항암치료 및 교통사고로 유발된 불안에 대한 한의학적 치료 1례: 증례 보고 (Korean Medicine Treatment for Anxiety Induced by Anticancer Therapy and Traffic Accident: A Case Report)

  • Objectives: The purpose of this study was to report the efficacy of using Korean medicine treatment, for anxiety induced by chemotherapy and traffic accident. Methods: We treated a patient's anxiety with Korean medicine treatments (Yangshimtang-Gamibang, ear acupuncture, acupuncture, cupping, interferential current therapy, and herbal acupuncture therapy). To evaluate the results of this treatment, we measured the Beck Anxiety Inventory, EuroQol-Five Dimensions, Numeric rating scale, Neck Disability Index, Oswestry Disability Index, total sleep duration, time duration to fall asleep, and the number of wake-up times during sleep. Results: Improvements of the Beck Anxiety Inventory, EuroQol-Five Dimensions, Numeric rating scale, Neck Disability Index, Oswestry Disability Index, and total sleep duration were observed after the treatments. Conclusions: This study showed one case of a patient who received Korean medicine treatment for the anxiety induced by anticancer therapy and traffic accident, and improvement was observed.

지역사회 일부 대학생의 수면습관이 건강에 미치는 영향 요인 (Effects on health by sleeping habits from few local university students)

  • Background & Objective, Methods: This study aims to analyze the reason why if university students feel lack of sleep and study how much we know about correct sleeping habits and thus increase attention on correct sleeping habits and prepare plans for applying this into our lives thus 210 male and female students from 2 universities in Daegu were studied during May 10 and 15 of 2006 and the summary and results are as follows. Results: Sleeping is very important since it as much as takes about 1/3 of our lives. However, most of students do not sleep well. Therefore, this study researched the reason for not sleeping well and the status on incorrect sleeping habits subjecting 210 university students. The results of this research showed that average sleeping hours are for $6{\sim}8$ hours and majority of students went to bed before lam and got up before 7am. Majority of students went to bed late to watch TV or doing computers from both male and female students followed by attending various gatherings. It took $10{\sim}20$ minutes to fall asleep and reasons for waking up while sleeping was with no special reason. The most sleepy time was after meals at 39% followed by during class, during daytime, while using transportations. More female students answered to take nap than male students and majority of them answered as taking nap for less than 2 hours. And, 51% answered that they slept well and 49% answered that they did not, and majority of them answered the reason for not sleeping well as being sensitive, or because of dreams and nightmares. Chronic fatigue was the biggest cause for lack of sleeping followed by fall in concentration, skin diseases. Conclusions: The results of this study showed that we need to take plenty of sleep by exercising regularly or sleeping for daily appropriate hours, maintaining peaceful mind before going to bed, avoid taking naps and limiting the time for watching TV and doing computers to develop correct sleeping habits. Also, we hope that it will Provide an opportunity to live healthy and quality lives mentally as well as physically by realizing and practicing the importance and necessity of sleeping.

불면증에 자하거(紫河車) 약침(藥針)을 응용한 1례 (The Case of regulation of Insomnia with Jahageo Herbal acupuncture)

  • The Common Symptoms of insomnia are to be hard to sleep, not to get deep sleep, often wake up at night, be easy not to fall asleep again after waking up. In serious conditions, an insomniac can never get to sleep overnight. Herbal acupuncture therapy is especially used for patients who cannot take an oral medication. Another advantage of herbal acupuncture therapy is immediate effect of the herbal potency which is not destroyed during digestion. One patient who have taken western drugs for long time, was treated with Jahageo herbal acupuncture at BaekRo and AnMyen. effectively his insomnia was regulated. So We consider that Jahageo herbal acupuncture is useful in regulation insomnia.

유리섬유 제조업체 근로자의 건강장해 (Medical Surveillance of Glass Fiber Workers in Korea)

  • The industrial use of MMMF(man-made mineral fibers), has been increasing, particularly since the banning of most asbestos products. Fibrous minerals can cause health abnormalities currently associated with occupational exposure to glass fiber. This study was conducted to evaluate health risks of glass fiber manufactory workers within the country. We examined questionaries, physical examination including auscultation, chest x-ray, pulmonary function test for 488 male workers, to go through their dermal itching symptoms and respiratory evaluation. we had the results as follows. 1. In 45% of the workers itching had been expressed at their entrance. At that time we was investigating, 18.5% had itching, and most of them complained it when they fall asleep and night. The Sequent itching site is waist and groin, upper and lower extremity in order, and it had been expressed mainly during summer and winter. 2. As the results of ventilatory functions test, 6.0% were obstructive type, 1.0% were restrictive type. So, glass fiber exposures should be controlled or elimination by protective devices in the workplace. 3. The means of FVC, $FEV_1,\;FEV_1%$ were in normal range. As the comparison of ventilatory functions by age groups, MMF was decreased significantly for the group, 50 years old and more than other groups. And the comparison by the serving periods at glass fiber producing factory, MMF was decreased for the workers had worked for $11\sim15$ years. Therefore, MMF be more sensitive index in the evaluation of ventilatory impairments caused by glass fiber workers.

비행시차와 일중리듬 (Jet Lag and Circadian Rhythms)

  • 누구나 시차가 큰 여행을 할 때 몇일 간 비행시차증이라고 불리우는 증상을 경험하게 된다. 비행시차증은 수면박탈, 비행요인, 지연요인의 복합적인 원인으로 인해 생기는 하나의 증상군이라고 말할 수 있다. 특히 빠른 시차변화로 인한 생리적 지연효과(Jet lag)는 외적 비동조화, 내적 비동조화, 그리고 수면상실의 결과를 낳는다. 인간의 수면을 조절하는 기전에 있어 일중체계가 중요하다. 즉, 평균적인 수면-각성주기는 중심체온의 주기와 내적 비동조화가 일어나더라도 수면경향, 졸리움, 자발적 수면 기간, 그리고 렘수면 경향은 중심체온의 내인성 일중주기에 따라 통제된다. 수면의 구성요소중에서 서파수면은 중심체온의 주기보다는 수면시작시간에 따라 나타나며 이전에 깨어있었던 기간이 길수록 강력하게 나타난다. 따라서 수면은 일중체계와 항상성 기전의 상호작용으로 조절된다. 비행시차 후에 변화되는 수면양상을 이해하는데 있어 일중 체계 이외에 도항상성 기전을 고려하여야한다. 수면에 대한 일중리듬체계의 영향과 수면의 항상성 과정이 비행시차후 도착지에서의 수면양상을 설 명할 수 있을 것이다. 도착지에서의 적응은 통과한 시간대 수, 여행 방향, 일주기 리듬의 부조화에 적응 할 수 있는 개인별 능력에 따라 다르다. 도착지의 시간적 단서에 빨리 노출되어 일중체계의 위상반응곡선에 의한 재동조화를 촉진시키고 수면의 항상성 과정을 고려하여 도착지의 밤 이전까지 충분히 깨어 있는 것이 Jet Lag를 극복하고 적응하는 지름길일 것이다.

순환제 교대근무자의 수면양상과 주간의 졸리움 및 성격요인과의 관련성 (Sleep Patterns, Daytime Sleepiness and Personality Factors in Rotating Shiftworkers)

  • 연구배경 : 본 연구는 순환제 교대근무가 전반적인 수면양상과 주간의 졸리움에 미치는 영향을 평가하고, 주간의 졸리움과 성격요인 사이의 상호관련성에 대해 알아보고자 시도되었다. 방법 : 20-30대 순환제 교대근무 간호사 41명과 대조군 39명을 대상으로 Beck 우울척도(BDI), 상태특성불안 척도(STAI), 고려대학교 병원 수면장애 클리닉 수면력 검사지, Epworth 졸리움 척도(ESS), 성인용 성격요인 검사(16PF) 등을 실시하였다. 결과 : 교대근무로 박탈된 수면시간은 평균 64.26분 ${\pm}\;14.54$이고, 수면에 어려움을 겪는 빈도($1.24\;{\pm}\;1.17$일/주 vs $0.67\;{\pm}\;1.31$ 일/주, p < 0.05), 잠들기 어려울 때 수면까지 걸리는 시간($103.05\;{\pm}\;73.48$ 분 vs $70.00\;{\pm}\;60.08$, p < 0.05), 불면증이 있을 때 취하는 수면시간($204.25\;{\pm}\;79.90$ 분 vs $280.44\;{\pm}\;111.59$ 분, p < 0.001), 최근들어서의 기력의 변화($x^2\;=\;4.16$, p < 0.05), 수면에 대한 걱정($x^2\;=\;11.08$, p < 0.05), 낮잠자는 여부($x^2\;=\;4.98$, p < 0.05) 등의 문항에서 교대근무를 시행한 대상군과 대조군 사이에 유의한 차이를 보였다. 또한 Epworth 졸리움 척도의 결과도 순환제 교대근무자에서 $8.68\;{\pm}\;3.04$로 대조군의 경우인 $6.86\;{\pm}\;3.04$인 경우보다 유의하게 높았다(p < 0.01). 성격요인검사에서 자아강도 요인(C factor, $R^2\;=0.283$), 민감성 요인(I factor, $R^2\;=0.358$), 도덕성 요인(G factor $R^2\;=0.470$)이 주간에 느끼는 졸리움의 정도에 대해 유의한 설명량을 보였다(p < 0.001). 결론 : 순환제 교대근무 간호사들은 불면증, 수면부족등과 같이 여러가지 수면곤란을겪고 있으며, 이는 심한 불안감, 기력 및 집중력 저하의 원인이 되는 것으로 사료된다. 또한 주간의 졸리움도 순환제 교대근무자에서 유의하게 많이 겪고 있었다. 순환제 교대근무로 인해 겪는 주간의 졸리움의 정도를 예측하는 성격요인으로는 신경증적 행동의 속성인 정서적 불안정을 나타내는 낮은 자아강도 요인, 정서적으로 민감하고 의존적인 성격을 나타내는 높은 민감성 요인, 그리고, 직업에 만족하지 못하는 속성을 의미하는 낮은 도덕성 요인 등이 보여졌다.
