Development of a Family Nursing Model for Prevention of Cancer and Other Noncommunicable Diseases through an Appreciative Inquiry

  • Background: Cancer and non-communicable diseases are a major issue not only for the developed but also developing countries. Public health and primary care nursing offer great potential for primary and secondary prevention of these diseases through community and family-based approaches. Within Thailand there are related established educational curricula but less is known about how graduate practitioners enact ideas in practice and how these can influence policy at local levels. Aim: The aim of this inquiry was to develop family nursing practice in primary care settings in the Isaan region or Northeastern Thailand and to distill what worked well into a nursing model to guide practice. Materials and Methods: An appreciative inquiry approach involving analysis of written reports, focus group discussions and individual interviews was used to synthesize what worked well for fourteen family nurses involved in primary care delivery and to build the related model. Results: Three main strategies were seen to offer a basis for optimal care delivery, namely: enacting a participatory action approach mobilizing families' social capital; using family nursing process; and implementing action strategies within communities. These were distilled into a new conceptual model. Conclusions: The model has some features in common with related community partnership models and the World Health Organization Europe Family Health Nurse model, but highlights practical strategies for family nursing enactment. The model offers a basis not only for planning and implementing family care to help prevent cancer and other diseases but also for education of nurses and health care providers working in communities. This articulation of what works in this culture also offers possible transference to different contexts internationally, with related potential to inform health and social care policies, and international development of care models.

  • The adult guardianship system has been introduced through amendments of Korean Civil Code for the first time in the March 2011(Act No. 10429, 7. 1. 2013. enforcement). The adult guardianship system has the main purposes to provide a lot of help vulnerable adults and elderly, and protect them on the welfare related with property act, treatment, care, etc. There could be a controversy about whether the protection Legal Guardian's consent(formerly known as the Mental Health Act) or permission of the Family Court(revised Civil Code) are required to, or the Mental Health Act should be revised, when mental patient will be hospitalized forcibly. The author proposes that mental patient with Adult guardians should be determined by Legal Guardian's consent and approval of the Family Court, but mental patient without Adult guardians could be determined by Legal Guardian's consent. The issue of Withdrawing of life-sustaining treatment could be occurred due to the aging society and the development of modern medicine, and this has provided difficult, various problems to mankind in Legal, ethical, and social welfare aspects. The need of Death with dignity law or Natural death law has been reduced for a revision of the Civil Code. Therefore, on the issue of Withdrawing of life-sustaining treatment, in the future, intervention of the court is necessary in accordance with the revised Civil Code Section, and Organ Transplantation Act and the brain death criteria may serve as an important criterion.

  • Along with the change in the values of marriage and the prevalence of non-marriage in Korean society, a new form of family composition called unmarried birth or non-maritalism childbirth has appeared, and social discussion in taking place in connection with the problem of a decrease in the birthrate. Using sentiment analysis and social network analysis, this research explored how the people's sentiment and perception has changed toward 'nonmarital birth.' The data used is comments on news articles from the period of November 2020 to August 2021. As a result of the study, there were a lot of positive comments during the social issue period by marriage, whereas there were many negative comments from the policy agenda to the policy making period. As a result of co-occurrence network analysis, the topic of family norm, policy, and personal aspect appeared. This study is significant in that it revealed that negative perceptions prevailed during the policy-making process after the issue of unmarried births after the issue of unmarried births, and it became a cornerstone of social discussion on unmarried births

Knowledge about Cervical Cancer Risk Factors and Pap Smear Testing Behavior among Female Primary Health Care Workers: A Study from South Turkey

  • Background: Cervical cancer is one of the ten most frequent cancers in Turkey. We here examined knowledge about cervical cancer in relation to Papanicolaou (Pap) testing among female primary health care workers in Hatay, a city is located in the south of Turkey. Materials and Methods: The study was completed on 261 women healthcare workers who were or had been sexually active and who accepted to participate to the study. The participants gave verbal informed consent and thereafter questionnaires prepared by the investigators were administered by personal interview. Results: Only 30.3% (n=79) of the participants regularly had a gynecologic examination. While 87.4% (n=228) of the participants reported that they had already heard about the Pap smear test, only 45.2% (n=118) had undergone this test. It was determined that had undergone an average of $1.66{\pm}0.89$ times (1-4) within the last five years. Some 56.0% (n=117) of the participants were well informed about the Pap smear test (p<0,001) and 81.1% (n=63) of the participants who regularly had gynecological examinations (p<0,001) had this test. Conclusions: For the early diagnosis of the cervical cancer, regularly having a Pap smear test is crucial. Healthcare workers should also demonstrate sensitivity about this issue. We think that the importance of the issue should be re-highlighted by organizing in-service training for female primary healthcare workers. Studies are warranted to determine the psychosociological factors that cause individuals to not have the test.

  • This study aims to investigate the Healthy Family Support Centers'(HFSCs) operational patterns and their programs by surveying 46 HFSC employees in order to prepare for the massive increase of the number of HFSCs nationwide. This study might contribute to not only rebuilding the HFSCs' operational strategies but suggesting visible ideas for the HFSCs' programs. Results were as followed: First, most HFSCs consisted of several teams, i.e., educational team, counseling team and cultural team. However, the number of employees in each team was different from one regional community to another. The most difficult task in the HFSCs' operation was publicity work. Citing the issue of system delivery difficulties, HFSC employees insisted that budgetary deficit was the most difficult obstacle in running the HFSCs. Second was in regards to the programs that each team was planning, performing, and evaluating for each program. The HFSCs' programs were mostly structured around family difficulties or problems that arise according to the family life cycle. Based on these study results, more unified and specialized programs for HFSCs should be developed. In order to achieve this related studies should continue to conducted.

Comparison of the Marginal Utility and Disease Burden of Hearing Loss and Other Chronic Diseases

  • Background: Compared with other chronic diseases, hearing loss is generally overlooked from the perspective of disease burden. However, hearing loss is emerging as an important issue in the current society. The objective of this study was to investigate disease burdens and marginal utilities associated with hearing loss and other chronic diseases. Methods: This study analyzed the cross-sectional data of 32,986 participants aged 19 years and older who completed the Korea National Health and Nutrition Examination Surveys between 2009 and 2013. Additionally, this study used the pure tone audiometric test, European Quality of Life-Five Dimensions Questionnaire, and chronic disease status. The data were analyzed using a multiple linear regression method. Results: The data of 23,297 people who underwent a pure tone audiogram and completed the European Quality of Life-Five Dimensions Questionnaire were used in this analysis. The marginal utility of hearing loss ranked fifth among nine chronic diseases. The estimated loss of quality-adjusted life years associated with hearing loss was -93.69 years per 100,000 people, which is similar to other chronic diseases. Conclusion: This study assessed the marginal utilities and public burdens of hearing loss and eight chronic diseases in a South Korean population. Although the values may vary depending on country and race, this study may provide an indispensable foundation for more detailed studies on hearing loss.

  • The problem of the formation of machine-part cells in FMS is a very important issue at the planning and operating stages of FMS. This problem is inherently a combinatorial optimization problem, proven to be NP-complete(or, NP-hard). Among the several kinds of approaches which have been applied to solve the combinatorial optimization problems, the Simulated Annealing(SA) algorithm, a technique of random search type with a flexibility in generating alternatives, is a powerful problem solving tool. In this paper, the SA algorithm is used to solve machine cell-part family formation problems. The primary purpose of the study is to find the near-optimal solution of machine cell-part family formation problem, whare the product volume and number of operations are prespecified, that can minimize the total material handling cost caused by exceptional elements and intercell moves as much as possible. The results show that the SA algorithm is able to find a near-optimal solution for practical problems of the machine cell-part family formation.

  • Previous studies have focused on service quality by examining customer satisfaction and retention. Little, if any work has been conducted exploring the emotional state of customers and that state in con-nection to perceived service quality. The objective of this study was to explore and determine how a customer's emotions perceive and respond to service quality within family restaurants. It was found that within a positive emotional field, product and personal service quality showed a high correlation to the positive image while customers experiencing negative emotions had the perception of poorer product and personal service quality as well as a worse impression of cleanliness. Cleanliness was not connected to the issue of customer satisfaction but each of these three factors, whether positive or negative, led to the creation and mediation of an image which in itself directly resulted in customer satisfaction. Customer emotional fields are not merely the by-product of quality issues, or satisfaction but in fact the foundation by which all other factors must be considered and analyzed. Family restaurateurs need to focus on the customer's emotional well-being to create customer satisfaction.

  • Wandering behavior is a serious problem among the elderly in nursing homes, yet it has received relatively little study As multi-family houses built before the '80s in South Korea were designed, it did not take account into issues related to parking so that at present parking became a big issue. The Seoul Metropolitan Government tried various ways to sort out the parking problem in multi-family house areas, but inadequate parking and illegal parking are still an ongoing problem. Thus, the purpose of this study was to examine current situation on resident parking only and illegal parking centered on a congested area of multi-family houses in Seoul and present the improvement plan of parking problems through case investigation home and abroad. As the result of the survey, it has been identified that the subject area consists of 506 households in total and 186 parking spaces and also, 100 cars in parking space or parking lot in housing space and 143 in illegal parking. Green parking business and open access parking system were suggested as solution plan for parking problem. Thus, it is expected that through green parking business for 6 houses and the parking lot of elementary schools around, the illegal parking problem would be sorted out.
