Camurati-Engelmann's Disease on $^{99m}Tc$-MDP Bone Scan (Camurati-Engelmann 병의 뼈스캔 소견)

  • A 24 year-old female presented for a $^{99m}Tc$-methylene diphosphonatae (MDP) whole body bone scan due to chronic pain in the bilateral lower extremities that has aggravated since 2002. She was diagnosed with Camurati-Engelmann disease (CED) based on the clinical and radiological findings in 2002, and she re-visited our institute to evaluate disease status at this time. CED is a rare autosomal dominant type of bone dysplasia characterized by progressive cortical thickening of long bones, and narrowing of medullary cavity, and thus presents with typical clinical symptoms and signs such as chronic pain in the extremities, muscle weakness, and waddling gait. On the $^{99m}Tc$-MDP bone scan performed to evaluate disease status, intense increased uptake was seen in the skull, facial bones, bilateral scapulae, bilateral long bones, and bilateral pelvic bones, which clearly demonstrated the extent of CED involvement.

  • The purpose of this study is to describe the clinical availability of a variety of intraoral local flaps in reconstruction of oral soft tissue defects, Forty patients with oral soft tissue defects were treated by tongue, buccinator, palatal, labial, facial artery musculomucosal, buccal fat pad, and masseter muscle crossover flap. Total 43 intraoral flaps were used to reconstruct a variety of intraoral soft tissue defects, such as oronasal fistula, oroantral fistula, traumatic deformities and other. The age of patients ranged from 7 to 72 years, with mean age of 39.6 years. Follow up period ranged from 2 to 66 months, mean follow up period of 21.6 months. There were 9 complications, of which four were partial necrosis, three infections, one total necrosis, and 1 speech problem. Except for total necrosis, most of the recipient sited healed uneventually without severe morbidity. We consider that a variety of intraoral local flaps can be available for reconstruction of small of moderate large intraoral soft tissue defects.

Traumatic Displacement of the Globe into the Maxillary Sinus: Case Report (외상성 상악동 안구탈출의 치험례: 증례보고)

  • Purpose: Globe displacement due to a blowout fracture is a rare clinical phenomenon. The authors present reduction of a globe displacement to the maxillary sinus due to trauma suffered in a fall and the reconstruction of a large defect left in the medial and inferior orbit. Methods: A 39-year-old male patient was unable to open his left eye after being struck on the periorbital area by a metal edge. Laceration was not noted in that area but we were unable to observe the intraorbital globe. A facial computed tomography (CT) scan showed that the globe was displaced through the maxillary sinus. A transconjunctival approach was used to access the infraorbital margin and the globe entrapped in the inferior margin of the orbit was successfully reduced. A large defect in the medial and inferior orbit was reconstructed using a graft from the iliac bone. Results: In 5 months after the operation, no atrophy of the globe was seen. Both sides retained a similar shape. A satisfactory functionality outcome in terms of improved extraocular muscle movement, and a satisfactory aesthetical outcome were achieved. Conclusion: The authors report the reduction of a globe displaced to the maxillary sinus following a fall and the reconstruction of the large defect left in the medial and inferior orbit.

Treatment of Severe Blepharoptosis after Blow Out Fracture (안와 파열골절 후 발생한 중증 안검하수의 치료)

  • Purpose: Blepharoptosis can result from either congenital or acquired causes. Blow out fracture or facial bone fracture including blow out fracture can be one of the causes. Authors experienced 3 cases of severe blepharoptosis after blow out fracture treated only with observation after reduction of associated fracture. Methods: Reconstruction of orbital wall was conducted on all cases diagnosed as blow out fracture using 3 dimensional computed tomography, and conservative treatment was done on accompanying severe blepharoptosis. Results: At the time of injury, all cases showed severe blepharoptosis requiring frontalis muscle transfer for correction. But blepharoptosis was recovered in an average of 18 weeks without any surgical procedure except reconstruction of orbital wall. Conclusion: Once Blepharoptosis occurred after blow out fracture, thorough evaluation must be done at first. If definitive cause of blepahroptisis cannot be found as authors' cases, injury of oculomotor nerve may result in blepharoptosis. So, as for blepharoptosis after blow out fracture, conservative treatment following reconstruction of fractured orbital wall can be one of good management.

  • A 2 year-old intact male Shih-tzu dog was presented with a history of snakebite on the left upper lip. He was depressed and had swelling, erythema and pain around the left muzzle. Both mandibular lymph nodes were swollen. Fang marks were not clear, but there were scratch wounds and prolonged bleeding. Hemogram revealed neither thrombocytopenia nor echinocytosis. Increased CK value in serum chemistry and myoglobinuria in urine analysis meant muscle injury. With careful monitoring, fluid, corticosteroid, antihistamine and antibiotics were administered. After the application of ice pack and suction, local symptoms changed for the worse. However, any other systemic disorders were not identified. After 5 days, the facial swelling had resolved completely.

The Effect of Korean Medicine Treatment on Left Basal Ganglia Infarction with Hemiplegia Symptoms: Case Report (기저핵 뇌경색 환자의 편마비 한방치료 1례)

  • Objectives : The purpose of this study was to report the effect of Korean medical treatment on left basal ganglia infarction. Methods : We performed acupuncture and administered herbal medicine to one patient to alleviate symptoms of hemiplegia such as motor disorder, facial palsy, and dysarthria. Manual Muscle Test(MMT), Hand Grip Test(HGT), Gait Level(GL), and Dysarthria grade were used to evaluate status of the patient. Results & Conclusions : The results of this research showed that overall symptoms of hemiplegia in the patient were improved. According to the results, the Korean medical treatment is considered to be effective on patients of left basal ganglia infarction to treat symptoms of hemiplegia. Further studies with larger sample sizes are needed to examine this issue.

  • Drug-induced dyskinesia is an involuntary muscle movement caused by various dopamine receptor-blocking drug exposure, such as antipsychotics, antidepressants, and antiemetics. Causative drug removal is the main treatment for drug-induced dyskinesia whenever possible because its pathophysiology lacks a universally accepted mechanism; however, the symptoms can persist for years or decades in many patients even after causative drug removal. Herein, we present a case of drug-induced dyskinesia in a 61-year-old female patient who consumed medication for approximately 10 years for her depression, anxiety, and insomnia. Cervical and facial dyskinesia was suggested to be related to perphenazine and levosulpiride administration. The patient received acupuncture, pharmacopuncture, herbal medicine, and chuna treatment for 81 days during hospitalization. The symptoms were evaluated using the Abnormal Involuntary Movement Scale, Toronto Western Spasmodic Torticollis Rating Scale, Tsui's score, and Numeric Rating Scale, which revealed remarkable improvement, suggesting the effectiveness of combined Korean medicine for drug-induced dyskinesia.

  • In case of treating the functional type of anterior cross bite, removing the cause in early stage prevents from turning to skeletal type, leads to perform normal function, and has improved facial appearance. Functional Regulator III (FR III), one kind of the Functional regulator(FR)s suggested by R$\ddot{o}$lf Fr$\ddot{a}$nkel in 1966, applied to patients with the functional and skeletal anterior crossbite in early mixed, and permanent dentition. This appliance improves unbalanced power condition by blocking abnormal muscle-power effect, so that normal growth can be expected. In this case report, favorable results were obtained by selecting clinical cases of children in their early mixed dentition with functional cross bite. 1. FR IIIs were applied to patients with anterior crossbite for 5~6 months. Anterior crossbite patients were corrected favorably, nevertheless they didn't show any horizontal skeletal-changes by buccal shields. 2. Normal occlusion and esthetic facial contour were achieved from dental movement of maxillary and mandibular anterior teeth while the mandible rotates to posterior and inferior direction.

  • $T4K^{TM}$(Myofunctional Research Co, Australia) is one of the myofunctional appliance developed to be used in children of mixed dentition. Myofuncitonal appliance stimulate the facial, masticatory and tongue muscle and help to balance the muscular force. Labial bow included in the device exerts strength in excessively labial inclineded upper jaw, Lip bumper blocks strength of the mouth to prevent abnormal strength exerted in lower jaw, Tongue tag secures proper position of tongue, and additional exercise is not required for child patients. For the more, simpler design and softer texture of device prmoted cooperation of patients during use. This case report is to present the satisfactory results gained by using $T4K^{TM}$ on Class II patients. Comment 1. $T4K^{TM}$ was applied in Class II malocclusion patients of mixed dentition with expected space insufficient to gain facial improvement. 2. Excessive overjet, overbite were improved. 3. Main effects are regarded to have been achieved by development of lingual slant of upper jaw, labial slant of lower jaw, and lower part of jawbone. 4. Bad habits, such as mouth breathing, can also be adjusted.

A Study of Surface Electromyography Measurement of Orbicularis oris motion in Healthy People (정상인의 구륜근 운동 시 표면근전도(SEMG) 측정연구)

  • Objectives : To find an acupuncture point where more exact Surface Electromyography(SEMG) measurement can be drown, through the study of measurement of orbicularis oris. Methods : Of healthy people from 19 to 40 years of age, who did not fall under exclusion criteria (22 males and 22 females), were selected as subjects, after relaxation for 10 minutes, and they were told how to pronounce 'O' and 'U' with their lips puckered. The SEMG figures were measured with attaching disposable electrode on acupuncture point of right-and-left Hwaryo(LI19) and 1 cun away from Seungjang(CV24) on both sides when the subjects pronounced 'O' and 'U'. Results : The average value was highest on left 1 cun away from Seungjang(CV24) in pronouncing 'O' and 'U', and the average SEMG value was higher in the order of right 1 cun away from Seungjang(CV24), right Hwaryo(LI19), left Hwaryo(LI19). Average of the lower orbicularis oris is statistically higher than that of the upper orbicularis oris, which has significant meaning. However, there was no significant difference when compared by pronunciation. Average of percentage mark of differences of right-and-left measured value of each pronunciation and each acupuncture point : Pronounced 'O' and the upper part of orbicularis oris: $16.76{\pm}11.29%$, pronounced 'O' and the lower part of orbicularis oris: $22.41{\pm}12.92%$, pronounced 'U' and the upper part of orbicularis oris: $17.10{\pm}9.89%$, pronounced 'U' and the lower part of orbicularis oris : $19.20{\pm}10.82%$. Conclusion : The difference of pronunciation will not affect the results in SEMG measurement. In addition, the average of the lower orbicularis oris is statistically and significantly higher than that of the upper orbicularis oris.
