  • Background : Sequelae symptoms of peripheral facial palsy include not only partial recovery of the paretic muscles but contracture, spasm, synkinesis and atrophy which cause significant functional, esthetic, and psychosocial disturbances to the lives of patients. Objective : This study is to investigate the prevalence, time of onset, patient's self assessment on the degree of understanding of the sequelae and the association of the degree of palsy with the appearance of sequelae. Methods : 106 patients with peripheral facial palsy were sequentially interviewed and examined. Results : 29 patients(27.4%) of 106 patients showed sequelae symptoms, of whom 19 patients(65.5%) showed sequelae symptoms during 4 to 6 months after the onset of palsy. The degree of understanding on the sequelae at the time of interview showed improvement compared to that at the time of first consultation to any physician but not to reach an sufficient understanding. No relation between the degree of palsy and the appearance of sequelae was observed.

  • Background: This retrospective study aimed to determine whether there were correlations between the number and type of accompanying symptoms of peripheral facial nerve palsy, and surface electromyography (SEMG) and clinical assessment scales to help diagnosis. Methods: There were 30, cases of peripheral facial nerve palsy at Visit 1 to the Korean Medicine Hospital, Dong-eui University, 22 cases at Visit 2 and 10 cases at Visit 3. The study period was from July 19, 2021 to November 31, 2021. Symptoms were evaluated three times (with two-week intervals which began 7 days from onset) using SEMG, clinical assessment scales and accompanying symptoms. In this study, the House-Brackmann grading system (HBGS), and the Yanagihara's unweighted grading system (Y-score) clinical assessment scales were used. The Pearson or Spearman correlation was used for statistical analysis. Results: On Visit 1, the number of accompanying symptoms of peripheral facial nerve palsy had no significant correlation with other measures. On Visits 1-3, the HBGS score had a significant negative correlation with the Y-score. On Visit 2, most of the mean values measured had significant correlations with each other although not between SEMG-Z and SEMG-O that Z means a zygomaticus muscle and O means a orbicularis oris muscle. On Visit 3, the number of accompanying symptoms significantly correlated with the clinical assessment scales. The HBGS score, Y-score, and SEMG measurements (except SEMG-Z) had significant correlations with each other. A significant positive correlation between SEMG-Z and SEMG-T was noted. Conclusion: We predict accompanying symptoms can be used to diagnose the peripheral facial nerve palsy including both clinical assessment scales and SEMG measurements at 2-5 weeks after onset.

  • This study was performed to evaluate the effect of high frequency therapy applied to the region branching to the external carotid artery for peripheral facial paralysis sequelae. A patient suffering with facial edema due to facial paralysis sequelae had been treated with acupuncture, high frequency therapy on the branch area to the external carotid artery for 7 weeks. The evaluation of clinical outcome was done by degree of swelling by measuring the distance of the face and skin temperature of face through digital infrared thermographic imaging. After treatment, the patient's degree of swelling and the temperature difference between the affected side and normal side was decreased. In addition, the temperature was changed in the entire facial area as well as the treatment point of high frequency therapy. This result shows that acupuncture combined with high frequency therapy at the region branching to the external carotid artery could be an effective way to improve facial blood flow, although further clinical studies will be needed.

  • Facial nerve palsy is not an uncommon disease encountered at the department of otolaryngology or pain clinic. It usually occurs following viral infection(Bell's palsy, BP). However, it is rare to develop bilaterally, and that not simultaneously. We experienced a rare case of bilateral facial nerve palsy. Patient first experienced left side paralysis, then right side paralysis approximately two months later. We treated the patient with serial bilateral stellate ganglion blocks(SGB). When left side paralysis improved, we performed unilatertal SGB for right side paralysis. Patient was also treated with intermittent electro-acupuncture stimulations, to right side first, then left side. Four months of treatment provided good results. This may be the first case, in Korea, of facial nerve paralysis bilaterally within a certain interval, treated with serial SGB and electro-acupuncture stimulation. However, the etiology of this bilateral BP was not fully confirmed as being a cause of viral origin.

  • Objectives : The purpose of this case is to report the improvement of two patients diagnosed with eight and a half syndrome after combination therapy of oriental and western medicine. Methods : We treated the patients with combination therapy of oriental and western medicine such as acupuncture, moxibustion, herbal medication, antithrombotic agents and steroid therapy. Changes of peripheral facial paralysis were evaluated using House-Brackmann facial grading system and the degree of dizziness, tenderness and pain of upper abdomen were assessed using numerous rating scale. Changes in motor grade of upper and lower extremities were evaluated using medical research council scale. Results : We have recently experienced two cases of eight and a half syndrome - a syndrome characterized by the coexistence of one and a half syndrome, a rare ophthalmoparetic syndrome characterized by a conjugate horizontal gaze palsy in one direction and an internuclear ophthalmoplegia in the other, and cranial nerve VII palsy. The one and a half syndrome was caused by acute cerebral infarction. Two patients in this report were improved through combination therapy of oriental and western medicine. Conclusions : We report the clinical course and treatment methods of eight and a half syndrome.

  • As indicators of life quality have recently shown great improvement, alternative medicine is becoming a mainstream health care pattern, in which early stage medical examinations and treatments are usually preformed before diseases occur. In this context, the ear-acupuncture method is increasingly used these day as one of important alternative medicine treatments. In this paper, we performed an experiment of facial region color analysis with kidney ear acupuncture point irritation. For this, we collected facial image samples before and after kidney ear acupuncture point irritation under the same condition. From the samples, the change rates of K, L values of CMYK color system and Lab color system, respectively, on kidney related facial parts were extracted and analyzed to investigate kidney function improvement.

  • Objective : In Modern society, patients with facial nerve paralysis are increasing because of many factors - irregular life, cold, overwork and stress etc. We have troubles in presuming the prognosis, though how to diagnosis and examine facial nerve paralysis are many. Methods : A clinical study was done on 89 patients who were diagnosed and treated as facial nerve paralysis(Bell's palsy) from January 2001 to May 2003 at the Dep. of Acupuncture and Moxibustion, college of Oriental Medicine, Woo-suk University. We classified 89 patients as the Sasang(四象) constitution, contributing factor, season, age and existence of diabetes items and analyzed, as we would research differences of changing point-Period from on set of Bell's palsy to the day which the change begins to be seen at the face- and improvement -Period which Bell's palsy is improved from on set to H-B grade II. Results : 1. Among the Sasang constitution, Soeumin($10.67{\pm}3.77days$) were faster than other constitutions, but Soyangin($16.25{\pm}6.75days$) were slower than other constitutions in changing point. Taeumin($4.12{\pm}1.49weeks$) were shorter than other constitutions, but Soyangin($4.88{\pm}2.11weeks$) were longer than other constitutions in improvement. 2. Among contributing factors, overwork and stress group were slower than other contributing factors in changing point($13.95{\pm}6.52days$), and longer than others in improvement($4.67{\pm}1.87weeks$). 3. Changing point and improvement of season, age, and existence of diabetes had the difference of the average according to an each item, but they were not statistically significant. 4. In 89 patients with Bell's palsy, average changing-point was $13.11{\pm}5.99$(days) and average improvement was $4.47{\pm}1.82$(weeks). Their correlation was 0.687 and statistically significance(P<0.01), therefore we could decide that their relation is highly correlation.
