The Application State of the Sunnybrook Facial Grading System for Facial Palsy Patients : A retrospective study (안면마비 환자에 대한 Sunnybrook Facial Grading System의 적용 실태 분석 : 후향적 관찰연구)

  • Objectives : Among the assessment tools for evaluating facial function, the House-Brackmann scale is used as a standard tool, but it has some shortcomings. The Sunnybrook Facial Grading System can assess the after effects of facial palsy and facial movement by each part of the face. By understanding the application state of this Sunnybrook Facial Grading System, we intend to analyze the relationship between House-Brackmann scale score and Sunnybrook Facial Grading System score so that we can examine the advantages of the Sunnybrook Facial Grading System as a more accurate tool. Methods : We screened both inpatients and outpatients who visited the Facial Palsy Center at Kyung Hee University Hospital for Korean medical treatment and were evaluated with the Sunnybrook Facial Grading System from December 2015 to October 2016. A total of 159 out of 166 patients were studied, including basic characteristics and missing data. We used descriptive statistics for general features of patients and SPSS Ver.18 for statistical analysis. Results : House-Brackmann scale and Sunnybrook Facial Grading System have high negative correlation through Pearson Correlation Coefficient with a score of -0.884. Analyzing outlier data resulting from relation analysis between the House-Brackmann scale and the Sunnybrook Facial Grading System showed many outliers when the damaged state of each part of the face is different. Conclusion : Sunnybrook Facial Grading System can make up for faults of the House-Brackmann scale, which is inferior in accuracy when each damage status of each part of the face is different. Sunnybrook Facial Grading System performs a detailed assessment of facial function and sequelae of facial palsy easier than the House-Brackmann scale.

A better facial contour accomplished by parotid duct preserving superficial parotidectomy

  • Depression of facial contour after parotidectomy is still challenging to many of surgeons. A 68-year-old man presented with a 4-month history of a painless swelling in both parotid area. The mass was multiple and fixed at the parotid region. We conducted a parotid duct preserving bilateral superficial parotidectomy by one-stage operation to remove the multiple tumors. A lazy S incision was made in both preauricular area and the peripheral branches of the facial nerve were identified using surgical landmark. After dissecting the branches of the facial nerve and parotid duct, main parotid duct was preserved but only small fine ductules from the superficial lobe were ligated. Parotid gland was excised from its anterior aspect with about 1 cm of normal parotid tissue margin. The patient was followed up for 6 years to evaluate postoperative parotid gland function and the computed tomography (CT) was taken. Patient was satisfied with no significant complication such as sunken changes in facial contour, facial nerve function. As far as we know, it is the first study to compare long-term soft tissue contours of soft tissue of duct preserving superficial parotidectomy with duct sacrificing superficial parotidectomy by means of CT findings.

  • Nowadays, eyes detection is required and considered as the most important step in several applications, such as eye tracking, face identification and recognition, facial expression analysis and iris detection. This paper presents the eyes detection in facial images using Harris corner detection. Firstly, Haar-like features for face detection is used to detect a face region in an image. To separate the region of the eyes from a whole face region, the projection function is applied in this paper. At the last step, Harris corner detection is used to detect the eyes location. In experimental results, the eyes location on both grayscale and color facial images were detected accurately and effectively.

  • Objectives : To report the patient bilateral facial palsy, because it is unusual. Methods : One patient with bilateral facial palsy was selected. Herbal medicine(Cheunggansoyosan, bubidongyeom-2) was administered and acupuncture was applied for twice a day in 73 days. Facial function was evaluated by House-Brackmann scale, Yanagihara's scale and Standardization of muscular paralysis. Results : Cheunggansoyosan, bubidongyeom-2 and acupuncture treatment improved facial function after 73 days of treatment. Adverse effects were not reported. Conclusions : This study shows that the Korean medicine treatment was effective in improving bilateral facial palsy.

  • This study was to determine the effects of a massage and oro-facial exercise program on spastic dysarthric patients' lip muscle function using an electromyogram (EMG). Three subjects with Spastic Dysarthria participated in the study. The surface electrodes were positioned on the Levator Labii Superior Muscle (LLSM), Depressor Labii Inferior Muscle (DLIM), and Orbicularis Oris Muscle (OOM). To examine lip muscle function improvement, the EMG signals were analyzed in terms of RMS (Root Mean Square) values and Median Frequency. In addition, the diadochokinetic movements and the rate of sentence reading were measured. The results revealed that the RMS values were decreased and the Median Frequency moved to a high frequency area. Diadochokinesis and sentence reading rates were improved.

Restoration of Blink Function in the Paralyzed Rabbit Using Electroactive Polymer (안면신경마비 동물모델에서 전기활성고분자를 이용한 눈 깜박임 기능의 회복)

  • There have been numerous modalities to recover blink function of orbicularis oculi muscle in patients with facial paralysis. However, there is still no optimal method for reanimation of eyelid. In this study, we tried to recover blink function of paralyzed rabbit's eyelid with the ion polymer metal composite (IPMC) which is one of the electroactive polymers that is spotlighted as artificial muscle. We manufactured IPMC by plating the platinum over perfluorosulphonic acid polymer ($Nafion^{(R)}$). IPMC was coated by Norland optical adhesive for the purpose of insulation and keeping it from dry. IPMC modifications by roughening the surface of Nafion, repetitive plating (maximum 4 times) with platinum, and lengthening the width of IPMC were done. The facial paralysis was induced in the rabbit by sectioning of facial nerve at the main trunk. After minimum period of 4 weeks, IPMC was inserted in the paralyzed rabbit's eyelid. By modification, the force generated by IPMC was enhanced. Restoration of blink function in paralyzed rabbit was achieved on electrical stimulation of the IPMC by 5 voltage direct current. IPMC can be promising option for facial reanimation, but further studies are needed to enhance the efficiency of IPMC.

  • A neurilemmoma of the facial nerve presenting as a parotid mass is a rare neoplasm and has been reported infrequently in the surgical literature. Diagnosis is usually by tissue biopsy intraoperatively and treatment is surgical with preservetion of facial function. This is a case of a solitary neurilemmoma involving the main trunk of the facial nerve in the posterior part of parotid gland continuing up to vertical segment of the facial nerve, in which the facial nerve had to be sacrificed and free autogenous nerve grafting was done.

Region-Based Facial Expression Recognition in Still Images

  • In Facial Expression Recognition Systems (FERS), only particular regions of the face are utilized for discrimination. The areas of the eyes, eyebrows, nose, and mouth are the most important features in any FERS. Applying facial features descriptors such as the local binary pattern (LBP) on such areas results in an effective and efficient FERS. In this paper, we propose an automatic facial expression recognition system. Unlike other systems, it detects and extracts the informative and discriminant regions of the face (i.e., eyes, nose, and mouth areas) using Haar-feature based cascade classifiers and these region-based features are stored into separate image files as a preprocessing step. Then, LBP is applied to these image files for facial texture representation and a feature-vector per subject is obtained by concatenating the resulting LBP histograms of the decomposed region-based features. The one-vs.-rest SVM, which is a popular multi-classification method, is employed with the Radial Basis Function (RBF) for facial expression classification. Experimental results show that this approach yields good performance for both frontal and near-frontal facial images in terms of accuracy and time complexity. Cohn-Kanade and JAFFE, which are benchmark facial expression datasets, are used to evaluate this approach.

  • In discussing the effects of adenoids on the development of the face and dentition, it is important to note their influence on the mode of breathing and to relate this to specific facial types and dentition. This study, therefore, assumed that the ability to adapt to individual's neuromuscular complex is various. And tried to investigate the effects of reduced nasal respiratory function on the development of dentition by facial type. This paper is based on children patients with enlarged adenoids and comparing them to data taken from a control group with normal respiratory function. Among the three facial types, the most statistical significant difference was observed from dolichofacial type between experimental and control group. In dolichofacial type, the experimental group showed labioversion of upper incisor, decrease in the width of upper arch, increase in overjet, increase in the rate of cross-bite, and increase in the height of palatal vault. No significant difference was observed between the two groups in the inclination of upper and lower incisors in mesofacial type, but the experimental group was observed to show decrease in the width of upper arch and increase in the height of palatal vault. On the other hand, in brachyfacial type, no significant difference was observed between the two groups in dentition variables except showing linguoversion of upper incisor. The results, which were observed in dolichofacial type, consist with Nordlund's theory of Compression.

Emotion Recognition Method Based on Multimodal Sensor Fusion Algorithm

  • Human being recognizes emotion fusing information of the other speech signal, expression, gesture and bio-signal. Computer needs technologies that being recognized as human do using combined information. In this paper, we recognized five emotions (normal, happiness, anger, surprise, sadness) through speech signal and facial image, and we propose to method that fusing into emotion for emotion recognition result is applying to multimodal method. Speech signal and facial image does emotion recognition using Principal Component Analysis (PCA) method. And multimodal is fusing into emotion result applying fuzzy membership function. With our experiments, our average emotion recognition rate was 63% by using speech signals, and was 53.4% by using facial images. That is, we know that speech signal offers a better emotion recognition rate than the facial image. We proposed decision fusion method using S-type membership function to heighten the emotion recognition rate. Result of emotion recognition through proposed method, average recognized rate is 70.4%. We could know that decision fusion method offers a better emotion recognition rate than the facial image or speech signal.
