Background: The aim of this study is to determine the correlation between clinical assessment scales, self-assessment scales, and surface electromyography (SEMG) for facial nerve palsy. Methods: This study assessed 32 cases of facial nerve palsy on the first visit, 11 cases on the second visit, and 9 cases on the third visit to the Korean medicine hospital, university. This study was conducted from October 22, 2022, to December 22, 2022. The patients were evaluated using SEMG, clinical assessment scales, and self-assessment scales 3 times. The House-Brackmann grading systems (HBGS), Yanagihara unweighted grading system (Y-score), facial disability index, numerous rating scale, and accompanying symptoms of facial nerve palsy were used for assessment. Moreover, statistical correlation was analyzed using Pearson correlation. Results: On Visit 1, Significant correlations were observed between the results of SEMG and other clinical assessment scales as well as between SEMG-F (frontalis) and different parts of the Y-score. On Visit 2, significant correlations were observed between the results of SEMG and HBGS as well as between SEMG-F and the detailed parts of the Y-score. On Visit 3, significant correlations were observed only between SEMG-F and the detailed parts of the Y-score. A significant correlation was also observed between the changes in the clinical assessment scales on Visits 1 and 3 and between the changes in SEMG-F and those in the patient self-assessment scales. Conclusion: These findings suggest that SEMG can be used to evaluate facial nerve palsy in conjunction with the use of other clinical assessment scales.
Background: The purpose of this study was to determine the effect of massage and muscle reeducation training with conventional treatment in patients with facial paralysis. Methods: Twenty-five patients with facial nerve paralysis were randomly allocated to 3 groups: massage, muscle reeducation training, and control groups. Therapeutic intervention for the massage (n=8) and muscle reeducation training (n=8) groups consisted of conventional therapy such as application of hot pack and electrical stimulation plus massage therapy and muscle reeducation training, respectively. The control group (n=9) received only conventional therapy. Therapeutic intervention for each group was performed 6 times per week for 4 weeks. The patients were assessed by using the House-Brackmann Grading System (H-B grade) and Yanagihara Unweighted Grading System (Y grade) once every week. Results: The H-B and Y grades improved significantly in all 3 groups after a 4-week intervention (p<.01). At 3 and 4 weeks, the H-B and Y grades of the massage group improved significantly when compared with those of the control group (p<.01). Muscle reeducation training group showed significant improvements in the scores of the two grades with time when compared with the massage and control groups (p<.01). The rate of change in the H-B grade was significantly different between the control and muscle reeducation training groups (p<.01), and that of change in the Y grade was significantly different between the control and muscle reeducation training groups (p<.01) and between the control and massage groups (p<.01). Conclusion: These findings suggest that massage and muscle reeducation training are more effective in improving the condition of patients with facial nerve paralysis than conventional therapy. In particular, the results of this study indicate that muscle reeducation training can be recommended by clinicians since it provides more benefits.
Objectives : This study was performed to evaluate the effect of facial rejuvenation acupuncture on cases of sequela of facial nerve palsy. Methods : This study was carried out in respect of 5 cases of sequela of facial nerve palsy patients which had been treated in Dong-Shin university oriental hospital from 2009 year with House-Brackmann grading system(HBGS) above Grade IV. Two patients with sequelae of peripheral facial nerve palsy were treated with Facial Chuna Manual Therapy (FCMT) and acupuncture over 6 months. The House-Brackmann (HB) scale, facial nerve grading system 2.0 (FNGS), the scale of Peitersen, the scale of Murata, and the Numeric Rating Scale (NRS) were used to assess the effects of treatment. The HB scale, FNGS and NRS scores showed improvement for both patients (Case 1: HB scale 5 to 3, FNGS 4 to 2, NRS 10 to 5; Case 2: HB scale 5 to 3, FNGS 4 to 3, NRS 10 to 2.5) following 6 months of treatment. The scores for the Peitersen and Murata scales showed improvement over 6 months in Case 1 (Peitersen 2 to 1, Murata 10 to 7), but there was no change in Case 2 over the test period (4 months). FCMT and acupuncture may help patients with sequelae of facial palsy.

Objectives : This study was to compare the incipient grade and improvement rate between each opposite gender and part on peripheral facial nerve paralysis. Methods : We investigated 64 cases of patient with peripheral facial nerve paralysis and divided into four groups with each gender and affected part and compared their incipient grade and rate of improvement. All groups were evaluated by Yanagihara's unweighted grading system before treatment and in everyday and after treatment. Results & Conclusion : 1. There's a difference of incipient grade between each opposite affected lesion in male. But there was not significant statistics. 2. There's no difference of incipient grade between each opposite affected lesion in female. 3. There's a difference of incipient grade between each opposite gender, affected their right side. But there was not significant statistics. Left side affecting palsy is more severe than right in male, and reversed results in female. But there were not significant statistics. There's no differences between each affected lesion in female. 4. There's no differences of improvement rate between each affected lesion in female. 5. The group, affected right side had better rate of improvement than another in male. But it's not significant statistics. 6. Male group had better rate of improvement than female in both gender, affected its left lesion. But it's not significant statistics.
Background: Idiopathic facial nerve palsy, or Bell's palsy (BP), is a common and important disease. Recurrent Bell's palsy has been known as a rare entity with only a few cases in the literature. Methods: A total of 111 consecutive patients with acute BP patients were enrolled at Daegu Catholic University Hospital from July 2005 to March 2007. We classified the patients into two groups - single BP and recurrent BP - and compared them by demographic data, clinical features, MRI findings and prognosis. The degree of BP was graded according to the House and Brackmann facial nerve grading system. Results: Recurrent BP was observed in 10 (9%) patients. The number of recurrence was varied from 2 to 5. The mean age of first attack in recurrent BP was $35.70{\pm}23.65$ years old and was earlier than that of the single BP ($50.94{\pm}16.21$ year). The larger proportion of the single BP had an abnormal enhancement of affected facial nerve (91.3%) than the recurrent BP (50%). The recurrent BP showed worse prognosis than the single BP. The associated conditions, etiology, and clinical features were similar between two groups. Conclusions: In comparison with single BP, recurrent BP showed earlier onset of first BP attack, less frequent abnormal enhancement of facial nerve on MRI, and worse prognosis.
Purpose: The purpose of study was to determine the effects of muscle reeducation training in patients with acute facial nerve paralysis. Methods: Thirty patients were randomly assigned to either the experimental group (n=15) or the control group (n=15). The experimental group received muscle reeducation training for 20 minutes after electrotherapy and the control group received conventional electrotherapy for only 40 minutes. Therapeutic intervention for each group was performed four times per week for four weeks. The patients were measured for recovery of paralysis using the House-Brackmann Grading System (H-B grade), the Movement Distance of Mouth, Nasolabial Angle (NA), and Facial Disability Index (FDI). Results: In within group comparison, the experimental group showed significant improvements for all variables (p<0.01). In comparison between two groups, the experimental group showed relatively greater significant improvements for all variables (p<0.01). Conclusion: These findings suggest that muscle reeducation training is more effective than conventional therapy in improving the condition of patients with facial nerve paralysis. In particular, the results of this study indicate that muscle reeducation training can be recommended by clinicians since it provides more benefits.

Herpes zoster oticus which is called Ramsay Hunt syndrome, geniculate ganglion herpes and otic herpes, is characterized by a viral prodrome with otalgia, vesicular eruptions, facial paralysis, hearing disturbance, tinnitus, vertigo and other symtoms. Among cranial nerves, trigeminal and facial nerves are the most commonly affected in patients with herpes zoster oticus, but on rare occasions 8th, 9th, 10th, 11th, 12th cranial nerves and even the upper cervical nerve are influenced as well.. In general, it has poor prognosis compared with Bell's palsy, leaving permanent facial nerve dysfunction. We report a case of Herpes zoster oticus, which was fully recovered by acupuncture and herbal medication within 12 days. We used $FEMA^{11)}$ and House-Brackmann's grading system to assess the degree of paralysis in each part of face.

Background and Objectives: The aim of the present study was to assess the feasibility of a PC-based facial asymmetry assessment program (PC-FAAP) and to compare the results of PC-FAAP with subjective regional scoring by raters in acute unilateral peripheral facial nerve paralysis (FNP). Subjects and Methods: Participants were divided into 3 groups with 8 participants per group: group I, normal; group II, mild to moderate FNP; and group III, severe FNP. Using the PC-FAAP, the mouth asymmetry ratio (MAR), eyebrow asymmetry ratio (EAR), and complete eye closure asymmetry ratio (CAR) were calculated by comparing the movement of tracking points on both sides. The FNP grading scale (FGS) integrated each score, and the scores were weighted with a ratio of 5:3:2 (MAR:CAR:EAR). Subjective regional scoring was measured on a 0-100 scale score by three otologists. PC-FAAP and subjective scoring were compared in each group regarding the consistency of the results. Results: The mean scores of the MAR, EAR, CAR, and FGS of each group were significantly different. PC-FAAP showed significant differences between the three groups in terms of MAR, EAC, CAR, and FGS. PC-FAAP showed more consistent results than subjective assessment (p<0.001). The PC-FAAP was significantly more consistent in group I and group III (p<0.001 and p=0.002, respectively). FGS in group III was the only parameter that showed a more consistent result in PC-FAAP than the subjective scoring (p=0.008). Conclusions: An FNP grading system using a PC-based program may provide more consistent results, especially for severe forms.
