of the tracheostomy tube. No abnormality was found by angiographic evaluation. The patient failed to regain consciousness and died 4 days later from sepsis.

  • In this paper a dispatching rule, $D^{*}$, for AGV operations in automated manufacturing system is proposed to improve the performance measures. And the algorithm is compared with existing heuristic rules via simulation to evaluate its capabilities. It is shown that the proposed rule enhances mean flow time, makespan and throughput, and avoids the locking phenominon which can be fatal to the operation of automated manufacturing systems.

  • Confusion of a patent ductus arteriosus (PDA) for the descending thoracic aorta is a fatal error occurring occasionally in infants or neonates. As a result, the left pulmonary artery (LPA) may be misconceived as the PDA, and ligated. This surgical mishap of other hospital leads to serious congestive heart failure and loss of left lung function due to the underdevelopment in the peripheral vascular and alveolar structures in neonates and premature infants. In this report, 3 cases of LPA ligation and subsequent treatment are presented.
