다발성감각운동자극 치료가 뇌졸중 환자의 보행과 낙상위험도에 미치는 효과: 무작위배정예비임상시험 (Effect of Multi-Sensorimotor Training on Gait Ability and Fall Risk in Subacute Stroke Patients: A Randomized Controlled Pilot Trial)

  • Purpose : To determine whether an advanced rehabilitation therapy combined with conventional rehabilitation therapy consisting of sensorimotor exercises that would be superior to a usual treadmill training in gait ability and fall risk in subacute stroke patients. Methods : Thirty subjects randomly assigned to either multi-sensorimotor training group (n=19) or treadmill training group (n=18). Both groups first performed conventional physical therapy for 30 min, after which the multi-sensorimotor training group performed multi-sensorimotor training for 30 min, and the treadmill training group performed treadmill gait training for 30 min. Both groups performed the therapeutic interventions 5 days per week for 8 weeks. Gait ability was evaluated using the GAITRite system and Fall risk was measured using the Biodex Balance system before intervention and after 8 weeks. Results : There were no intergroup differences between demographic and clinical characteristics at baseline (p>.05). Both groups showed a significant improvement in gait ability (p<.05) and Fall risk (p<.05). In particular, the multi-sensorimotor training group showed more significant differences in gait velocity (p=.05), step length (p=.01) and stride length (p=.014) than the treadmill training group. Conclusion : The multi-sensorimotor training program performed on multiple types of sensory input had beneficial effect on gait ability. A large-scale randomized controlled study is needed to prove the effect of this training.

Comparison of Balance and Fall Efficacy of Virtual Reality Program in Elderly Women with Fall Experience

  • Purpose: The objective of this study was a virtual reality-based balance training program effective for improvement in physical function, examined the balance ability and fall efficacy of elderly women with experienced falls, intending to examine the program's usefulness as an exercise program to prevent the recurrence of a fall. Methods: The participants were 30 elderly women aged 65 or older who met the conditions. The participants were randomly assigned to either a balance training group (BT) or a virtual reality-based balance training group (VT) and received the training three times per week, 30 minutes per day, for six weeks. To measure static balance, the Tetrax Portable Multiple System (Tetrax Ltd, Israel) were used. To measure dynamic balance, the Berg Balance Scale (BBS) and functional reach test (FRT) was used, and regarding fall efficacy, the Korean Fall Efficacy Scale (K-FES) was used. Results: Tetrax significantly improved after the intervention in both the BT group and the VT group (p<0.05). The comparison between the two groups was not significantly. BBS and FRT result significantly improved after the intervention in both the BT and VT groups (p<0.001), while K-FES was significantly ameliorated in the VT group only (p<.05). Comparing the groups, there were more significant changes in the BBS (p<0.05) and FRT (p<0.01) result of the VT group than the BT group. Conclusion: A virtual reality-based balance training program is considered to be usable as an exercise program to prevent recurrence of falls in elderly women.

  • Recently, social technologies have been created to solve problems from businesses for the establishment of generational solidarity ecosystem in terms of employment, residential space, network and social capital, age, cognitive and environmental aspects. This is senior-friendly healthcare business system aimed at meeting the senior needs for health life to enjoy active consumption culture life even after retirement, becoming a catalyst for minimizing generational conflicts, preventing the cognitive and physical deterioration of seniority in the areas of life healthcare, fitness and well-aging, and expanding into systems necessary for seniority self-reliance. We would like to draw up the development and requirements of the concept of the service platform for the study of collective characteristics for generation solidarity with senior class and the establishment of a customized senior health life system for generation solidarity. This system is characterized by a platform that can prevent the decline of seniors' cognitive and physical functions and enhance emotional stability. It is significant in providing feedback on the risk perception index, fall index, and prevention training index information to the child through the analysis and extraction of the senior health index for risk perception, fall probability, and fall prevention.

Effects of Visual Cue Deprivation Balance Training with Head Control on Balance Function and Fall Index in Older People

  • Purpose: This study sought to investigate the effects of visual cue deprivation balance training by applying head control feedback to the balance function and the fall index in older people. Methods: The study was conducted on 26 older people at the S Hospital in Gyeongsansi. The patients were randomly divided into the experimental groupI (EGI, n=9), the experimental groupII (EGII, n=9), and the control group (CG, n=8). The three groups were trained for 30 minutes three times a week for four weeks. To assess the patients' static balance function, their limits of stability (LOS) was measured using the BioRescue system, (RM Ingenierie, France), composed of a pressure platform that can measure force in diverse ways, a computer, and a monitor. The dynamic balance function was measured using the Berg Balance Scale (BBS). Also, the fall index was measured to evaluate the risk of falling. A paired t-test was performed to compare pre- and post-training performance within the groups. One-way ANOVA was performed for comparing the three groups. A post hoc least significant difference (LSD) test was also performed. Results: The results of the LOS showed a significant difference after training in the EGI and EGII groups (p<0.05). As a result of the difference between the three groups, there was a significant difference after training (p<0.05). The results of the BBS showed a significant difference after training in the EGI group (p<0.05). As a result of the difference between the three groups, there was a significant difference after training (p<0.05). Conclusion: Visual cue deprivation balance training applying head control feedback is effective in improving the dynamic balance function in older people. It is also necessary to constantly maintain the head orientation by feedback and to properly control the head movement.

가상현실 기반 운동이 파킨슨병 환자의 균형, 보행 및 낙상효능감에 미치는 영향: 예비연구 (Effects of Virtual Reality-Based Exercise on Balance, Gait, and Falls Efficacy in Patients with Parkinson's Disease: A pilot study)

  • Purpose : The purpose of this study was to identify whether virtual reality-based exercise could improve on balance, gait and fall efficacy in patients with Parkinson's disease. Methods : Ten patients with Parkinson's disease were randomly divided into either an experimental or control group. The experimental subjects performed vertual reality-based exercise, whereas the control subjects performed conventional physical therapy for 4 weeks. The balance, gait and fall efficacy of all subjects were assessed by using the Measurement Training and Documentation (MTD) balance system, force platform system, Korean version of Berg Balance scale (K-BBS), 6 Minute Walk Test (6MWT), and Korean version of Fall efficacy scale (K-FES) at pre training and post training. Wilcoxon signed rank test was used to analyze change before and after intervention in intra-group. Mann Whitney U test was used to analyze changes of all variables in inter-groups. Results : Subjects in the experimental group showed significant improvements in difference of weight distribution, K-BBS scores, antero-posterior and medio-lateral sway length, ground reaction force (GRF), 6MWT, and step length following training. The changes of difference of weight distribution, K-BBS scores, AP Sway Length, GRF, 6MWT, step length and K-FES scores in the experimental group were significantly more than them of the control group. Conclusion : The result of this study suggest that virtual reality-based exercise training is an intervention to improve on balance, gait, and falls efficacy in patients with Parkinson's disease.

  • 본 논문에서는 낙상상태를 감지할 수 있는 장단기 메모리(Long Short-Term Memory)를 이용한 낙상감지 시스템에서 은닉층 노드 수 변경에 따른 영향을 소개한다. 3축 가속도 센서를 이용하여 x, y, z축 데이터를 중력 방향과 이루는 각도를 나타내는 파라미터 theta(θ)를 이용하여 훈련을 진행한다. 학습에서는 validation이 진행되어 8:2의 비율로 훈련 데이터와 테스트 데이터로 나뉘며, 효율성을 높이기 위해 은닉층의 노드 수를 변화하며 훈련을 진행한다. 노드 수가 128일 때 Accuracy 99.82%, Specificity 99.58%, Sensitivity 100%로 가장 좋은 정확도를 나타내었다.

노인의 낙상방지를 위한 바이오피드백 훈련의 효과 (Effects of Biofeedback Training for Prevention of Falling in Elderly Persons)

  • Background: The purpose of this study was to investigate effects of six-week biofeedback training for prevention of falling in elderly persons. Biofeedback training for using the TETRAX system. Methods: Thirty healthy elderly persons(men=17, women=13) who were 79 years of mean age participated in sensory regulation training. They were trained for thirty minutes a day, three times per week. We measured subjects sensory regulated function by TETRAX system, and analyzed mean difference of observed variables by paired t-test between the pre and post test. Results: The first experimental group were significantly difference between pre and post test. The second control group were not significantly between the pre and post test. The third there are significant between group. Conclusion: The results of this study reveal that biofeedback training exercise will improve sensory balance function, and further studies needs to identify which specific factors are related to fall in the elderly population, and it is expected this study may contribute in reducing fall and therapeutic exercise in falling.

가상현실기반 낙하훈련시스템 개발 (Virtual Reality Based Fall Training System)

  • 가상현실은 게임, 오락, 통신, 스포츠, 건축 분야에서 활발히 적용되고 있다. 특히 교육 분야에도 많은 가상현실 기반의 교육 시스템이 개발되고 있어 효율적인 학습효과를 만들어내고 있다. 그러나 기술적 한계, 콘텐츠의 부족, 이론적 연구 부족 등으로 인해 아직은 군사용 교육 훈련 부분 적용되기에는 수준이 부족한 상태이다. 본 논문에서는 군사용 집단 강하 훈련에 적용 가능한 수준 높은 낙하훈련시스템을 개발하고, 개발에 필요한 핵심 기술과 구현방법을 연구하였으며, 개발 결과에 따른 효과를 분석하여 군사용 훈련시스템으로서의 가상현실기반 교육 시스템의 발전에 이바지하고자 한다.

  • In this research, new conceptual model is being established to advocate a hypothesis during the hands-on to guarantee an effectiveness of the training with a scientific procedures and techniques under the management of KCTC training system. From this, current requirements of facilitator's qualifications and technical standards can be correspond with educate and their individual characteristics. Establishing education model basis will increase facilitator's training and conservatism in the representative KCTC training operative as the actual fight; contribute to effective training procedures. Like this model, it can be flexibly applied to the units in armies' actual training in circumstantial situations other than KCTC training; also can be applicable in many quarters.

균형능력 운동치료 시스템을 이용한 치료가 급성기 뇌졸중 환자의 균형과 이동능력에 미치는 영향 (The Effects of Treatment With a TETRAX on Balance and Mobility in Acute Stroke Patients)

  • The purpose of the study was to determine the effects of balance training with 'TETRAX' system, a balance training and assessment tool, on balance and mobility in acute hemiplegic patients. Nineteen matched subjects were assigned randomly into either an experimental group or a control group. An experimental group with 10 subjects received balance training with 'TETRAX' exercise program and conventional physical therapy interventions 5 times per week during 4 weeks. A control group with 9 subjects received conventional physical therapy interventions 5 times per week during 4 weeks. Outcome measures were taken before and after 4 weeks of interventions using the Stroke Rehabilitation Assessment of Movement (STREAM), the Berg Balance Scale (BBS), gait speed, and the fall down index. Results indicated that both exercise groups improved significantly in STREAM, BBS, and gait speed (p<.05). The experimental group had a little improvement than the control group. Both exercise groups did not show statistical significance in fall down index (p<.05). Following 4 weeks of intervention, except gait speed there was no statistically significant difference between two groups. However, these findings suggest that conventional physical therapy interventions with visual feedback training could be effective on improving balance and mobility than conventional physical therapy alone in acute hemiplegic patients.
