Autologous Fat Graft in the Reconstructed Breast: Fat Absorption Rate and Safety based on Sonographic Identification

  • Background Autologous fat graft has become a useful technique for correction of acquired contour deformity in reconstructed breasts. However, there remains controversial regarding the efficacy and safety of the practice for reconstructive breast surgery. Methods A retrospective review was performed on 102 patients who had secondary fat grafting after breast reconstruction. Fat harvest, refinement and injection were done by Coleman's technique. All patients were followed up postoperatively within 1 month and after 6 months including physical examination and ultrasonography. In 38 patients, the reabsorption rate was calculated by serial changes of thickness between skin and pectoral fascia in the ultrasonic finding. Locoregional recurrence rate was compared with control group of 449 patients who had breast reconstruction without fat graft in the same time period. Results Average 49.3 mL fat was injected into each breast. The most common location of fat graft was upper pole, followed by axilla, lower and medial breasts. During 28.7 months of average follow-up period, 2.9% of total patients had symptoms of palpable mass on fat graft side and ultrasonography identified fat necrosis and cyst formation in 17.6% of the patients. Calculated fat reabsorption rate was 32.9%. Locoregional recurrence was occurred in 1 patient (0.9%) and the rate was not different significantly with control group (2%). Conclusions Although further studies are required to provide surgeons with definitive guidelines for the implementation of fat grafting, we propose autologous fat graft is an efficient and safe technique for secondary breast reconstruction.

Analysis of gluteal subcutaneous and muscle thickness in infants and children for application to intramuscular injection, autologous fat grafting, and gluteal artery perforator flaps

  • Background To achieve improvements in intramuscular injection, autologous fat grafting, and gluteal artery perforator flaps in infants and children, the relationships of computed tomography (CT)-determined gluteal muscle thickness (MT) and subcutaneous tissue thickness (SCT) with age and body mass index (BMI) were analyzed. Methods Gluteal SCT and MT at ventrogluteal (VG) and dorsogluteal (DG) sites were measured in a standardized manner in 350 patients aged 0 to 6 years who had visited a tertiary hospital and had undergone abdominopelvic CT between January 2005 and December 2016. Recorded measurements were analyzed using one-way analysis of variance and stepwise multiple regression to identify the factors that were most closely related to MT and SCT. Results Subcutaneous tissue at VG sites was thinner than at DG sites, but not significantly so in any age group. Muscles tended to be thinner at VG sites in 4 to 7-year-old, but thicker at VG sites in 1 to 3-year-old, though the differences were not significant. MT in the VG and DG regions was found to be related to age, and SCT in these regions to be related to age and BMI. Conclusions The VG and DG sites should be considered as alternatives for intramuscular injection in infants and children when the anterolateral site is problematic. In addition, considering the gluteal MT and SCT of infants and children should help produce good results in autologous fat grafting and gluteal artery perforator flap harvesting.

Abdominal skin subcutaneous fat thickness over the gestational period in Korean pregnant women: a descriptive observational study

  • Purpose: Although insulin is usually injected into the abdominal subcutaneous fat, in pregnancy women tend to avoid abdominal injections due to concern about fetal damage. Prior studies have been limited to only measuring skin-subcutaneous fat thickness (S-ScFT) at one site at specific pregnancy points. This study aimed to measure S-ScFT across several abdominal sites and over the gestational period in Korean pregnant women. This can identify which site would be relatively safe for subcutaneous injection during pregnancy. Methods: Healthy women over 24 weeks of pregnancy in Korea were invited to voluntarily participate in this descriptive study. For the 142 women, S-ScFT of 12 sites in the abdomen were measured by ultrasound, several times over the pregnancy. Each incidence was treated as a case and a total of 262 cases were analyzed. Results: The mean S-ScFT during pregnancy was 1.14±0.47 cm (1.25±0.54 cm at 24+0-27+6 weeks; 1.17±0.48 cm at 28+0-31+6 weeks; 1.09+0.40 cm at 32+0-35+6 weeks; and 1.06±0.47 cm at 36+0-40 weeks of pregnancy). Most S-ScFT were thicker than 10 mm. But S-ScFTs in the lateral abdomen and some sites were suboptimal (<6 mm), especially in the pre-pregnancy underweight body mass index group, who had a high rate of suboptimal thickness (27.1% overall and 33.9% in the lateral side). Conclusion: The whole abdomen seems to be appropriate for subcutaneous injection in most Korean women during pregnancy, with a 4 to 5-mm short needle. However, for the lateral abdomen, making the skin fold might be needed for fetal safety.

편측 성대마비에서 Restylane을 이용한 성대내 주입치료 1예 -증 례 보 고- (Restylane Injection into the Vocal Cord of the Patient with Unilateral Vocal Cord Paralysis -A Case Report-)

  • When a person was suffered from vocal cord paralysis or glottic insufficiency, injection materials (e.g Teflon, Bovine collagen, Autologous fat & tendon, Gelfoam) into the vocal cord have been widely used. But each injection material has some disadvantage. We introduce the Restylane which is composed of a hyaluronic acid, artificially producted. It has advantage of rate foreign body reaction, proper endurance, easy to injection. The patient was 55-year-old woman who showed left vacal cord paralysis after pneumonectomy due to aspergillosis, taken the type I thyroplasty and arytenoid adduction. The middle portion of left vocal cord has some atropic mucosal change, slight chink was noted. The restylane injection into vocal cord was done with suspension laryngoscopy under general anesthesia. In the 3 month follow-up after Restylane injection, the quality of voice has been better progressively. We report a case of Restylane injection as a new method for the improvement of quality of voice.

Effects of the Diabetic Condition on Grafted Fat Survival: An Experimental Study Using Streptozotocin-Induced Diabetic Rats

  • Background Autologous fat grafts have been widely used for cosmetic purposes and for soft tissue contour reconstruction. Because diabetes mellitus is one of the major chronic diseases in nearly every country, the requirement for fat grafts in diabetes patients is expected to increase continuously. However, the circulation complications of diabetes are serious and have been shown to involve microvascular problems, impairing ischemia-driven neovascularization in particular. After injection, revascularization is vital to the survival of the grafted fat. In this study, the authors attempted to determine whether the diabetic condition inhibits the survival of injected fat due to impaired neovascularization. Methods The rat scalp was used for testing fat graft survival. Forty-four seven-week-old male Sprague-Dawley rats were allocated to a diabetic group or a control group. 1.0 mL of processed fat was injected subcutaneously into the scalp of each rat. The effect of diabetes was evaluated by calculating the volume and the weight of the grafted fat and by histologically analyzing the fat sections. Results The surviving fat graft volume and weight were considerably smaller in the diabetic group than in the control group (P<0.05), and histological evaluations showed less vascularity, and more cysts, vacuoles, and fibrosis in the diabetic group (P<0.05). Cellular integrity and inflammation were not considerably different in the two groups. Conclusions As the final outcome, we found that the presence of diabetes might impair the survival and the quality of fat grafts, as evidenced by lower fat graft weights and volumes and poor histologic graft quality.

Nicolau Syndrome after Intramuscular Injection: 3 Cases

  • Nicolau syndrome is a rare complication of intramuscular injection consisting of ischemic necrosis of skin, soft tissue, and muscular tissue that arises locoregionally. The characteristic pattern is pain around the injection site, developing into erythema, a livedoid dermatitis patch, and necrosis of the skin, subcutaneous fat, and muscle tissue. Three patients were injected with drugs (diclofenac sodium, ketoprofen, meperidine) for pain relief. Three patients complained of pain, and a skin lesion was observed, after which necrosis developed on their buttocks. Each patient underwent debridement and coverage. The wound healed uneventfully. We report three cases of Nicolau syndrome in the buttocks following diclofenac intramuscular injection.

Streptozotocin 유발 당뇨병 쥐에서의 인삼 지용성분획의 혈당 강하작용 (Hypoglycemic Action of the Fat Soluble Fraction of Panax Ginseng C.A. Meyer in Streptozotocin Induced Diabetic Rats)

  • This study was made to understand a hypoglycemic action of the fat soluble fraction of Panax ginseng C.A. Meyer in streptozotocin induced diabetic rats by determining the activities of several enzymes related to carbohydrate and lipid metabolism as well as several blood component levels such as glucose and ketone bodies, and non-esterified fatty acids. Albino rats (Sprague Dawley, 170-200g, 3) were injected once with 70mg streptozotocinhg body weight intraperitoneally and fed with ordinary diet for 7 days, and then the fat soluble fraction (5 mg~20 mg/day/rat) was injected intraperitoneally once a day for three days to rats having high blood glucose level over 340 mg/100ml. After a final injection of the fat soluble fraction, rats u.ere starved for 16 hours followed by the analysis of blood serum and liver enzymes. It was found that increased levels of glucose, ketone bodies and free fatty acids in streptozotocin induced rats were decreased appreciably by administration of the fat soluble fraction. However, the amount of administered fat soluble fraction did not show any significantly different hypoglycemic action. Decreased activities of glucokinase, phosphofructokinase, pyruvate kinase, 6-phosphogluconate dehydrogenase and acetyl CoA carboxylase of the liver of streptozotocin induced diabetic rats were greatly modified suggesting that a hypoglycemic action of the fat soluble fraction was also appreciable as ginseng saponin fraction. We also compared a hypoglycemic action of the fat soluble fraction prepared from American ginseng and Chinese ginseng with that of Korean pain ginseng. 핀o significant difference of the hypoglycemic activity was observed between the above ginseng fat soluble fractions, suggesting that a study of the fat soluble fraction might be one of the most interesting subjects relating to diabetic hyperglycemia in the near future.

Effects of Dietary Modification on Plasma Glucose and Insulin Sensitivity in Streptozotocin-induced Diabetic Rats

  • There is substantial evidence that insulin sensitivity can be enhanced through appropriate dietary management . In this study, insulin sensitivity was evaluated using and insulin suppression test. Male Sprague-Dawley rats, were caused to be in a diabetic condition by the injection of streptozotocin, and divided into four groups. They were fed one of the following diets for 2 weeks : (group 1) a high-carbohydrate(CHO) low-fat low-fiber diet, (group 2) a high CHO low-fat and high-fiber diet, (group 3) a low-CHO high-fat and low-fiber diet, and (group 4) a low-CHO low-fat diet groups (as comparison between group 1 and group 2 shows). In the low-CHO high-fat diet groups, dietary fiber tended to decrease plasma glucose levels at the end of the experiment, but not significantly (as comparison between group 3 and group 4 shows). The average steady state plasma glucose level in rats on the group 3 diet was the highest among all four groups(p<0.05), indicating the poorest insulin sensitivity . However, high fiber increased insulin sensitivity in rats on the low-CHO high-fat diets(as shows by a comparison between group 3 and group 4). On the other hand , the high-CHO low-fat enhanced insulin sensitivity in rats on the low fiber diet(group 1 and group 3). The degree of enhancement of insulin sensitivity depends on the combination of CHO, fat , and fiber in the diet. In conclusion, this study demonstrates that a low-CHO high-fat low -fiber diet may be deleterious to diabetic rats. In view of insulin sensitivity enhancement , dietary fiber level is irrelevant, as long as the diet has a high-CHO and low-fat level.

Plasma Gel을 이용한 성대 주입술의 예비적 보고 (Preliminary Reports of Injection Laryngoplasty with Plasma Gel)

  • Background : Vocal fold injection using autologous material (fat or collagen) is very useful. However, Autologous material have variable resorption times and results, Plasma gel is a new injection material. The purpose of this study is to introduce a new injection material and discuss the effectiveness and complications. Subjects and Method: Eleven cases with vocal cord paralysis were analyzed after plasma gel injection, The plasma gel was acquired from patient's own blood. The preoperative and postoperative parameters including maximum phonation time (MPT) and subject aspiration score were analyzed. Results: There was a significant improvement in MPT and aspiration score in the case of vocal cord palsy. There was only one laryngeal complication. Conclusion : According to these preliminary results, the injection laryngoplasty with Plasma gel is a simple, safe, cheap procedure for temporally vocal fold palsy.

Effects of Whey Protein Injection as a Curing Solution on Chicken Breast Meat

  • The quality characteristics and storage stability of chicken breast meat (CBM) was investigated following the injection of whey protein (WP) as a curing ingredient. The moisture content of CBM decreased with increasing concentration of WP. The highest concentration of WP (7%) resulted in the lowest moisture and fat content and the highest protein content of CBM. Injection of WP elevated the pH and water holding capacity (WHC) of CBM. The cooking loss of CBM was significantly decreased with WP injections of 3% and higher. All WP injections increased the $L^*$ of the CBM but decreased the $a^*$ and $b^*$. WP injection increased the springiness, cohesiveness, and chewiness and decreased the hardness of the CBM. WP injection increased 2-thiobarbituric acid reactive substances (TBARS) after 3 and 7 days of storage. The volatile basic nitrogen (VBN) content of the CBM increased with increased concentrations of WP. The total microbial count (TMC) of CBM injected with WP was higher initially and after 3 days of storage. Our results showed WP injection improved the WHC of CBM but decreased the storage stability by increasing TBARS, VBN and TMC.
