Participation and Influencing Factors in the Decision-Making of Life-Sustaining Treatment: A Focus on Deceased Patients with Hematologic Neoplasms

  • Purpose: This study aimed to investigate the involvement of patients who died from hematologic neoplasms in the decision-making process surrounding the withdrawal of life-sustaining treatment (LST). Methods: A total of 255 patients diagnosed with hematologic neoplasms who ultimately died following decisions related to LST during their end-of-life period at a university hospital were included in the study. Data were retrospectively obtained from electronic medical records and analyzed utilizing the chi-square test, independent t-test, and logistic regression. Results: In total, 42.0% of patients participated in the decision-making process regarding LST for their hematologic neoplasms, while 58.0% of decisions were made with family involvement. Among these patients, 65.1% died in general wards and 34.9% in intensive care units (ICUs) as a result of decisions such as the suspension of LST. The period from the LST decision to death was longer when the decision was made by the patient (average, 27.15 days) than when it was made by the family (average, 7.48 days). Most decisions were made by doctors and family members in the ICU, where only 20.6% of patients exercised their right to make decisions regarding LST, a rate considerably lower than 79.4% observed in general wards. Decisions to withhold or withdraw LST were more commonly made by patients themselves than by their families. Conclusion: The key to discussing the decision to suspend hospice care and LST is respecting the patient's self-determination. If a patient is lucid prior to admission to the ICU, considerations about suspending LST should involve the patient input.

  • Purpose: The purpose of this study, which was guided by the Resiliency Model of Family Stress, Adjustment, and Adaptation, was twofold: (a) to explore family and parental adaptation and factors influencing family adaptation in Korean families of children with Down syndrome (DS) through a quantitative methodology and (b) to understand the life with a Korean child with DS through a qualitative method. Methods: A mixed-methods design was adopted. A total of 147 parents of children with DS completed a package of questionnaires, and 19 parents participated in the in-depth interviews. Quantitative and qualitative data were analyzed using stepwise multiple regression and content analysis respectively. Results: According to the quantitative data, the overall family adaptation scores indicated average family functioning. Financial status was an important variable in understanding both family and parental adaptation. Family adaptation was best explained by family problem solving and coping communication, condition management ability, and family hardiness. Family strains and family hardiness were the family factors with the most influence on parental adaption. Qualitative data analysis showed that family life with a child with DS encompassed both positive and negative aspects and was expressed with 5 themes, 10 categories, and 16 sub-categories. Conclusion: Results of this study expand our limited knowledge and understanding concerning families of children with DS in Korea and can be used to develop effective interventions to improve the adaptation of family as a unit as well as parental adaptation.

  • The purpose of this study is to get preeminent data from comparative analysis of multi-family housing unit plans between Korea and China nowadays. Even though Korea and China belong to common cultured band in Asia, there are many different Housing aspects and unit plans. Through this study we want to grasp the characteristics of both nation's apartment unit plans and to contribute for advance into the China apartment market. Analysis target is constructing and lotting out apartment at South Korea and China in 2004, and the space syntax model is used as an analysis tool. First, analyze kind, number and special quality of each room by use area, and second, analyze area and scale of the rooms. Through the integration analysis we know the grade of rank and centrality of each rooms, and search difference of South Korean and Chinese apartment.

  • This study attempts to suggest new directions of urban multi-family housing design in the recent era of information, in which the representation of publicity and privacy of the housing reflects new meanings and relationships of the current digital society. The study examines cases of Dutch multi-family housing and investigates the meanings and relationships of publicity and privacy in the building as well as unit design of the housings. Borneo-Sporenburg housing development is a good example that suggests new interpretation and design solution for low-rise, high-density multi-family housing. Thus, this study analyzes street patterns, facade design, and ways of housing unit combination in Borneo-Sporenburg housing and concludes as follows. First, public space in Borneo-Sporenburg housing, which was designed differently from hierarchical and centripetal organization of modern architecture, is divided into small units and spread into the inside, which provides high potential for personal control of space and personal programming of space by space choices. Second, street pattern and facade design of Borneo-Sporenburg housing provide visual publicity and privacy simultaneously as they maintain unity as a whole as well as articulate individual unit in many different ways and provide clues to neighboring. Streets as a public domain have self-controlled boundaries for residents and introduce voluntary use-programs for residents. Third, facades of the housing have no inter-mediate space or common space, and confront streets directly. Space in-between is composed inside the facade and extends into the streets by residents' own choices. Fourth, privacy and individuality of the housing is strengthened. Units of the housing have individual entrances, unique plan type and complicated combination of space that all together emphasize individuality of units, however they are not often notified from the outside.

  • The purpose of this study is to analyze and assess the family policy of the Lee administration and to suggest agendas for sustainable future family policy. Traditionally, families are a unique unit of production and reproduction of family members and social laborers. In addition, families are the social safety net help people survive in a society. Therefore, family policies play an important role in contemporary society. In this context, we review the literature related to family and social policy, and the contents of Ministry for Health, Welfare and Family Affairs, which is the main authority in Korea. The results are as follow: Firstly, the Lee administration ignored the data showing that family policy should be a unique policy, and not a sub-division of welfare policy. Secondly, there is no difference between the active welfare policy of the Lee administration and policies of past administrations. Finally, this study suggests that subjects of future oriented family policy should focus on co-developing programs for the individual, family, and society, to develop problem-prevention and family needs- ordered policy, and to make integrated family policy through laws and delivery systems such as Healthy Family Centers.

대학주변 원룸형 다가구주택의 실내소음수준 실태 (Present Condition of Indoor Noise Level in One-Room Type Multi-Family Housings around Campus)

  • The present study is a preliminary research improving the dwelling quality of one-room type multi-family housings around the university campus. The purpose of the study is to investigate the present condition of Indoor noise level using · residents' responses and field measurements. The respondents are 104 residents living in one-room type multi-family housings. The field measurements on equivalent noise level of indoor and outdoor were carried out in 6 subject house units during the $26th\~28th$ of November 2002. The results are as follows. 1) The residents show relatively non-positive responses at evening and night on the present condition of indoor noise. 2) They answer 'living equipment foise' and 'water hammer' as major types of indoor noise of house unit. 3) Outdoor noise levels, basic factor of noise environment in 6 subject buildings were distributed $52.8\~65.3dB(A)Leq_{5min}$ and were inappropriate to the standard for environmental noise, $55 dB(A)Leq_{5min}$. 4) Indoor noise levels of subject house units were measured as $27.5\~63.5dB(A)Leq_{5min}$, the average of each house unit except one house unit was higher than the level feeling as noise, 40dB(A). 5) It was found that the differences of indoor noise levels between subject house units were caused by 'residents' living noise', 'living equipment noise', 'water hammer', and 'walking and talking noise in stairs and corridors'. 6) Therefore, it is required to plan for improving the quality of noise environment in one-room type multi-family housing around the campus. For example, soundproof construction (including double window with pair glass and balcony), outdoor garden with trees and water for increasing natural sound, interior materials with sound absorbing power to absorb living noise, soundproof pipe or double surface pipe for decreasing 'water hammer', and noiseproof floors, etc. are required.

  • According as the modern form of the family becomes nuclearized, inevitable is the isolation of the family. The relationship of the neighborhood and the kinsmen are being dissolved, making the isolated and unsteady nuclear family. We need a community in the modern society, for we are facing a lot of social problems as the nuclear family. The problems are as follows ; the rearing of the children, the alienation of a woman and the woman's roles in the society. In addition to those problems, there are also such some other problems as the alienation of the old man and the social roles of them. That is to say, the community appeared as a kind of attempt to overcome the above-mentioned problems, Kibbutz may be itself called and enlarged family to function as one family even though it is not just a blood relationship. The American commune is a large family community as a group to overcome the alienation and solitude of the nuclear family. On the other hand, the urban community is the small-scaled and cooperative family to solve the problem that the modern nuclear family is a unit living solitarily and unsteadily. But in fact, even in such a community, there are much tension and conflict against its establishing purpose. In my opinion, if those communities have to surmount the tension and conflict which are derived from the process in a bid to overcome the troubles like the alienation and solitude of the nuclear family. The bond of family must be consolidated among all the members of the community. And then community may be co-existed with the traditional family as the alternative family in the future, which can supply the deficiency of the unsteady unclear family.

고위험 신생아 가족의 스트레스, 가족의미 및 적응에 영향을 미치는 요인 (Influencing Factors on Family Stress, Family Meaning and Family Adaptation in Families with High Risk Neonates)

  • Purpose: The purpose of this study was to identify the relationship among family stress, family meaning and family adaptation of families with high risk neonates. Method: The date was collected on the basis of self- report questionnaires (August 2004 to March 2005); Tow-hundred twelve parents, who had high risk neonates in C hospital's neonatal intensive care unit, participated on request. Results: Family sense of coherence, family meaning, social support, family stress, marital communication and patient condition had a significant, direct effect on family adaptation. Family cohesion, religion, confidence in the health professional, and length of stay had a significant, direct effect on family meaning. Conclusion: The results of this study suggest the consequences associated with high risk neonates may be alleviated by a family support intervention designed to improve parental communication skills as well as to maintain family cohesiveness. Medical care could also encourage more emotional support of parents towards their neonate.

맞벌이 가족 라이프스타일에 따른 아파트 단위평면에 관한 연구 (A Study on the Apartment Unit Plans by Double-income Households Considering Life Style)

  • With the change of family type and diversify of life style, residents need various unit plan in quality. A rate of economically productive women has increased, and generally double-income households will continuously increase in the future. So a research is requested to consider the double-income households's satisfaction for a unit plan based on their life characteristics. The purpose of this study is to find out double-income households's lifestyles, and to search and analyze users' needs for spatial composition according to their lifestyles, and then, embody spatial composition of Apartment Unit plans. The findings are as follows. (1) Housing lifestyles of users living in apartment arc categorized into 5 types: pursuing the trend and personality, pursuing the convenience and leisure, pursuing being domestic, making a livelihood and practicality, the type of unspecification. (2) The type of pursing the trend and individuality requests new plan and a shower bath, the type of pursing the convenience and leisure requests facility with home appliances and likes traveling and sports, the type of making a hearth and home wants a talk with their family and discuss some problem, the type of making a livelihood and practicality requests storage space, the type of unspecification wants a comfortable master bedroom zone. Consequently factors of residental space plan are need for double-income households.

소형 임대주택 가구특성 및 실내공간 이용실태 조사 연구 (A Study on the Residents Characteristic and the Inner Space Use of Small-Sized Rental Multi-family Housing)

  • As the provision of National Rental Housing has led the consciousness on rental housing changed, it becomes more necessary to improve the quality of rental housing. The purpose of this study is to analyze the inner space use of a unit and residents' consciousness on the results of surveying the residents in order to construct rental housing which provides comfort residental environment for the residents. Data was collected through questionnaire surveys(459 residents) from 5 estates including 3 estates of National Rental Housing and 2 estates of Public Rental Housing, which is $36m^{2}\;and\;45m^{2}$ of net area of a unit, for the actual use conditions and their requirements on the inner space of a unit. The results are as follows ; 1) The rental housing differentiated according to the needs from the residents' characteristics should be developed. 2) The main demand household for rental housing is established to that of 4 families including a 40s year-old couple and that of 3 or 4 families including a 30s year-old couple, and a double income family. But the type of household becomes more diverse than before. 3) It is necessary to plan the living room for the Multi-functional inner space emphasizing commom space 4) The various and efficient storage space and system for the residents' demands.
