A Comparative Quantitative Analysis of IDEAL (Iterative Decomposition of Water and Fat with Echo Asymmetry and Least Squares Estimation) and CHESS (Chemical Shift Selection Suppression) Technique in 3.0T Musculoskeletal MRI

  • Patients who underwent hip arthroplasty using the conventional fat suppression technique (CHESS) and a new technique (IDEAL) were compared quantitatively to assess the effectiveness and usefulness of the IDEAL technique. In 20 patients who underwent hip arthroplasty from March 2009 to December 2010, fat suppression T2 and T1 weighted images were obtained on a 3.0T MR scanner using the CHESS and IDEAL techniques. The level of distortion in the area of interest, the level of the development of susceptibility artifacts, and homogeneous fat suppression were analyzed from the acquired images. Quantitative analysis revealed the IDEAL technique to produce a lower level of image distortion caused by the development of susceptibility artifacts due to metal on the acquired images compared to the CHESS technique. Qualitative analysis of the anterior area revealed the IDEAL technique to generate fewer susceptibility artifacts than the CHESS technique but with homogeneous fat suppression. In the middle area, the IDEAL technique generated fewer susceptibility artifacts than the CHESS technique but with homogeneous fat suppression. In the posterior area, the IDEAL technique generated fewer susceptibility artifacts than the CHESS technique. Fat suppression was not statistically different, and the two techniques achieved homogeneous fat suppression. In conclusion, the IDEAL technique generated fewer susceptibility artifacts caused by metals and less image distortion than the CHESS technique. In addition, homogeneous fat suppression was feasible. In conclusion, the IDEAL technique generates high quality images, and can provide good information for diagnosis.

  • We report two cases of spinal subdural hematoma in the lumbar region after trauma. They developed in a 27-years old man and a 32-year-old woman. They had no other causes such as bleeding tendency, preexisting spinal lesions, lumbar puncture, vascular malformation and anticoagulant therapy. There lesions were diagnosed with MRI(fat suppression) and treated by lumbar puncture.

  • 자기공명영상(Magnetic Resonance Imaging:MRI)은 물과 지방 사이에 화학적 이동 현상을 가지고 있고 지방에 의해 구조적 현상이 나타난다. 또한 지방은 기본적으로 강한 신호를 가지고 있어 잘못된 인공물을 형성한다. 그래서 지방소거(fat suppression)에 대해 연구하고자 한다. MRI에서 지방소거(fat suppression)방법만을 사용하여 진단하는데 약간의 어려움은 있지만 기본은 화학적 이동 선택 포화(chemical shift selective saturation : CHESS) pulse 방법을 이용하여 물과 지방 phantom을 가지고 지방소거(fat suppression)를 연구하는데 목적이 있다. 본 연구에서 CHESS pulse를 matlap 프로그램을 이용 Map pulse로 알맞게 design하고 Pre saturation을 하기 위한 pulse diagram이 가능하였고, fat phantom과 water phantom을 실험하여 지방소거 기법(fat saturation)의 효과를 볼 수가 있었으며 임상 적용 부위로 경추(C-SPINE), 요추(L-SPINE), 가습(BREAST)에 적용하여 의료영상진단에 있어 화학적 이동 선택 포화 CHESS pulse의 지방소거(fat suppression)는 유용한 방법임을 증명하였다.(충남대학병원 G.E 1.5T MR를 이용하여 실험하였다.)

T1 강조 경추자기공명영상에 대한 최적의 지방소거기법의 정량적 평가: TSE-CHESS 과 TSE-SPAIR 의 비교 (Quantitative Evaluation of Optimized Fat-Suppression Techniques for T1 Weighted Cervical Spine MR Imaging: Comparison of TSE-CHESS and TSE-SPAIR)

  • 본 실험의 목적은 TSE-CHESS 지방소거와 TSE-SPAIR 지방소거 기법을 비교하여 인체굴곡부위 지방소거에 있어서 TSE-SPAIR 기법의 임상적 유용성을 알아보고자 하였다. 총 25명의 경추질환이 없는 정상인을 대상으로 3.0 T MRI 기기를 이용하여 검사를 하였고, 사용된 두 기법에 대한 인체굴곡 부위 지방소거에 대한 정량적 분석으로 PSNR과 CNR을 평가하였다. 실험결과로 굴곡부위에 대한 PSNRs 과 CNRs 값은 TSE-SPAIR 기법이 유의성 있는 결과를 얻었다. 결론적으로, TSE-CHESS 지방소거와 TSE-SPAIR 지방소거 기법을 비교했을 때, 인체굴곡 부위에 대한 지방소거 평가에서 증가된 PSNRs 과 CNRs 값을 얻었다. 이러한 결과는 향후 인체굴곡 부위 지방소거 진단에 유용성 있는 정보를 제공 할 것이다.

  • 이 강의의 목적은 fat suppresion 과 water suppression에 관계되는 수소자기공명영상법의 기본 원리를 포괄적으로 이해하는데 있다. 임상적으로 이런 Technique의 수행이 필요한 가장 간단한 이유는 다른 병소의 신호와 구별을 하고 불필요한 artifact을 제거하기 위함이다.

복부의 T2강조 영상에서 지방소거기법의최적의 평가 (The Evaluation of Optimized Inversion-Recovery Fat-Suppression Techniques for T2-Weighted Abdominal MR Imaging : Preliminary report)

  • To test the real image quality of a spectral attenuated inversion-recovery (SPAIR) fat-suppression (FS) techniquein clinical abdominal MRI by comparison to turbo spin echo inversion-recovery (TSEIR) fat-suppression (FS) technique. 3.0T MRI studies of the abdomen were performed in 30 patients with liver lesions (hemangiomas n: 15; HCC n: 15). T2W sequences were acquired using SPAIR TSEIR. Measurements included retroperitoneal and mesenteric fat signal-to-noise (SNR) to evaluate FS; liver lesion contrast-to-noise (CNR) to evaluate bulk water signal recovery effects; and bowel wall delineation to evaluate susceptibility and physiological motion effects. SPAIR-TSEIR images produce significantly improved FS and liver lesion CNR. The mean SNR of the retroperitoneal and mesenteric fat for SPAIR were 20.5, 10.2 and TSEIR were 43.2, 24.1 (P<0.05). SPAIR-TSEIR images produced higher CNR for both hemangiomas CNR 164.88 vs 126.83 (P<0.05) and metastasis CNR 75.27 vs 53.19 (P<0.05). Bowel wall visualization was significantly improved using in both SPAIR-TSEIR (P< 0.05). The real image quality of SPAIR was better than over conventional TSEIR FS on clinical abdominal MRI scans.

Improvement of Fat Suppression and Artifact Reduction Using IDEAL Technique in Head and Neck MRI at 3T

  • Purpose: To quantitatively and qualitatively compare fat-suppressed MRI quality using iterative decomposition of water and fat with echo asymmetry and least-squares estimation (IDEAL) with that using frequency selective fat-suppression (FSFS) T2- and postcontrast T1-weighted fast spin-echo images of the head and neck at 3T. Materials and Methods: The study was approved by our Institutional Review Board. Prospective MR image analysis was performed in 36 individuals at a single-center. Axial fat suppressed T2- and postcontrast T1-weighted images with IDEAL and FSFS were compared. Visual assessment was performed by two independent readers with respect to; 1) metallic artifacts around oral cavity, 2) susceptibility artifacts around upper airway, paranasal sinus, and head-neck junction, 3) homogeneity of fat suppression, 4) image sharpness, 5) tissue contrast of pathologies and lymph nodes. The signal-to-noise ratios (SNR) for each image sequence were assessed. Results: Both IDEAL fat suppressed T2- and T1-weighted images significantly reduced artifacts around airway, paranasal sinus, and head-neck junction, and significantly improved homogeneous fat suppression in compared to those using FSFS (P < 0.05 for all). IDEAL significantly decreased artifacts around oral cavity on T2-weighted images (P < 0.05, respectively) and improved sharpness, lesion-to-tissue, and lymph node-to-tissue contrast on T1-weighted images (P < 0.05 for all). The mean SNRs were significantly improved on both T1- and T2-weighted IDEAL images (P < 0.05 for all). Conclusion: IDEAL technique improves image quality in the head and neck by reducing artifacts with homogeneous fat suppression, while maintaining a high SNR.

상완신경총 환자에서 최적의 자기공명영상 지방소거 기법에 관한 연구 : SPAIR, STIR 지방소거 중심으로 (A Study on Optimized MRI Fat-Saturation Technique for Brachial Plexus Patients : Focused on SPAIR and STIR Fat-Saturation)

  • 상완신경총에서 STIR(short TI inversion recovery) 지방소거 강조영상과 SPAIR(spectral adiabatic inversion recovery) T2, T1 지방소거 강조영상기법을 비교 평가하여 최적의 지방소거 기법을 알아보고자 하였다. 총 30 명 상완신경총에 질환이 없는 정상인 지원자를 대상으로 3.0 T MRI 기기를 이용하여 검사를 하였고, 분석방법으로 세 기법에 대하여 SNR, CNR 및 4단계 점수로 평가를 하였다. 정량적 분석 결과로 SPAIR T1 기법에 대한 SNR, CNR 값은 상완신경총기시부($03.07{\pm}0.98$, $-2.25{\pm}0.54$), 상완신경총몸체($06.70{\pm}1.81$, $36.31{\pm}2.17$)에서 높은 값을 제공하였다 (p<0.05). 정성적 분석결과 상완신경총 묘출도와 지방소거 정도, 영상의 인공물은 SPAIR T1($3.2{\pm}0.70$, $3.6{\pm}0.51$, $3.4{\pm}0.10$)기법이 의미 있게 높게 나타났다(p<0.05).

복부 MRI검사에서 m-DIXON기법의 유용성 평가: e-THRIVE기법과 비교 분석 (Evaluation of Usefulness of an m-DIXON Technique during an Abdomen MRI Examination : A Comparison with an e-THRIVE Technique)

  • 이 연구의 목적은 복부 MRI검사에 유용한 m-DIXON기법이 기존 e-THRIVE기법에 비해 임상적으로 유용한 검사인지 알아보기 위해 2013년 9월부터 2014년 2월까지 간 질환을 주소로 복부 MRI검사를 시행한 84명을 대상으로 분석하였다. 연구결과에서 SNR은 m-DIXON기법 $90.42{\pm}16.90$, e-THRIVE $60.42{\pm}11.54$로 나타났으며, CNR은 m-DIXON기법 $52.38{\pm}22.58$, e-THRIVE기법 $46.31{\pm}20.25$로 나타났다. 정성적 평가는 m-DIXON기법에서 영상의 질 $4.06{\pm}0.34$, 인공물 $3.64{\pm}0.22$, 지방소거 $4.16{\pm}0.15$로 나타났으며, e-THRIVE기법에서 영상의 질 $3.14{\pm}0.35$, 인공물 $3.06{\pm}0.38$, 지방소거 $3.14{\pm}0.30$으로 나타났다. 결론적으로 복부 MRI검사에서 m-DIXON기법은 e-THRIVE 기법에 비해 정량적 평가 및 정성적 평가에서 우수한 것으로 나타나, 향후 복부질환의 진단에 유용하게 활용할 것을 제안한다.

Chest Wall Lipogranuloma after Hydrogel Implant Rupture: Case Report

  • We present a 53-year-old woman with a large chest wall mass in the interpectoral space, which was eventually confirmed as a lipogranuloma resulting from hydrogel implant rupture. Ultrasonography (US) showed reduced implant volume with surrounding peri-implant fluid collection, suggesting the possibility of implant rupture. A heterogeneously hypoechoic mass was found between the pectoralis major and minor muscles adjacent to the ruptured implant. On magnetic resonance imaging (MRI), there was a large mass in the left interpectoral space of the upper inner chest wall. The mass showed slightly high signal intensity (SI) on pre-contrast T1-weighted image (WI) with mixed iso and high SI on T2-WI. The signal of the mass was suppressed using the water suppression technique but not with the fat suppression technique on T2-WI. The mass showed diffuse enhancement upon contrast enhancement. The enhancing kinetics showed persistent enhancement pattern. US-guided core needle biopsy revealed a lipogranuloma and removal confirmed a ruptured PIP hydrogel implant.
