Five cases of ocular toxocariasis confirmed by serology

  • We report 5 cases of ocular toxocariasis in Korean adults complaining of visual impairment along with floating or bubbling sensation. Fundoscopic examination revealed a retinal detachment along with exudate in 4 cases. They all showed typical reaction by ELISA and immunoblot against Toxocra excretory-secretory antigen. One case showed high level of anti-Toxocara IgE antibodies (34.000 Toxocara units/L) as well as increased level of serum total IgE antibodies and the specific IgE antibodies for 3 inhalant antigens, suggesting that high level of anti-Toxocara IgE antibodies was associated with an atopic status. Clinical manifestations were improved after the sequential use of steroids then mebendazole. We also suggest that ocular toxocariasis should be thoroughly investigated even when an evocative uniocular inflammatory lesion is encountered in peripheral retina without a systematic disease.

Effects of Kumsooyukkun-jeon on Asthma Induced by Ovalbumin in Mice (금수육군전이 Ovalbumin으로 수발된 Mice의 천식에 미치는 영향)

  • Objectives : To study the effects of Kumsooyukkun-jeon on asthma. Methods : Asthma was induced to Balb/c mice with ovalbumin using the method of Hatfield et al. We measured the histological profiles of lung and trachea, numbers of cellular compartments in the bronchoalveolar lavage fluid (BALF), numbers and morphology of the mast cells in the trachea, numbers of mucous-secretory cells in the bronchus, morphology of the bronchus, ultramicroscopical appearance of surface of trachea and number of cilia and mucous-secretory cells by scanning electron microscope. Results : 1. Hypertrophy of mucous membrane of trachea and bronchus and bronchioles in the lung, peritracheal, peribronchus and peribronchiolar inflammatory cell infiltration, and mucoid exudate deposition the lumen were observed in control groups but these phenomena were recovered in the Kumsooyukkun-jeon groups. 2. Cellular compartments including neutrophil and eosinophil were increased in the BALF of control groups but these phenomena were recovered in the Kumsooyukkun-jeon groups. 3. Degranulation and decrease of the numbers of mast cells were detected in the trachea of control groups. However, these phenomena were recovered in the Kumsooyukkun-jeon groups. 4. Shedding, decrease of cilia cell and increase of mucous-secretory cells in the surface of the trachea were measured in control groups but these phenomena were recovered in the Kumsooyukkun-jeon groups. Conclusions : It is considered that Kumsooyukkun-jeon has somewhat favorable effects on asthma.

A Study on The Lesions of The Mammary Glands of Slaughtered Holstein Cows in Gwangju Area (광주지역에서 도태되는 홀스타인 소 유방의 병변 조사)

  • Macroscopic investigations were made on the lesions of a total of 101 udders from slaughtered Holstein cows in Gwangju area. Incidence of supernumerary teats was 9.9%. Gross lesions were found in the surface of teats of 29.4% of cows; these lesions comprised papilomatosis (16.0%), fronds (8.9%) and injury at the tip (3.2%). Approximately 95% of the cows had slight to severe teat orifice keratosis. On the mucous membranes of the teat cisterns, fibrosis (22.7%) and congestion (8.9%) were observed. Gross lesions were found in the mammary glands of 68% of cows; these lesions comprised fibrosis (18.8%), serous exudate (4.9%), fatty tissue (4.9%), pus (4.0%), congestion (2.0%).

  • A 60-year-old male complained of headache, nasal discharge, and diplopia for over one month with a history of left upper molar extraction, and he had recently experienced severe discharge of purulent exudate from his left antrum. Under the diagnosis of maxillary sinusitis, the Caldwell-Luc operation was performed, and several fragments of amorphous white mucoroid materials were removed. In the histological observation, sinus mucosa was relatively well preserved, but showed diffuse infiltration with eosinophilic polymorphonuclears. Huge molds of mucormycosis were associated with the surface of mucosa. He was treated with amphotericin-B deoxycholate, resulting in the uneventful healing of the antral lesion. The current case of antral mucormycosis was very rare but effectively treated by surgical removal of antral mucosa and the following antibiotic therapy for the strong inhabitants of fungal molds. We also presumed that the patient was superinfected with commensal fungus of mucormycosis during broad spectrum antibiotic therapy for the previous dental infection.

Deep Neck Space Infection Caused by Keratocystic Odontogenic Tumor

  • Keratocystic odontogenic tumor (KCOT) is a benign cystic intraosseous tumor of odontogenic origin. An infection of a KCOT is not common because KCOT is a benign developmental neoplasm. Moreover, a severe deep neck space infection with compromised airway caused by infected KCOT is rare. This report presents a 60-year-old male patient with a severe deep neck space infection related to an infected KCOT due to cortical bone perforation and rupture of the exudate. Treatment of the deep neck space infection and KCOT are reported.

A Necropsy Case of the Huge Pseudo-diverticulous Cyst at the Rumen of a Cattle (소의 제1위(第一胃)에 형성(形成)된 거대(巨大)한 가성게실(假性憩室)에 관(關)한 일부검례(一部檢例))

  • In a cattle which had died of extreme emaciation caused by cachexic condition, a huge cyst of $45{\times}35{\times}20$cm, in size was found at the rumen. The cyst having with the narrow stalk which ended in blind sac attached firmly to the rumen by fibrinous or fibrous adhesion and, contained the lightly yellowish, clear exudative fluid of about 20 l, in volume. Grossly, the thickness of the cyst wall was 10~14mm, and its inner portion of the half was very rigid and colored with milky white in contrast with soft and edematous outer portion of the half. Microscopically the inner portion of the cyst wall was consisted of scarred fibrous tissue and possessed at its inner margin a little amount of the muscle fibers which had degenerated passably. These were confirmed as the muscle fibers in specific staining property by Van-Gieson's and Mallory-Azan stain. On regarding to above findings of gross and microscopic pictures, it was considered to be a pseudo-diverticulous cyst composed of the serosa and the muscle layer deriving from the wall of the rumen. And it was suggested that the cyst had been growing up to big size by storage of the plenty exudate arisen from its wall and was separated from the rumen at the end of its stalk.

Anti-Inflammatory Effect of Fermented Artemisia princeps Pamp in Mice

  • Essential oil-excluded Artemisia princeps Pamp var Ssajuarissuk (AP) was fermented with Lactobacillus brevis K-1, which was isolated from cabbage Kimchi, and the anti-inflammatory effects of AP and fermented AP (FAP) on lipopolysaccharide (LPS)-induced inflammatory response in peritoneal macrophages were investigated. AP and FAP inhibited LPS-induced TNF-$\alpha$, IL-$1{\beta}$, COX-2, iNOS and COX-2 expression, as well as NF-${\kappa}B$ activation. AP and FAP also reduced ear thickness, inflammatory cytokine (TNF-$\alpha$, IL-$1{\beta}$ and IL-6) expression and NF-${\kappa}B$ activation with 12-O-tetradecanoylphorbol-13-acetate (TPA) induced dermatitis in mice. Furthermore, AP and FAP also reduced exudate volume, cell number, protein amount, inflammatory cytokines (TNF-$\alpha$, IL-$1{\beta}$ and IL-6) expression and NF-${\kappa}B$ activation in carrageenan-induced air pouch inflammation in mice. The inhibitory effects of FAP were more potent than those of non-fermented AP. Based on these findings, we propose that FAP can improve inflammatory diseases, such as dermatitis, by inhibiting the NF-${\kappa}B$ pathway.

A Case of Transpalpebral Enucleation in Shih-tzu with Chronic Unilateral Glaucoma (편측성 만성 녹내장을 지닌 Shih-tzu의 안구적출술 치유예)

  • A 4-year-old female Shih-tzu with severe episcleral congestion and buphthalmos in left eye was referred to the Veterinary Teaching Hospital, Chonnam National University in October 1998. During two months in local veterinary clinics, the dog had been treated unsuccessfully with tarsorrhaphy and antibiotics. By history taking, cause of the glaucoma was ascertained as accidental nonpenetrated cornea trauma by owner, Ophthalmic examination revealed that the left eye was buphthalmic with severe episcleral congestion, pain and fever. The direct pupillary light response could not be evaluated in the left eye, but right eye had normal reaction. The intraocular pressure was 4.5 mmHg in the right eye and 33 mmHg in the left eye, as measured with a Schiotz tonometer, Ultrasonographic finding revealed hyperechoic vitrous body and central echofree material. The Schirmer tear test showed 20 mm in the left eye and 8 mm in the right eye. The hematologic examination revealed monocytosis. The initial medical treatment was admitted, but there was no response. Transpalpebral enucleation in the left eye under inhalation anesthesia with enflurane 1.2 vol% in oxygen was performed. The day after operation the left eye ravealed no exudate, and pain, and the systemic vital signs were normal. The prognosis is excellent.

Candidiasis in layer chickens in Korea (산란계에서의 칸디다증 국내 발생 사례)

  • Candidiasis is a mycosis caused by the mycelial yeast of the Candida genus which is opportunistic pathogen of humans, animals, and birds. Under some conditions such as prolonged antibiotic therapy, overcrowding, and immunosuppression, the opportunistic Candida can cause disease. Chicken candidiasis is sporadically occurred and characterized by unsatisfactory growth, listlessness, roughness of feathers, and death. A case of 23 weeks old layer with history of increased mortality and anemia was submitted to our Lab. At necropsy, the characteristic lesions were observed in the crop and proventriculus. The whitish pseudomembrane, that are peeled easily, was found in the crop. Proventriculus was swollen and the mucosa was covered with hemorrhagic exudate. The histological changes of the affected crop are epithelial hyperplasia, hydropic degeneration, and mycelia formation. Smears made from the necrotic mucosal surfaces of the crop revealed the presence of large number of yeast cells and mycelia. Pure cultures of yeast colonies were obtained from the potato dextrose agar. The yeast cells were identified as Candida albicans by gene sequencing. To our knowledge, this is the first report of candidiasis in chickens with anemia in Korea.

The Review of the Blood-Letting Therapy Written in the Donguibogam (동의보감(東醫寶鑑)에 나타난 자락요법(刺絡療法)에 대한 고찰)

  • Objectives : This study is designed to improve the practical possibility using Blood-Letting therapy through reviewing the literature recorded in the Donguibogam. Methods : Records on Blood-Letting therapy have been extracted from Donguibogam. First all articles containing the key words 'Hyul (血)' and 'Rak (絡)' have been extracted. Of them, records associated with Blood-Letting therapy had been sited and classified in terms of searching patterns for the Blood-Letting area, instruments for Blooding-Letting, cautions and side effects for the practice, applicable symptoms, acupoints/extraordinary acupoints, and Blood-Letting areas. Results : The regions capable of Blood-Letting are generally engaged with abnormal skin color such as black, bluish red, dark purple, dark blue, etc, and localized collection of fibrous exudate. There are several instruments used for Blood-Letting therapy; Samreongchim (三陵鍼), Chogyong (草莖), Pichim (鈹鍼), Lodaeguan (蘆管尖), Sachim (砂鍼), Saechim (細鍼), Gichim (?鍼), Eunjam (銀簪), nail, etc. Several cautions and side effects of the Blood-Letting therapy are presented. The Blood-Letting therapy could be applied to broad spectrum of symptoms. Body parts being applied were not only several acupoints but also effective points of the body. Conclusions : This study shows that Donguibogam represents Blood-Letting therapy in clear fashion so as to enhance the convenience and its practicality.
