  • Fine needle aspiration cytology (FNAC) is an easy convenient non-invasive method in the diagnosis of superficial palpable masses. The cytologic findings by FNAC of reactive and neoplastic lesions in various organs including breast, lymph node, thyroid, salivary gland, etc., have been described, but, those of soft tissue lesions including proliferative fasciitis are relatively rare to find. We recently experienced a case of FNAC of proliferative fasciitis in the left back of a 72-year-old male. The FNAC smears were scant in cellularity and contained large cells with abundant basophilic cytoplasm, one to two nuclei lying at the periphery, and prominent nucleoli that resemble ganglion cells.

Nodular Fasciitis of the Chest in a Young Woman

  • Nodular fasciitis is a benign reactive proliferation that usually involves the deep fascia. Although it is relatively common in the adult population, it is often misdiagnosed as sarcoma due to its rapid growth and pathological features. It rarely presents as a chest wall tumor in young patients. Here, we report a case of nodular fasciitis involving the chest wall of an 18-year-old woman and its surgical management. This case underscores the need to consider nodular fasciitis in the differential diagnosis of chest wall tumors in young patients.

요족형 족저근막염 환자에 대한 도침요법과 근막 추나요법 병행 치료의 임상적 효과: 증례 보고 (A Case Report of Talipes Cavus-Type Plantar Fasciitis Treated with Acupotomy and Fascia Chuna Therapy)

  • Objectives This case study aimed to investigate the effect of acupotomy and fascia chuna therapy on talipes cavus-type plantar fasciitis. Methods We classified the foot arch type using a podoscope and estimated the outcome by evaluating the numeric rating scale, pain disability index, and EuroQol-5-dimension measurement. Results After treatment, heel pain decreased and the quality-of-life score improved. Conclusions This study suggests that treatment with acupotomy and muscle energy techniques based on the foot arch type may be effective for plantar fasciitis. A limitation of this study is the small number of cases. Further clinical studies are required.

Effect of Korean Medicine Treatment Combined with Conventional Medicine in Patients Diagnosed with Plantar Fasciitis

  • This study examined the effectiveness of Korean-Western cooperative treatment for patients with plantar fasciitis. Fifty patients received Korean medicine treatments (acupuncture, pharmacopuncture, herbal medicine) and Western medicine treatments (polydeoxyribonucleotide, and extracorporeal shock wave therapy). Evaluation methods used were comparison before and after ultrasound (P9), and numeric rating scale scores. Results revealed a significant improvement in the level of pain and evaluation of improvement using ultrasound. Moreover, it was suggested that Korean-Western cooperative medicine treatment may be effective for the treatment of plantar fasciitis.

족저 근막염의 초음파 영상학적 형태 (Ultrasonographic Appearances of the Plantar Fasciitis)

  • Purpose: This retrospective study was designed to evaluate the appearances of plantar fasciitis by ultrasonography. Materials and Methods: 48 cases of unilateral plantar fasciitis were enrolled in this study. Plain radiograph and realtime sonography of both feet were perfomred and the results were compared between the affected feet and controlateral symptomless feet. Calcaneal spur were observed on plain radiograph and thickness of plantar fascia, hypoechogenecity, blurring of border of plantar fascia, perifascial effusion, wavy plantar fascia were observed on sonography. Results: Women (35 cases) and left feet (30 cases) were more frequent than men and right feet. Thickness of plantar fascia in affected site was thicker than normal site (p<0.01). Hypoechogenecity was observed only in 39 cases (81%) affected site, blurring of border of plantar fascia in affected site was 30 cases (62%) and 7 cases (15%) in normal site, perifascial effusion was observed only in 38 cases (79%) affected site, wavy plantar fascia in affected site was 43 cases (90%) and 2 cases (4%) in normal site. Calcaneal spur in affected site was 36 cases (75%) and 33 cases (69%) in normal site. Conclusion: Sonography is a useful diagnostic procedure for the plantar fasciitis especially in the unilateral plantar fasciitis.

평지와 오르막경사의 트레드밀 걷기와 달리기 동안 발보장구가 발바닥근막염과 관련된 발아치 스트레인에 미치는 영향 (Effects of Foot Orthotics on the Foot Arch Strain related to Plantar Fasciitis During Treadmill Level and Uphill Walking and Running)

  • There is some evidence that one of major factors to produce plantar fasciitis depends on the magnitude of the foot arch strain. The orthotics that can reduce the foot arch strain during locomotion may be effective to prevent or treat plantar fasciitis. Therefore, the purpose of this study was to investigate the effect of control condition and three types of foot orthotics on 3-dimensional foot arch strain that can produce plantar fasciitis during treadmill level and uphill walking and running. Sixteen male subjects are recruited and the arch length and height strain according to three types of foot orthotics with respect to control condition were measured by using two digital video cameras. The first hypothesis which the comfort of foot orthotics would be increased from arch pad, half length orthotics to full length orthotics was mostly accepted. It suggested that the types of the foot orthotics could be properly prescribed according foot regions that is pain or abnormal. The second hypothesis which the foot arch strain can be reduced by foot orthotics during level heel-toe walking and running and the third hypothesis which the foot arch strain can be reduced by foot orthotics during uphill heel-toe walking and running were rejected. The foot arch length and height strain during walking and running showed small and subject-specific characteristics and could not be optimal biomechanical variable to prove the overall comfort. The forth hypothesis which the foot arch strain cannot be reduced by foot orthotics during uphill toe walking and running was accepted. With the foot arch length and height strain during uphill toe walking and running the windlass mechanism suggested by Hicks can be explained successfully and excessive uphill toe walking and running can be one of cause of plantar fasciitis. The dynamic investigation on the foot arch such as walking and running should be carefully observed with integrated insights considering ligaments and foot bones as well as plantar fascia, extrinsic muscles and tendons, and intrinsic muscles and tendons.

족저근막 파열의 임상양상 (The Clinical Features of Plantar Fascia Rupture)

  • Purpose: The purpose of this study is to analyze the clinical features of plantar fascia rupture. Materials and Methods: We retrospectively reviewed 312 patients with plantar fasciitis between March 2008 and February 2013. We investigated age, sex, site, visual analogue scale (VAS), body mass index (BMI), characteristics of pain, awareness of rupture, and duration of symptoms. Acute rupture was defined as a rupture that occurred during exercise; chronic rupture was defined as a degenerative rupture after plantar fasciitis. We investigated the frequency of acute and chronic rupture. Results: Among 312 patients, 38 patients (12.2%) were diagnosed with plantar fascia rupture. Thirty-eight patients consisted of 14 men (36.8%) and 24 women (63.2%). The mean age of plantar fascia rupture was $58.29{\pm}12.54years$. The mean VAS score was 5.92 points (3~9 points). The mean BMI was $25.92{\pm}1.59kg/m^2$. Among the 38 patients, 2 patients had acute plantar fascia rupture and 36 had chronic plantar fascia rupture. In 34 patients-out of 36 chronic plantar fascia rupture, there were no subjective symptoms. Conclusion: Chronic rupture of the plantar fascia that occurred after plantar fasciitis was more common than acute rupture. Chronic rupture occurred at approximately 12% of patients treated with plantar fasciitis. In chronic rupture of the plantar fascia, there were no subjective symptoms of rupture. Therefore, we should doubt chronic rupture of plantar fascia when plantar fasciitis is prolonged.

대상포진 후 발생한 비부의 괴사성 근막염 치험례 (Necrotizing Fasciitis of Nose Skin Following Herpes Zoster)

  • Purpose: Varicella-zoster virus (VZV) infection is a common childhood disease. However, old and immune compromised patients are also at risk. Necrotizing fasciitis is a life threatening infection of the subcutaneous tissues, rapidly extending along the fascial planes. It is associated with a significant mortality rate, reported between 20% and 50%, and is therefore regarded as a surgical emergency. The authors treated a patient, who developed skin necrosis of her nose and left hemifacial area, following VZV infection. There are few literatures concerning this case; therefore, we present a rare case with review of literature. Methods: A 39-year-old woman had shown a localized, painful, multiple bullae and eschar formation in her nose and left hemifacial area for several days. Her skin lesion had rapidly worsened in size and morphology. Results: We diagnosed her as a necrotizing fasciitis, following herpes zoster, and then we performed a debridement of necrotic tissue and took a full thickness skin graft on her nose and left hemifacial area. Now, she was followed up with acceptable aesthetic result after 6 months. Conclusion: Secondary bacterial skin infection following VZV, can cause a result in a higher risk of complications. Among the complication, a necrotizing fasciitis of the head and neck is uncommon, and involvement of the nose is even more rare. Through this uncommon case report, we intend to emphasize the fact that early diagnosis of necrotizing fasciitis is very important, since it frequently necessitates surgical treatment which improves morbidity and leads to good recovery.

청소년 시기의 두개 근막염 1례: 증례보고 (A Case Report of Cranial Fasciitis in an Adolescent Male)

  • Purpose: Cranial fasciitis is a rare type of benign tumor that occurs mostly in children younger than 6 years. It arises from the deep fascia, periosteum, or fibromembranous layer that covers fontanelles. The etiology is unknown, although prior trauma has been postulated to be an underlying cause. There is a 2:1 male predominance. Despite its rapid growth, this tumor has a benign clinical course and can be cured by total excision. Methods: A 16-year-old male presented with a 3 cm-sized palpable mass in the left lateral eyebrow region that he first noticed 4 months before presentation. The mass had grown rapidly since it was first noticed. Preoperative brain computed tomography showed a well-demarcated mass approximately 3 cm in size extending from the subcutaneous layer to the periosteum. Preoperatively, the presumed diagnosis was a dermoid cyst. An operation was performed with the patient under general anesthesia. The subcutaneous mass was completely excised by periosteal dissection. Results: Histological diagnosis revealed the presence of cranial fasciitis. After 20 months of follow-up, there have been neither complications nor evidence of local recurrence besed on clinical examination. Conclusion: Although cranial fasciitis is quite rare, it should be considered in the differential diagnosis for lytic skull lesions in patient whose clinical presentation suggests this possibility. This condition could be occasionally mistaken for malignant or locally aggressive lesions. To prevent local recurrence, curettage of the underlying bone is recommended for patients with bone involvement.

  • Objectives This research aimed to investigate recent clinical treatment of herbal medicine fumigation for plantar fasciitis. Methods We searched recent clinical studies regarding herbal medicine fumigation in traditional Chinese medical journals on plantar fasciitis through the China Academic Journal (CAJ). Clinical articles published from 2012 to July 2022 were analyzed. This study examined the types of study, periods, herbal medicine, assessment, and risk of bias and conducted a comparative evaluation of herbal medicine fumigation added to other treatments. Results Ten studies were selected from 52 studies. Eight studies were classified as randomized controlled trial. Various assessment scales were used. Most treatments were administered for approximately 20 to 30 min once daily. Most studies revealed that herbal medicine fumigation is effective for plantar fasciitis. Conclusions Most studies showed that herbal medicine fumigation is effective for plantar fasciitis. More studies are needed for Korean medicine development.
