  • Objectives : Placebo phenomena have been considered as a confounding factor of clinical trial. Expectancy and belief of acupuncture have not been evaluated quantitatively. The present study was performed to analyze the emotional and cognitive factor .of acupuncture and investigate whether the expectancy of acupuncture treatment is associated with the cognition of acupuncture. Methods : The expectancy and the perception of bodily sensation (PBS) of 22 participants were assessed using self-reported questionnaire. The subjects used the self assessment manikin (SAM) to rate each of the standard affective image of the international affective picture system (lAPS) and other acupuncture-related image. Based on the degree of expectancy, the high expectant (HE) and the low expectant (LE) group were classified. The thermal and pressure pain threshold was objectively evaluated using radiant-heat device and algometer. The degree of expected pain of acupuncture and the actual pain of painful stimulation was subjectively evaluated using facial pain scales (FPS). Results : Using SAlVI analysis, we identified the negative correlation between hedonic valence and arousal dimension on acupuncture-related visual cue. The degree of the PBS and general pain threshold did not show any significant difference between the HE and the LE group. The HE group rated the acupuncture images as more pleasant, more arousing, than the LE group. In addition, we also found that the higher expectancy marked the lower FPS of the expected pain of acupuncture, but not of the actual pain of painful stimulation. Conclusions : Our preliminary study identified the psychological dimensions of acupuncture-related visual cue. These findings indicate that the expectancy of acupuncture could affect the cognition of acupuncture.

  • Involvement of the facial nerve(herpes zoster oticus, Ramsay Hunt Syndrome) is a rather common clinical syndrome. It begins with unilateral ear pain, followed shortly by a peripheral facial palsy. Paresis or paralysis may affect the muscles of facial expression, which also close the eyelids. The levator palpebrae which is innervated by the 5th cranial nerve is spared, so the eye may remain open. The rash is usually confined to the tympanic membrane and the external auditory canal. It may spread to involve the outer surface of the lobe of the ear, anterior pillar or the fauces and mastoid. There also may be a loss of taste in the anterior two thirds of tongue. At time, the auditory nerve involvement produces tinnitus, deafness and vertigo. The 5th, 8th and 10th nerves and even the upper cervical spinal nerve can be involved presumedly on the base of spread of the infective process along anastomotic connections between the facial nerve. The facial paralysis is identical to that of Bells palsy. Frequently the recovery of facial nerve function is incomplete, leaving the patient with some residual facial weak ness. We experienced 2 cases of Ramsay Hunt Syndrome. The first patients, 55 year old male, visited our pain clinic on the day when his left facial nerve start to paralyze. We injected 6 ml of 0.25% bupivacaine into his left stellate ganglion 15 times. TENS was also applicated simultaneously. His facial paralysis was recovered completely 3 weeks after treatment without any complications. Another one, 53 year old male, visited us 7 weeks after onset of facial paralysis. He has been treated conventional oriental method(acupuncture, massage, warm application, etc). But the degree of his left facial paralysis didn't improve at all He has been treating with SGB 50 times and TENS for 2 months. Temporal and zygomatic branch of his left facial nerve recovered nearly completely but buccal and mandibular branch did not recover completely. We are willing to insist on the early treatment is the best choice in managing of Ramsay Hunt Syndrome.

  • Objective : Miller-Fisher syndrome(MFS) is a type of acute inflammatory polyneuropathy, a disease characterized by abnormal muscle coordination, absence of tendon reflexes, and paralysis of the eye muscle. This study is to report a case of delayed facial palsy and dizziness in Miller-Fisher syndrome, treated with traditional Korean medicine. Methods : A 57-year-old male was diagnosed with Miller-Fisher syndrome accompanied by neurological symptoms such as right facial palsy and dizziness. He received traditional Korean medicine treatment such as acupuncture(including pharmacopuncture, needle embedding therapy, electroacupuncture) and herbal medicine for 17 days. Results : Each neurological symptoms were improved after traditional Korean medicine. In case of facial palsy, House Brackmann Grading System was reduced form grade 3 to grade 2, and Sunnybrook Scale was improved from 39 to 55. Also, other symptoms such as dizziness and headache was also improved. Conclusions : This case report suggests that traditional Korean medicine may have a role in treating neurological symptoms such as facial palsy and dizziness in patient diagnosed with Miller-Fisher syndrome.

  • Objectives: The purpose of this study was to investigate the effect of Korean medicine treatment on facial asymmetric treatment in 5 cases of facial asymmetry correction by non - surgical treatment such as acupucture, chuna treatment, FCST (Functional cerebrospinal technique) and cranial osteopathy. Methods: We analyzed the initial charts of 5 patients who had undergone facial asymmetry in a Korean medicine clinic and measured the position and distance using the photograph, lateral cephalograms, and whole body radiograms. The results were as follows. Results: To quantify both soft and hard tissues to confirm the results of Korean medicine treatment of facial asymmetry, soft tissues quantitatively measure the displacement of the face, the slope of the left and right eyes, and the slope of the lip in order to grasp the positional displacement of the mandible. As a result, on the average, the correction effect as measured by the angle difference between A and C is $1.8{\pm}0.57$, the correction effect as measured by the angle difference between B and C is $1.4{\pm}0.89$, and the angle difference between D and the horizontal plane is $1.9{\pm}0.89$, and the angle difference between E and the horizontal plane is $1.9{\pm}0.89$. The result of reduced angle difference between A and C means that the head position shifted from the center of the body to the unilateral side was shifted to the center. The decrease in the angle difference between B and C means the restoration of the maxillary distortion relative to the mandible. In hard tissues, numerical values were measured based on the skull standard. The average distortion of the skull was $1.9{\pm}0.67$, and the distortion of the lower eye was $1.4{\pm}0.41$. Conclusion: General studies on facial asymmetric treatment are limited to treatments such as surgery and orthodontics. However, this study confirmed the possibility that facial asymmetry could be corrected by Korean medical treatment consisting of reversible non-surgical treatment rather than irreversible treatment such as surgery or orthodontic treatment. In particular, Korean medicine treatment is effective for muscular asymmetry, soft asymmetry, functional asymmetry, etc. The facial asymmetric treatment of Korean medicine is not limited to the face-centered correction, but the asymmetry of the whole body may be corrected as well.

  • 'Needle-embedding therapy', also called 'Acupoint Embedding Therapy' is the newly induced therapy which uses specialized tools. Embedded alien substance stimulates the acupoint and helps to keep the stimulation up. Needle-embedding therapy is mainly used for chronic diseases and deficiency syndromes, and also for other diseases including acute pain and excessive syndromes. Recently, clinical doctors are studying this therapy vigorously on the foundation of 'theory of meridian pathway' and applying for facial and body esthetics such as face-lift, correction skin troubles, depilation and so on. In spite of all these effect, theories for 'Needle-embedding Therapy' doesn't seem clearly arranged yet. So we, here, present the philological basis of 'Needle-embedding Therapy' from referring to old records and newly publicated papers and consider the new trend of this therapy in Korean and China.

  • Objective : There was no report on the treatment of Fascioscapulohumeral (FSH) Muscular Dystrophy by Oriental medicine. But the treatment conducted on the patient admitted to the Sangji Oriental Medicine Hospital from January 9, 2001 to February 23, 2001, a significant treatment result was yielded and would like to suggest treatment plan for the future treatments. Methods : Under the assumption that Korean Bee-Venom Therapy may be affective for treating FSH Muscular Dystrophy, the following points were administered : SI10(노유), SI11(天宗), BL23(腎兪), BL26(關元兪), ST36(足三里), LI4(合谷), Liv3(太衝), SI9(肩貞). CFC(Carthami Flos;紅花 and Cervi Pantotrichum Cornu;鹿茸) herbal extract was treated on the other acupuncture points. Sa-Am(Four needle technique) Acupuncture (tonifying SI5 and ST42, sedating GB41 and ST43) was done every day. For herbal medicine, TaeEumIn ChoWiSeungChung-Tang was given based on the constitutional diagnosis. Results : After 7 weeks of treatment, a remarkable improvement was made for facial muscular movement and muscular strength of the scapular and another regions. Conclusions : 1. Significant improvement in the muscular strength for the case of FSH Muscular Dystrophy was obtained with through Korean Bee-Venom Therapy, Four needle technique, and herbal medication. 2. For progressive muscular dystrophy, it is necessary to practice muscular strength recovery exercise in conjunction with Korean Bee-Venom Therapy. 3. Although this case yielded favorable result, further observation and study must be made to concretely prove the effectiveness of Korean Bee-Venom Therapy for treating muscular dystrophies.

  • The wrinkle is a typical sign showing aging. These days lots of people want to have a young and healthy appearance and many medical doctors are studying about wrinkle cure. In fact a young face without wrinkle looks more healthy and energetic. Therefore the wrinkle cure is important at the point of medical view. This research is an investigation of several studies about wrinkle cure using acupuncture needle. We have to consider the problem of the whole body which is related 12 meridians and meridian muscles as well as topical skin and tissues. There are three parts affecting wrinkle formation and cure. The first is a meridian and meridian muscle, the second is a local muscle and fascia on the head and neck, the third is a local skin structure. There are three kinds of method to cure the wrinkle locally. The first one is the perpendicular treatment of acupuncture, called the microneedle therapy, the process of this method is that lots of microneedles stab in the skin to dermis. The second one is the transverse treatment of acupuncture. The process of this method is that an acupuncture needle inserts just under the wrinkle skin. The third one is the incision treatment of acupuncture, called subcision, the process of this method is to cut the fibrous band which connect from the facial muscle to SMS(superficial musculoaponeurotic system) using cuttable needle. The hematoma after treatment let a collagen increase.
