20인승급 소형 위그선의 안전성 평가 (The Safety Assessment of Small WIG Craft in the 20-Passenger Class)

  • WIG crafts are a high speed vessel with features of dynamic supported craft. These crafts, which are predominantly of light weight and operate any substantially greater speeds than conventional craft such as bulk carrier, tanker, container ship, etc., could not be accommodated under traditional maritime safety instruments. It means that there is the need for risk and safety levels to be assessed on a holistic basis, recognizing that high levels of operator training, comprehensive and thoroughly implemented procedures, high levels of automation and sophisticated software can all make significant contributions to risk reduction. To response this requirement, the Interim Guideline for WIG craft(MSC/Circ.1054) were developed in the view of the configuration of WIG craft, which fall between the maritime and aviation regulatory regimes. This paper reviews a safety assessment process and methodology to be used in the design phase of a new ship. The process and methodology is based on the risk-based approach and is applied to safety assessment in concept development phase of small WIG craft in the 20-person class.

Correlations between Foot Sensory Impairment, Timed Up and Go Test and Berg Balance Scale, and Mental Activities for Prevention of Falling in Elderly People

  • PURPOSE: This research was designed to study the correlations between physical activities such as foot sensory impairment, balance and ambulatory abilities, as well as fall risk factors and mental activities such as cognitive performance and depression in order to provide fundamental data for the prevention of falling in the elderly. METHODS: A total of 36 elderly people over the age of 65 years who voluntarily joined the research were selected as subjects. Sensory perception in both feet was measured by using a monofilament in 10 parts with each part given one point. The points for each foot were integrated to analyze the correlations between physical and mental activities indices, and the results were analyzed by using Pearson's correlation coefficient. RESULTS: The results of this research show that the extent to which senses in feet were impaired was correlated to TUG, a functional activity index involved in tranfers, gait, and turning movements, which was correlated to BBS (balance index) as well as MMSE-K and K-GDS (mental activities index). BBS was interrelated to FES-K (physical activities index) and MMSE-K. The muscular strength of the right ankle dorsiflexor had mutual relations with FES-K. CONCLUSION: The results of this research indicate that TUG is widely correlated to foot sensory impairment as well as general physical and mental activities in elderly people. BBS was also shown to be correlated with TUG, FES-K, and MMSE-K. Thus, it might be necessary that TUG and BBS be included as two items in physical and mental check-ups for the elderly, and further studies on correlations using evaluation items for physical and mental activities should lead to the simplification of the evaluation criteria.

국내 가계대출의 특징과 결정요인 분석: COVID-19를 중심으로 (Analysis of Characteristics and Determinants of Household Loans in Korea: Focusing on COVID-19)

  • Purpose - Since COVID-19, the government's expansion of liquidity to stimulate the economy has resulted in an increase in private debt and an increase in asset prices of such as real estate and stocks. The recent sharp rise of the US Federal fund rate and tapering by the Fed have led to a fast rise in domestic interest rates, putting a heavy burden on the Korean economy, where the level of household debt is very high. Excessive household debt might have negative effects on the economy, such as shrinking consumption, economic recession, and deepening economic inequality. Therefore, now more than ever, it is necessary to identify the causes of the increase in household debt. Design/methodology/approach - Main methodology is regression analysis. Dependent variable is household loans from depository institutions. Independent variables are consumer price index, unemployment rate, household loan interest rate, housing sales price index, and composite stock price index. The sample periods are from 2017 to May 2022, comprising 72 months of data. The comparative analysis period before and after COVID-19 is from January 2017 to December 2019 for the pre-COVID-19 period, and from Jan 2020 to December 2022 for the post-COVID-19 period. Findings - Looking at the results of the regression analysis for the entire period, it was found that increases in the consumer price index, unemployment rate, and household loan interest rates decrease household loans, while increases in the housing sales price index increase household loans. Research implications or Originality - Household loans of depository institutions are mainly made up of high-credit and high-income borrowers with good repayment ability, so the risk of the financial system is low. As household loans are closely linked to the real estate market, the risk of household loan defaults may increase if real estate prices fall sharply.

입원 노인의 연령별 영양불량 위험요인 (Risk Factors of Malnutrition by Age in Hospitalized Older Adults)

  • Purpose: Malnutrition affects all age groups, but older adults are particularly more vulnerable to nutritional deficiencies. This study evaluated the age-specific factors affecting malnutrition in hospitalized older adults. Methods: A retrospective study was conducted on inpatient elderly people who received artificial nutrition from 2010 to 2017. Data of demographics, diagnosis, type of nutrition therapy, number of comorbidity, fall risk assessment, Acute Physiologic Assessment and Chronic Health Evaluation (APACHE) score, and intensive care unit admission were collected. Malnutrition was defined as a body mass index (BMI) of less than 18.5 kg/m2. Patients were classified as the young-old (65~74 years old), the old-old (75~84 years old), or the oldest-old (85 years old or older). Results: A total of 7,130 older adults were included, and 4,028 patients were classified as the young-old, 2,506 into the old-old, and 596 into the oldest-old. Proportion of malnutrition was higher in the oldest-old compared to the other groups. In multivariate analysis, parenteral nutrition, alcohol, and high risk of falls were factors affecting malnutrition in all groups. Parenteral nutrition and alcohol in the young-old, high risk of falls in the old-old, and male sex in the oldest-old were the factors affecting malnutrition by the age group. Conclusion: Older age was the most significant factor affecting malnutrition. Specific strategies by age are needed to improve nutritional status in hospitalized older adults as influencing factors for malnutrition vary among different age groups.

입원환자의 낙상 실태 및 위험요인 조사연구 (Incidence of Falls and Risk Factors of Falls in Inpatients)

  • Purpose: The objective of this study was to report the incidence of falls in hospitals and analyze the risk factors for falls. Methods: This study used data on 1,216 patients who experienced falls from 2015 to 2017 during their hospitalization. The data was collected from the falls incident reports and patient' electronic medical record of hospital. Data were analyzed with descriptive statistics using Chi-square test, Fisher's exact test and multiple Poisson regression analysis with the SAS 9.4 Results: The incidence of falls was 1.38 per 1,000 patients days (2015), 1.81 per 1,000patients days (2016) and 1.99 per 1,000patients days (2017). The incidence of injury caused by falls (level III~V) was 0.05 per 1,000patients days (2015), 0.04 per 1,000patients days (2016) and 0.06 per 1,000patients days (2017). The largest number of falls occurred during night shift (42.5%), specifically in the patients' room (70.8%), and medical unit (66.0%). Average age of fallers was 69.1 years and 61.7% of them were older than 71 years. CCI and the patient's department have statistically significant differences in injury or injury levels from falls, but the integrated nursing care services had no significant difference in injury or injury levels from falls. Conclusion: The result of this study can be used as a reference for establishing a fall prevention strategy for hospitalized patients by presenting index values such as the fall rate.

한국 노인의 넘어짐과 연계된 인체손상에 대한 이해와 예방: 체계적 문헌 고찰 (Understanding and Prevention of Fall-related Injuries in Older Adults in South Korea: A Systematic Review)

  • Background: Fall-related injuries in older adults are a major health problem, and the risks and mechanisms of these injuries should be affected by race, culture, living environment, and/or economic status. Objects: Research articles have been systematically reviewed to understand fall-related injuries in older adults in South Korea. Methods: 128 published research papers have been found through the Korea Citation Index and the Korean Studies Information Service System, and reviewed in various perspectives, including incidents, fall death rates, medical costs, causes, injury sites and types, locations where falls occurred, prevention strategies, scholarly fields interested in fall injuries, and the role of physical therapy. Results: Fall-related injuries were found to be more common in women than in men, and the number of incidents increased with age, with the highest rate found in individuals over 85 years old. Risk of fall injury was associated with education level, comorbidities, and fear of falling. Common places where falls occurred included the bathroom, living room, stairs, and hallway. Common types of injury included bruises, fractures, and sprains in the lower extremities. Intervention strategies included exercise programs, education, and protective clothing. Scholarly fields interested in fall-related injuries in older adults included medicine, nursing, physical therapy, occupational therapy, physical education, pharmacology, oriental medicine, biomedical engineering, design, clothing, and textiles. Physical therapy intervention using proprioceptive neuromuscular facilitation has been used to improve one's balance. Conclusion: Any movement during the activities of daily living can lead to a fall. Physical therapists are highly educated to analyze human movements and should be involved in more research and practices to solve fall-related injuries in older adults.

시각적 바이오피드백 시뮬레이션훈련이 불완전 척수손상환자의 균형에 미치는 효과 (Effect of Visual Biofeedback Simulation Training for Balance in Patients with Incomplete Spinal Cord Injury)

  • 불완전 척수손상환자에게 시각적 바이오피드백 시뮬레이션훈련의 적용을 통해 균형에 미치는 영향을 알아보고자 하였다. 불완전 척수손상환자 중 기능향상을 기대할 수 있는 ASIA장애척도 C, D환자 15명을 실험군 7명, 대조군 8명으로 무작위 분류하였다. 균형진단과 바이오피드백 훈련이 가능한 Tetrax Interactive Balance System을 이용하여 시각적 바이오피드백 시뮬레이션훈련을 6주간 일주에 3회씩, 1회에 15분간 총 18회를 실시하였다. 분석방법은 집단 내 전과 후의 차이는 대응표본 t검정을 시행하였고, 두집단 사이 전후 변화량의 차이는 독립표본 t검정을 사용하였다. 실험군은 자세 흔들림을 측정하는 안정성지수와 낙상의 위험정도를 나타내는 낙상지수 및 각각의 체중 변화를 감지하는 체중분포도(A, C, D, LEFT, HEEL)와 체중분포지수에서 유의하게 향상되었고, 자세조절능력을 나타내는 동조화에서는 좌우뒤꿈치(AC)간과 좌우앞꿈치(BD)간을 제외하고 모두 유의하게 향상되었다(p<.05). 대조군은 체중분포도(A, C, LEFT)에서만 유의하게 향상되었다(p<.05). 실험군과 대조군간의 변화량에서 체중분포도(A, B, C, HEEL)와 안정성지수 및 낙상지수, 그리고 동조화 AD에서만 유의한 향상을 보였다(p<.05). 이 연구의 결과로 볼 때 불완전 척수손상환자에게 부가적으로 균형증진을 위한 시각적 바이오피드백 시뮬레이션훈련은 균형능력향상에 있어서 유용할 것이며 추후 더 많은 연구들이 시행되어야 할 것이다.

뒤로 걷기 운동에 따른 뇌졸중 편마비 환자의 균형능력 및 보행능력의 변화 (Changes in Balance and Gait Following Backward Walking Exercise in Hemiplegic Stroke Patients)

  • PURPOSE: The purpose of this study was to identify whether backward walking exercise was more effective than conventional physical therapy for balance and gait in hemiplegic stroke patients. METHODS: Eighteen patients with chronic stroke were randomly assigned to the experimental (n=9) or control (n=9) group. The experimental and control group performed backward walking exercise and conventional physical therapy, respectively, for 8 weeks. Stability Index (SI) and Weight Distribution Index (WDI) during standing were assessed using the Tetrax Balance System. The Timed Up and Go (TUG) test and Korean version of the Berg Balance Scale (K-BBS) were used to evaluate balance and fall risk. Walking speed, stride length, and step length on the affected side were measured using the 10-Meter Walk and ink foot printing tests. Wilcoxon signed-rank and Mann-Whitney U tests were used for within- and between- group comparisons, respectively. RESULTS: The experimental group showed significantly higher changes in SI (p<.01), WDI (p<.01), TUG (p<.001), and BBS score (p<.001) following intervention compared with the control group. The experimental group also showed significantly greater improvements in walking speed (p<.01), stride length (p<.001), and step length on the affected side (p<.001) after intervention compared with the control group. CONCLUSION: Backward walking exercise is an effective intervention to improve balance and gait in hemiplegic stroke patients.

우리나라 치매노인의 낙상 유병률과 위험요인: 고령화연구패널조사 결과를 이용하여 (Prevalence and Risk Factors for falls of Older Adults with Dementia in Korea: Based on the Korean Longitudinal Study of Aging)

  • 본 연구는 치매 노인의 치매 관련 요인 및 동반질환이 낙상 위험도에 미치는 영향을 파악하고자 실시한 자료분석 연구이다. 한국고용정보원에서 실시한 2018년 제7차 고령화연구패널조사를 이용하여 치매 노인 119명을 분석 대상으로 포함하였다. 치매 관련 요인은 치매 유병 기간 및 치매로 인한 일상생활 제한 여부에 대한 응답자료를 활용하였고, 동반질환은 고혈압, 당뇨, 비만 자료를 사용하였다. 수집된 자료의 통계분석은 SPSS statistics 22.0을 사용하여 로지스틱 회귀 분석을 실시하였다. 분석대상자의 치매관련 요인 및 동반질환 모두 낙상 위험도 증가에 유의한 영향을 미쳤으며, 특히 비만, 당뇨, 고혈압, 치매로 인한 일상생활 제한, 치매 유병 기간 순으로 영향력이 큰 것으로 도출되었다. 본 연구는 치매 노인의 낙상 위험을 높이는 요인을 확인함으로써 낙상 예방 프로그램의 구성에 우선되어야 할 요인을 제시하였다. 본 연구의 결과를 바탕으로 치매노인 케어에 치매유병기간과 일상생활 제한에 따른 낙상예방 전략과 동반질환으로 인한 낙상 고위험군의 집중 관리와 보행 보조도구 등 안전 보조도구 사용 훈련이 필요할 것이다.

The Effects of Repetitive Sit-to-Stand Training with a Paretic-side Asymmetrical Foot Position on the Balance of Chronic Stroke Subjects

  • Purpose: This study aimed to improve the asymmetrical weight-bearing ratio, by applying different repetitive sit-to-stand training methods to the paretic-side foot of hemiplegic patients, as well as to provide the necessary information for applying balance training with hemiplegic patients. Methods: The subjects were divided into two groups: a spontaneous foot group and an asymmetrical foot group. They all performed repetitive sit-to-stand training five times a week for a total of six weeks. The sit-to-standing movement was studied using standardized clinical tests. The Biodex Balance System, Time up and go test (TUG), 5 times sit-to-stand test (5XSST), and functional reach test (FRT) were used to measure the static and dynamic standing balance of the patients. Results: In the balance system measurement, the results for the overall index, ant-post index, med-lat index, fall risk index, 5XSST, and FRT after the training differed significantly between the comparison groups (p<0.05). In the evaluation of dynamic balance, the differences in TUG did not differ significantly between the comparison groups after the training (p>0.05). Conclusion: The study found that the asymmetrical group showed significant increases in static and dynamic balance in comparison to the spontaneous group after repetitive sit-to-stand training. Based on this result, it is clear that training in an asymmetrical position with the paretic foot back can increase the left-right stability limit and the anterior-posterior stability limit, thus improving balance control.
