Osteopetrosis Tards (만발형골화석증)

  • Osteopetrosis is an uncommon hereditary bone disorder whose prominent radiologic feature characterized by increased bone density. The authors reported a 7-year-old male patient who referred from local dental clinic for dental problems such as early exfoliation of deciduous teeth(#54,73,83) and delayed eruption of permanent teeth(#31.41.36.46). The patient appeared as a poorly developed. Dental X-ray films showed early exfoliation of deciduous teeth, delayed eruption of permanent teeth, and rampant caries. Lateral view of skull demonstrated increased opacity of calvarium, facial bones, and skull base. Generally the skeletal density is greatly increased throughout all bones. Facial CT showed poor development of paranasal sinuses and mastoid air cells. No hematopoietic and neurologic complications such as anemia, thrombocytopenia, blindness and deafness were found. Also mental retardation was not found. The final diagnosis of this case was a osteopetrosis tarda. Sometimes patient with osteopetrosis tarda may be developed dental problems prior to severe systemic symptoms. The dentist can be the first clinician to see the patient. It is very important for the dentist to have the knowledge of the osteopetrosis and to care the patient's dental problems to prevent complication such as osteomyelitis of jaws.

  • Periventricular nodular heterotopia (PNH) is a malformation of cortical development in which normal neurons inappropriately cluster in periventricular areas. Patients with Mowat-Wilson syndrome (MWS) typically present with facial gestalt, complex neurologic problems (e.g., severe developmental delay with marked speech impairment and epilepsy), and multiple anomalies (e.g., Hirschsprung disease, urogenital anomalies, congenital heart defects, eye anomalies, and agenesis of the corpus callosum [CC]). MWS is mostly caused by haploinsufficiency of the gene encoding zinc-finger E-box-binding homeobox 2 (ZEB2) due to premature stops or large deletions. We present a case report of a 9-year-old girl with PNH, drug-responsive epilepsy, severe intellectual disability, and facial dysmorphisms only in whom we performed whole-exome sequencing and found a de novo heterozygous missense mutation (c.3134A>C; p.His1045Pro) of ZEB2 (NM_014795.3; NP_055610.1). This mild case of MWS caused by a rare novel missense mutation of ZEB2 represents the first report of MWS with isolated PNH.

  • Osteochondroma is a common benign tumor of the axial skeleton, especially the distal metaphysis of the femur and the proximal metaphysis of the tibia, however, is occurred rarely on the facial skeleton. Development of the tumor is most frequently seen in the second or third decades of life. Typically, it arises directly from the cortex of the underlying bone, without any intervening zone of abnormal osseous tissue, and is covered by a cap composed of cartilage undergoing calcification. In case of osteochondroma of the mandibular condyle, its clinical features are occulusal change, facial asymmetry, headache, pain and click on temporomandibular joint, mouth opening limitation, and jaw deviation to involved site. This is a case report of a 13-year old woman who had mouth opening limitation and severe temporomandibular joint pain. We obtained successful results with surgical removal of the osteochondroma on the mandibular condyle.

Development of a Hybrid Recognition System Using Biometrics to Manage Smart Devices based on Internet of Things

  • In this paper, we propose a hybrid-recognition system to obtain the state information and control the Internet of Things (IoT) based smart devices using two recognitions. First, we use a facial recognition for checking the owner of the mobile devices, i.e., smartphones, tablet PCs, and so on, and obtaining the state information of the IoT based smart devices, i.e., smart cars, smart appliance, and so on, and then we use a fingerprint recognition to control them. Further, in the conventional system, the message of the state and control information between the mobile devices and smart devices is only exchanged through the cellar mobile network. Thus, we also propose a direct communication to reduce the total transmission time. In addition, we develop a testbed of the proposed system using smartphones, desktop computers, and Arduino vehicle as one of the smart devices. We evaluate the total transmission time between the conventional and direct communications and show that the direct communication with the proposed system has better performance.

Clinical Experience of Maxillary Fibrous Dysplasia: Shaving, Partial Maxillectomy & Maxillary Sinus Formation (상악골에 생긴 섬유성이형성증의 치료 경험: 안면윤곽술, 부분적 상악골 절제술과 상악동 형성)

  • Fibrous dysplasia(FD) of the bone is a slowly progressive, benign disease of unknown cause where normal architectures are replaced with fibrous and osteoid tissue. FD of the maxilla usually manifests as a bony enlargement with painless swelling and bone deformity, contouring to facial asymmetry. The lesion may involve the nasal fossae, orbits, or alveolus bone, causing diverse functional disturbance. Treatment options range from shaving to total maxillectomy and reconstruction depending on the presenting symptoms. Shaving, partial maxillectomy and maxillary sinus formation was performed in 5 patients with fibrous dysplasia in the past 2 years. Follow up period ranged from 1 month to 11 months. Aesthetic appearance, CT findings, and relief from symptoms were compared. In all patients, facial asymmetry was restored to symmetry and nasal obstructive symptoms were improved. With this procedure, expansion of the lesion will be controlled until puberty, preventing the development of new functional disturbances. After puberty, no further treatment can be anticipated due to the growth arrest inherent to the disease.

  • The osteochondroma, also known as osteocartilaginous exostosis, is one of the most common benign tumors of the axial skeleton. These tumours rarely affect the facial skeleton and a true osteochondroma of the mandibular condyle is a very uncommon entity. The tumors are usually slow growing, and symptoms develop over a long time. The most common presentation of the condylar osteochondroma consists of a changing occlusion, the development of facial asymmetry, and a posterior open bite on the affected side. In this case, we treated an osteochondroma of Lt. mandibular condyle through excision of the lesion via the temporal approach to the temporomandibular joint.

  • OpenBR is a framework for researching new facial recognition methods, improving existing algorithms, interacting with commercial systems, measuring perceived performance, and deploying automated biometric systems. Designed to facilitate rapid algorithm prototyping, it features a mature core framework, flexible plug-in system, and open and closed source development support. The established algorithms can be used for specific forms such as face recognition, age estimation, and gender estimation. In this paper, we describe the framework of OpenBR and implement facial recognition, gender estimation, and age estimation using supported programs.

  • With the rapid development of mobile technology the use of smartphone is spreading. In order to activate mobile banking and market in the future, the most important key is a secure financial transactoin. However, because many apps are developed without security check in proportional to the spread of smartphone, security threat is inevitably high. Current smartphone banking is processed as the way of the existing public certificate or OTP technique in the mobile environment, but many security hole about current technology is pointed out steadily. Therefore, in this paper we are to improve a existing security hole by reinforcing the security through multi-factor authentication and providing a physical non-repudiation.

  • Vascular malformations (VMs) in the head and neck region are present at birth and grow commensurately with the child, they can result in significant cosmetic problems for the patient, and some may lead to even serious life threatening hemorrhage. Although the molecular mechanisms underlying the formation of these VMs remain unclear, lesions are known to result from abnormal development and morphogenesis. Histologically, there are no evidence of cellular proliferation, but rather progressive dilatation of abnormal channels, which VMs are designated to their prominent channel types such as capillary, venous, lymphatic, arterial, and combined malformations. VMs with an arterial component are rheologically fast-flow, whereas capillary, lymphatic, and venous components are slow-flow. In this article, we review the clinical presentations, diagnosis, and management of VMs of facial regions with author's embolization and surgical treatment cases.

Recent advances in dental implants

  • Dental implants are a common treatment for the loss of teeth. This paper summarizes current knowledge on implant surfaces, immediate loading versus conventional loading, short implants, sinus lifting, and custom implants using three-dimensional printing. Most of the implant surface modifications showed good osseointegration results. Regarding biomolecular coatings, which have been recently developed and studied, good results were observed in animal experiments. Immediate loading had similar clinical outcomes compared to conventional loading and can be used as a successful treatment because it has the advantage of reducing treatment times and providing early function and aesthetics. Short implants showed similar clinical outcomes compared to standard implants. A variety of sinus augmentation techniques, grafting materials, and alternative techniques, such as tilted implants, zygomatic implants, and short implants, can be used. With the development of new technologies in three-dimension and computer-aided design/computer-aided manufacturing (CAD/CAM) customized implants can be used as an alternative to conventional implant designs. However, there are limitations due to the lack of long-term studies or clinical studies. A long-term clinical trial and a more predictive study are needed.
